To assess the effect of the injection duration of subcutaneous low-molecular-weight heparin(LMWH)on pain and bruising in patients.Randomized controlled trials and quasiexperimental studies were searched for in four el...To assess the effect of the injection duration of subcutaneous low-molecular-weight heparin(LMWH)on pain and bruising in patients.Randomized controlled trials and quasiexperimental studies were searched for in four electronic databases.The pooled effect size was expressed as relative risk(RR)andmeandifference(MD)with95%confidence intervals(CI)for dichotomous and continuous data.Cochrane Q and p value were used to assess heterogeneity and the I2 statistic was adopted to quantify the level.Finally,eight studies involving a total of 532 participants met our inclusion criteria.The slow(30 second)injection was associated with a reduction in pain intensity and duration,and lower bruising occurrence at 48-72 hours and 48 hours post injection.The bruising area was also smaller at 48 hours and 60 hours post injection.Nodifferenceswere identified betweenthe slowand fast(10 second)injectionin bruising area and bruising occurrence at 24 hours and 60 hours post injection.With present evidences,slow injection of LMWH is beneficial to the patient's well being,but further studies to identify the feasibility and standardization of the technique is recommended.展开更多

Methods: Eighteen databases were searched for randomized controlled trials, focusing on evaluation of fast track surgery care on knee joint function in patients with knee joint replacement surgery. Two people independently screened literatures and extracted data according to the inclusion criteria. Then the available data were analyzed with RevMan software (version 5.3.0). Descriptive techniques were performed when no available data could be used. Results: A total of 8 studies were included, for a total sample of 635 patients. The results showed that fast track surgery care can improve Hospital for Special Surgery Knee score at 2 months, 3 months, 6 months, and 1 years after operation [after 2 months (weighted mean difference (WMD), 15.2;95%CI, 13.58 to 16.82;P < 0.01);after 3 months (WMD, 9.42;95%CI, 7.80 to 11.04;P < 0.01);after 6 months,(WMD, 11.01;95%CI, 9.73 to 12.28;P < 0.01);after 1 years (WMD, 10.78;95%CI, 9.33 to 12.24;P < 0.01)]. Fast track surgery care can improve the knee score of American Knee Society Knee Score(WMD, 9.23;95%CI,0.86 - 17.6;P < 0.05), and reduce the incidence of postoperative deep vein thrombosis, constipation, pulmonary infection, urinary tract infection and other complications. Conclusion: Fast track surgery care can promote knee joint function recovery, reduce postoperative deep venous thrombosis, constipation, pulmonary infection and urinary tract infection. However, rigorous large sample and multicenter randomized controlled trials are needed for further validation.展开更多

BICOMB 2.0 and SPSS 20.0 software were used to analyze high-frequency keywords and conduct co-word clustering analysis. Results: We searched for totally 2353 articles related to our topic and extracted 19 high-frequency keywords (27.50%). Five research focuses were concluded, including: study on nursing safety culture; team work to promote nursing safety; practice of nursing safety management; workplace violence against nursing staffs; nursing safety and quality evaluation standard. Conclusion: Analysis of the hotspots of nursing safety management in the past 10 years will contribute to understanding the research emphases and trend of development, and provide reference for the study and practice of nursing safety management.展开更多

Background:Honey dressing has been applied to clinical practice for many types of disease for centuries.Many researchers have studied the effects of honey dressing for the treatment of DFUs(diabetic foot ulcers),and n...Background:Honey dressing has been applied to clinical practice for many types of disease for centuries.Many researchers have studied the effects of honey dressing for the treatment of DFUs(diabetic foot ulcers),and no systematic review has considered effects of honey dressing on DFUs.A systematic review performed to objectively evaluate the effectiveness of honey dressing in the treatment of DFUs.Methods:We include all original studies found for the key words honey and diabetic foot ulcers.Mean effect sizes and confidence intervals are pooled from study effect sizes according to standard methods,and these are considered for various common types of honey dressing interventions separately.Results:A total of 4 RCTs involving 258 participants were included,and 3 trails involving 228 participants met the quantitative analysis and 1 study involving 30 participants met qualitative analysis.Results of meta and descriptive analyses showed that total treatment time,Mean purge time of ulcers,ratio of purging germ,healed area of ulcers in honey dressing group are better than that of control group,respectively,and with statistically significant differences.Conclusions:Honey dressing was superior of traditional dressing for treatment of DFUs.Due to limitations in the quantity of published studies,this conclusion has yet to be carried out in large,multicenter study to validate.展开更多

