Recent studies on the pathogenesis and progression of multiple sclerosis have highlighted the multiple roles of mononuclear macrophages in the neuroinflammatory process. Monocytes play a significant role in neuroinflammation, and managing neuroinflammation by manipulating peripheral monocytes stands out as an effective strategy for the treatment of multiple sclerosis, leading to improved patient outcomes. This review outlines the steps involved in the entry of myeloid monocytes into the central nervous system that are targets for effective intervention: the activation of bone marrow hematopoiesis, migration of monocytes in the blood, and penetration of the blood–brain barrier by monocytes. Finally, we summarize the different monocyte subpopulations and their effects on the central nervous system based on phenotypic differences. As activated microglia resemble monocyte-derived macrophages, it is important to accurately identify the role of monocyte-derived macrophages in disease. Depending on the roles played by monocyte-derived macrophages at different stages of the disease, several of these processes can be interrupted to limit neuroinflammation and improve patient prognosis. Here, we discuss possible strategies to target monocytes in neurological diseases, focusing on three key aspects of monocyte infiltration into the central nervous system, to provide new ideas for the treatment of neurodegenerative diseases.展开更多

However, few studies have comprehensively investigated the clini...Background: Explosions can produce blast waves, high-speed medium, thermal radiation, and chemical spatter,leading to complex and compound eye injuries. However, few studies have comprehensively investigated the clinical features of different eye injury types or possible risk factors for poor prognosis.Methods: We retrospectively reviewed all consecutive records of explosive eye injuries(1449 eyes in 1115 inpatients)in 14 tertiary referral hospitals in China over 12 years(between January 1, 2008 and December 31, 2019). Data on demographics, eye injury types, ocular findings, treatments, and factors affecting visual prognosis were extracted from a standardized database of eye injuries and statistically analyzed.Results: Mechanical ocular trauma accounted for 94.00% of explosion-related eye injuries, among which intraocular foreign bodies(IOFBs) resulted in 55.17% of open globe injuries(OGIs) and contusion caused 60.22% of close globe injuries(CGIs). Proliferative vitreous retinopathy(PVR) was more common in perforating(47.06%) and IOFB(26.84%)than in penetrating(8.79%) injuries, and more common with laceration(24.25%) than rupture(9.22%, P<0.01).However, no difference was observed between rupture and contusion. Ultimately, 9.59% of eyes were removed and the final vision was ≤4/200 in 45.82% of patients. Poor presenting vision [odds ratio(OR)=5.789], full-thickness laceration of the eyeball ≥5 mm(OR=3.665), vitreous hemorrhage(OR=3.474), IOFB(OR=3.510), non-mechanical eye injury(NMEI, OR=2.622), rupture(OR=2.362), traumatic optic neuropathy(OR=2.102), retinal detachment(RD,OR=2.033), endophthalmitis(OR=3.281), contusion(OR=1.679), ciliary body detachment(OR=6.592), zone Ⅲ OGI(OR=1.940), and PVR(OR=1.615) were significant negative predictors for poor visual outcomes(P<0.05).Conclusions: Explosion ocular trauma has complex mechanisms, with multiple eyes involved and poor prognosis. In lethal level Ⅰ explosion injuries, eyeball rupture is a serious condition, whereas contusion is more likely to improve. In level Ⅱ injuries, IOFBs are more harmful than penetrating injuries, and level Ⅳ represents burn-related eye injuries.PVR is more associated with penetrating mechanisms than with OGI. Identifying the risk predictors for visual prognosis can guide clinicians in the evaluation and treatment of ocular blast injuries.展开更多

