BACKGROUND The clinical advantages of painless colonoscopy can reduce the fear and discomfort of patients and increase the detection rate of diseases.Propofol has the characteristics of fast effect and short action ti...BACKGROUND The clinical advantages of painless colonoscopy can reduce the fear and discomfort of patients and increase the detection rate of diseases.Propofol has the characteristics of fast effect and short action time.It is a common choice for painless endoscopic sedation and anesthetics.However,propofol can cause severe respiratory and circulatory depression.Therefore,it is important to find a way to reduce the dose of propofol.AIM To explore the effect of wrist-ankle acupuncture on propofol dose during colonoscopy.METHODS Two hundred patients who were going to receive selective painless colonoscopy in Hebei Hospital of Traditional Chinese Medicine were selected and divided into wrist-ankle acupuncture group(WAA group,n=100)and control group(CON group,n=100).After entering the operation room,patients were given 0.025 mg/kg nabufine intravenously and propofol at the initial dose of 0.5 mg/kg.In patients who did not fall asleep,propofol(10 mg/time)was given until loss of consciousness.Prior to anesthesia,patients in WAA group were punctured by specialist in the inferior 1,2 and 3 regions according to the zoning principle of wrist-ankle acupuncture.The primary endpoint was required dose of propofol,and the secondary endpoints were the incidence of hypoxemia and hypotension.Furthermore,the following data were recorded:The operation time,wake-up time,incidence of nausea and vomiting,incidence of abdominal distention,post colonoscopy pain,examiners'satisfaction,patients'satisfaction and Borg fatigue index.This study has been registered in the Chinese Clinical Trial Registry(Registration Code:ChiCTR1900022177).RESULTS The induced dose of propofol and the total dose of propofol in WAA group were 80 mg and 110 mg,respectively,which were significantly lower than those in CON group(P<0.05).The incidences of hypoxemia and hypotension in the WAA group were 2.2%and 3.3%,respectively,significantly lower than those in the CON group(P<0.05).The incidence of abdominal distension in the WAA group was 8.8%,which was significantly lower than that in the CON group(P<0.05,28.9%).The waking time of WAA group was 3.26±0.87 min,which was significantly lower than that of CON group(6.06±0.88 min,P<0.05).CONCLUSION Wrist-ankle acupuncture can reduce the induction dose and total dose of propofol as well as the incidence of adverse reactions in painless colonoscopy without affecting the satisfaction of examiners and patients.This procedure is simple in operation and easy to promote in clinical practice.展开更多

BD was originally described in 1937 as a syndrome involving oral and genital ulceration in ...Behcet's disease(BD) is a rare and life-long disorder characterized by inflammation of blood vessels throughout the body. BD was originally described in 1937 as a syndrome involving oral and genital ulceration in addition to ocular inflammation. Intestinal BD refers to colonic ulcerative lesions documented by objective measures in patients with BD. Many studies have shown that over 40% of BD patients have gastrointestinal complaints. Symptoms include abdominal pain, diarrhea, nausea, anorexia and abdominal distension. Although gastrointestinal symptoms are common, the demonstration of gastrointestinal ulcers is rare. This so-called intestinal BD accounts for approximately 1% of cases. There is no specific test for BD, and the diagnosis is based on clinical criteria. The manifestations of intestinal BD are similar to those of other colitis conditions such as Crohn's disease or intestinal tuberculosis, thus, it is challenging for gastroenterologists to accurately diagnose intestinal BD in patients with ileocolonic ulcers. However, giant ulcers distributed in the esophagus and ileocecal junction with gastrointestinal hemorrhage are rare in intestinal BD. Here, we present a case of untypical intestinal BD. The patient had recurrent aphthous ulceration of the oral mucosa, and esophageal and ileo-colonic ulceration, but no typical extra-intestinal symptoms. During examination, the patient had massive acute lower gastrointestinal bleeding. The patient underwent ileostomy after an emergency right hemicolectomy and partial ileectomy, and was subsequently diagnosed with incomplete-type intestinal BD by pathology. The literature on the evaluation and management of this condition is reviewed.展开更多