Objective:To systematically evaluate the therapeutic effects of inspiratory muscle training(IMT)on cardiopulmonary functions in patients with heart failure.Methods:PubMed,EMbase,the Cochrane Library,Web of Science,CBM...Objective:To systematically evaluate the therapeutic effects of inspiratory muscle training(IMT)on cardiopulmonary functions in patients with heart failure.Methods:PubMed,EMbase,the Cochrane Library,Web of Science,CBM,WanFang Data,VIP,and CNKI databases were searched to collect randomized controlled trials about the effects of IMT on cardiopulmonary function in patients with heart failure.Meta-analysis was performed using RevMan 5.3 software after two researchers independently screened the literatures,extracted the data,and evaluated the risk of bias.Results:A total of 20 articles with 1,415 patients were included.Meta-analysis showed that IMT was able to increase the 6-minute walking distance(mean difference(MD)=59.41,95%confidence interval(CI)(51.02–67.80),P<0.001)and maximum inspiratory pressure(MD=15.59,95%CI(12.96–18.21),P<0.001)in patients with heart failure compared with the control group.But there was no statistical difference in peak oxygen uptake(MD=1.37,95%CI(?0.57–3.30),P=0.17),first second forced expiratory volume(MD=?5.79,95%CI(?12.23–0.65),P=0.08)and forced vital capacity(MD=?0.45,95%CI(?6.39–5.49),P=0.88)between the control and the experimental group.Conclusion:Available evidence suggested that IMT seemed to be a useful strategy for improving exercise capacity and inspiratory muscle strength in heart failure patients.Limited by the quantity and quality of the included studies,the above conclusions need to be verified by more high-quality studies.展开更多

Managed care requires the incorporation of information pertaining to patient evaluation, treatment planning, referrals, and follow-up care to ensure that payment for services is received and that care is ongoing and comprehensive. The objective of this review was to assess the efficacy of case management in patients with type 2 diabetes mellitus with respect to outcomes such as glycosylated hemoglobin (HbA1c), systolic blood pressure (SBP), diastolic blood pressure (DBP), and low-density lipoprotein (LDL). Methods: Databases including PubMed, Embase, Web of Science, the Cochrane Library, the China Na- tional Knowledge Infrastructure (CNKI), VIP, Wan Fang and the Chinese Biomedical Literature Database (CBM) were searched for randomized controlled trials (RCTs) dating as late as Jan, 2015, Reference sections of the included studies were also searched. Results: Twelve studies, involving 11 RCTs that evaluated a total of 4000 patients, were included in this analysis. Two of the 12 studies evaluated the same RCT. Seven of the 12 studies reported HbAlc as an outcome, and three trials reported changes in SBP, DBP and LDL levels as outcomes. The pooled results indicated that statistically significant improvements in HbAlc (MD = -0.35, 95% CI (-0.68, -0,02), P = 0.04) and LDL levels (MD = -2,49, 95% CI (-4.04, -0.93), P - 0.002) were associated with the case management group compared with control group; however, no statistically significant differences in DBP (MD = -0.08, 95% CI (-0.68, 0.52), P = 0.8) and SBP (MD = -0.96, 95% CI ( 5.77, 3.84), P = 0.69) were observed. Conclusions: Case management was effective in improving HbA1c and LDL levels in patients with type 2 diabetes mellitus. Although no statistically significant differences in DBP and SBP between the case management group and the control group were observed, further research is required to draw a conclusion about the effect of managed care on these outcomes. Based on this meta-analysis of clinical trials, we conclude that case management offers an effective clinical method for the treatment of type 2 diabetes.展开更多

