目的:应用在循证基础上的妊娠期恶心呕吐预防及管理的最佳证据临床护理方案,观察妊娠剧吐病人的循证护理临床实施效果。方法:通过运用复旦大学循证护理中心的PIPOST模型来确立循证问题;通过检索BMJ Best Practice、PubMed、万方数据库...目的:应用在循证基础上的妊娠期恶心呕吐预防及管理的最佳证据临床护理方案,观察妊娠剧吐病人的循证护理临床实施效果。方法:通过运用复旦大学循证护理中心的PIPOST模型来确立循证问题;通过检索BMJ Best Practice、PubMed、万方数据库、中国知网、美国妇产科医师学会、美国国立指南数据库、英国国家卫生与临床优化研究所等数据库收集妊娠剧吐护理的相关证据,构建妊娠剧吐的循证护理方案,并应用该方案。选取2018年10月—2021年2月在某院收治的妊娠剧吐病人作为研究对象,按随机数字表法将其分为试验组和对照组。两组病人均接受基础补液治疗,试验组实施妊娠剧吐循证护理实践方案,对照组行常规专科护理,观察两组病人的临床效果。结果:实施妊娠剧吐循证护理方案后,试验组病人的依从性提高,出院时呕吐频次及尿酮体转阴时间均优于对照组,差异均有统计学意义(P<0.05);同时医护人员的执行力也有所提升。结论:基于循证的妊娠剧吐护理方案可提高医护人员的执行力和病人的依从性,并能改善妊娠剧吐病人的恶心呕吐程度等症状,提高病人的生活质量。展开更多
目的探讨延续性护理对全髋关节置换术后患者功能康复的影响。方法选取2016年1~12月在本科尚未开展延续性护理时行全髋关节置换术的患者70例为对照组,2017年1~12月在本科已开展延续性护理时行全髋关节置换术的患者70例为观察组,共140例。...目的探讨延续性护理对全髋关节置换术后患者功能康复的影响。方法选取2016年1~12月在本科尚未开展延续性护理时行全髋关节置换术的患者70例为对照组,2017年1~12月在本科已开展延续性护理时行全髋关节置换术的患者70例为观察组,共140例。2组患者在住院期间均给予相同的护理方式。在患者出院后6个月内,对照组给予常规护理,而观察组给予延续性护理。比较6个月期间2组患者的Harris髋关节功能评分、日常生活自理能力(the barthel index of activities dailyliving)简称为Barthel指数评分[1]以及术后并发症发生率。结果2组患者出院时、出院后1个月Harris评分无统计学意义(P> 0.05 ),出院后3、6个月观察组Harris评分及Barthel指数评分均明显优于对照组,差异有统计学意义(P<0.01),观察组并发症发生率明显低于对照组,差异有统计学意义(P<0.01)。结论延续性护理可促进全髋关节置换术后患者髋关节功能的恢复和生活自理能力的提高,大大降低了术后并发症的发生率,值得在临床上推广。展开更多

We study the structure ...We theoretically investigate the influence of the shape of nanoholes on plasmonic behaviours in coupled elliptical metallic nanotube arrays by the finite-difference time-domain (FDTD) method. We study the structure in two cases: one for the array aligned along the minor axis and the other for the array aligned along the major axis. It is found that the optical properties and plasmonic effects can be tuned by the effective surface charges as a result of the variation in the minor axis length. Based on the localized nature of electric field distributions, we also clearly show that the presence of localized plasmon resonant modes originates from multipolar plasmon polaritons and a large magnitude of opposing surface charges build up in the gap between adjacent nanotubes.展开更多

The results show that the transmission properties can...We investigate the relationship between the transmission and the layer distance of double-layer gold slit arrays by using the finite-difference time-domain method. The results show that the transmission properties can be influenced strongly by layer distance. We attribute the two types of resonant modes to surface plasmon resonance and the localised waveguide resonance. We find that the localised waveguide transmission peak redshifts and becomes broader with increasing layer distance D. We also describe and explain the splitting, shift, and degeneration of the surface plasmon resonant transmission peak theoretically. In addition, to clarify the physical mechanism of the transmission behaviours, we analyse the distributions of electric field and total energy for the three transmission peaks with distance D = 45 nm for the double-layer system. Light transporting behaviours are mostly concentrated in the region of the slits as well as the interspaces of the two layers, and for different resonant wavelengths the electric field and energy distributions are different. It is expected that the results obtained here will be helpful for designing subwavelength metallic grating devices.展开更多
