BACKGROUND Cervical haemorrhage due to spontaneous rupture of a parathyroid adenoma is a rare complication that may cause life-threatening acute airway compromise.CASE SUMMARY A 64-year-old woman was admitted to the h...BACKGROUND Cervical haemorrhage due to spontaneous rupture of a parathyroid adenoma is a rare complication that may cause life-threatening acute airway compromise.CASE SUMMARY A 64-year-old woman was admitted to the hospital 1 day after the onset of right neck enlargement, local tenderness, head-turning difficulty, pharyngeal pain, and mild dyspnoea. Repeat routine blood testing showed a rapid decrease in the haemoglobin concentration, indicating active bleeding. Enhanced computed tomography images showed neck haemorrhage and a ruptured right parathyroid adenoma. The plan was to perform emergency neck exploration, haemorrhage removal, and right inferior parathyroidectomy under general anaesthesia. The patient was administered 50 mg of intravenous propofol, and the glottis was successfully visualised on video laryngoscopy. However, after the administration of a muscle relaxant, the glottis was no longer visible and the patient had a difficult airway that prevented mask ventilation and endotracheal intubation. Fortunately, an experienced anaesthesiologist successfully intubated the patient under video laryngoscopy after an emergency laryngeal mask placement. Postoperative pathology showed a parathyroid adenoma with marked bleeding and cystic changes. The patient recovered well without complications.CONCLUSION Airway management is very important in patients with cervical haemorrhage. After the administration of muscle relaxants, the loss of oropharyngeal support can cause acute airway obstruction.Therefore, muscle relaxants should be administered with caution. Anaesthesiologists should pay careful attention to airway management and have alternative airway devices and tracheotomy equipment available.展开更多

The effectiveness ...A rotating pre-twisted and inclined cantilever beam model (RPICBM) with the flapwise-chordwise-axial-torsional coupling is established with the Hamilton principle and the finite element (FE) method. The effectiveness of the model is verified via comparisons with the literatures and the FE models in ANSYS. The effects of the setting and pre-twisted angles on the dynamic responses of the RPICBM are analyzed. The results show that:(i) the increase in the setting or pre-twisted angle results in the increases in the first-order flapwise and torsional frequencies while the decrease in the first-order chordwise frequency under rotating conditions;(ii) a positive/negative setting angle leads to a positive/negative constant component, while a positive/negative pre-twisted angle leads to a negative/positive constant component;(iii) when the rotation speed is non-zero, the pre-twisted angle or non-zero setting angle will result in the coupled flapwise-chordwiseaxial- torsional vibration of the RPICBM under axial base excitation.展开更多
AIM: To conduct a comparative study of effectiveness of silicone hydrogel contact lens and hydrogel contact lens, which are used in patients after laser-assisted subepithelial keratomileusis(LASEK).·METHODS: Sixt...AIM: To conduct a comparative study of effectiveness of silicone hydrogel contact lens and hydrogel contact lens, which are used in patients after laser-assisted subepithelial keratomileusis(LASEK).·METHODS: Sixty-three patients(121 eyes) with a spherical equivalent ≤-5.0 D were chosen after undergoing LASEK in 2012 at Guangdong General Hospital. They were randomly divided into 2 groups. The silicone hydrogel group included 32 cases(61 eyes) that wore silicone hydrogel contact lenses for 4-6d after the operation, while the hydrogel group included 31 cases(60 eyes) who wore hydrogel contact lenses for 4-6d after the operation. Patients’ self-reported postoperative symptoms(including pain, photophobia, tears, and foreign body sensation) were evaluated. The healing time of the corneal epithelium, the visual acuity of patients without contact lens after epithelial healing, and the incidence of delayed corneal epithelial shedding were also assessed. The follow-up time was 1mo.·RESULTS: Postoperative symptoms were milder in the silicone hydrogel group than in the hydrogel group.There were significant differences in pain, foreign body sensation, and photophobia between the 2 groups(P <0.05),although there was no significant difference in postoperative tearing(P >0.05). The healing time of the corneal epithelium in the silicone hydrogel lens group was markedly shorter than that in the hydrogel group(4.07±0.25 vs 4.33±0.82 d, t =2.43, P =0.02). Visual acuity without contact lenses after healing of the corneal epithelium was better in the silicone hydrogel group compared with the hydrogel group(χ~2=7.76, P =0.02).There was no significant difference in the occurrence ofdelayed corneal epithelial shedding between the 2 groups(P >0.05).· CONCLUSION: Patients with LASEK using silicon hydrogel contact lenses had less discomfort and shorter corneal epithelial healing time compared with those using hydrogel contact lenses, suggesting that silicon hydrogel contact lenses may be considered to be a better choice of bandage contact lens after LASEK.展开更多

