利多卡因气管内喷雾药物动力学研究 被引量:1

摘要 为观察利多卡因在抑制气管插管时的心血管反应中的量效关系及毒性,对24例无心、肺、肝、肾疾患,行全麻手术的病人进行了血药浓度监测。病人随机分成4组,于气管内喷利多卡因后定时取动脉血,用TDx快速血药浓度检测仪测定血药浓度,同时连续监测收缩压、平均动脉压和心率。血药浓度用计算机拟合符合一室模型。3个用药组平均半衰期[t_(1/2)(kα)]分别为1.58 min(4%利多卡因),2.17 min(2%利多卡因);2.73 min(1%利多卡因),平均消除半衰期[t_(1/2)(ke)]为55.24 min、57.38 min、39.48 min;平均达峰时间(T(Peak))分别为8.10 min、8.43 min、3.69 min;平均峰浓度[Cmax]分别为2.14μg/ml、1.15μg/ml和0.45μg/ml.其中Ⅰ、Ⅱ2个剂量组可明显抑制气管插管时的心血管反应(P<0.05)。 Serum concentrations of Lidocaine were determined in 24 general anesthesia patients with no cardiac,pulmonary,hepatic and renal disorders in order to observe the dose—effect and toxicity of Lidocaine for inhibiting the cardovas-cular reaction during tracheal intubation.The patients were divided into 4 groups in andom.The serum concentrations of Lidocaine were determined by TDx Assay after the tracheal administration of this drug,the systolic pressure,average arterial pressure and heart rate were monitored simultaneously.The pharmacokinetic pa-rameters were analyzed by computer.The cardiovascular reaction can be inhibited significantly in 4% Lidocaine group and 2% Lidocaine group compared with the contrast group(P<0.05).
