糖尿病下肢动脉病变辅助诊断方法研究进展

摘要 糖尿病(diabetes mellitus, DM)是一种由多病因引起的以慢性高血糖为主要特征的代谢综合征,多数是由胰岛素分泌不足和(或)利用障碍导致。其长期的代谢紊乱引起了多器官损伤,包括眼、肾脏、神经、心脏、血管等,从而引发一系列的并发症。在糖尿病中,比较常见的并发症为下肢动脉病变(lower extremity arterial disease, LEAD),若病人病情严重,将会伴有肢体溃疡以及坏疽等症状,采取早期识别LEAD可以评估是否需要血运重建手术或血管成形术来改善血流状况,从而降低肢体溃疡、坏疽甚至截肢的风险,因此对于下肢血管病变的早期筛查或提前预警就尤为重要,目前已有多种早期筛查LEAD的检查方式,部分检查方法已在临床上广泛应用,现就对已有的糖尿病下肢血管病变检查方法做出综述。 Diabetes mellitus (DM) is a metabolic syndrome characterized by chronic hyperglycemia caused by multiple etiologies, most of which are caused by insufficient insulin secretion and/or dysfunction of insulin utilization. Its long-term metabolic disorder causes multiple organ damage, including eyes, kidneys, nerves, heart, blood vessels, etc., which leads to a series of complications. In diabetes, the most common complication is lower extremity arterial disease (LEAD). If the patient’s condition is severe, it will be accompanied by limb ulcer and gangrene and other symptoms. Early identification of LEAD can assess whether revascularization or angioplasty is needed to improve blood flow, thereby reducing the risk of limb ulcer, gangrene and even amputation. Therefore, early screening or early warning of lower extremity vascular disease is particularly important. At present, there are a variety of early screening methods for LEAD, and some methods have been widely used in clinical practice. This article reviews the existing detection methods of diabetic lower extremity vascular disease.