Methodology Fluorescently labeled dextrans with different molecular mass (3 kD,10 kD,40 kD,70 kD,2 000 kD) were used as a series of diffusion probes. Streptococcus mutans,Streptococcus sanguinis,Actinomyces naeslundii and Fusobacterium nucleatum were used as inocula for biofilm formation. The diffusion processes of different probes through the in vitro biofilm were recorded with a confocal laser microscope. Results Mathematical function of biofilm penetration was constructed on the basis of the inverse problem method. Based on this function,not only the relationship between average concentration of steady-state and molecule weights can be analyzed,but also that between penetrative time and molecule weights. Conclusion This can be used to predict the effective concentration and the penetrative time of anti-biofilm medicines that can diffuse through oral biofilm. Further-more,an improved model for large molecule is proposed by considering the exchange time at the upper boundary of the dental biofilm.展开更多

However, existing word segmentation methods have problems when applying to Chinese judicial documents:(1) existing methods rely on large-scale labeled data which is typically unavailable in judicial documents, and (2) judicial document has its own language features and writing formats. In this paper, a word segmentation method is proposed for Chinese judicial documents. The proposed method consists of two steps:(1) automatically generating some labeled data as legal dictionaries, and (2) applying a hybrid multilayer neural networks to do word segmentation incorporating legal dictionaries. Experiments are conducted on a dataset of Chinese judicial documents showing that the proposed model can achieve better results than the existing methods.展开更多

BACKGROUND Although external penetrating laryngeal trauma is rare in the clinic,such cases often result in a high mortality rate.The early recognition of injury,protection of the airway,one-stage laryngeal reconstruct...BACKGROUND Although external penetrating laryngeal trauma is rare in the clinic,such cases often result in a high mortality rate.The early recognition of injury,protection of the airway,one-stage laryngeal reconstruction with miniplates and interdisciplinary cooperation are important in the treatment of such patients.CASE SUMMARY A 58-year-old male worker sustained a penetrating injury in the left neck.After computed tomography scanning at a local hospital,he was transferred to our hospital,where he underwent tracheotomy,neck exploration,extraction of the foreign object,debridement and repair of the thyroid cartilage using titanium miniplates.An endo laryngeal stent was inserted,which was removed 12 days later.The patient recovered well and his voice rapidly improved after surgery.CONCLUSION Penetrating laryngeal trauma is uncommon.We successfully treated a patient with early laryngeal reconstruction and management by interdisciplinary cooperation.展开更多
There exists a lot of research on the nonlinear vibration of the pipeline system with different boundary conditions.To the best of our knowledge,little research on the actual constraint of the clamp has been performed...There exists a lot of research on the nonlinear vibration of the pipeline system with different boundary conditions.To the best of our knowledge,little research on the actual constraint of the clamp has been performed.In this paper,according to hysteresis loops of the clamp obtained from experimental test,the simplified bilinear stiffness and damping model is proposed.Then the Finite Element(FE)model of L-type pipeline system with clamps is established using Timoshenko beam theory in combination with aforementioned stiffness-damping model.Both hammering and shaker tests verify the FE model via the comparisons of natural frequencies and vibration responses.The results show that the maximum errors of natural frequencies and vibration responses are about 8.31%and 17.6%,respectively.The proposed model can simulate the dynamic characteristics of the L-type pipeline system with clamps well,which is helpful to provide some guidance for the early design stage of pipeline in aero-engine.展开更多

文摘This paper introduced the biological characteristics and medicinal value of medlar(Lycium chinense Miller).It studied Chinese wolfberry industry and its agricultural intellectual property resources in Xinjiang.In addition,it analyzed main problems such as the lag in the construction of authentic medicinal materials bases and insufficient deep processing of related products.Finally,it proposed eight recommendations including cultivating biological species with more medicinal value and creating an advantageous area of characteristic authentic medicinal materials.