We observed species composition, species richnes...Understanding the effect of biodiversity on ecosystem function is critical to promoting the sustainability of ecosystems and species conservation in natural ecosystems. We observed species composition, species richness and aboveground biomass,and simulated the competitive assemblages in a natural grassland ecosystem of China, aiming to test some assumptions and predictions about biodiversity–stability relationships. Our results show that aboveground productivity and temporal stability increased significantly with increasing species richness, and via a combination of overyielding, species asynchrony, and portfolio effects. Species interactions resulted in overyielding caused by trait-independent complementarity, and were not offset by a negative dominance effect and trait-dependent complementarity effect. Therefore, the mechanisms underlying the biodiversity effect shifted from the selection effect to the complementarity effect as diversity increased, and both effects were coexisted but the complementarity effect represent a mechanism that facilitates long term species coexistence in a natural grassland ecosystem of China.展开更多
Carex brunnescens(Pers.)Poir.is considered to be the only clonal herb found to date that can develop and form fixed dunes in Maqu alpine degraded grasslands of northwestern China.However,due to strong dormant characte...Carex brunnescens(Pers.)Poir.is considered to be the only clonal herb found to date that can develop and form fixed dunes in Maqu alpine degraded grasslands of northwestern China.However,due to strong dormant characteristics of C.brunnescens seeds,the sand-fixing effect of the plant is severely limited.This study explores a technique that can rapidly promote the seed germination of C.brunnescens,and also investigates the adaptation and sand-fixing effect by cultivating C.brunnescens seedlings to establish living sand barriers in the sand ridges of moving sand dunes.Results show that the seed germination rate obtained a maximum of 63.7%or 65.1%when seeds were treated with 150 mg/L gibberellic acid(GA3)for 24 h followed by soaking in sulfuric acid(98%H2SO4)for 2.5 min or sodium hydroxide(10%NaOH)for 3.5 h,and then germinated(25°C in daytime and 5°C at nighttime)in darkness for 10 d.After breaking seed dormancy of C.brunnescens,the living sand barrier of C.brunnescens(plant spacing 15−20 cm;sand barrier spacing 10−20 m)was established in the perpendicular direction to the main wind in the middle and lower parts of the sand ridges on both sides of the moving sand dunes.When the sand ridges were leveled by wind erosion,the living sand barrier(plant spacing 15−20 cm;sand barrier spacing 0.5−1.0 m)of C.brunnescens was reestablished on the wind-eroded flat ground.Finally,a stable sand-fixing surface can be formed after connecting the living sand barriers on both sides,thus achieving a good sand-fixing effect.These findings suggest that rapid seed germination technology combined with the sand−fixing method of C.brunnescens can shorten the seed germination period and make the seedling establishment become much easier which may be an effective strategy to restore and reconstruct Maqu degraded grasslands.展开更多
Cigarette smoking contributes to the development of pulmonary artery hypertension(PAH).As the basic pathological change of PAH,pulmonary vascular remodeling is considered to be related to the abnormal proliferation of...Cigarette smoking contributes to the development of pulmonary artery hypertension(PAH).As the basic pathological change of PAH,pulmonary vascular remodeling is considered to be related to the abnormal proliferation of pulmonary artery smooth muscle cells(PASMCs).However,the molecular mechanism underlying this process remains not exactly clear.The aim of this research was to study the molecular mechanism of PASMCs proliferation induced by smoking.Human PASMCs(HPASMCs)were divided into 6 groups:0%(control group),cigarette smoking extract(CSE)-treated groups at concentrations of 0.5%,1%,2%,5%,10%CSE respectively.HPASMCs proliferation was observed after 24 h.HPASMCs were divided into two groups:0(control group),0.5%CSE group.The mRNA and protein expression levels of transient receptor potential channel 1(TRPC1)and cyclin D1 in HPASMCs after CSE treatment were respectively detected by RT-PCR and Western blotting.The intracellular calcium ion concentration was measured by the calcium probe in each group.In the negative control group and TRPC1-siRNA transfection group,the proliferation of HPASMCs and the expression of cyclin D1 mRNA and protein were detected.Data were compared with one-way ANOVA(for multiple-group comparison)and independent t-test(for two-group comparison)followed by the least significant difference(LSD)test with the computer software SPSS 17.0.It was found that 0.5%and 1%CSE could promote the proliferation of HPASMCs(P<0.05),and the former was more effective than the latter(P<0.05),while 3%and above CSE had inhibitory effect on HPASMCs(P<0.05).The mRNA and protein expression levels of TRPC1 and cyclin D1 in 0.5%and 1%CSE groups were significantly higher than those in the control group(P<0.05),while those in 3%CSE group were significantly decreased(P<0.05).Moreover,the proliferation of HPASMCs and the expression of cyclin D1 mRNA and protein in TRPC1-siRNA transfection group were significantly reduced as compared with those in the negative control group(P<0.05).It was concluded that low concentration of CSE can promote the proliferation of HPASMCs,while high concentrations of CSE inhibit HPASMCs proliferation.These findings suggested that CSE induced proliferation of HPASMCs at least in part via TRPC1-mediated cyclin D1 expression.展开更多

The calculation results show that optical band gap of AlxGa1-xN crystal is widened that the Eg of AlxGa1-xN can be adjusted by the x, and the absorption spectrum shifts to high-energy direction with the increase of the x, and then the Fermi energy moves to the top of valence band slightly which leads to that conductivity weakened after mixed with the Al. At the same time, the variation trends of complex dielectric function, absorption spectrum and transitivity have been made clear, and the results show that AlxGa1-xN compounds can achieve the theoretical design of photoelectric performance. In a word, AlxGa1-xN crystals are potential semiconductors with very remarkable photoelectric properties, which can be applied in the development of the diversified GaN devices.展开更多

