BACKGROUND Pacemaker lead-induced heart perforation is a rare but life-threatening complication of pacemaker implantation,and timely diagnosis remains a challenge for clinicians.Here,we report a case of pacemaker lead...BACKGROUND Pacemaker lead-induced heart perforation is a rare but life-threatening complication of pacemaker implantation,and timely diagnosis remains a challenge for clinicians.Here,we report a case of pacemaker lead-induced cardiac perforation rapidly diagnosed by a“bow-and-arrow”sign on point-of-care ultrasound(POCUS).CASE SUMMARY A 74-year-old Chinese woman who had undergone permanent pacemaker implantation 26 d before suddenly developed severe dyspnea,chest pain,and hypotension.The patient had received emergency laparotomy for an incarcerated groin hernia and was transferred to the intensive care unit 6 d before.Computed tomography was not available due to unstable hemodynamic status,so POCUS was performed at the bedside and revealed severe pericardial effusion and cardiac tamponade.Subsequent pericardiocentesis yielded a large volume of bloody pericardial fluid.Further POCUS by an ultrasonographist revealed a unique“bow-and-arrow”sign indicating right ventricular(RV)apex perforation by the pacemaker lead,which facilitated the rapid diagnosis of lead perforation.Given the persistent drainage of pericardial bleeding,urgent off-pump open chest surgery was performed to repair the perforation.However,the patient died of shock and multiple organ dysfunction syndrome within 24 h post-surgery.In addition,we also performed a literature review on the sonographic features of RV apex perforation by lead.CONCLUSION POCUS enables the early diagnosis of pacemaker lead perforation at the bedside.A step-wise ultrasonographic approach and the“bow-and-arrow”sign on POCUS are helpful for rapid diagnosis of lead perforation.展开更多
The understanding of the weakening mechanism of tensile strength of rock subjected to cyclic wetting-drying is critical for rock engineering.Tensile strength tests were conducted on a total of 35 sandstone specimens w...The understanding of the weakening mechanism of tensile strength of rock subjected to cyclic wetting-drying is critical for rock engineering.Tensile strength tests were conducted on a total of 35 sandstone specimens with different wetting-drying cycles.The crack propagation process and acoustic emission characteristics of the tested samples were obtained through a high-speed camera and acoustic emission system.The results indicate that the tensile strength is observably reduced after cyclic wetting-drying,and the extent of the reduction is not only related to the number of wettingdrying cycle,but also closely related to the clay mineral content of the sample.In addition,as the cycles of wetting-drying increase,the effect of each single cycle on tensile strength get reduced until it becomes constant.Moreover,the crack initiation and penetration time is prolonged as the number of wetting-drying cycle increases,which indicates that cyclic wetting-drying weakens the rock stiffness and enhances the ductility of sandstone.Meanwhile,the acoustic emission characteristics of the tested samples further confirmed the ductile behaviour of the sandstone samples with increasing wetting-drying cycle.Furthermore,through the analysis of the microstructure and mineral composition of the samples with different wetting-drying cycles,it is concluded that the main weakening mechanisms of sandstones containing clay minerals are frictional reduction,chemical and corrosive deterioration.展开更多
AIM: To report the outcomes of penetrating canaloplasty for corticosteroid-induced glaucoma in a case series.METHODS: Penetrating canaloplasty is a blebindependent filtering surger y unifying canaloplasty and trabecul...AIM: To report the outcomes of penetrating canaloplasty for corticosteroid-induced glaucoma in a case series.METHODS: Penetrating canaloplasty is a blebindependent filtering surger y unifying canaloplasty and trabeculectomy. In this study, the surger y was performed to restore the natural outflow through surgically expanded Schlemm’s canal and generated trabeculum ostium. A total of 10 eyes of 8 patients were treated with penetrating canaloplasty for corticosteroid-induced glaucoma. Intraocular pressure(IOP) and the number of glaucoma medications at postoperative 3, 6, 12, 18, 24,36, and 48mo were documented as primary endpoint.Complications after the surgery were recorded as secondary endpoint.RESULTS: Penetrating canaloplasty was accomplished successfully for all 10 eyes, with a mean follow-up of 20.4±13.0mo(range 6-48mo). The mean preoperative IOP and number of anti-glaucoma medications were 45.1±6.5 mm Hg and 3.3±0.5 respectively. The mean post-operative IOP at 3, 6, 12, 18, 24, 36, and 48mo were 15.8±6.0, 14.7±3.3,15.3±2.0, 15.6±2.6, 17.5±1.8, 16.5±4.9, and 14.0 mm Hg.The number of anti-glaucoma medications at these time points were all 0. This surgery failed to control the IOP in 1 eye at 1mo after surgery. Hyphaema occurred in 3 eyes on the first day after surgery. Postoperative transient IOP increasing was encountered with in two eyes from 1wk to 1mo after surgery. Choroidal detachment developed in one eye but responded well to conservative treatment.CONCLUSION: Penetrating canaloplasty is effective for corticosteroid-induced glaucoma without serious complications, making it a viable or preferred alternative option.展开更多

Big Earth data are produced from satellite observations,Internet-ofThings,model simulations,and other sources.The data embed unprecedented insights and spatiotemporal stamps of relevant Earth phenomena for improving o...Big Earth data are produced from satellite observations,Internet-ofThings,model simulations,and other sources.The data embed unprecedented insights and spatiotemporal stamps of relevant Earth phenomena for improving our understanding,responding,and addressing challenges of Earth sciences and applications.In the past years,new technologies(such as cloud computing,big data and artificial intelligence)have gained momentum in addressing the challenges of using big Earth data for scientific studies and geospatial applications historically intractable.This paper reviews the big Earth data analytics from several aspects to capture the latest advancements in this fast-growing domain.We first introduce the concepts of big Earth data.The architecture,various functionalities,and supporting modules are then reviewed from a generic methodology aspect.Analytical methods supporting the functionalities are surveyed and analyzed in the context of different tools.The driven questions are exemplified through cutting-edge Earth science researches and applications.A list of challenges and opportunities are proposed for different stakeholders to collaboratively advance big Earth data analytics in the near future.展开更多
Epilepsy is a common and severe brain disease affecting>65 million people worldwide.Recent studies have shown that kinesin superfamily motor protein 17(KIF17)is expressed in neurons and is involved in regulating th...Epilepsy is a common and severe brain disease affecting>65 million people worldwide.Recent studies have shown that kinesin superfamily motor protein 17(KIF17)is expressed in neurons and is involved in regulating the dendrite-targeted transport of N-methyl-D-aspartate receptor subtype 2B(NR2B).However,the effect of KIF17 on epileptic seizures remains to be explored.We found that KIF17 was mainly expressed in neurons and that its expression was increased in epileptic brain tissue.In the kainic acid(KA)-induced epilepsy mouse model,KIF17 overexpression increased the severity of epileptic activity,whereas KIF17 knockdown had the opposite effect.In electrophysiological tests,KIF17 regulated excitatory synaptic transmission,potentially due to KIF17-mediated NR2B membrane expression.In addition,this report provides the first demonstration that KIF17 is modified by SUMOylation(SUMO,small ubiquitin-like modifier),which plays a vital role in the stabilization and maintenance of KIF17 in epilepsy.展开更多