Two features with complex formation processes, the zonal E1 Nifio-like pattern and the meridional equatorial peak warm- ing (EPW), are investigated. The climatological evaporation is the main contributor to the E1 Nifio-like pattern, while the ocean dynamical thermostat effect plays a comparable negative role. The cloud-shortwave-radiation-SST feedback and the weakened Walker circulation play a small positive role in the E1 Nifio-like pattern. The processes associated with ocean dynamics are confined to the equator. The climatological evaporation is also the dominant contributor to the EPW pattern, as suggested in previous studies. However, the effects of some processes are inconsistent with previous studies. For example, changes in the zonal heat advection due to the weakened Walker circulation have a remarkable positive contribution to the EPW pattern, and changes in the shortwave radiation play a negative role in the EPW pattern.展开更多
Atrophic fracture nonunion poses a significant clinical problem with limited therapeutic interventions.In this study,we developed a unique nonunion model with high clinical relevance using serum transfer-induced rheum...Atrophic fracture nonunion poses a significant clinical problem with limited therapeutic interventions.In this study,we developed a unique nonunion model with high clinical relevance using serum transfer-induced rheumatoid arthritis(RA).Arthritic mice displayed fracture nonunion with the absence of fracture callus,diminished angiogenesis and fibrotic scar tissue formation leading to the failure of biomechanical properties,representing the major manifestations of atrophic nonunion in the clinic.Mechanistically,we demonstrated that the angiogenesis defect observed in RA mice was due to the downregulation of SPP1 and CXCL12 in chondrocytes,as evidenced by the restoration of angiogenesis upon SPP1 and CXCL12 treatment in vitro.In this regard,we developed a biodegradable scaffold loaded with SPP1 and CXCL12,which displayed a beneficial effect on angiogenesis and fracture repair in mice despite the presence of inflammation.Hence,these findings strongly suggest that the sustained release of SPP1 and CXCL12 represents an effective therapeutic approach to treat impaired angiogenesis and fracture nonunion under inflammatory conditions.展开更多
Glucose metabolism is fundamental for the functions of all tissues,including cartilage.Despite the emerging evidence related to glucose metabolism in the regulation of prenatal cartilage development,little is known ab...Glucose metabolism is fundamental for the functions of all tissues,including cartilage.Despite the emerging evidence related to glucose metabolism in the regulation of prenatal cartilage development,little is known about the role of glucose metabolism and its biochemical basis in postnatal cartilage growth and homeostasis.We show here that genetic deletion of the glucose transporter Glutl in postnatal cartilage impairs cell proliferation and matrix production in growth plate(GPs)but paradoxically increases cartilage remnants in the metaphysis,resulting in shortening of long bones.On the other hand,articular cartilage(AC)with Glutl deficiency presents diminished cellularity and loss of proteoglycans,which ultimately progress to cartilage fibrosis.Moreover,predisposition to Glutl deficiency severely exacerbates injury-induced osteoarthritis.Regardless of the disparities in glucose metabolism between GP and AC chondrocytes under normal conditions,both types of chondrocytes demonstrate metabolic plasticity to enhance glutamine utilization and oxidation in the absence of glucose availability.However,uncontrolled glutamine flux causes collagen overmodification,thus affecting extracellular matrix remodeling in both cartilage compartments.These results uncover the pivotal and distinct roles of Glutl-mediated glucose metabolism in two of the postnatal cartilage compartments and link some cartilage abnormalities to altered glucose/glutamine metabolism.展开更多
Background:The number of papers published in the field of nursing practice has greatly increased in recent years in China's Mainland,yet the quality of these papers is highly variable.There has been no attempt to ...Background:The number of papers published in the field of nursing practice has greatly increased in recent years in China's Mainland,yet the quality of these papers is highly variable.There has been no attempt to comprehensively estimate the overall quantity and quality of these papers.Objectives:To systematically retrieve nursing intervention studies published in simplified Chinese from 1979 to 2012,critically appraise their quality,and provide suggestions for future development.Methods:All of the papers were retrieved from China Biology Medicine disc database.The preliminary screening of retrieved publications was performed prior to conducting a rigorous quality evaluation of the remaining publications.10 characteristics would be included in consideration in quality evaluations.Results:69,150 papers were retrieved according to searching strategy.7391 of them were included after preliminary screening and appraised critically.Among the 10 characteristics considered in quality evaluations,the lowest ratings were observed for the factors of“utilisation of blind method”(13 articles),“description of loss of follow-up”(499 articles),“appropriate calculation of sample size”(511 articles)and“randomised assignment of patients to treatments”(652 articles).Conclusions:Chinese papers published in the field of nursing practice have increased over time,but improvements remain needed to ensure that thorough studies with high-quality research methodologies are being performed.Future nursing researchers should not only improve the design of their intervention studies but also clearly describe the methodology they used,especially in group randomisation,blinded research designs,and estimations of required sample sizes.展开更多

