摘要
目的 评价弥散加权成像 (DWI)以及表观弥散系数 (ADC)值测量在脑脓肿与坏死囊变性胶质瘤鉴别诊断中的作用。方法 12例经手术病理 (10例 )或临床随访 (2例 )证实的脑脓肿和 2 0例经手术病理证实的坏死囊变性胶质瘤 ,在术前或治疗前接受了常规MRI检查及DWI(弥散敏感系数即b值采用 :b =0s/mm2 及b =10 0 0s/mm2 ) ,测量了脑脓肿、胶质瘤坏死囊变区、对侧相应正常白质区以及脑脊液的ADC值 ,并计算其平均值。结果 脓腔于DWI上表现为明显的高信号 ,而胶质瘤坏死囊变区表现为低信号。脑脓肿、胶质瘤坏死囊变区、对侧相应正常白质区以及脑脊液的ADC平均值分别为(0 .5 3± 0 .15 )× 10 -3 mm2 /s、(2 .92± 0 .2 2 )× 10 -3 mm2 /s、(0 .75± 0 .0 5 )× 10 -3 mm2 /s以及 (3 .16± 0 .3 0 )× 10 -3 mm2 /s ,脑脓肿与胶质瘤坏死囊变区之间ADC值存在显著差异 (T =78.0 ,P <0 .0 0 1)。结论 DWI以及ADC值测量在脑脓肿与坏死囊变性胶质瘤的鉴别诊断中具有重要作用 ,可作为常规MR序列的重要补充。
Objective To evaluate the role of diffusion weighted imaging (DWI) and apparent diffusion coefficient (ADC) value in differentiation of brain abscess and necrotic or cystic glioma. Methods Twelve patients with brain abscess (10 histopathology verified and 2 clinical follow-up confirmed) and 20 patients with necrotic or cystic glioma (confirmed by histopathology) were studied using routine MRI and DWI (with the b values of 0 s/mm 2 and 1000 s/mm 2) before operations or medical treatments. ADC values of abscesses,necrotic or cystic parts of gliomas,opposite normal white matters and cerebrospinal fluid were measured,and their mean ADC values were calculated. Results On DWI,all twelve brain abscesses showed marked high intensity signal,and all twenty necrotic or cystic glioma demonstrated low intensity signal. The mean ADC values of abscesses,necrotic or cystic parts of gliomas,opposite normal white matters and cerebrospinal fluid were ( 0.53 ±0.15)×10 -3 mm 2/s,(2.92±0.22)×10 -3 mm 2/s,(0.75±0.05)×10 -3 mm 2/s and (3.16±0.30)×10 -3 mm 2/s,respectively. The difference of mean ADC values of abscesses and necrotic or cystic gliomas was significant ( T = 78.0 ,P < 0.001 ). Conclusion DWI and ADC value can play an important role in differentiating brain abscess and necrotic or cystic glioma. DWI and ADC value measuring will be an important supplementary for routine MRI.