Control measures during the coronavirus disease 2019(COVID-19)outbreak may have limited the spread of infectious diseases.This study aimed to analyze the impact of COVID-19 on the spread of hand,foot,and mouth disease...Control measures during the coronavirus disease 2019(COVID-19)outbreak may have limited the spread of infectious diseases.This study aimed to analyze the impact of COVID-19 on the spread of hand,foot,and mouth disease(HFMD)in China.A mathematical model was established to fit the reported data of HFMD in six selected cities in China's Mainland from 2015 to 2020.The absolute difference(AD)and relative difference(RD)between the reported incidence in 2020,and simulated maximum,minimum,or median incidence of HFMD in 2015-2019 were calculated.The incidence and R effof HFMD have decreased in six selected cities since the outbreak of COVID-19,and in the second half of 2020,the incidence and R effof HFMD have rebounded.The results show that the total attack rate(TAR)in 2020 was lower than the maximum,minimum,and median TAR fitted in previous years in six selected cities(except Changsha City).For the maximum,median,minimum fitted TAR,the range of RD(%)is 42·20-99·20%,36·35-98·41%48·35-96·23%(except Changsha City)respectively.The preventive and control measures of COVID-19 have significantly contributed to the containment of HFMD transmission.展开更多

This study aimed to quantify the transmissibility ofMycobacterium tuberculosis (MTB) among students and non-students using a mathematical model to determine characteristics of TB transmission.Methods: We constructed a dataset of reported TB cases from four regions (Jilin Province, Xiamen City, Chuxiong Prefecture, and Wuhan City) in China from 2005 to 2019. We classified the population and the reported cases under student and non-student groups, and developed two mathematical models [nonseasonal model (Model A) and seasonal model (Model B)] based on the natural history and transmission features of TB. The effective reproduction number (R(eff)) of TB between groups were calculated using the collected data.Results: During the study period, data on 456,423 TB cases were collected from four regions: students accounted for 6.1% of cases. The goodness-of-fit analysis showed that Model A had a better fitting effect (P < 0.001). The average R_(eff) of TB estimated from Model A was 1.68 [interquartile range (IQR): 1.20–1.96] in Chuxiong Prefecture, 1.67 (IQR: 1.40–1.93) in Xiamen City, 1.75 (IQR: 1.37–2.02) in Jilin Province, and 1.79 (IQR: 1.56–2.02) in Wuhan City. The average R_(eff) of TB in the non-student population was 23.30 times (1.65/0.07) higher than that in the student population.Conclusions: The transmissibility of MTB remains high in the non-student population of the areas studied, which is still dominant in the spread of TB. TB transmissibility from the non-student-to-student-population had a strong influence on students. Specific interventions, such as TB screening, should be applied rigorously to control and to prevent TB transmission among students.展开更多

Introduction:Vaccination booster shots are completely necessary for controlling breakthrough infections of severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)in China.The study aims to estimate effectiveness o...Introduction:Vaccination booster shots are completely necessary for controlling breakthrough infections of severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)in China.The study aims to estimate effectiveness of booster vaccines for high-risk populations(HRPs).Methods:A vaccinated Susceptible-Exposed-Symptomatic-Asymptomatic-Recovered/Removed(SEIAR)model was developed to simulate scenarios of effective reproduction number(Reff)from 4 to 6.Total number of infectious and asymptomatic cases were used to evaluated vaccination effectiveness.Results:Our model showed that we could not prevent outbreaks when covering 80%of HRPs with booster unless Reff=4.0 or the booster vaccine had efficacy against infectivity and susceptibility of more than 90%.The results were consistent when the outcome index was confirmed cases or asymptomatic cases.Conclusions:An ideal coronavirus disease 2019(COVID-19)booster vaccination strategy for HRPs would be expected to reach the initial goal to control the transmission of the Delta variant in China.Accordingly,the recommendation for the COVID-19 booster vaccine should be implemented in HRPs who are already vaccinated and could prevent transmission to other groups.展开更多

