摘要
[目的 ]探讨冠心病、高血压患者心导纳微分环 (CADL)的环体面积变化、环形改变的特点及差异 ,进而评价其左心功能 ,为配合临床诊断与治疗提供参考。 [方法 ]用心导纳微分环 (CADL)对30例正常人、30例冠心病、30例高血压患者作左心功能检测。从检测结果中选取CADL的总环面积、I相面积、III相面积、V相面积及I相离心支的改变程度 5项指标 ,比较 2个患者组与正常组以及 2个患者组之间的差异。 [结果 ]冠心病、高血压组与正常组比较CADL的总环面积、I相面积均较正常组明显减小 ,III、V相面积较正常组亦明显增大 ,有显著差异 (P <0 .0 1 ) ;另外 ,2个患者组中CADL环的I相离心支都程度不同的出现切迹或波浪状改变。这些改变与正常组的CADL环形有明显不同 ,在两个患者组间则无明显差异 (P >0 .0 5 )。 [结论 ]CADL的环体面积及环形变化可反映左心功能的变化 ,其敏感度高于心电图。从左心功能受损程度上看以冠心病组最为严重 ,高血压组次之。
Discuss about characteristics and differences of area and shape variability of CADL between the individuals who suffer from coronary heart disease and individuals suffering from hypertension and estimate for their left cardiac function . Make use of CADL to detect left cardiac function of 30 normal persons and 60 patients (30 cases of coronary heart disease and 30 cases of hypertension). From the results ,five targets about shape variability in centrifugal branches of phase I and area variability of total loop ,phase I ,phase III and phase V are selected to contrast the difference between two groups of the patients and the group of the normal persons ,and the difference among two patient's groups. Compared to the normal group ,two patient's groups indicate that CADL's total loop area and the area of phase I decrease significantly; the area of phase III and phase V increase remarkably .Furthermore , waved changes with various degrees appear at the centrifugal branches of phase I. These change shows difference ( P <0.01) with the normal groups ,while there is no significant difference ( P >0.05)among two groups of patients.[Conclusion] Changes of left cardiac function can be indicated by the variability of area and shapes of CADL, whose sensitivity is higher than ECG's. Regarding the damage extend of left cardiac function, the group of coronary heart disease is more serious than the group of hypertension.
