摘要
目的 :探讨突出髓核的占位与神经根受压程度的关系 ,及在决定是否采用手法或手术治疗中的意义。方法 :选择 2组腰椎间盘突出症患者 ,其中 a组患者突出髓核巨大 ,b组患者突出较轻 ,同时观察 2组患者直腿抬高高度 ,和手法治疗前后 b组患者直腿抬高高度及 CT上髓核占位的改变。结果 :2组患者直腿抬高高度无显著差异 (P>0 .0 5 ) ,b组患者手法治疗前后直腿抬高高度有显著性差异 (P<0 .0 1 ) ,而 b组患者治疗前后 CT各层面叠加对照 ,无 1例患者髓核形态大小发生变化 ,其矢状径无显著性差异。结论 :突出髓核占位大小不是决定神经根所受到的挤压的程度的唯一因素。突出髓核大小并不是判断是否采取手法或手术治疗的指征。
Objective:To study the relation between the space occupation of protruded nucleus pulposus inside spinal canal and the degree of the nerve root compression which might play a key role in the decition of the treatment-surgery or nosurgery.Methods:Two groups subjects of with lumber disc herniation were selected in the study , group a with enormous protruded nucleus pulposus and b with slight protrusion, Both were observed SLR test ane CT scanning of lumbar spine.Results: There was no significant difference in the SLR test between group a and b(P>0.05).In the group, b there was a significant difference in the SLR test after treatment by spinal manipulation.(P<0.01)There was no significant changes in the size of nucleus pulposus Before and after the successful manipulative treatment by CT scanning in group b patiants.Conclusion:The size of protruded nucleus pulposus may not necsecerily be on index of greater chance to be included into surgical consideration.