This study aimed to simulate the optimal vaccination strategy to control the disease by developing an age-specific model based on the current transmission patterns of COVID-19 in Wuhan City, China.Methods: We collected two indicators of COVID-19, including illness onset data and age of confirmed case in Wuhan City, from December 2, 2019, to March 16, 2020. The reported cases were divided into four age groups: group 1, ≤ 14 years old;group 2, 15 to 44 years old;group 3, 44 to 64 years old;and group 4, ≥ 65 years old. An age-specific susceptible-exposed-symptomatic-asymptomatic-recovered/removed model was developed to estimate the transmissibility and simulate the optimal vaccination strategy. The effective reproduction number (R_(eff)) was used to estimate the transmission interaction in different age groups.Results: A total of 47 722 new cases were reported in Wuhan City from December 2, 2019, to March 16, 2020. Before the travel ban of Wuhan City, the highest transmissibility was observed among age group 2 (R_(eff) = 4.28), followed by group 2 to 3 (R_(eff) = 2.61), and group 2 to 4 (R_(eff) = 1.69). China should vaccinate at least 85% of the total population to interrupt transmission. The priority for controlling transmission should be to vaccinate 5% to 8% of individuals in age group 2 per day (ultimately vaccinated 90% of age group 2), followed by 10% of age group 3 per day (ultimately vaccinated 90% age group 3). However, the optimal vaccination strategy for reducing the disease severity identified individuals ≥ 65 years old as a priority group, followed by those 45-64 years old.Conclusions: Approximately 85% of the total population (nearly 1.2 billion people) should be vaccinated to build an immune barrier in China to safely consider removing border restrictions. Based on these results, we concluded that 90% of adults aged 15-64 years should first be vaccinated to prevent transmission in China.展开更多

Objective:Mumps is a seasonal infectious disease,always occurring in winter and spring.In this study,we aim to analyze its epidemiological characteristics,transmissibility,and its correlation with meteorological varia...Objective:Mumps is a seasonal infectious disease,always occurring in winter and spring.In this study,we aim to analyze its epidemiological characteristics,transmissibility,and its correlation with meteorological variables.Method:A seasonal Susceptiblee Exposede Infectious/Asymptomatice Recovered model and a next-generation matrix method were applied to estimate the time-dependent reproduction number(Rt).Results:The seasonal double peak of annual incidence was mainly in May to July and November to December.There was high transmission at the median of Rt¼1.091(ranged:0 to 4.393).Rt was seasonally distributed mainly from February to April and from September to November.Correlations were found between temperature(Pearson correlation coefficient[r]ranged:from 0.101 to 0.115),average relative humidity(r¼0.070),average local pressure(r¼-0.066),and the number of new cases.In addition,average local pressure(r¼0.188),average wind speed(r¼0.111),air temperature(r ranged:-0.128 to-0.150),average relative humidity(r¼-0.203)and sunshine duration(r¼-0.075)were all correlated with Rt.Conclusion:A relatively high level of transmissibility has been found in Xiamen City,leading to a continuous epidemic of mumps.Meteorological factors,especially air temperature and relative humidity,may be more closely associated with mumps than other factors.展开更多
文摘BACKGROUND The clinical advantages of painless colonoscopy can reduce the fear and discomfort of patients and increase the detection rate of diseases.Propofol has the characteristics of fast effect and short action time.It is a common choice for painless endoscopic sedation and anesthetics.However,propofol can cause severe respiratory and circulatory depression.Therefore,it is important to find a way to reduce the dose of propofol.AIM To explore the effect of wrist-ankle acupuncture on propofol dose during colonoscopy.METHODS Two hundred patients who were going to receive selective painless colonoscopy in Hebei Hospital of Traditional Chinese Medicine were selected and divided into wrist-ankle acupuncture group(WAA group,n=100)and control group(CON group,n=100).After entering the operation room,patients were given 0.025 mg/kg nabufine intravenously and propofol at the initial dose of 0.5 mg/kg.In patients who did not fall asleep,propofol(10 mg/time)was given until loss of consciousness.Prior to anesthesia,patients in WAA group were punctured by specialist in the inferior 1,2 and 3 regions according to the zoning principle of wrist-ankle acupuncture.The primary endpoint was required dose of propofol,and the secondary endpoints were the incidence of hypoxemia and hypotension.Furthermore,the following data were recorded:The operation time,wake-up time,incidence of nausea and vomiting,incidence of abdominal distention,post colonoscopy pain,examiners'satisfaction,patients'satisfaction and Borg fatigue index.This study has been registered in the Chinese Clinical Trial Registry(Registration Code:ChiCTR1900022177).RESULTS The induced dose of propofol and the total dose of propofol in WAA group were 80 mg and 110 mg,respectively,which were significantly lower than those in CON group(P<0.05).The incidences of hypoxemia and hypotension in the WAA group were 2.2%and 3.3%,respectively,significantly lower than those in the CON group(P<0.05).The incidence of abdominal distension in the WAA group was 8.8%,which was significantly lower than that in the CON group(P<0.05,28.9%).The waking time of WAA group was 3.26±0.87 min,which was significantly lower than that of CON group(6.06±0.88 min,P<0.05).CONCLUSION Wrist-ankle acupuncture can reduce the induction dose and total dose of propofol as well as the incidence of adverse reactions in painless colonoscopy without affecting the satisfaction of examiners and patients.This procedure is simple in operation and easy to promote in clinical practice.
文摘Behcet's disease(BD) is a rare and life-long disorder characterized by inflammation of blood vessels throughout the body. BD was originally described in 1937 as a syndrome involving oral and genital ulceration in addition to ocular inflammation. Intestinal BD refers to colonic ulcerative lesions documented by objective measures in patients with BD. Many studies have shown that over 40% of BD patients have gastrointestinal complaints. Symptoms include abdominal pain, diarrhea, nausea, anorexia and abdominal distension. Although gastrointestinal symptoms are common, the demonstration of gastrointestinal ulcers is rare. This so-called intestinal BD accounts for approximately 1% of cases. There is no specific test for BD, and the diagnosis is based on clinical criteria. The manifestations of intestinal BD are similar to those of other colitis conditions such as Crohn's disease or intestinal tuberculosis, thus, it is challenging for gastroenterologists to accurately diagnose intestinal BD in patients with ileocolonic ulcers. However, giant ulcers distributed in the esophagus and ileocecal junction with gastrointestinal hemorrhage are rare in intestinal BD. Here, we present a case of untypical intestinal BD. The patient had recurrent aphthous ulceration of the oral mucosa, and esophageal and ileo-colonic ulceration, but no typical extra-intestinal symptoms. During examination, the patient had massive acute lower gastrointestinal bleeding. The patient underwent ileostomy after an emergency right hemicolectomy and partial ileectomy, and was subsequently diagnosed with incomplete-type intestinal BD by pathology. The literature on the evaluation and management of this condition is reviewed.