The objective of this meta-analysis was to assess the effect of physical exercise on patients with dementia. Methods: The Pubmed, EMbase, Web of Science, Cochrane Library, China National Knowledge Infra- structure (CNK1), V1P and Wan Fang databases were searched for randomized controlled trials (RCTs) through July 2016. References of eligible citations were also searched by hand. Articles were screened and data were extracted. The methodological quality was independently assessed by two reviewers. Then, a meta-analysis was performed by RevMan 5.3. Results: A total of 14 RCTs, involving 1546 participants, were included. The pooled results suggested that physical exercise effectively improved the Time up and go (seconds) [MD = -2.87, 95%U (-3.25, -2.5)], Function Reach (cm) [MD = 4.25, 95%CI (3.52, 4.98)], Cadence, steps]min [MD = 12.71, 95%U (6.92, 18.51)], NPl-caregiver total score [MD - -2.33, 95%C/( 3.65, 1.01)] and Berg Balance Scale [MD - 3.62, 95%Cl (1.51, 5.73)1. Conclusions: Physical activity training effectively improved the condition of dementia patients. Physical activity training effectively improved the condition of Time up and go (seconds), NPl-caregiver total score, Function Reach (cm), Cadence, steps/min and Berg Balance Scale. 文摘Background:Honey dressing has been applied to clinical practice for many types of disease for centuries.Many researchers have studied the effects of honey dressing for the treatment of DFUs(diabetic foot ulcers),and no systematic review has considered effects of honey dressing on DFUs.A systematic review performed to objectively evaluate the effectiveness of honey dressing in the treatment of DFUs.Methods:We include all original studies found for the key words honey and diabetic foot ulcers.Mean effect sizes and confidence intervals are pooled from study effect sizes according to standard methods,and these are considered for various common types of honey dressing interventions separately.Results:A total of 4 RCTs involving 258 participants were included,and 3 trails involving 228 participants met the quantitative analysis and 1 study involving 30 participants met qualitative analysis.Results of meta and descriptive analyses showed that total treatment time,Mean purge time of ulcers,ratio of purging germ,healed area of ulcers in honey dressing group are better than that of control group,respectively,and with statistically significant differences.Conclusions:Honey dressing was superior of traditional dressing for treatment of DFUs.Due to limitations in the quantity of published studies,this conclusion has yet to be carried out in large,multicenter study to validate.
文摘Dear Editor,We read with great interest the systematic review and meta-analysis publishedby Yanget al.[1],assessing the impact of various mindfulness-based therapies(MBTs),including mindfulness stress reduction(MBSR)and mindfulness-based cognitive therapy(MBCT),on patients with HIV/AIDS[1].While we congratulate the authors for investigating this important topic,several issues pertaining to this study require further discussion.
文摘Neonatal pain management is an important issue which should have great attention.More and more researches have proved that neonates can feel pain when undergoes painful procedures such as vaccination,heel stick,and so on,and it will result in shortterm and long-term outcomes.So it is very important to manage neonatal pain.This article summarized some non-pharmaceutical interventions,including sucrose or glucose,non-nutritional sucking(NNS),breastfeeding,facilitated tucking(FT),kangaroo mother care(KMC),swaddling,heel warming,sensorial saturation(SS),and music therapy,which showed obvious effects for neonatal pain.In addition,this article summarized the progress of neonatal pain intervention in various countries and showed that many countries have not paid enough attention to this problem,while some countries have carried out promotion programs for neonatal pain management which give some clinical enlightenment to our country that we need to pay more attention to this problem.
文摘Objective:To systematically evaluate the therapeutic effects of inspiratory muscle training(IMT)on cardiopulmonary functions in patients with heart failure.Methods:PubMed,EMbase,the Cochrane Library,Web of Science,CBM,WanFang Data,VIP,and CNKI databases were searched to collect randomized controlled trials about the effects of IMT on cardiopulmonary function in patients with heart failure.Meta-analysis was performed using RevMan 5.3 software after two researchers independently screened the literatures,extracted the data,and evaluated the risk of bias.Results:A total of 20 articles with 1,415 patients were included.Meta-analysis showed that IMT was able to increase the 6-minute walking distance(mean difference(MD)=59.41,95%confidence interval(CI)(51.02–67.80),P<0.001)and maximum inspiratory pressure(MD=15.59,95%CI(12.96–18.21),P<0.001)in patients with heart failure compared with the control group.But there was no statistical difference in peak oxygen uptake(MD=1.37,95%CI(?0.57–3.30),P=0.17),first second forced expiratory volume(MD=?5.79,95%CI(?12.23–0.65),P=0.08)and forced vital capacity(MD=?0.45,95%CI(?6.39–5.49),P=0.88)between the control and the experimental group.Conclusion:Available evidence suggested that IMT seemed to be a useful strategy for improving exercise capacity and inspiratory muscle strength in heart failure patients.Limited by the quantity and quality of the included studies,the above conclusions need to be verified by more high-quality studies.

