术前睡眠质量对老年患者无痛结肠镜检查术中丙泊酚用量的影响 被引量:15

目的评估术前睡眠质量对老年患者无痛结肠镜检查术中丙泊酚用量的影响。方法择期行无痛结肠镜检查术患者60例,年龄65~76岁,美国麻醉医师协会(ASA)分级Ⅱ~Ⅲ级。术前根据匹兹堡睡眠质量指数(PSQI)诊断有无睡眠障碍,其中,PSQI≥5分为睡眠... 目的评估术前睡眠质量对老年患者无痛结肠镜检查术中丙泊酚用量的影响。方法择期行无痛结肠镜检查术患者60例,年龄65~76岁,美国麻醉医师协会(ASA)分级Ⅱ~Ⅲ级。术前根据匹兹堡睡眠质量指数(PSQI)诊断有无睡眠障碍,其中,PSQI≥5分为睡眠障碍;PSQI<5分为睡眠正常。据PSQI评分将患者分为睡眠障碍组和睡眠正常组,每组30例。两组患者诱导前5 min均静脉注射羟考酮0.05 mg/kg。静脉注射丙泊酚负荷剂量0.50~1.00 mg/kg,待睫毛反射消失后麻醉维持:静脉泵注丙泊酚3~5 mg/(kg·h)。检查过程中发生体动反应时追加丙泊酚0.50 mg/kg。主要观察指标为操作过程中丙泊酚用量。次要观察指标:术中警觉与镇静评分(OAA/S评分);术中呼吸抑制、体动反应、腹痛和恶心呕吐等发生情况;诱导时间、苏醒时间、手术时间;血氧饱和度降低[脉搏血氧饱和度(SpO2)分别小于95%和90%的发生情况]、检查结束至进入恢复室的时间。用疼痛视觉模拟评分(VAS)(0~10 cm)评价结肠镜检查后的疼痛和虚弱程度。记录两组患者术后胃肠功能指标。结果与睡眠障碍组比较,睡眠正常组患者操作过程中的丙泊酚用量明显减少(P<0.05)。两组患者SpO2下降低于95%的例数和下降低于90%的例数均相近(P>0.05)。术中OAA/S评分、呼吸抑制、体动反应、腹痛和恶心呕吐等发生情况和检查结束至进入恢复室的时间及VAS评分结果在两组患者间差异无统计学意义(P>0.05)。与睡眠障碍组比较,睡眠正常组结肠镜检查后苏醒时间明显缩短(P<0.05),两组术后疼痛评分和虚弱程度均接近(P>0.05)。两组患者术后胃肠功能指标差异均无统计学意义(P>0.05)。结论术前睡眠障碍可明显增加老年患者无痛结肠镜检查术中的丙泊酚用量,并延缓患者麻醉苏醒。 展开更多

In order to prevent severe pollution by de-icing salt on greenery along urban roads, a half lethal dose (LD_50)for a plant population was confirmed through stress simulation of chloride de-icing salt on Euonymus japonicus, with an ianalysis of physiological changes, statistics on mortality rate on plant populations and mathematic modeling during a 30- day subacute toxicity test. The results indicate that a significant positive correlation in the early stages and a significant negative correlation in the later stages were observed between the amount of chlorophyll a and b in plants and a cumulative dose of de-icing salt. The amounts of free proline in plants and the dose of de-icing salt were positively correlated Over the entire period. No significant correlation in the initial stage, but a significant negative correlation in later stages was observed between the soluble protein and the dose of de-icing salt. LDs0 of this chloride agent on E. japonicus is 5 kg.(L·m2)-1 over 30 days. 展开更多

An effective sponge city construction evaluation system plays a crucial role in evaluating sponge city construction schemes.The construction of a sponge city evaluation system still faces challenges related to incomplete index selection and unscientific weight division.Limited studies have focused on the comprehensive assessment of sponge city construction in the early stages.This study constructed a scientific assessment indicator system and a quantitative indicator weight at all levels by literature review and statistical analysis methods from an objective perspective.To demonstrate how to utilize our evaluation methods,three construction schemes randomly generated by MATLAB were evaluated under evaluation states of constant weight and variable weight,respectively.Scheme 3 had the highest score of 0.638 under the constant weight assessment,but it cannot practically be the final construction scheme due to the imbalance between indicators.Compared to the constant weight assessment,a variable weight assessment can effectively balance the states of the evaluation index with changes in the decision variable.Among the three schemes,Scheme 2 is the best choice with a value of 0.0355 under variable weight evaluation due to punishment and incentives in the variable weight method.The concept of“punishing”a disadvantageous indicator and“motivating”an advantageous indicator increases the relative advantages of the indices,ultimately affecting the assessment results of schemes and leading to a more balanced state.This study provides reasonable analysis and decision-making mechanisms to support decision-making and guide the scientific selection of a construction scheme. 展开更多

Background:NAC,as a unique transcription factor to plants,plays important roles in multiple biological functions,such as regulation of plant growth and development,hormone levels,and responses to various kinds of stresses.However,there is a lack of research of NAC genes in Chinese herbs.Objective:The study aimed to evaluate the potential functions of NAC genes in Scutellaria baicalensis by bioinformatics and expression analysis,and provide evidence of the molecular regulation mechanism involved in flavonoid biosynthesis in S.baicalensis.Methods:The genes of NAC transcription factors in S.baicalensis were obtained from cDNA library and their functions were explored using bioinformatic methods.The NAC genes were screened from the cDNA library of S.baicalensis using BLAST comparison software.Then,the open reading frame(ORF) finder online tool was used to predict the full-length ORFs of NAC genes and their protein characteristics were explored by bioinformatic methods.The expression of NAC genes was then detected by quantitative polymerase chain reaction in different parts of S.baicalensis and different leaves treated by gibberellin GA3 treatment.Results:Six genes of NAC transcription factors were cloned,two of which had complete ORFs.NAC genes cloned in this study were mainly expressed in the flowers of S.baicalensis.The expression levels of NAC2,NAC3,NAC4,NAC5,NAC6 were increased firstly and then decreased gradually after 100 μM GA3 treatment.Meanwhile,some NACs and PAL2 in S.baicalensis showed strong correlation.Conclusion:This study suggested that NACs cloned in this study were mainly regulated the flavonoid biosynthesis in the flowers of S.baicalensis;NAC6 in S.baicalensis might be involved in the regulation of PAL2 transcription and affected the accumulation of flavonoids in the root of S.baicalensis.Our results provided a basis for further understanding the molecular regulation mechanism of flavonoid biosynthesis in S.baicalensis. 展开更多