文摘Understanding the effect of biodiversity on ecosystem function is critical to promoting the sustainability of ecosystems and species conservation in natural ecosystems. We observed species composition, species richness and aboveground biomass,and simulated the competitive assemblages in a natural grassland ecosystem of China, aiming to test some assumptions and predictions about biodiversity–stability relationships. Our results show that aboveground productivity and temporal stability increased significantly with increasing species richness, and via a combination of overyielding, species asynchrony, and portfolio effects. Species interactions resulted in overyielding caused by trait-independent complementarity, and were not offset by a negative dominance effect and trait-dependent complementarity effect. Therefore, the mechanisms underlying the biodiversity effect shifted from the selection effect to the complementarity effect as diversity increased, and both effects were coexisted but the complementarity effect represent a mechanism that facilitates long term species coexistence in a natural grassland ecosystem of China.

文摘Carex brunnescens(Pers.)Poir.is considered to be the only clonal herb found to date that can develop and form fixed dunes in Maqu alpine degraded grasslands of northwestern China.However,due to strong dormant characteristics of C.brunnescens seeds,the sand-fixing effect of the plant is severely limited.This study explores a technique that can rapidly promote the seed germination of C.brunnescens,and also investigates the adaptation and sand-fixing effect by cultivating C.brunnescens seedlings to establish living sand barriers in the sand ridges of moving sand dunes.Results show that the seed germination rate obtained a maximum of 63.7%or 65.1%when seeds were treated with 150 mg/L gibberellic acid(GA3)for 24 h followed by soaking in sulfuric acid(98%H2SO4)for 2.5 min or sodium hydroxide(10%NaOH)for 3.5 h,and then germinated(25°C in daytime and 5°C at nighttime)in darkness for 10 d.After breaking seed dormancy of C.brunnescens,the living sand barrier of C.brunnescens(plant spacing 15−20 cm;sand barrier spacing 10−20 m)was established in the perpendicular direction to the main wind in the middle and lower parts of the sand ridges on both sides of the moving sand dunes.When the sand ridges were leveled by wind erosion,the living sand barrier(plant spacing 15−20 cm;sand barrier spacing 0.5−1.0 m)of C.brunnescens was reestablished on the wind-eroded flat ground.Finally,a stable sand-fixing surface can be formed after connecting the living sand barriers on both sides,thus achieving a good sand-fixing effect.These findings suggest that rapid seed germination technology combined with the sand−fixing method of C.brunnescens can shorten the seed germination period and make the seedling establishment become much easier which may be an effective strategy to restore and reconstruct Maqu degraded grasslands.
文摘Cigarette smoking contributes to the development of pulmonary artery hypertension(PAH).As the basic pathological change of PAH,pulmonary vascular remodeling is considered to be related to the abnormal proliferation of pulmonary artery smooth muscle cells(PASMCs).However,the molecular mechanism underlying this process remains not exactly clear.The aim of this research was to study the molecular mechanism of PASMCs proliferation induced by smoking.Human PASMCs(HPASMCs)were divided into 6 groups:0%(control group),cigarette smoking extract(CSE)-treated groups at concentrations of 0.5%,1%,2%,5%,10%CSE respectively.HPASMCs proliferation was observed after 24 h.HPASMCs were divided into two groups:0(control group),0.5%CSE group.The mRNA and protein expression levels of transient receptor potential channel 1(TRPC1)and cyclin D1 in HPASMCs after CSE treatment were respectively detected by RT-PCR and Western blotting.The intracellular calcium ion concentration was measured by the calcium probe in each group.In the negative control group and TRPC1-siRNA transfection group,the proliferation of HPASMCs and the expression of cyclin D1 mRNA and protein were detected.Data were compared with one-way ANOVA(for multiple-group comparison)and independent t-test(for two-group comparison)followed by the least significant difference(LSD)test with the computer software SPSS 17.0.It was found that 0.5%and 1%CSE could promote the proliferation of HPASMCs(P<0.05),and the former was more effective than the latter(P<0.05),while 3%and above CSE had inhibitory effect on HPASMCs(P<0.05).The mRNA and protein expression levels of TRPC1 and cyclin D1 in 0.5%and 1%CSE groups were significantly higher than those in the control group(P<0.05),while those in 3%CSE group were significantly decreased(P<0.05).Moreover,the proliferation of HPASMCs and the expression of cyclin D1 mRNA and protein in TRPC1-siRNA transfection group were significantly reduced as compared with those in the negative control group(P<0.05).It was concluded that low concentration of CSE can promote the proliferation of HPASMCs,while high concentrations of CSE inhibit HPASMCs proliferation.These findings suggested that CSE induced proliferation of HPASMCs at least in part via TRPC1-mediated cyclin D1 expression.
文摘Objective:The clinical characteristics and microbiological data of patients with K.pneumoniae bloodstream infections(BSI)from January 2018 to December 2020 were retrospectively analyzed to study the molecular epidemiology of Carbapenem-resistant Klebsiella pneumoniae(CRKP).We also aimed to identify the risk factors for the development of CRKP BSI.Methods:This retrospective study was conducted at Renmin Hospital of Wuhan University from January 2018 to December 2020.The date of non-duplicate K.pneumoniae isolates isolated from blood samples was identified using the microbiology laboratory database.The data from patients diagnosed with K.pneumoniae BSI were collected and analyzed.

