聚-乳酸-乙醇酸涂层冠状动脉支架的生物相容性实验研究 被引量:7

Biocompatibility study of poly-D, L-lactide/glycolic acid coated coronary stent in porcine model

摘要 目的 了解聚 乳酸 乙醇酸 (PLGA)涂层支架的生物相容性。方法 PLGA(D、L LA :GA为 6 0∶4 0 )浸涂Dragon支架制成PLGA涂层支架组 ,Dragon支架作为对照组 ,支架置入小型猪冠状动脉 ,7d期和 30d期各 8只猪 ,每只猪置入上述各 1枚支架。支架置入前、后即刻及终点时行冠状动脉造影。分析支架置入前及终点时的全血细胞计数、生化指标、血压、心率、体重、全程的行为状态。处死动物后取出支架置入段冠状动脉 ,制备标本 ,行扫描电子显微镜观察及组织学观察测量 ,取肺、胃、小肠、大肠、肝、肾以及支架血管段供血的心室壁行组织学观察。结果 两组均未发现骨髓抑制、溶血 ,对肝、肾功能及代谢无明显影响 ,两组中心率、血压、体重的变化差异无显著性 ,对行为状态无影响。所有支架置入成功 ,7d期两组中冠状动脉无狭窄 ,30d期两组中各有 1支冠状动脉狭窄 (>5 0 %直径狭窄 )。 7d期两组均见完全内皮化。 7d期对照组 2个、涂层支架组 1个支架血管段形成血栓 ,30d期内膜面积及内膜面积与中膜面积的比率两组间差异无显著性 ,未见明确的炎症反应细胞。未发现肺、胃、小肠、大肠、肝、肾以及支架血管段供血的心室壁与PLGA涂层支架有关的病理损害。结论 PLGA涂层支架置入小型猪冠状动脉后 30d的血液相容性及组织相容性? Objective To evaluate the biocompatibility of Poly-D, L-lactide/glycolic acid (PLGA) coated stent in porcine model. Methods Dragon stent coated with PLGA(DL-LA:GA for 60:40 in mole) and Dragon stent (as control group) were implantated into porcine coronary arteries, 2 stents for each pig, set two endpoint times of 7 days and 30 days each with 8 pigs.Coronary angiography was performed before and immediately after the implantations and at the end points.The analysis on blood cell count, 27 biochemistry items, blood pressure(BP), heart rate(HR), body weight(BW), status of behavior of pigs was made before the implantations and at the end points.Stented-coronary, stented-coronary related ventricular wall, lung, stomach, big bowel and small intestine, liver and kidney were collected after euthanasia of animals on each endpoint.Scanning electron microphotography, histopathology, and histomorphometry had been done.Results No bone marrow inhibition and hemolysis occurred, no damage to the function and metabolism of liver and kidney was discovered, no significant difference was found in variation of HR,BP and BW between the two groups.All the stents were implanted into porcine coronary arteries successfully.No stenosis was found in pigs for 7 days, but stenosis was found in only 1 artery in each group at 30 days.All stented-coronary arteries were reendothelialized completely at 7 days.Thrombosis was found in 2 stented-arteries in control group and in 1 stented-artery in PLGA group at 7 days.No inflammation cell was found in any stented-coronary wall for each endpoint in both groups.No damage to stented-coronary related ventricular wall, lung, stomach, big bowel and small intestine, liver and kidney due to PLGA was found.There was no significant difference in neointimal area and ratio of neointimal area/ medium area between the two groups.Conclusion Blood or tissue compatibility of Dragon stent coated with PLGA (D, L-LA:GA for 60∶40 in mole) in porcine model in one month seems good and acceptable.
