摘要
目的 探讨慢性肾脏病患者肾组织蛋白酶 B(cathepsin B,CB)、胱抑素 C(cystatin C,CC)的表达及其与肾组织纤维化的关系。方法 采用免疫组化技术和计算机图像分析系统检测肾组织 CB、CC和 型胶原的表达情况 ,并计算硬化肾小球比率及病变肾小管肾间质比率。结果 (1)无纤维化组 CB、CC在近曲小管上皮细胞胞浆内表达最高。 (2 )轻度纤维化组、中度纤维化组肾组织 CB表达较无纤维化组明显减少 ,肾组织 CB表达与硬化肾小球比率及病变肾小管间质比率呈显著负相关。(3)中度纤维化组患者肾组织 CC表达较无纤维化明显增高 ,肾组织 CC表达与病变肾小管肾间质比率呈显著正相关。结论 肾组织 CB、CC主要在近曲小管表达。CB表达减少 (或 ) CC表达增高 ,与慢性肾脏疾病时细胞外基质降解减少有关 ,参与肾组织纤维化的发生机制。
Objective To elucidate the localization of cathepsin B and cystatin C in human kidney and their role in the genesis of renal fibrosis. Methods The expression of cathepsin B, cystatin C and type Ⅳ collagen in the renal biopsy specimens was analysed by computer image analysis system on the immunohistochemical stained section. The relationship between the expression of cathepsin B and cystatin C in kidneys and the degree of glomerulosclerosis and tubulointerstitial injury was analyzed. Results (1)Cathepsin B and cystatin C were highy expressed in renal proximal tubular epithelial cells in the group without fibrosis. (2)The expression of cathepsin B was significantly decreased in groups with mild or moderate fibrosis compared with that of no fibrosis group. The expression of cathepsin B was significantly negatively correlated with the degree of glomerulosclerosis and tubulointersitial injury. (3)The expression of cystatin C in renal tissue of light fibrosis group and middle fibrosis group was significantly higher than that of no fibrosis group. The renal cystatin C expression was positively correlated with the degree of tubulointerstitial injury. Conclusion The expression of cathepsin B and cystatin C correlates with the ECM degradation and contributes to the formation of renal fibrosis.
