组织工程化表皮膜片的构建及其在增殖性疤痕治疗中的应用 被引量:25

摘要 目的 构建组织工程化表皮细胞膜片并应用于增殖性疤痕治疗。方法 利用患者少量自体正常皮肤分离、培养、扩增表皮细胞,接种于壳聚糖-明胶膜构建成表皮细胞膜片;断层削除增殖性疤痕,膜片移植于创面、适度加压,术后10、30、90 d行大体观察、组织学检查及广谱角蛋白、包壳蛋白、层粘连蛋白免疫组织化学检测。结果 利用自体表皮细胞和壳聚糖/明胶膜能够构建出人表皮细胞膜片,应用于临床增殖性疤痕治疗6例,经过3月随访,创面愈合时间(16.2±15)d,移植膜片存活良好,结构较完整,90 d随访无明显疤痕增生,疗效肯定。结论 表皮细胞/壳聚糖-明胶膜片对增殖性疤痕具有良好治疗效果,组织工程皮肤进入临床具有现实的可行性及广阔的应用前景。 Objective Construction tissue engineered epidermal membrane for clinical treatment of hypertrophic scar. Methods Construct epidermal cells/chitosan-gelatin (CG) membrane was constructed with cultured human epidermal cells (EC) and CG menbrane; Hypertrophic scar was partedly resected and EC/CG membrane was grafted on the wound. Gross observation , HE and immunohistochemistry test were performed ( pre-oprating,and post-operating 10days,30 days,90 days) . Results It was proved that human tissue engineered epidermis could be constructed with cultured human EC and CG membrane , and this artificial epidermal membrane could be used for clinical treatment of hypertrophic scar successfuly (6 cases with 3 monthes'oberservation) . The average healing time is (16.2 ±15) days. The artificial epidermis was well survived with normal structure . No further scar develement was observed in treated area in 90 days. Conclusion The constructed EC-CG membrane has good clinical application in treatment of hypertrophic scar .
