摘要
目的 探讨青少年肥胖与代谢综合征的关系。方法 从年龄 17~ 19岁的 2 4 0 0例学生中筛选出体重指数(BMI)≥ 2 5kg/m2 的肥胖学生 (肥胖组 ) 116例 ,BMI在 (18.5~ 2 3.0 )kg/m2 之间体重正常学生 (正常组 ) 6 0例 ,测定血糖、血脂、谷氨酰转肽酶 (GGT)水平。同时做肝脏B超 ,测量血压及腰围 ,比较两组差异。结果 肥胖组血糖、甘油三酯 (TG)和高密度脂蛋白胆固醇 (HDL C)水平虽然在正常范围内 ,但血糖和TG水平高于正常组 (P <0 .0 1) ,分别为 [(4.72± 0 .85 )mmol/L比 (4.13± 0 .33)mmol/L ,(1.5 7± 0 .93)mmol/L比 (0 .93± 0 .32 )mmol/L]。HDL C水平低于正常组 (P <0 .0 1) [(1.14± 0 .2 4 )mmol/L比 (1.38± 0 .2 6 )mmol/L]。肥胖组GGT水平异常检出率及脂肪肝的检出率分别是 2 1.5 4 %和 4 0 .31% ,均明显高出正常组。结论 肥胖青少年可能存在潜在的糖代谢和脂代谢异常等代谢综合征改变。
Objective To investigate the metabolic changes in obese adolescents and the relationship between obese adoleescents and metabolic syndrome.Methods 116 obese adolescents (body mass index, BMI≥25㎏/m 2) and 60 normal adolescents (BMI 18.5 ~ 23.0 ) were chosen form 2 400 middle school students of 17-19 years old. Blood glucose, lipid, and GGT were determined. Hepatic ultrasonic examination was carried. Blood pressure and waistline was measured. The difference between the obesity group and control group was compared.Results The levels of blood glucose and triglyceride (TG) in the obesity group were significantly higher than in control group (P< 0.01 ) [( 4.72 ± 0.85 ) mmol/L vs ( 4.13 ± 0.33 ) mmol/L and (1.57±0.93) mmol/L vs ( 0.93 ± 0.32 ) mmol/L respectively], but the levels of normal serum high-density lipoprotein (HDL-C) in obesity group were significantly lower than in control group (P< 0.01 ) [( 1.14 ± 0.24 ) mmol/L vs ( 1.38 ± 0.26 ) mmol/L]. Furthermore the abnormal rate of GGT and fatty liver by B ultrasound in obesity group was 21.54% and 40.31% separately.Conclusion It is suggested that there are potential disorders of blood glucose and lipid metabolism in obese adolescents.
