Objective To systematically analyze the current status of outcomes reporting in clinical trials on treating stasis acute mastitis with Traditional Chinese Medicine breast massage.Methods We searched CNKI,Wanfang,VIP,S...Objective To systematically analyze the current status of outcomes reporting in clinical trials on treating stasis acute mastitis with Traditional Chinese Medicine breast massage.Methods We searched CNKI,Wanfang,VIP,Sino Med,Pub Med,Web of Science,Embase,Cochrane library,JBI,CINAHL,Psyc INFO,Clinical Trials Registry Platform portal,Clinical Trials Registry,Australian New Zealand Clinical Trials Registry,Center Watch Registry from inception to May 15,2022 to find randomized controlled trials,non-randomized controlled trials,case series and cohort studies which reported the outcomes of stasis acute mastitis managed with Traditional Chinese Medicine breast massage,with search terms of mastitis,acute mastitis,lactation mastitis,puerperal mastitis,breast problem,breast engorgement,milk stasis,blocked ducked,breast pain,breast massage,and acupoint massage.Outcomes and the measurement schemes(measurement methods,timing of assessing outcome,frequency of assessing outcome,measurers)were extracted from the included studies.We used the Management of Otitis Media with Effusion in Children with Cleft Palate(MOMENT)to assess the quality of each study,then categorized outcomes derived from the included studies into different domains according to the Outcome Measures in Rheumatology Arthritis Clinic Trials(OMERACT)Filter 2.1 framework.Results We identified 85 clinical trials,in which 54 different outcomes were reported.A total of 81.2%(69/85)of studies were assessed as medium quality with a mean score of 2.6,and 18.8%(16/85)as low quality with a mean score of 0.9.These outcomes were organized in three core areas.Lump size(89.4%,76/85)was the most frequently reported outcome,followed by breast pain(69.4%,59/85)and milk excretion(68.2%,58/85).Five methods were used to assess lump size and four methods to assess breast pain.Conclusions The outcomes reported in clinical trials regarding stasis acute mastitis treated by Traditional Chinese Medicine breast massage are heterogeneous.Developing a core outcome set to achieve consistent standards for reporting outcomes and modalities for validation of the outcomes is clearly warranted.展开更多
Objective:The objective of this study is to explore the current situation,clinical research hot spots,and trends of traditional Chinese medicine(TCM)nursing technology in rheumatoid arthritis(RA),to draw a panorama of...Objective:The objective of this study is to explore the current situation,clinical research hot spots,and trends of traditional Chinese medicine(TCM)nursing technology in rheumatoid arthritis(RA),to draw a panorama of clinical research,to provide the basis and clues for subsequent high-level evidence integration and further in-depth research.Materials and Methods:Seven databases including China National Knowledge Infrastructure,Chinese Scientific Journal Database(VIP),Wanfang Database,Chinese Biomedical Literature Database(Sino-Med),PubMed,Embase,and the Cochrane Library were searched.The bibliometric method and visualization software CiteSpace were used to conduct a multi-dimensional analysis of the included literature.Results:A total of 805 pieces of literature were included(of them,one was written in German and four in English),and the number of published literature showed an increasing trend year by year.There were only 30(3.73%)pieces of literature published in nursing journals.The hot spots of the co-occurrence map were concentrated in:(1)the nursing of fumigation in RA;(2)the efficacy evaluation of TCM nursing technology on pain,joint malformation,and joint dysfunction caused by RA.The included literature themes focused on five TCM technologies:fumigation,moxibustion,acupoint patching,acupoint injection,and Chinese herbal soaking.The beginning year and strength in TCM fumigation were 2012 and 5.2,and those in moxibustion were 2011 and 3.38.Conclusion:The related studies are on the rise.It has entered the field of international readers with clear research hot spots.However,there are still shortcomings such as little literature published in Chinese nursing core journals,few non-Chinese-related documents,and a lack of international exchanges and cooperation.The current research hot spots in this field are TCM fumigation,and the cutting-edge trend of future research may be fumigation and moxibustion technology.It is suggested that further research can focus on evidence integration and original research on the self-optimization of these two techniques.展开更多