文摘With the development of information and communication technology and the advent of the Internet of Things(IoT)era,cyber-physical system(CPS)is becoming the trend of products or systems.The deep integration and real-time interaction between the physical world and the virtual world expand system functions.Although there are some CPS implementation guidelines,the virtual world is still relatively abstract compared to the concrete physical world that can be touched through the IoT.Besides that,human is a non-negligible CPS endogenous interactive intelligent component.In this paper,we propose a triple human-digital twin architecture,where the physical objects and the digital twins that are the projections of the physical entities establish the cornerstone of human functioning together.And the hierarchically distributed digital twins grow dynamically with the physical entities along the lifecycle.Furthermore,the interaction and collaboration among the physical objects,the digital twins,and the humans in their respective worlds(the expected world,the interpreted world,and the physical world)integrate the full value chain of the products in anticipation of seamless synergy.Finally,we present a power management digital companion platform for the lunar probe to demonstrate the efficacy of the architecture.
文摘By making use of the density functional theory (DFT) of the first principles and generalized gradient approximation method, the electronic structures and properties of ideal GaN and AlxGa1-xN crystals (x = 0.25, 0.5, 0.75) have been calculated and analyzed, and the influence of the doping quantity of x on the characteristics of AlxGa1-xN has been studied. The calculation results show that optical band gap of AlxGa1-xN crystal is widened that the Eg of AlxGa1-xN can be adjusted by the x, and the absorption spectrum shifts to high-energy direction with the increase of the x, and then the Fermi energy moves to the top of valence band slightly which leads to that conductivity weakened after mixed with the Al. At the same time, the variation trends of complex dielectric function, absorption spectrum and transitivity have been made clear, and the results show that AlxGa1-xN compounds can achieve the theoretical design of photoelectric performance. In a word, AlxGa1-xN crystals are potential semiconductors with very remarkable photoelectric properties, which can be applied in the development of the diversified GaN devices.

文摘Garlic,an economically important vegetable,spice,and medicinal crop,produces highly enlarged bulbs and unique organosulfur compounds.Here,we report a chromosome-level genome assembly for garlic,with a total size of approximately 16.24 Gb,as well as the annotation of 57561 predicted protein-coding genes,making garlic the first Allium species with a sequenced genome.Analysis of this garlic genome assembly reveals a recent burst of transposable elements,explaining the substantial expansion of the garlic genome.We examined the evolution of certain genes associated with the biosynthesis of allicin and inulin neoseries-type fructans,and provided new insights into the biosynthesis of these two compounds.Furthermore,a large-scale transcriptome was produced to characterize the expression patterns of garlic genes in different tissues and at various growth stages of enlarged bulbs.The reference genome and large-scale transcriptome data generated in this study provide valuable new resources for research on garlic biology and breeding.

