羟基磷灰石义眼台纤维血管化的实验研究 被引量:21

摘要 目的 研究手术方式对兔眼窝植入羟基磷灰石义眼台纤维血管化的影响。方法 将 3 6只新西兰大白兔随机分为 4组 ,分别对其左眼实施半巩膜壳内 (A组 )、多孔巩膜壳内 (B组 )、全巩膜壳内 (C组 )或巩膜瓣遮盖肌锥内 (D组 ) 4种不同术式义眼台植入术。不同时间采用Tc -99MDP显像或病理检查法检测各组义眼台纤维血管化程度。结果 第 1、2周及第 8周 ,各组间平均L/R值差异无显著性意义 ;第 4周 (F =7.482 ,P =0 .0 0 9)和第 6周 (F =6.676,P =0 .0 3 4) ,C组低于其他 3组 ,而A、B、D组间比较差异无显著性意义。第 8周病理检查各组义眼台纤维血管化程度差异无显著性意义。结论 半巩膜壳内或多孔巩膜壳内植入义眼台效果最佳。 Objective To investigate the effects of surgery procedure on the fibrovascular ingrowth into rabbits hydroxyapatite(HA) orbital implant.Methods 36 New Zealand white rabbits were divided into four(A, B, C, D) groups at randam and received HA orbital implant surgery in their left eyes. The orbital implants were implanted into semi-sclera shell for group A, into multi-hole sclera shell for group B, into omni-sclera shell for group C and into the extrabulbar muscle surrounding space for group D. The animals received technetium-99 scan at 1,2, 4,6 and 8 weeks postoperatively. The mean radioactivity counts within region of interest(ROI) of the surgery eye(L) and the non-surgery eye(R) in the same animal were tested, and the L/R ratios were calculated. The implants were harvested at 8 weeks and examined histopathologically. Results At 1 and 2 weeks, the mean L/R ratios difference among 4 groups were not significant. At 4 ( F=7.482, P=0.009 ) and 6 ( F=6.676, P =-0.034) weeks the mean L/R ratio of group C was lower than group A,B and D; while there were no significant difference among group A,B and D. At 8 weeks, the mean L/R ratios difference among 4 groups were not significant. At 8 weeks, the mean fibrovascularization rates of 4 groups were 98.5% (group A) ,100% (group B) ,97.25% (group C) ,98% (group D) respectively, but there were no statistical significance.Conclusion Implanting the HA orbital implant into sclera shell can get the same fibrovascular ingrowth progress as other surgery procedure, but it improved the motility. HA orbital implant should be implanted into sclera shell for better effect.