Objective To compare the efficacy of Chinese herbal medicine (therapy A) or Chinese herbal medicine plus indomethacin (therapy B) with that of indomethacin alone (therapy C) in treating polyhydramnios.Methods Literatu...Objective To compare the efficacy of Chinese herbal medicine (therapy A) or Chinese herbal medicine plus indomethacin (therapy B) with that of indomethacin alone (therapy C) in treating polyhydramnios.Methods Literatures published up to April 2012 were retrieved from PubMed,Embase and Cochrane library,Chinese National Knowledge Infrastructure (CNKI),Chinese Scientific and Technological Periodical Database (VIP),Wangfang,and Traditional Chinese Medicine online.Two researchers collected data independently.The assessment of methodological quality was based on Cochrane handbook and the materials were analyzed with software RevMan 5.1.2.The outcome measure index was relative risk or difference of mean value (95% confidence interval).The following outcomes were evaluated:(1) general clinical improvement rate;(2) maximum vertical pocket depth;(3) amniotic fluid index (AFI) value;(4) rate of fetal ductus arteriosus constriction;(5) incidence of adverse events.Results Based on the search strategy,5 trails involving 1017 patients were finally included.Compared with therapy C,therapy A decreased the rate of fetal ductus arteriosus constriction (P<0.01).Therapy B was more effective than therapy C in general clinical improvement and decrease of AFI for polyhydramnios (P< 0.01).No serious adverse events were reported in therapy A and therapy B.Conclusions Compared with therapy C,therapy A and therapy B may appear to be more effective for polyhydramnios.However,the exact effect needs to be confirmed with well-designed large-scale clinical trials.展开更多
Objective:Methotrexate(MTX)can be safely administered to most patients but may cause severe toxicity in others.This study aimed to summarize the characteristics of high-dose methotrexate(HD-MTX)chemotherapy and to eva...Objective:Methotrexate(MTX)can be safely administered to most patients but may cause severe toxicity in others.This study aimed to summarize the characteristics of high-dose methotrexate(HD-MTX)chemotherapy and to evaluate whether the modified dose-adjustment program was able to improve the maintenance of sufficient MTX exposure levels while minimizing toxicities.Methods:We evaluated 1172 cycles of high-dose MTX chemotherapy from 294 patients who were treated according to the CCCG-ALL-2015 protocol(clinical trial number:ChiCTR-IPR-14005706)and analyzed the data of actual MTX dosage,MTX concentration,toxicity,and prognosis.We compared data between the dose-adjustment Program 1(fixed 20%reduction in dose)and the dose-adjustment Program 2(dose-individualization based on reassessment of the creatine clearance rate and the MTX concentration-monitoring point at 16 h),which were applied if the MTX clearance was delayed in the previous cycle.Results:The patients who used Program 2 had higher actual MTX infusion doses and infusion rates and were able to better maintain the MTX concentration at 44 h at the established target value than those on Program 1(P<0.001).No significant differences in toxicities were found between these two programs except that abnormal serum potassium levels and prolonged myelosuppression in intermediate-risk/high-risk patients were more frequently observed in patients using Program 2(P<0.001).No significant correlations were observed between the MTX dose,dose-adjustment programs,or MTX concentrations and relapse-free survival.Conclusion:Adjusting the MTX dose using Program 2 is more efficient for maintaining sufficient MTX exposure without significantly increasing the toxicity.展开更多
Objective At present,a number of very severe aplastic anemia(VSAA)patients cannot receive hematopoietic stem cell transplantation(HSCT)or standard immunosuppressive therapy(IST)due to the high cost of therapy,shortage...Objective At present,a number of very severe aplastic anemia(VSAA)patients cannot receive hematopoietic stem cell transplantation(HSCT)or standard immunosuppressive therapy(IST)due to the high cost of therapy,shortage of sibling donors,and lack of resources to support the HSCT.In addition,some VSAA patients with autoantibodies have no life-threatening infections or bleeding at the time of initial diagnosis.Considering the disease condition,economics and other factors,the present study designed a new and relatively mild treatment strategy:cyclosporine A plus pulsed high-dose prednisone(CsA+HDP).Methods The present study retrospectively analyzed 11 VSAA patients,who were treated with CsA+HDP in our hospital from August 2017 to August 2019.Results The median follow-up time for these patients was 24.9 months.The overall response rate was 54.5%(6/11)at six months after the initiation of IST and 81.8%(9/11)at deadline.Five patients achieved complete remission and four patients met the criteria for partial response at the last follow-up.The median time to response for responders was 110 days.Three patients underwent HSCT due to the poor effect of CsA+HDP or to find a suitable transplant donor.Recurrence and clonal evolution were not found in any of these patients.The estimated 3-year overall survival rate and 3-year failure-free survival rate were 100.0%and 72.7%,respectively.In addition,the results revealed that the cyclosporine-prednisone-associated toxicity was mild and well-tolerated by most patients.Conclusion The novel CsA+HDP regimen has good therapeutic effect and safety for VSAA patients with autoantibodies,who have no serious life-threatening infections or bleeding at the time of initial diagnosis.展开更多

