摘要 目的 :探讨Er,Cr∶YSGG激光根管内照射对根管壁的作用 ,为此激光在根管治疗领域的临床应用提供实验依据。方法 :选用 2 4颗新拔除的单根离体牙 ,去除牙冠 ,常规根管预备后 ,随机分成 6组。分别使用 1,2 ,3,4 ,6WEr,Cr∶YSGG激光于有水条件下进行根管内照射 ;另选一组不做任何处理 ,作为阳性对照。扫描电镜下观察根管内壁的形态学改变。结果 :与对照组相比 ,激光照射组可有效去除根管内壁的玷污层及碎屑 ;牙本质小管口清晰可见 ;未见熔融及炭化等热损伤改变 ,其中使用 3W激光处理最为理想。与 1~ 3W相比 ,4W ,6W时根管表面更为粗糙不平 ,且偶见裂纹。结论 :应用 3WEr ,Cr∶YSGG激光进行根管内照射可起到较为理想的根管清洁作用 ,有望在临床上应用提高根管治疗效果。 Objective: To discuss the morphological changes in root canal walls as a result of intracanal irradiation with Er,Cr ∶YSGG laser, and to provide an important experimental basis for clinical application of Er,Cr ∶YSGG laser. Methods: Twenty-four freshly extracted human teeth of single-root were randomly divided into 6 groups. The anatomical crowns were removed and the remaining roots were prepared endodontically with conventional methods. Laser irradiation was performed at different output powers with water spray cooling. Specimens were evaluated by using scanning electron microscopy. Results: Er,Cr ∶YSGG laser treatment resulted in a clean surface, with no smear layer or debris. Little or no fusing and carbonization was observed in samples irradiated with cooling. In all samples, the effect of laser irradiation for 3 W was the best. Compared with the samples treated for 1 to 3 weeks, those at 4 and 6 weeks appeared rougher and occasionally with microcracks. Conclusion: Er,Cr ∶YSGG laser irradiation with water spray cooling at 3 W can depurate root canal perfectly, which may be applied as a therapeutic tool for root canal therapy.
