摘要
为了解脑外伤病人的体液免疫动态,我们对35例各类脑外伤病人分别于伤后或术后第24小时,第3,5,7,10和第14天抽外周静脉血行免疫球蛋白测定,结果表明,非手术组伤后各日各类免疫球蛋白浓度均无显著差异(P>0.05),手术组术后各日IgG血清浓度有显著差异(P<0.01),具体表现为:术后24小时与术后第10天比较(P<0.05),术后第3天与术后第10天比较(P<0.01),术后第3天与术后第14天比较(P<0.05),而IgA、IgM术后各日血清浓度虽可见变化趋势,但统计学处理表明无显著差异。我们认为手术组术后血清免疫球蛋白下降可能既为严重脑外伤本身所致,也不能排除手术的创伤所引起,减轻脑外伤后的免疫功能损害有着重要的临床意义。
In order to understand the humoral immunity function of the patients with brain trauma, the concentrations of the serum immunoglobulin were measured in 35 patients with acute brain injures at different times of postoperation or posttrauma. There were no significant changes in serum immunoglobulin (IG) level in 10 patients with nonoperative treatment (P<0.05). A characteristic finding was the significant change of IgG in 25 patients treated by operation (p<0.01). There were significant discrepancies to compare the IgG measured at 24 hrs postoperation with that mersured at 10 days postoperation (P<0.05), and the IgG of 3 days with that of 10 days (P<0.01) and 14 days (P<0.05). The change tendencies of serum IgA and IgM. in the 25 patients had observed, but there were no significant differences. This study suggests that the severe head injury may be responsible for the decrease in Ig level and the operation followed the head injury may be also responsible for the decremetnts. Reducing operation injures and protecting immuity functions have a great value for patients with serious head injuries
