摘要
通过对健康人216例、高血压病、冠心病、心肌炎各48例按病种随机分组,在农历每月朔日(初一)和望日(十五)进行24h 动态心电图监测,观察心律失常、心肌缺血的季日和时辰的变化趋势。结果表明:ST 段水平型压低、≥1mm 的检出率(%)和压低平均程度(mm·min/例)、冬季(分别为39.7%和345.2%)、春季(分别为25.6%和305.6%)明显高于夏秋两季(P<0.005—0.05)。室性早搏检出率和平均程度则分别以春季(67.8%)和冬季(321.7搏·级/例)为最重(P<0.005—0.05)。无论何季,在一日之中 ST 段压低和室性早搏时辰例次比率以卯-已时最高(分别为41.7%和34.2%)。房性早搏检出率无季间差异,但其平均程度以冬季(94.4搏/例)为最重(P<0.05)。房性早搏在春季以子-寅时、冬季以酉-亥时发生例次比率最高(分别为45.6%和48.4%)。
Two hundred and sixteen normal healthy individuals and three groups of patients with hypertension,
coronary heart disease and myocarditis(48 cases each)were divided at random into 24 groups to be under-
gone a 24-hour ambulatory monitoring on the first and the fifteenth date of each Chinese lunar month for
ischemic ST segment changes and arrhythmias to investigate the seasonal and diurnal variation on their re-
spective monitoring days.The result showed that,the incidence rate(%)and mean extent(in mm·min
per subject)of ischemic ST segment depression in the winter(39.7 and 345. 2 respectively)and the spring
(25. 6 and 305. 6 respectively)were both more serious than those in the summer and in the autumn(p<0.
005—0.05).The incidence rate(%)in the spring(67.8)and the mean extent(in beat grade per
subject)in the winter(321.7)of ventricular premature beat were both proved to be the most serious in the
whole year(p<0.005—0.05).A significant circadian increase in ischemic ST segment depression and
ventricular premature beat were noted,the highest incidence rates(41.7% and 34.2% respectively)being
recorded between Mao Shichen(5:00)and Si Shichen(11:00)during the whole days of the four sea-
sons.Although no evident seasonal difference was found in the incidence rate of artrial premature beat,the
average severity was usually the greatest(in beat per subject)in the winter(94. 4)(p<0.05).In addi-
tion,it was also noted that the highest episode of artrial premature beat during the whole day was detected
between Zi Shichen(23:00)and Yin Shichen(5:00)in the spring and between You Shichen(17:
00)and Hai Shichen(23:00)in the winter(45.6% and 48. 4% respectively).
