摘要
目的 建立桡神经深支的体表定位方法并明确其应用解剖学特征。方法 取成人尸体 48具 ,显露桡神经深支 ,在前臂旋前旋后中立位 ,分别测量桡神经深支发出点与肱桡关节线的距离、桡神经深支穿出旋后肌的部位与肱桡关节线的距离、桡骨茎突与肱桡关节线的距离、桡神经深支穿出旋后肌的部位与肱骨外上髁和Lister结节连线的水平距离。结果 桡神经深支自桡神经主干发出的位置在肱桡关节上方平均为 ( 15 4± 2 5 )mm。桡神经深支穿出旋后肌的部位均位于肱骨外上髁与桡骨Lister结节连线的尺侧 ,与连线的水平距离平均为 ( 6 1± 2 1)mm ,与肱桡关节线的平均距离为 ( 61 3±17 6)mm ,肱桡关节线与桡骨茎突尖端之距平均为 ( 2 3 0 5± 19 4)mm ,后二者之比平均为 ( 0 2 7± 0 0 3 )mm。结论 桡骨小头正前方或略偏外侧可作为桡神经深支近端的定位标志 ,肱桡关节线下方约 60mm ,肱骨外上髁与桡骨Lister结节连线尺侧约 6mm可作为桡神经深支远端的定位标志 。
Objective To build up a method for positioning the radial nerve deep branch (RNDB) on the body surface and to clarify the applied anatomic characteristics Methods Forty eight embalmed adult cadaver arms were dissected RNDB was found between the brachioradialis and the brachialis at the upper arms and between the extensor carpi radialis brevis and the extensor digiti at the forearms The following measurements were performed at the neutral position with a vermer caliper (accurate to 0 1 mm): distance between the origin points of RNDB and the radial humeral joints (L 1), distance between the exit points of RNDB from the supinator muscles to the radial humeral joints (L 2), distance between the styloid processes of radius and the radial humeral joints (L 3), and horizontal distance from the exit points of RNDB to the line between the lateral humeral epicondyles and the lister tubercles (L 4) Results The average distance between the origin points of DBRN and the radial humeral joints (L 1) was (15 6±2 5) mm All the RNDBs exited from the distal edge of supinator at the ulnar side, and the average distance between the exit points and the radial humeral joints (L 2) was (61 3±17 6) mm The average distance between the styloid processes of radius and the radial humeral joints (L 3) was (230 5±19 4) mm, and the average horizontal distance between the exit points of RNDB and the line between the lateral humeral epicondyles and the lister tubercles (L 4) was (6 1±2 1) mm The average ratio of L 2 to L 3 was 0 27±0 03 Conclusion The front of the head of radius or a bit of lateral to that can be considered a positioning sign of the proximal end of RNDB at the body surface, and the position 0 6 cm ulnar lateral to the line between the lateral humeral epicondyles and lister tubercles can be considered as the distal end of RNDB Based on the combination of the two means, we can perform accurate positioning of RNDB at the body surface relatively