To evaluate the effects of maternal pre-pregnancy body mass index (pre-BMI) and gestational weight gain (GWG) on neonatal birth weight (NBW) in the population of Chinese healthy pregnant women, attempting to gui...To evaluate the effects of maternal pre-pregnancy body mass index (pre-BMI) and gestational weight gain (GWG) on neonatal birth weight (NBW) in the population of Chinese healthy pregnant women, attempting to guide weight control in pregnancy. A retrospective cohort study of 3772 Chinese women was conducted. The population was stratified by maternal pre-BMI categories as underweight (〈18.5 kg/m2), normal weight (18.5-23.9 kg/m2), overweight (24.0-27.9 kg/m2), and obesity (〉28.0 kg/m2). The NBW differences were tested among the four groups, and then deeper associations among maternal pre-BMI, GWG, and NBW were investigated by multivariate analysis. NBW increased significantly with the increase of maternal pre-BMI level (P〈0.05), except overweight to obesity (P〉0.05). The multivariate analysis showed that both pre-BMI and GWG were positively correlated with NBW (P〈0.05). Compared with normal pre-BMI, underweight predicted an increased odds ratio of small-for-gestational-age (SGA) and decreased odds ratio for macrosomia and large-for-gestational-age (LGA), and the results were opposite for overweight. With the increase of GWG, the risk of SGA decreased and the risks of macrosomia and LGA increased. In addition, in different pre-BMI categories, the effects of weight gain in the first trimester on NBW were different (P〈0.05). NBW is positively affected by both maternal pre-BMI and GWG, extreme pre-BMI and GWG are both associated with increased risks of abnormal birth weight, and maternal pre-BMI may modify the effect of weight gain in each trimester on NBW. A valid GWG guideline for Chinese women is an urgent requirement, whereas existing recommendations seem to be not very suitable for the Chinese.展开更多

TARA progressing int...Background:Takayasu arteritis-induced renal arteritis (TARA), commonly seen in Takayasu arteritis (TA), has become one of the main causes of poor prognosis and early mortality in patients with TA. TARA progressing into Takayasu arteritis-induced renal artery stenosis (TARAS), could lead to severe complications including malignant hypertension, cardiac-cerebral vascular disease, and ischemic nephropathy. Since there existed no guidelines on treatments, this study aimed to review the comprehensive treatments for TARA.Methods:We searched systematically in databases including PubMed, Ovid-Medline, EMBASE, Web of Science, China National Knowledge Infrastructure, Wanfang, and SinoMed, from inception to May 2018. Literature selection, data extraction, and statistical analysis were performed.Results:Eighty-two literatures were recruited focusing on medical treatments ( n = 34) and surgical treatments ( n = 48). We found that combined medical treatments of glucocorticoids and conventional synthetic disease-modifying anti-rheumatic drugs could reach high rates of remission in patients with TARA, and biological disease-modifying anti-rheumatic drugs were preferred for refractory patients. After remission induction, surgical treatment could help reconstruct renal artery and recover renal function partly. Percutaneous transluminal angioplasty was the first choice for patients with TARAS, while open surgery showed a good longterm survival. Conclusions:Patients with TARA should benefit both from medical treatments and from surgical treatments comprehensively and sequentially. Multidisciplinary team coordination is recommended especially in patients with severe complications.展开更多

