摘要
目的:研究SLE患者的骨髓象及血象特点。方法:对43例SLE病人的血象及骨髓象进行涂片染色镜检并进行临床观察。结果:SLE临床表现以贫血为主,占86%;另有不同程度的发热,约占53.48%;关节痛者69.76%;面部红斑65.11%,部分患者合并有口腔溃疡、脱发、四肢指趾端坏死、肝大、脾大及淋巴结肿大。SLE患者血象WBC、HB及PLT均有不同程度减少,仅1例正常;骨髓象增生活跃程度不一,部分患者可出现原始细胞、早幼粒细胞及嗜酸细胞增多,可见病态造血及核浆发育不平衡,部分患者有粒系成熟受阻。红系80%以上增生良好,但也有部分患者表现为缺铁性贫血、溶血性贫血及混合性贫血。巨核细胞增生正常的占65.11%,另有少数增生减低或缺如。结论:SLE临床表现不一,多以贫血、血小板减少等为首发症状,部分患者骨髓象可出现原始细胞及早幼粒细胞增多,对此类“SLE相关性血液病”应与原发性血液病相区别。
Objective To observe the morphologic characteristics of SLE patients' bone marrow and peripheral blood smears.Method A total of 43cases of SLE patients' bone marrow and peripheral blood smears were observed,at the same time a deep clinical investigation on the patients was conducted.Results The major clinical sign of SLE patients was anemia,accounting for 86%.About 53.48%of SLE patients had a fever to some extent.Moreover,there were69.76%and65.11%of patients presented arthralgia and erythema on the face respectively.Some patients companied with symptoms such as:ulceration in the mouth,losing hair,necrotic tip of fingers and toes,swelling of liver,spleen and lymph nodes.All SLE patients' WBC,HB and PLT almost decrease at different de-grees except one is normal.What's more,the marrow proliferative activities were different among SLE patients.In some patients' bone marrow smears,some plasts,progranulocytes and eosinophilia were saw,In others',there were morbid hemopoiesis,off balance of nucleus-plasma development and blocking of granulocytopoiesis.About 80%of SLE patients erythrocytic series proliferate were good state but some patients had iron deficiency anemia,hemolytic anemia or compounding anemia yet.Further,65.11%of SLE patients' megalocytes developed normally except few develop depressed.Conclusion The results showed that the clinical symptoms were differ-ent among SLE patients and the most primary clinical signs were anemia,platelet decrease and so on.And there was an increase of plast and progranulocyte in some SLE patients' bone marrow,so primary hematologic diseases with this kind of ″hematologic disease correlating to SLE″should be distinguished.