Venous thromboembolism(VTE)is a complication in children with acute lymphoblastic leukemia(ALL).The Chinese Children’s Cancer Group-ALL-2015 protocol was carried out in China,and epidemiology,clinical characteristics...Venous thromboembolism(VTE)is a complication in children with acute lymphoblastic leukemia(ALL).The Chinese Children’s Cancer Group-ALL-2015 protocol was carried out in China,and epidemiology,clinical characteristics,and risk factors associated with VTE were analyzed.We collected data on VTE in a multiinstitutional clinical study of 7640 patients with ALL diagnosed in 20 hospitals from January 2015 to December 2019.First,VTE occurred in 159(2.08%)patients,including 90(56.6%)during induction therapy and 108(67.92%)in the upper extremities.T-ALL had a 1.74-fold increased risk of VTE(95%CI 1.08–2.8,P=0.022).Septicemia,as an adverse event of ALL treatment,can significantly promote the occurrence of VTE(P<0.001).Catheter-related thrombosis(CRT)accounted for 75.47%(n=120);and,symptomatic VTE,58.49%(n=93),which was more common in patients aged 12–18 years(P=0.023),non-CRT patients(P<0.001),or patients with cerebral thrombosis(P<0.001).Of the patients with VTE treated with anticoagulation therapy(n=147),4.08%(n=6)had bleeding.The VTE recurrence rate was 5.03%(n=8).Patients with VTE treated by non-ultrasoundguided venous cannulation(P=0.02),with residual thrombus(P=0.006),or with short anticoagulation period(P=0.026)had high recurrence rates.Thus,preventing repeated venous puncture and appropriately prolonged anticoagulation time can reduce the risk of VTE recurrence.展开更多
Background Chemotherapy and hematopoietic stem cell transplantation(HSCT)can damage the immune system,and may result in a loss of protection from infectious diseases.This study aimed to evaluate the impact of these tr...Background Chemotherapy and hematopoietic stem cell transplantation(HSCT)can damage the immune system,and may result in a loss of protection from infectious diseases.This study aimed to evaluate the impact of these treatments on the decrease in antibody titers of the measles,mumps,and rubella(MMR)vaccine and seroconversion post-revaccination of MMR.Methods After completion of treatment for primary diseases,participants received an MMR revaccination.Antibody titers for MMR before revaccination were analyzed for all 110 children.After revaccination,68 participants received a follow-up evaluation of antibody titer and adverse reaction.Results Multivariable analysis showed that therapeutic schedules were the only factor correlated with lack of antibody titers for measles after completing treatment(P=0.008),while for mumps and rubella,no statistically significant difference was observed.Importantly,our study clearly demonstrated positive seroconversion rates for measles(97.5%),mumps(81.0%),and rubella(93.2%),with antibody levels rising across the board and peaking at around 6 months following revaccination.However,6 months after revaccination,a downtrend of antibody titer levels was observed,which is comparatively earlier than the waning immunity observed in healthy children.Furthermore,we found MMR revaccination to be safe,with only a single adverse reaction(local pain at the injection site)reported.Conclusions MMR revaccination is immunogenic for the population.We suggest periodic monitoring of antibody titers,in addition to a booster vaccination,although the optimal timing of booster vaccination remains to be investigated further.展开更多
The influences of equimolar substitution of yttria by gadolinia on the phase constituent and stability for Y_2O_3—Ta_2O_5—ZrO_2 ceramics have been investigated.The ceramics with the Gd content lower than 8 nnol%exhi...The influences of equimolar substitution of yttria by gadolinia on the phase constituent and stability for Y_2O_3—Ta_2O_5—ZrO_2 ceramics have been investigated.The ceramics with the Gd content lower than 8 nnol%exhibit pure tetragonal phase as well as high tetragonal phase stability.However,the high Gd containing ceramics consist of t-ZrO_2,m-ZrO_2 and monoclinic GdTaO_4 and show complicated phase evolution behaviors.The fractions of monoclinic ZrO_2 and GdTaO_4 increase with increasing Gd content,indicating that the excessive substitution of Gd for Y results in a reduction in the tetragonal phase stability.In addition,the lower Gd containing ceramics show an almost invariable tetragonality,while the higher Gd containing ceramics reveal a composition-dependent tetragonality.Accordingly,an association of the tetragonal phase stability and tetragonality with the Gd concentration is established,which provides us a clue to understand the phase stability of these ceramics.展开更多

