糖尿病母亲婴儿心脏损害的临床研究 被引量:6

摘要 [目的 ]研究妊娠期糖尿病对新生儿的心脏损害。 [方法 ]检测糖尿病母亲婴儿 3 2例 (观察组 )和正常婴儿3 0例 (对照组 )的血清血糖、肌酸激酶 (CK)、肌酸激酶同功酶 (CKMB)、谷草转氨酶 (AST)和乳酸脱氢酶 (LDH )水平 ,并对临床表现进行比较研究。[结果 ]①观察组 8例低血糖 ,对照组无低血糖病例 ,有低血糖者均有不同程度心脏损害的临床表现 ;②观察组与对照组CK、CKMB、AST和LDH的水平差异有显著性 ,且前者有心脏损害临床表现 2 3例(71 9% ) ,后者无心脏损害表现 ;③观察组中有心脏损害临床表现者 ,上述 4种酶异常率高于无心脏损害临床表现者和对照组 ;④糖尿病母亲妊娠期血糖控制不良者 ,新生儿心脏损害和心肌酶水平高于血糖控制良好者。 [结论 ]妊娠期糖尿病可引起围产儿心脏损害 ;糖尿病母亲婴儿的心脏损害程度与母亲妊娠期糖尿病严重程度呈正相关 ;血清心肌酶改变可作为糖尿病母亲婴儿心脏损害的早期诊断实验室指标之一 ,结合心脏损害的临床表现 ,可早期诊断和治疗。 Objective\] To study the effect of gestational diabetes mellitus on cardiac injury of newborns \ Serum glucose, CK, CKMB, AST and LDH concentration were detected in 32 infants of diabetic mother (IDMs) (case group) and compared with 30 normal infants (control group) \ 1 There were 8 cases of IDMs with hypoglycemia which had symptoms of cardiac injury and no case in control group 2 The levels of CK, CKMB, AST and LDH in case group were higher than those in control group significantly There were 23 cases (71 9%) with clinical cardiac injury in case group and no case in control group 3 Abnormal rates of the above enzymes in IDMs with clinical cardiac injury were higher than those in IDMs with non clinical symptoms and control group 4 The rate of cardiac injury and abnormal rate of myocardiac enzyme were lower in the mothers with better effect of diabetes treatment \[ Conclusions\] Gestational diabetes mellitus may result in cardiac injury of newborns and the degree of injury is positively correlated with the severity of diabetes The changing of myocardiac enzymes may be the one of indications for cardiac injury of IDMs And the enzymes changing combined with clinical symptoms will be important indicators for early diagnosis and treatment
