摘要
目的 分析听觉诱发电位指数 (AAI)、双频指数 (BIS)与异丙酚镇静作用的相关性。方法 选择 2 0例ASAⅠ~Ⅱ级 ,腰段硬膜外阻滞 /腰 硬联合麻醉下行妇科、骨科等手术的成年患者。监测AAI、BIS ,依据OAA/S评分法评估镇静程度。手术开始后用微泵注入异丙酚 6~ 8mg/ (kg·h) ,直至OAA/S评分达到 1分 ,分析AAI、BIS与OAA/S评分值之间的相关性。结果 AAI、BIS随患者镇静程度加深明显降低 ,且与OAA/S评分有显著相关性 ,诱导期Spearman等级相关系数r =0 933、 0 5 33,和恢复期r =0 94 3、 0 5 92 (均为P <0 0 1) ,诱导期与恢复期AAI的相关系数与BIS之间有显著性差异 (P <0 0 1)。结论 AAI、BIS均可较好地反映异丙酚的镇静效果 。
Objective To analyze the correlation of the autoregressive model with exogenous input extracted auditory evoked potentials index (AAI), bispectral index (BIS) with propofol-induced sedation. Methods Twenty ASAⅠ-Ⅱ adult patients undergoing elective gynecological surgery under lumbar epidural anesthesia and CESA were randomly studied. When operation started, propofol 6-8 mg/(kg·h) was perfused until the score level 1 observer's assessment of the alertness/sedation scale (OAA/S) reached. AAI and BIS were monitored on different OAA/S score levels and their correlation with propofol induced sedation was analyzed. Results AAI and BIS were distinctly reduced as patients OAA/S scores were decreased, and had a signficnat correlation with sedation (during the onset phase, the coefficients of Spearman's rank correlation r=0 933, 0 533, P =0 01; during the recovery phase r=0 943, 0 592, P =0 000). There was signficnat difference in coefficient between AAI and BIS during the onset and recovery phase ( P <0 01). Conclusion AAI and BIS correlated well with propofol-induced sedation quantitatively and accurately.