However, curr...Background: Acupoint herbal patching(AHP) alone or as an adjuvant therapy with conventional treatment(CT) has been widely used for prevention and treatment of chronic obstructive pulmonary disease(COPD). However, current clinical evidence from a systematic review of RCTs is lacking.Objective: To evaluate the effectiveness and safety of AHP with or without CT for people with COPD at stable stage.Methods: We searched randomized controlled trials comparing AHP(with or without CT) with no intervention, placebo, or CT from six databases. Two authors selected studies, extracted data and evaluated risk of bias of included trials. Rev Man 5.2 software was used to analysis data.Results: Twenty one RCTs(2327 participants) were included. AHP of non sanfu applied on no fixed dates with CT significantly decreased the mean frequency of acute exacerbation of COPD(times per year)(MD:-1.24; 95% CI:-2.02 to-0.46; 2 trials), and improved lung function parameters and quality of life. The AHP with CT showed no better effect in 6-minute walking distance(6 MWD) that CT alone. AHP applied at sanfu(specific dates based on lunar calendar) with CT had significant effect for 6 MWD(MD: 11.20; 95% CI: 0.83 to 21.56; I^2= 0%;3 trials). One trial reported skin irritation from AHP. Another trial reported two patients had eye discomfort, which was inferred as the adverse effects of seretide.Conclusion: AHP used as an adjunct to CT, appears to be more effective than CT alone in patients with stable COPD. However, further large,rigorously designed trials are warranted to confirm these potential effects.展开更多
Rare earth Er^(3+)doped(Sm_(1-x)Er_(x))_(2)Zr_(2)O_(7)(x=0.1,0.2,and 0.3)ceramic samples were synthesized using a solid state reaction method.The microstructure and thermal properties of these ceramics were investigat...Rare earth Er^(3+)doped(Sm_(1-x)Er_(x))_(2)Zr_(2)O_(7)(x=0.1,0.2,and 0.3)ceramic samples were synthesized using a solid state reaction method.The microstructure and thermal properties of these ceramics were investigated to evaluate their potential as thermal barrier coating materials.The results show that ceramics are compact with regular-shaped grains of 1-5μm size.(Sm_(1-x)Er_(x))_(2)Zr_(2)O_(7)has a pyrochlore structure mainly determined by ionic radius ratio,but the ordering degree decreases with increase of the Er_(2)O_(3)content.There is no phase transformation from 1000 to 1200℃,and the(Sm_(1-x)Er_(x))_(2)Zr_(2)O_(7)ceramics exhibit excellent phase stability during thermal treatment at 1200℃for 100 h and 1400℃for 50 h.The thermal conductivities of dense(Sm_(1-x)Er_(x))_(2)Zr_(2)O_(7)ceramics range from 1.52 to 1.59 W/(m·K),which is lower than that of Sm_(2)Zr_(2)O_(7),and decrease as the Er2O3content increases.Besides,the thermal expansion coefficient of(Sm_(1-x)Er_(x))_(2)Zr_(2)O_(7)is higher than that of Sm_(2)Zr_(2)O_(7).展开更多
Background:Chimeric antigen receptor T(CAR-T)therapy has substantially revolutionized the clinical outcomes of patients with hematologic malignan-cies,but the cancer-intrinsic mechanisms underlying resistance to CAR-T...Background:Chimeric antigen receptor T(CAR-T)therapy has substantially revolutionized the clinical outcomes of patients with hematologic malignan-cies,but the cancer-intrinsic mechanisms underlying resistance to CAR-T cells remain yet to be fully understood.This study aims to explore the molecular deter-minants of cancer cell sensitivity to CAR-T cell-mediated killing and to provide a better understanding of the underlying mechanisms and potential modulation to improve clinical efficacy.Methods:The human whole-genome CRISPR/Cas9-based knockout screening was conducted to identify key genes that enable cancer cells to evade CD19 CAR-T-cell-mediated killing.The in vitro cytotoxicity assays and evaluation of tumor tissue and bone marrow specimens were further conducted to confirm the role of the key genes in cancer cell susceptibility to CAR-T cells.In addition,the specific mechanisms influencing CAR-T cell-mediated cancer clearance were elucidated in mouse and cellular models.Results:The CRISPR/Cas9-based knockout screening showed that the enrich-ment of autophagy-related genes(ATG3,BECN1,and RB1CC1)provided protec-tion of cancer cells from CD19 CAR-T cell-mediated cytotoxicity.These findings were further validated by in vitro cytotoxicity assays in cells with genetic and pharmacological inhibition of autophagy.Notably,higher expression of the three autophagy-related proteins in tumor samples was correlated with poorer respon-siveness and worse survival in patients with relapsed/refractory B-cell lymphoma after CD19 CAR-T therapy.Bulk RNA sequencing analysis of bone marrow samples from B-cell leukemia patients also suggested the clinical relevance of autophagy to the therapeutic response and relapse after CD19 CAR-T cell ther-apy.Pharmacological inhibition of autophagy and knockout of RB1CC1 could dramatically sensitize tumor cells to CD19 CAR-T cell-mediated killing in mouse models of both B-cell leukemia and lymphoma.Moreover,our study revealed that cancer-intrinsic autophagy mediates evasion of CAR-T cells via the TNF-α-TNFR1 axis-mediated apoptosis and STAT1/IRF1-induced chemokine signaling activation.Conclusions:These findings confirm that autophagy signaling in B-cell malig-nancies is essential for the effective cytotoxic function of CAR-T cells and thereby pave the way for the development of autophagy-targeting strategies to improve the clinical efficacy of CAR-T cell immunotherapy.展开更多