文摘Background: Case management is a term used to describe the activities performed by a physician or other health care professional to ensure the coordination of medical services required by a patient. Managed care requires the incorporation of information pertaining to patient evaluation, treatment planning, referrals, and follow-up care to ensure that payment for services is received and that care is ongoing and comprehensive. The objective of this review was to assess the efficacy of case management in patients with type 2 diabetes mellitus with respect to outcomes such as glycosylated hemoglobin (HbA1c), systolic blood pressure (SBP), diastolic blood pressure (DBP), and low-density lipoprotein (LDL). Methods: Databases including PubMed, Embase, Web of Science, the Cochrane Library, the China Na- tional Knowledge Infrastructure (CNKI), VIP, Wan Fang and the Chinese Biomedical Literature Database (CBM) were searched for randomized controlled trials (RCTs) dating as late as Jan, 2015, Reference sections of the included studies were also searched. Results: Twelve studies, involving 11 RCTs that evaluated a total of 4000 patients, were included in this analysis. Two of the 12 studies evaluated the same RCT. Seven of the 12 studies reported HbAlc as an outcome, and three trials reported changes in SBP, DBP and LDL levels as outcomes. The pooled results indicated that statistically significant improvements in HbAlc (MD = -0.35, 95% CI (-0.68, -0,02), P = 0.04) and LDL levels (MD = -2,49, 95% CI (-4.04, -0.93), P - 0.002) were associated with the case management group compared with control group; however, no statistically significant differences in DBP (MD = -0.08, 95% CI (-0.68, 0.52), P = 0.8) and SBP (MD = -0.96, 95% CI ( 5.77, 3.84), P = 0.69) were observed. Conclusions: Case management was effective in improving HbA1c and LDL levels in patients with type 2 diabetes mellitus. Although no statistically significant differences in DBP and SBP between the case management group and the control group were observed, further research is required to draw a conclusion about the effect of managed care on these outcomes. Based on this meta-analysis of clinical trials, we conclude that case management offers an effective clinical method for the treatment of type 2 diabetes.
文摘Background: Activity in a favorable environment excites the brain on a bodily and cognitive level and is beneficial for brain plasticity in dementia patients. Although dementia is an incurable disease, its pro- gression can be slowed with exercise and some patient functionality may be improved through physical exercise. The objective of this meta-analysis was to assess the effect of physical exercise on patients with dementia. Methods: The Pubmed, EMbase, Web of Science, Cochrane Library, China National Knowledge Infra- structure (CNK1), V1P and Wan Fang databases were searched for randomized controlled trials (RCTs) through July 2016. References of eligible citations were also searched by hand. Articles were screened and data were extracted. The methodological quality was independently assessed by two reviewers. Then, a meta-analysis was performed by RevMan 5.3. Results: A total of 14 RCTs, involving 1546 participants, were included. The pooled results suggested that physical exercise effectively improved the Time up and go (seconds) [MD = -2.87, 95%U (-3.25, -2.5)], Function Reach (cm) [MD = 4.25, 95%CI (3.52, 4.98)], Cadence, steps]min [MD = 12.71, 95%U (6.92, 18.51)], NPl-caregiver total score [MD - -2.33, 95%C/( 3.65, 1.01)] and Berg Balance Scale [MD - 3.62, 95%Cl (1.51, 5.73)1. Conclusions: Physical activity training effectively improved the condition of dementia patients. Physical activity training effectively improved the condition of Time up and go (seconds), NPl-caregiver total score, Function Reach (cm), Cadence, steps/min and Berg Balance Scale. Hence, physical activity training offers many advantages for dementia patients, including improving their balance ability, athletic ability and relieve caregivers' burden.

Aged patients are especially prone to suffering from various chronic diseases, and many elderly patients select to receive home-based care. Thus, professional care in elderly patients' homes is needed, which also represents a mark of social development. Today, home health nursing is widely applied, including nursing of post labor mothers and their infants, and osteoporosis patients, among others. The aim of the present study was to explore the home health nursing model and its applicable range, as well as existing problems and the impact of the model in the domestic settings.