文摘The development of diversified foods such as melons under the big food concept can reduce the pressure on staple food grain.Xinjiang has the advantages of building a new development pattern of dual circulation of diversified food industry and establishing a national diversified food safety and high-quality assurance center.The construction of an agricultural power needs the strategic support of an intellectual property power.This paper introduced and studied Xinjiang s melon industry and its agricultural intellectual property resources,analyzed the six main problems,including many idle land resources have not been developed into melon fields and the role of melons in diversified food supply systems not well played.Finally,it proposed to vigorously develop diversified food industries such as melons in Xinjiang and establish an eight-point strategy such as national diversified food safety and high-quality assurance center.

文摘As one of the most important parts in the engine,the structure and state of the rotating blade directly affect the normal performance of the aeroengine.In order to monitor engine crack failure and ensure flight safety,it is necessary to carry out research on the dynamic modeling of the cracked blade and breathing crack-induced vibration mechanisms.This paper summarizes the current research status on the dynamics of cracked blade,and the related topics mainly include four aspects:crack propagation path,mechanical model of open and breathing cracks,dynamic modeling methods of cracked blades such as lumped mass model,semi-analytical model and finite element model,and dynamic characteristics of cracked blades.The review will provide valuable references for future studies on dynamics and fault diagnosis of cracked blade in aeroengine.

文摘A rotating pre-twisted and inclined cantilever beam model (RPICBM) with the flapwise-chordwise-axial-torsional coupling is established with the Hamilton principle and the finite element (FE) method. The effectiveness of the model is verified via comparisons with the literatures and the FE models in ANSYS. The effects of the setting and pre-twisted angles on the dynamic responses of the RPICBM are analyzed. The results show that:(i) the increase in the setting or pre-twisted angle results in the increases in the first-order flapwise and torsional frequencies while the decrease in the first-order chordwise frequency under rotating conditions;(ii) a positive/negative setting angle leads to a positive/negative constant component, while a positive/negative pre-twisted angle leads to a negative/positive constant component;(iii) when the rotation speed is non-zero, the pre-twisted angle or non-zero setting angle will result in the coupled flapwise-chordwiseaxial- torsional vibration of the RPICBM under axial base excitation.

文摘AIM: To conduct a comparative study of effectiveness of silicone hydrogel contact lens and hydrogel contact lens, which are used in patients after laser-assisted subepithelial keratomileusis(LASEK).·METHODS: Sixty-three patients(121 eyes) with a spherical equivalent ≤-5.0 D were chosen after undergoing LASEK in 2012 at Guangdong General Hospital. They were randomly divided into 2 groups. The silicone hydrogel group included 32 cases(61 eyes) that wore silicone hydrogel contact lenses for 4-6d after the operation, while the hydrogel group included 31 cases(60 eyes) who wore hydrogel contact lenses for 4-6d after the operation. Patients’ self-reported postoperative symptoms(including pain, photophobia, tears, and foreign body sensation) were evaluated. The healing time of the corneal epithelium, the visual acuity of patients without contact lens after epithelial healing, and the incidence of delayed corneal epithelial shedding were also assessed. The follow-up time was 1mo.·RESULTS: Postoperative symptoms were milder in the silicone hydrogel group than in the hydrogel group.There were significant differences in pain, foreign body sensation, and photophobia between the 2 groups(P <0.05),although there was no significant difference in postoperative tearing(P >0.05). The healing time of the corneal epithelium in the silicone hydrogel lens group was markedly shorter than that in the hydrogel group(4.07±0.25 vs 4.33±0.82 d, t =2.43, P =0.02). Visual acuity without contact lenses after healing of the corneal epithelium was better in the silicone hydrogel group compared with the hydrogel group(χ~2=7.76, P =0.02).There was no significant difference in the occurrence ofdelayed corneal epithelial shedding between the 2 groups(P >0.05).· CONCLUSION: Patients with LASEK using silicon hydrogel contact lenses had less discomfort and shorter corneal epithelial healing time compared with those using hydrogel contact lenses, suggesting that silicon hydrogel contact lenses may be considered to be a better choice of bandage contact lens after LASEK.