文摘Objective To systematically analyze the current status of outcomes reporting in clinical trials on treating stasis acute mastitis with Traditional Chinese Medicine breast massage.Methods We searched CNKI,Wanfang,VIP,Sino Med,Pub Med,Web of Science,Embase,Cochrane library,JBI,CINAHL,Psyc INFO,Clinical Trials Registry Platform portal,Clinical Trials Registry,Australian New Zealand Clinical Trials Registry,Center Watch Registry from inception to May 15,2022 to find randomized controlled trials,non-randomized controlled trials,case series and cohort studies which reported the outcomes of stasis acute mastitis managed with Traditional Chinese Medicine breast massage,with search terms of mastitis,acute mastitis,lactation mastitis,puerperal mastitis,breast problem,breast engorgement,milk stasis,blocked ducked,breast pain,breast massage,and acupoint massage.Outcomes and the measurement schemes(measurement methods,timing of assessing outcome,frequency of assessing outcome,measurers)were extracted from the included studies.We used the Management of Otitis Media with Effusion in Children with Cleft Palate(MOMENT)to assess the quality of each study,then categorized outcomes derived from the included studies into different domains according to the Outcome Measures in Rheumatology Arthritis Clinic Trials(OMERACT)Filter 2.1 framework.Results We identified 85 clinical trials,in which 54 different outcomes were reported.A total of 81.2%(69/85)of studies were assessed as medium quality with a mean score of 2.6,and 18.8%(16/85)as low quality with a mean score of 0.9.These outcomes were organized in three core areas.Lump size(89.4%,76/85)was the most frequently reported outcome,followed by breast pain(69.4%,59/85)and milk excretion(68.2%,58/85).Five methods were used to assess lump size and four methods to assess breast pain.Conclusions The outcomes reported in clinical trials regarding stasis acute mastitis treated by Traditional Chinese Medicine breast massage are heterogeneous.Developing a core outcome set to achieve consistent standards for reporting outcomes and modalities for validation of the outcomes is clearly warranted.

文摘Objective:The objective of this study is to explore the current situation,clinical research hot spots,and trends of traditional Chinese medicine(TCM)nursing technology in rheumatoid arthritis(RA),to draw a panorama of clinical research,to provide the basis and clues for subsequent high-level evidence integration and further in-depth research.Materials and Methods:Seven databases including China National Knowledge Infrastructure,Chinese Scientific Journal Database(VIP),Wanfang Database,Chinese Biomedical Literature Database(Sino-Med),PubMed,Embase,and the Cochrane Library were searched.The bibliometric method and visualization software CiteSpace were used to conduct a multi-dimensional analysis of the included literature.Results:A total of 805 pieces of literature were included(of them,one was written in German and four in English),and the number of published literature showed an increasing trend year by year.There were only 30(3.73%)pieces of literature published in nursing journals.The hot spots of the co-occurrence map were concentrated in:(1)the nursing of fumigation in RA;(2)the efficacy evaluation of TCM nursing technology on pain,joint malformation,and joint dysfunction caused by RA.The included literature themes focused on five TCM technologies:fumigation,moxibustion,acupoint patching,acupoint injection,and Chinese herbal soaking.The beginning year and strength in TCM fumigation were 2012 and 5.2,and those in moxibustion were 2011 and 3.38.Conclusion:The related studies are on the rise.It has entered the field of international readers with clear research hot spots.However,there are still shortcomings such as little literature published in Chinese nursing core journals,few non-Chinese-related documents,and a lack of international exchanges and cooperation.The current research hot spots in this field are TCM fumigation,and the cutting-edge trend of future research may be fumigation and moxibustion technology.It is suggested that further research can focus on evidence integration and original research on the self-optimization of these two techniques.
文摘Background:Traditional Chinese medicine nursing(TCM nursing)is one of the highlights of nursing profession in China,which plays a significant role in Chinese healthcare system.However,bottlenecks exist in TCM nursing development.It’s necessary to explore the discipline connotation and advantages of TCM nursing in nursing education and clinical practice and introduce modern nursing concepts.Methods:We analysis the connotations of TCM nursing,the bottlenecks it faces,and strategies to facilitate TCM nursing development.Results:Qualitative research method and evidence-based nursing concepts are key strategies to solve the dilemma TCM nursing confronts with and are quite essential in TCM nursing development.Conclusion:We should embed qualitative research method and evidence-based nursing concepts into TCM nursing advancement so as to facilitate the advancement of TCM nursing and promote its globalization.

文摘Objective To compare the efficacy of Chinese herbal medicine (therapy A) or Chinese herbal medicine plus indomethacin (therapy B) with that of indomethacin alone (therapy C) in treating polyhydramnios.Methods Literatures published up to April 2012 were retrieved from PubMed,Embase and Cochrane library,Chinese National Knowledge Infrastructure (CNKI),Chinese Scientific and Technological Periodical Database (VIP),Wangfang,and Traditional Chinese Medicine online.Two researchers collected data independently.The assessment of methodological quality was based on Cochrane handbook and the materials were analyzed with software RevMan 5.1.2.The outcome measure index was relative risk or difference of mean value (95% confidence interval).The following outcomes were evaluated:(1) general clinical improvement rate;(2) maximum vertical pocket depth;(3) amniotic fluid index (AFI) value;(4) rate of fetal ductus arteriosus constriction;(5) incidence of adverse events.Results Based on the search strategy,5 trails involving 1017 patients were finally included.Compared with therapy C,therapy A decreased the rate of fetal ductus arteriosus constriction (P<0.01).Therapy B was more effective than therapy C in general clinical improvement and decrease of AFI for polyhydramnios (P< 0.01).No serious adverse events were reported in therapy A and therapy B.Conclusions Compared with therapy C,therapy A and therapy B may appear to be more effective for polyhydramnios.However,the exact effect needs to be confirmed with well-designed large-scale clinical trials.

