摘要
目的 研究乙肝 5项免疫标志物 (HBV -M)与乙肝核酸 (HBV -DNA)之间的关系 ,从而评价检测HBV-DNA载量的临床应用价值。方法 血清HBV -DNA采用美国PE公司生产 5 70 0荧光定量PCR仪进行定量检测。5项免疫标志物采用ELISA法。结果 表面抗原 (HBsAg)、e抗原 (HBeAg)和核心抗体 (抗 -HBc)阳性者 ,HBV -DNA阳性率为 99.5 %、e抗体、抗 -HBc阳性者 ,HBV -DNA阳性率为 5 6 % ;单项HBsAg阳性者 ,HBV -DNA阳性率为 5 0 .1% ;HBsAg和抗 -HBc阳性者 ,HBV -DNA阳性率为 4 8.3% ;5项免疫标志物皆阴性、含有 3个抗体或 2个抗体以及注射乙肝疫苗后单项抗 -HBs阳性者 ,HBV -DNA阳性率为 0。结论 HBV -DNA载量与抗原存在呈正相关性 ,而与抗体含量呈负相关性 ;慢性乙型肝炎患者机体无法清除乙肝病毒 ,HBV复制是激发机体免疫损害的关键环节 ,因此抗病毒治疗对减少或阻断肝病的发作和病情进展是十分必要的 ;HBV -DNA检测对乙肝早期诊断、传染性识别。
Objectives To study the relationship between 5 types of immune markers for hepatitis B virus(HBV-M) and HBV-DNA,and to evaluate the clinical significance of determining HBV-DNA.Methods Serum HBV-DNA was determined by PCR assay while 5 types of HBV-M by ELISA methods.Results Of patients positive for HBsAg,HBeAg and Anti-HBc,the HBV-DNA positive rate was 99.5%. Of patients positive for HBeAb and Anti-HBc,the HBV-DNA positive rate was 56%.Of patients positive for HBsAg,and Anti-HBc,the HBV-DNA positive rate was 48.3%.HBV-DNA positive rate was 0 in patients negative for 5 types of immune markers,containing three or two types of antibodies and positive for Anti-HBs after injection with hepatitis B vaccine.Conclusion There was a positive good correlation between carrying capacity of HBV-DNA and the antigens content.Instead,a negative correlation was observed between carrying capacity of HBV-DNA and the antibody content.HBV-DNA determination can be used to monitor the true state of HBV infection and replication,and it is useful for judging effectiveness of Anti-virus treatment.