文摘Hepatocellular carcinoma(HCC)is an aggressive human cancer with increasing incidence worldwide.Multiple efforts have been made to explore pharmaceutical therapies to treat HCC,such as targeted tyrosine kinase inhibitors,immune based therapies and combination of chemotherapy.However,limitations exist in current strategies including chemoresistance for instance.Tumor initiation and progression is driven by reprogramming of metabolism,in particular during HCC development.Recently,metabolic associated fatty liver disease(MAFLD),a reappraisal of new nomenclature for nonalcoholic fatty liver disease(NAFLD),indicates growing appreciation of metabolism in the pathogenesis of liver disease,including HCC,thereby suggesting new strategies by targeting abnormal metabolism for HCC treatment.In this review,we introduce directions by highlighting the metabolic targets in glucose,fatty acid,amino acid and glutamine metabolism,which are suitable for HCC pharmaceutical intervention.We also summarize and discuss current pharmaceutical agents and studies targeting deregulated metabolism during HCC treatment.Furthermore,opportunities and challenges in the discovery and development of HCC therapy targeting metabolism are discussed.

文摘Control measures during the coronavirus disease 2019(COVID-19)outbreak may have limited the spread of infectious diseases.This study aimed to analyze the impact of COVID-19 on the spread of hand,foot,and mouth disease(HFMD)in China.A mathematical model was established to fit the reported data of HFMD in six selected cities in China's Mainland from 2015 to 2020.The absolute difference(AD)and relative difference(RD)between the reported incidence in 2020,and simulated maximum,minimum,or median incidence of HFMD in 2015-2019 were calculated.The incidence and R effof HFMD have decreased in six selected cities since the outbreak of COVID-19,and in the second half of 2020,the incidence and R effof HFMD have rebounded.The results show that the total attack rate(TAR)in 2020 was lower than the maximum,minimum,and median TAR fitted in previous years in six selected cities(except Changsha City).For the maximum,median,minimum fitted TAR,the range of RD(%)is 42·20-99·20%,36·35-98·41%48·35-96·23%(except Changsha City)respectively.The preventive and control measures of COVID-19 have significantly contributed to the containment of HFMD transmission.

文摘Background: Recently, despite the steady decline in the tuberculosis (TB) epidemic globally, school TB outbreaks have been frequently reported in China. This study aimed to quantify the transmissibility ofMycobacterium tuberculosis (MTB) among students and non-students using a mathematical model to determine characteristics of TB transmission.Methods: We constructed a dataset of reported TB cases from four regions (Jilin Province, Xiamen City, Chuxiong Prefecture, and Wuhan City) in China from 2005 to 2019. We classified the population and the reported cases under student and non-student groups, and developed two mathematical models [nonseasonal model (Model A) and seasonal model (Model B)] based on the natural history and transmission features of TB. The effective reproduction number (R(eff)) of TB between groups were calculated using the collected data.Results: During the study period, data on 456,423 TB cases were collected from four regions: students accounted for 6.1% of cases. The goodness-of-fit analysis showed that Model A had a better fitting effect (P < 0.001). The average R_(eff) of TB estimated from Model A was 1.68 [interquartile range (IQR): 1.20–1.96] in Chuxiong Prefecture, 1.67 (IQR: 1.40–1.93) in Xiamen City, 1.75 (IQR: 1.37–2.02) in Jilin Province, and 1.79 (IQR: 1.56–2.02) in Wuhan City. The average R_(eff) of TB in the non-student population was 23.30 times (1.65/0.07) higher than that in the student population.Conclusions: The transmissibility of MTB remains high in the non-student population of the areas studied, which is still dominant in the spread of TB. TB transmissibility from the non-student-to-student-population had a strong influence on students. Specific interventions, such as TB screening, should be applied rigorously to control and to prevent TB transmission among students.
文摘Background:Novel coronavirus disease 2019(COVID-19)causes an immense disease burden.Although public health countermeasures effectively controlled the epidemic in China,non-pharmaceutical interventions can neither be maintained indefinitely nor conveniently implemented globally.Vaccination is mainly used to prevent COVID-19,and most current antiviral treatment evaluations focus on clinical efficacy.Therefore,we conducted population-based simulations to assess antiviral treatment effectiveness among different age groups based on its clinical efficacy.
文摘Introduction:Vaccination booster shots are completely necessary for controlling breakthrough infections of severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)in China.The study aims to estimate effectiveness of booster vaccines for high-risk populations(HRPs).Methods:A vaccinated Susceptible-Exposed-Symptomatic-Asymptomatic-Recovered/Removed(SEIAR)model was developed to simulate scenarios of effective reproduction number(Reff)from 4 to 6.Total number of infectious and asymptomatic cases were used to evaluated vaccination effectiveness.Results:Our model showed that we could not prevent outbreaks when covering 80%of HRPs with booster unless Reff=4.0 or the booster vaccine had efficacy against infectivity and susceptibility of more than 90%.The results were consistent when the outcome index was confirmed cases or asymptomatic cases.Conclusions:An ideal coronavirus disease 2019(COVID-19)booster vaccination strategy for HRPs would be expected to reach the initial goal to control the transmission of the Delta variant in China.Accordingly,the recommendation for the COVID-19 booster vaccine should be implemented in HRPs who are already vaccinated and could prevent transmission to other groups.