文摘Dear Editor,Dogs(Canis familiaris)serve as human companions and are raised to herd livestock,aid hunters,guard homes,perform police and rescue work,and guide the blind.Dogs exhibit close similarities to humans in terms of metabolic,physiological,and anatomical characteristics,and thus are ideal genetic and clinical models to study human diseases(Tsai et al.,2007).Gene target technology is a powerful tool to create new strains of animals with favorable traits.However,thus far,gene-target dogs have not been developed due to their unique species-specific reproductive characteristics,which limits the applications of dogs especially in the field of biomedical research.Recently,clustered regularly interspaced short palindromic repeats(CRISPRs)/CRISPR-associated(Cas)9 system was applied to edit specific genes with a high efficiency(Cong et al.,2013;Mali et al.,2013).Here we attempt to explore the feasibility of producing gene knockout(KO)dogs by using this technology.Beagle dog,the most widely used breed in biomedical research,was used as our animal model.Myostatin(MSTN)was chosen as the first gene of interest.

文摘To evaluate the effects of maternal pre-pregnancy body mass index (pre-BMI) and gestational weight gain (GWG) on neonatal birth weight (NBW) in the population of Chinese healthy pregnant women, attempting to guide weight control in pregnancy. A retrospective cohort study of 3772 Chinese women was conducted. The population was stratified by maternal pre-BMI categories as underweight (〈18.5 kg/m2), normal weight (18.5-23.9 kg/m2), overweight (24.0-27.9 kg/m2), and obesity (〉28.0 kg/m2). The NBW differences were tested among the four groups, and then deeper associations among maternal pre-BMI, GWG, and NBW were investigated by multivariate analysis. NBW increased significantly with the increase of maternal pre-BMI level (P〈0.05), except overweight to obesity (P〉0.05). The multivariate analysis showed that both pre-BMI and GWG were positively correlated with NBW (P〈0.05). Compared with normal pre-BMI, underweight predicted an increased odds ratio of small-for-gestational-age (SGA) and decreased odds ratio for macrosomia and large-for-gestational-age (LGA), and the results were opposite for overweight. With the increase of GWG, the risk of SGA decreased and the risks of macrosomia and LGA increased. In addition, in different pre-BMI categories, the effects of weight gain in the first trimester on NBW were different (P〈0.05). NBW is positively affected by both maternal pre-BMI and GWG, extreme pre-BMI and GWG are both associated with increased risks of abnormal birth weight, and maternal pre-BMI may modify the effect of weight gain in each trimester on NBW. A valid GWG guideline for Chinese women is an urgent requirement, whereas existing recommendations seem to be not very suitable for the Chinese.

文摘Background:Takayasu arteritis-induced renal arteritis (TARA), commonly seen in Takayasu arteritis (TA), has become one of the main causes of poor prognosis and early mortality in patients with TA. TARA progressing into Takayasu arteritis-induced renal artery stenosis (TARAS), could lead to severe complications including malignant hypertension, cardiac-cerebral vascular disease, and ischemic nephropathy. Since there existed no guidelines on treatments, this study aimed to review the comprehensive treatments for TARA.Methods:We searched systematically in databases including PubMed, Ovid-Medline, EMBASE, Web of Science, China National Knowledge Infrastructure, Wanfang, and SinoMed, from inception to May 2018. Literature selection, data extraction, and statistical analysis were performed.Results:Eighty-two literatures were recruited focusing on medical treatments ( n = 34) and surgical treatments ( n = 48). We found that combined medical treatments of glucocorticoids and conventional synthetic disease-modifying anti-rheumatic drugs could reach high rates of remission in patients with TARA, and biological disease-modifying anti-rheumatic drugs were preferred for refractory patients. After remission induction, surgical treatment could help reconstruct renal artery and recover renal function partly. Percutaneous transluminal angioplasty was the first choice for patients with TARAS, while open surgery showed a good longterm survival. Conclusions:Patients with TARA should benefit both from medical treatments and from surgical treatments comprehensively and sequentially. Multidisciplinary team coordination is recommended especially in patients with severe complications.
