摘要
目的:观察毛细支气管炎(毛支)的肺功能动态变化情况,探讨布地奈德气泵吸入佐治毛支的疗效。方法:48例毛支患儿随机分为2组,每组各24例,均采用综合治疗,观察组加用布地奈德气泵吸入。治疗前测定肺功能,观察组19例、对照组17例患儿在治疗后7~14天复查肺功能,以24例同龄健康儿童做正常组对照。结果:①观察组在治愈率、缩短哮鸣音、湿口罗音及咳嗽持续时间的作用优于对照组(P<0.05)。②观察组治疗后潮气量、25%潮气量时的潮气呼气流速(TEF25%)增加,呼吸频率、功能残气量(FRCpleth)、有效气道阻力(Reff)下降;对照组治疗后VT增加,RR、Reff下降(P<0.01,P<0.05)。两组治疗后达峰容积比(VPTEF/VE)、达峰时间比(tPTEF/tE)均有不同程度上升,但未恢复正常。结论:毛支患儿急性期存在不同程度的气道高反应性及肺功能损害。布地奈德气泵吸入能改善肺功能,修复气道,是佐治毛支的主要药物。
Objective: To observe the dynamics of pulmonary function in infan ts with bronchiolitis and investigate the clinical effects of Budesonide in halati on. Methods:Forty-eight cases with bronchiolitis were divided into two groups r andomly and their pulmonary functions were tested. All the cases were treated wi th combination therapy. One group had an adjunctive treatment of Budesonide inha lation. Pulmonary function test was repeated after 7 to 14 days of therapeatics in 17 cases of the control group and in 19 of the group with inhaled Budesonide. Twenty-four healthy children were as the normal controls. Results: ①The group with inhaling Budesonide showed a rapid improvement in respiratory symptoms and airway obstruction. ②The tidal volume (VT), TEF25%remaining were significantl y increased in the group with inhaling Budesonide after therapeutics, while the respiratory rate(RR),function residual capacity(FRC)and resistance effective(Ref f) were decreased. In the control group VT were significantly increased, RR and Reff were significantly decreased. VPTEF/VE and tPTEF/tE were still lower as com pared with the normal control. The tidal breathing flow-volume loop displayed a concave expiratory curve. Conclusion:In the acute phase of bronchiolitis there was an abnormal pulmonary function. With the treatment of Budesonide inhalation the pulmonary function can be improved. Budesonide inhalation can be one of the principal drugs in the adjunctive-treatment of infants with bronchiolits. [