文摘Objective:Methotrexate(MTX)can be safely administered to most patients but may cause severe toxicity in others.This study aimed to summarize the characteristics of high-dose methotrexate(HD-MTX)chemotherapy and to evaluate whether the modified dose-adjustment program was able to improve the maintenance of sufficient MTX exposure levels while minimizing toxicities.Methods:We evaluated 1172 cycles of high-dose MTX chemotherapy from 294 patients who were treated according to the CCCG-ALL-2015 protocol(clinical trial number:ChiCTR-IPR-14005706)and analyzed the data of actual MTX dosage,MTX concentration,toxicity,and prognosis.We compared data between the dose-adjustment Program 1(fixed 20%reduction in dose)and the dose-adjustment Program 2(dose-individualization based on reassessment of the creatine clearance rate and the MTX concentration-monitoring point at 16 h),which were applied if the MTX clearance was delayed in the previous cycle.Results:The patients who used Program 2 had higher actual MTX infusion doses and infusion rates and were able to better maintain the MTX concentration at 44 h at the established target value than those on Program 1(P<0.001).No significant differences in toxicities were found between these two programs except that abnormal serum potassium levels and prolonged myelosuppression in intermediate-risk/high-risk patients were more frequently observed in patients using Program 2(P<0.001).No significant correlations were observed between the MTX dose,dose-adjustment programs,or MTX concentrations and relapse-free survival.Conclusion:Adjusting the MTX dose using Program 2 is more efficient for maintaining sufficient MTX exposure without significantly increasing the toxicity.

文摘Objective At present,a number of very severe aplastic anemia(VSAA)patients cannot receive hematopoietic stem cell transplantation(HSCT)or standard immunosuppressive therapy(IST)due to the high cost of therapy,shortage of sibling donors,and lack of resources to support the HSCT.In addition,some VSAA patients with autoantibodies have no life-threatening infections or bleeding at the time of initial diagnosis.Considering the disease condition,economics and other factors,the present study designed a new and relatively mild treatment strategy:cyclosporine A plus pulsed high-dose prednisone(CsA+HDP).Methods The present study retrospectively analyzed 11 VSAA patients,who were treated with CsA+HDP in our hospital from August 2017 to August 2019.Results The median follow-up time for these patients was 24.9 months.The overall response rate was 54.5%(6/11)at six months after the initiation of IST and 81.8%(9/11)at deadline.Five patients achieved complete remission and four patients met the criteria for partial response at the last follow-up.The median time to response for responders was 110 days.Three patients underwent HSCT due to the poor effect of CsA+HDP or to find a suitable transplant donor.Recurrence and clonal evolution were not found in any of these patients.The estimated 3-year overall survival rate and 3-year failure-free survival rate were 100.0%and 72.7%,respectively.In addition,the results revealed that the cyclosporine-prednisone-associated toxicity was mild and well-tolerated by most patients.Conclusion The novel CsA+HDP regimen has good therapeutic effect and safety for VSAA patients with autoantibodies,who have no serious life-threatening infections or bleeding at the time of initial diagnosis.

文摘Reference-frame-independent quantum key distribution(RFI-QKD)can allow a quantum key distribution system to obtain the ideal key rate and transmission distance without reference system calibration,which has attracted much attention.Here,we propose an RFI-QKD protocol based on wavelength division multiplexing(WDM)considering finite-key analysis and crosstalk.The finite-key bound for RFI-QKD with decoy states is derived under the crosstalk of WDM.The resulting secret key rate of RFI-QKD,which is more rigorous,is obtained.Simulation results reveal that the secret key rate of RFI-QKD based on WDM is affected by the multiplexing channel number,as well as crosstalk between adjacent channels.

文摘We propose a fully symmetrical QKD system that enables quantum states to be prepared and measured simultaneously without compromising system performance.Over a 25.6 km fiber channel,we demonstrate point-to-point QKD operations with asymmetric Mach–Zehnder interferometer modules.Two interference visibilities of above99%indicate that the proposed system has excellent stability.Consequently,the scheme not only improves the feasibility of distributing secret keys,but also enables QKD closer to more practical applications.

文摘Objective:Lactation mastitis is a kind of disease with high incidence during puerperium.In the early stage,breast massage can effectively eliminate the symptoms.However,the value of breast massage is not fully known to the public,which restricts its promotion and popularization.One important reason is the absence of a multi-dimensional and overall evaluation of outcome.COS is strongly advocated internationally as the outcome in clinical research to promote the transformation of clinical evidence into practice.At present,there is no systematic review of the existing outcome.This protocol aims to provide a set of outcomes and outcome evaluation schemes for developing COS of breast massage in lactation mastitis early stage.Methods:We will search PubMed,Embase,Web of Science,CNKI,Wan-fang,VIP,SinoMed,Grey literature databases(master and doctoral thesis databases and conference literature databases of CNKI,Wan-fang and SinoMed),Clinical Trials Registry Platform portal,the Clinical Trials Registry,Australian,New Zealand Clinical Trials Registry,the Center Watch Registry,before March 1,2021.We will include randomized controlled trials and non-randomized trials,cohort studies,case series,case reports.We will assess the quality of outcome reporting according to the MOMENT,classify all outcomes using the OMERACT Filter 2.0 conceptual framework.Descriptive analysis will be performed for all categorical variables using frequency and mean.Result:This protocol aims to help guide the design of systematic review to summarize the existing outcomes,and comprehensively evaluate existing outcomes,classify outcomes and provide schemes of outcome evaluation for developing COS of breast massage in the early stage of lactation mastitis.Percentage and mean will be used to describe the extracted data.Conclusion:This is the first systematic review protocol to summarize outcomes,measurement instruments,assessment times,measurement frequencies,measurers about breast massage in the early stage of lactational mastitis.