文摘Aim The purpose of this study was to develop a mathe-matical model to quantitatively describe the passive trans-port of macromolecules within dental biofilms. Methodology Fluorescently labeled dextrans with different molecular mass (3 kD,10 kD,40 kD,70 kD,2 000 kD) were used as a series of diffusion probes. Streptococcus mutans,Streptococcus sanguinis,Actinomyces naeslundii and Fusobacterium nucleatum were used as inocula for biofilm formation. The diffusion processes of different probes through the in vitro biofilm were recorded with a confocal laser microscope. Results Mathematical function of biofilm penetration was constructed on the basis of the inverse problem method. Based on this function,not only the relationship between average concentration of steady-state and molecule weights can be analyzed,but also that between penetrative time and molecule weights. Conclusion This can be used to predict the effective concentration and the penetrative time of anti-biofilm medicines that can diffuse through oral biofilm. Further-more,an improved model for large molecule is proposed by considering the exchange time at the upper boundary of the dental biofilm.
文摘Word segmentation is an integral step in many knowledge discovery applications. However, existing word segmentation methods have problems when applying to Chinese judicial documents:(1) existing methods rely on large-scale labeled data which is typically unavailable in judicial documents, and (2) judicial document has its own language features and writing formats. In this paper, a word segmentation method is proposed for Chinese judicial documents. The proposed method consists of two steps:(1) automatically generating some labeled data as legal dictionaries, and (2) applying a hybrid multilayer neural networks to do word segmentation incorporating legal dictionaries. Experiments are conducted on a dataset of Chinese judicial documents showing that the proposed model can achieve better results than the existing methods.

文摘The aim of this paper is to gain insight into the nonlinear vibration feature of a dynamic model of a gas turbine.First,a rod fastening rotor-bearing coupling model with fixed-point rubbing is proposed,where the fractal theory and the finite element method are utilized.For contact analysis,a novel contact force model is introduced in this paper.Meanwhile,the Coulomb model is adopted to expound the friction characteristics.Second,the governing equations of motion of the rotor system are numerically solved,and the nonlinear dynamic characteristics are analyzed in terms of the bifurcation diagram,Poincarémap,and time history.Third,the potential effects provided by contact degree of joint interface,distribution position,and amount of contact layer are discussed in detail.Finally,the contrast analysis between the integral rotor and the rod fastening rotor is conducted under the condition of fixed-point rubbing.
文摘BACKGROUND Although external penetrating laryngeal trauma is rare in the clinic,such cases often result in a high mortality rate.The early recognition of injury,protection of the airway,one-stage laryngeal reconstruction with miniplates and interdisciplinary cooperation are important in the treatment of such patients.CASE SUMMARY A 58-year-old male worker sustained a penetrating injury in the left neck.After computed tomography scanning at a local hospital,he was transferred to our hospital,where he underwent tracheotomy,neck exploration,extraction of the foreign object,debridement and repair of the thyroid cartilage using titanium miniplates.An endo laryngeal stent was inserted,which was removed 12 days later.The patient recovered well and his voice rapidly improved after surgery.CONCLUSION Penetrating laryngeal trauma is uncommon.We successfully treated a patient with early laryngeal reconstruction and management by interdisciplinary cooperation.

文摘There exists a lot of research on the nonlinear vibration of the pipeline system with different boundary conditions.To the best of our knowledge,little research on the actual constraint of the clamp has been performed.In this paper,according to hysteresis loops of the clamp obtained from experimental test,the simplified bilinear stiffness and damping model is proposed.Then the Finite Element(FE)model of L-type pipeline system with clamps is established using Timoshenko beam theory in combination with aforementioned stiffness-damping model.Both hammering and shaker tests verify the FE model via the comparisons of natural frequencies and vibration responses.The results show that the maximum errors of natural frequencies and vibration responses are about 8.31%and 17.6%,respectively.The proposed model can simulate the dynamic characteristics of the L-type pipeline system with clamps well,which is helpful to provide some guidance for the early design stage of pipeline in aero-engine.