文摘Introduction:The coronavirus disease 2019(COVID-19)pandemic recently affected Taiwan,China.This study aimed to calculate the transmissibility of COVID-19 to predict trends and evaluate the effects of interventions.Methods:The data of reported COVID-19 cases was collected from April 20 to May 26,2021,which included daily reported data(Scenario I)and reported data after adjustment(Scenario II).A susceptibleexposed-symptomatic-asymptomatic-recovered model was developed to fit the data.The effective reproductive number(Reff)was used to estimate the transmissibility of COVID-19.Results:A total of 4,854 cases were collected for the modelling.In Scenario I,the intervention has already taken some effects from May 17 to May 26(the Reff reduced to 2.1).When the Reff was set as 0.1,the epidemic was projected to end on July 4,and a total of 1,997 cases and 855 asymptomatic individuals would have been reported.In Scenario II,the interventions were projected as having been effective from May 24 to May 26(the Reff reduced to 0.4).When the Reff was set as 0.1,the epidemic was projected to end on July 1,and a total of 1,482 cases and 635 asymptomatic individuals would have been reported.Conclusion:The epidemic of COVID-19 was projected to end after at least one month,even if the most effective interventions were applied in Taiwan,China.Although there were some positive effects of intervention in Taiwan,China.

文摘Background: Reaching optimal vaccination rates is an essential public health strategy to control the coronavirus disease 2019 (COVID-19) pandemic. This study aimed to simulate the optimal vaccination strategy to control the disease by developing an age-specific model based on the current transmission patterns of COVID-19 in Wuhan City, China.Methods: We collected two indicators of COVID-19, including illness onset data and age of confirmed case in Wuhan City, from December 2, 2019, to March 16, 2020. The reported cases were divided into four age groups: group 1, ≤ 14 years old;group 2, 15 to 44 years old;group 3, 44 to 64 years old;and group 4, ≥ 65 years old. An age-specific susceptible-exposed-symptomatic-asymptomatic-recovered/removed model was developed to estimate the transmissibility and simulate the optimal vaccination strategy. The effective reproduction number (R_(eff)) was used to estimate the transmission interaction in different age groups.Results: A total of 47 722 new cases were reported in Wuhan City from December 2, 2019, to March 16, 2020. Before the travel ban of Wuhan City, the highest transmissibility was observed among age group 2 (R_(eff) = 4.28), followed by group 2 to 3 (R_(eff) = 2.61), and group 2 to 4 (R_(eff) = 1.69). China should vaccinate at least 85% of the total population to interrupt transmission. The priority for controlling transmission should be to vaccinate 5% to 8% of individuals in age group 2 per day (ultimately vaccinated 90% of age group 2), followed by 10% of age group 3 per day (ultimately vaccinated 90% age group 3). However, the optimal vaccination strategy for reducing the disease severity identified individuals ≥ 65 years old as a priority group, followed by those 45-64 years old.Conclusions: Approximately 85% of the total population (nearly 1.2 billion people) should be vaccinated to build an immune barrier in China to safely consider removing border restrictions. Based on these results, we concluded that 90% of adults aged 15-64 years should first be vaccinated to prevent transmission in China.

文摘Objective:Mumps is a seasonal infectious disease,always occurring in winter and spring.In this study,we aim to analyze its epidemiological characteristics,transmissibility,and its correlation with meteorological variables.Method:A seasonal Susceptiblee Exposede Infectious/Asymptomatice Recovered model and a next-generation matrix method were applied to estimate the time-dependent reproduction number(Rt).Results:The seasonal double peak of annual incidence was mainly in May to July and November to December.There was high transmission at the median of Rt¼1.091(ranged:0 to 4.393).Rt was seasonally distributed mainly from February to April and from September to November.Correlations were found between temperature(Pearson correlation coefficient[r]ranged:from 0.101 to 0.115),average relative humidity(r¼0.070),average local pressure(r¼-0.066),and the number of new cases.In addition,average local pressure(r¼0.188),average wind speed(r¼0.111),air temperature(r ranged:-0.128 to-0.150),average relative humidity(r¼-0.203)and sunshine duration(r¼-0.075)were all correlated with Rt.Conclusion:A relatively high level of transmissibility has been found in Xiamen City,leading to a continuous epidemic of mumps.Meteorological factors,especially air temperature and relative humidity,may be more closely associated with mumps than other factors.