文摘Venous thromboembolism(VTE)is a complication in children with acute lymphoblastic leukemia(ALL).The Chinese Children’s Cancer Group-ALL-2015 protocol was carried out in China,and epidemiology,clinical characteristics,and risk factors associated with VTE were analyzed.We collected data on VTE in a multiinstitutional clinical study of 7640 patients with ALL diagnosed in 20 hospitals from January 2015 to December 2019.First,VTE occurred in 159(2.08%)patients,including 90(56.6%)during induction therapy and 108(67.92%)in the upper extremities.T-ALL had a 1.74-fold increased risk of VTE(95%CI 1.08–2.8,P=0.022).Septicemia,as an adverse event of ALL treatment,can significantly promote the occurrence of VTE(P<0.001).Catheter-related thrombosis(CRT)accounted for 75.47%(n=120);and,symptomatic VTE,58.49%(n=93),which was more common in patients aged 12–18 years(P=0.023),non-CRT patients(P<0.001),or patients with cerebral thrombosis(P<0.001).Of the patients with VTE treated with anticoagulation therapy(n=147),4.08%(n=6)had bleeding.The VTE recurrence rate was 5.03%(n=8).Patients with VTE treated by non-ultrasoundguided venous cannulation(P=0.02),with residual thrombus(P=0.006),or with short anticoagulation period(P=0.026)had high recurrence rates.Thus,preventing repeated venous puncture and appropriately prolonged anticoagulation time can reduce the risk of VTE recurrence.
文摘Background Chemotherapy and hematopoietic stem cell transplantation(HSCT)can damage the immune system,and may result in a loss of protection from infectious diseases.This study aimed to evaluate the impact of these treatments on the decrease in antibody titers of the measles,mumps,and rubella(MMR)vaccine and seroconversion post-revaccination of MMR.Methods After completion of treatment for primary diseases,participants received an MMR revaccination.Antibody titers for MMR before revaccination were analyzed for all 110 children.After revaccination,68 participants received a follow-up evaluation of antibody titer and adverse reaction.Results Multivariable analysis showed that therapeutic schedules were the only factor correlated with lack of antibody titers for measles after completing treatment(P=0.008),while for mumps and rubella,no statistically significant difference was observed.Importantly,our study clearly demonstrated positive seroconversion rates for measles(97.5%),mumps(81.0%),and rubella(93.2%),with antibody levels rising across the board and peaking at around 6 months following revaccination.However,6 months after revaccination,a downtrend of antibody titer levels was observed,which is comparatively earlier than the waning immunity observed in healthy children.Furthermore,we found MMR revaccination to be safe,with only a single adverse reaction(local pain at the injection site)reported.Conclusions MMR revaccination is immunogenic for the population.We suggest periodic monitoring of antibody titers,in addition to a booster vaccination,although the optimal timing of booster vaccination remains to be investigated further.

文摘The influences of equimolar substitution of yttria by gadolinia on the phase constituent and stability for Y_2O_3—Ta_2O_5—ZrO_2 ceramics have been investigated.The ceramics with the Gd content lower than 8 nnol%exhibit pure tetragonal phase as well as high tetragonal phase stability.However,the high Gd containing ceramics consist of t-ZrO_2,m-ZrO_2 and monoclinic GdTaO_4 and show complicated phase evolution behaviors.The fractions of monoclinic ZrO_2 and GdTaO_4 increase with increasing Gd content,indicating that the excessive substitution of Gd for Y results in a reduction in the tetragonal phase stability.In addition,the lower Gd containing ceramics show an almost invariable tetragonality,while the higher Gd containing ceramics reveal a composition-dependent tetragonality.Accordingly,an association of the tetragonal phase stability and tetragonality with the Gd concentration is established,which provides us a clue to understand the phase stability of these ceramics.

文摘Reference frame independent and measurement device independent quantum key distribution(RFI-MDI-QKD)has the advantages of being immune to detector side loopholes and misalignment of the reference frame.However,several former related research works are based on the unrealistic assumption of perfect source preparation.In this paper,we merge a loss-tolerant method into RFI-MDI-QKD to consider source flaws into key rate estimation and compare it with quantum coin method.Based on a reliable experimental scheme,the joint influence of both source flaws and reference frame misalignment is discussed with consideration of the finite-key effect.The results show that the loss-tolerant RFI-MDI-QKD protocol can reach longer key rate performance while considering the existence of source flaws in a real-world implementation.

