摘要
目的 :探讨瘦素促胎儿生长作用及其与胰岛素和胰岛素样生长因子 1(IGF 1)的关系。方法 :采用放射免疫法检测 10 0例足月新生儿脐血血清瘦素、胰岛素和IGF 1水平 ,根据新生儿出生体重分为大于胎龄儿 (LGA)组 19例 ,适于胎龄儿 (AGA)组 6 5例 ,小于胎龄儿 (SGA)组 16例。结果 :(1)脐血瘦素水平与新生儿出生体重呈正相关关系 (r =0 .5 7,P <0 .0 1) ,其中LGA组脐血瘦素水平为 (13.38± 6 .75 ) μg/L ,显著高于AGA组 (7.4 0±4 .4 5 ) μg/L ;SGA组 (2 .79± 1.5 4 ) μg/L低于AGA组 ,差异有显著性 (P<0 .0 1) ;(2 )脐血胰岛素和IGF 1水平分别与新生儿出生体重呈显著正相关关系 ,相关系数 (r )分别为 0 .30和 0 .35 ;(3)脐血瘦素和胰岛素水平呈正相关关系 (r =0 .32 ,P <0 .0 1) ,脐血瘦素与IGF 1水平之间无相关关系 (r =0 18,P >0 .0 5 )。结论 :脐血瘦素水平与胎儿生长发育状态密切相关 ,瘦素和胰岛素对胎儿体重有双向调节作用 ,IGF 1和瘦素具有各自独立的调控胎儿生长发育体系。
Objective: To investigate the effect of leptin on promoting fetus growth and the relationship between the effect and insulin or insulin-like growth factor-1(IGF-1).Methods:Radio immunoassay was used to measure the levels of leptin, insulin, and IGF-1 in umbilical blood of 100 full-term newborns. According to the neonatal weight, the newborns were divided into the large for gestational age (LGA) group(19 cases), the appropriate for gestational age (AGA) group(65 cases), the small for gestational age (SGA) group(16 cases). Results:(1) The leptin level in umbilical blood was positively related to the neonatal weight(r=0.57,P<0.01).The leptin level of LGA group(13.38±6.75)μg/L, was significantly higher than that of AGA group, which was (7.40±4.45)μg/L( P<0.01). The leptin level of SGA group was (2.79±1.54)μg/L , significantly lower than that of AGA group( P<0.01). (2)The levels of insulin and IGF-1 in umbilical blood were positively related to the neonatal weight(r=0.30,P<0.01;r=0.35,P<0.01).(3)The leptin level in umbilical blood was positively related to the level of insulin(r=0.32,P<0.01),while was not related to the level of IGF-1(r=0.18,P>0.05).Conclusion:The leptin level in umbilical blood is closely related to the growth of fetus. Leptin and insulin have bidirectional regulatory function on fetus weight. Leptin and IGF-1 regulate fetus growth in their individual way.