文摘Background: Acupoint herbal patching(AHP) alone or as an adjuvant therapy with conventional treatment(CT) has been widely used for prevention and treatment of chronic obstructive pulmonary disease(COPD). However, current clinical evidence from a systematic review of RCTs is lacking.Objective: To evaluate the effectiveness and safety of AHP with or without CT for people with COPD at stable stage.Methods: We searched randomized controlled trials comparing AHP(with or without CT) with no intervention, placebo, or CT from six databases. Two authors selected studies, extracted data and evaluated risk of bias of included trials. Rev Man 5.2 software was used to analysis data.Results: Twenty one RCTs(2327 participants) were included. AHP of non sanfu applied on no fixed dates with CT significantly decreased the mean frequency of acute exacerbation of COPD(times per year)(MD:-1.24; 95% CI:-2.02 to-0.46; 2 trials), and improved lung function parameters and quality of life. The AHP with CT showed no better effect in 6-minute walking distance(6 MWD) that CT alone. AHP applied at sanfu(specific dates based on lunar calendar) with CT had significant effect for 6 MWD(MD: 11.20; 95% CI: 0.83 to 21.56; I^2= 0%;3 trials). One trial reported skin irritation from AHP. Another trial reported two patients had eye discomfort, which was inferred as the adverse effects of seretide.Conclusion: AHP used as an adjunct to CT, appears to be more effective than CT alone in patients with stable COPD. However, further large,rigorously designed trials are warranted to confirm these potential effects.

文摘Rare earth Er^(3+)doped(Sm_(1-x)Er_(x))_(2)Zr_(2)O_(7)(x=0.1,0.2,and 0.3)ceramic samples were synthesized using a solid state reaction method.The microstructure and thermal properties of these ceramics were investigated to evaluate their potential as thermal barrier coating materials.The results show that ceramics are compact with regular-shaped grains of 1-5μm size.(Sm_(1-x)Er_(x))_(2)Zr_(2)O_(7)has a pyrochlore structure mainly determined by ionic radius ratio,but the ordering degree decreases with increase of the Er_(2)O_(3)content.There is no phase transformation from 1000 to 1200℃,and the(Sm_(1-x)Er_(x))_(2)Zr_(2)O_(7)ceramics exhibit excellent phase stability during thermal treatment at 1200℃for 100 h and 1400℃for 50 h.The thermal conductivities of dense(Sm_(1-x)Er_(x))_(2)Zr_(2)O_(7)ceramics range from 1.52 to 1.59 W/(m·K),which is lower than that of Sm_(2)Zr_(2)O_(7),and decrease as the Er2O3content increases.Besides,the thermal expansion coefficient of(Sm_(1-x)Er_(x))_(2)Zr_(2)O_(7)is higher than that of Sm_(2)Zr_(2)O_(7).

文摘Background:Chimeric antigen receptor T(CAR-T)therapy has substantially revolutionized the clinical outcomes of patients with hematologic malignan-cies,but the cancer-intrinsic mechanisms underlying resistance to CAR-T cells remain yet to be fully understood.This study aims to explore the molecular deter-minants of cancer cell sensitivity to CAR-T cell-mediated killing and to provide a better understanding of the underlying mechanisms and potential modulation to improve clinical efficacy.Methods:The human whole-genome CRISPR/Cas9-based knockout screening was conducted to identify key genes that enable cancer cells to evade CD19 CAR-T-cell-mediated killing.The in vitro cytotoxicity assays and evaluation of tumor tissue and bone marrow specimens were further conducted to confirm the role of the key genes in cancer cell susceptibility to CAR-T cells.In addition,the specific mechanisms influencing CAR-T cell-mediated cancer clearance were elucidated in mouse and cellular models.Results:The CRISPR/Cas9-based knockout screening showed that the enrich-ment of autophagy-related genes(ATG3,BECN1,and RB1CC1)provided protec-tion of cancer cells from CD19 CAR-T cell-mediated cytotoxicity.These findings were further validated by in vitro cytotoxicity assays in cells with genetic and pharmacological inhibition of autophagy.Notably,higher expression of the three autophagy-related proteins in tumor samples was correlated with poorer respon-siveness and worse survival in patients with relapsed/refractory B-cell lymphoma after CD19 CAR-T therapy.Bulk RNA sequencing analysis of bone marrow samples from B-cell leukemia patients also suggested the clinical relevance of autophagy to the therapeutic response and relapse after CD19 CAR-T cell ther-apy.Pharmacological inhibition of autophagy and knockout of RB1CC1 could dramatically sensitize tumor cells to CD19 CAR-T cell-mediated killing in mouse models of both B-cell leukemia and lymphoma.Moreover,our study revealed that cancer-intrinsic autophagy mediates evasion of CAR-T cells via the TNF-α-TNFR1 axis-mediated apoptosis and STAT1/IRF1-induced chemokine signaling activation.Conclusions:These findings confirm that autophagy signaling in B-cell malig-nancies is essential for the effective cytotoxic function of CAR-T cells and thereby pave the way for the development of autophagy-targeting strategies to improve the clinical efficacy of CAR-T cell immunotherapy.
