BACKGROUND Due to the prolonged life expectancy and increased risk of colorectal cancer(CRC)among patients with human immunodeficiency virus(HIV)infection,the prognosis and pathological features of CRC in HIV-positive...BACKGROUND Due to the prolonged life expectancy and increased risk of colorectal cancer(CRC)among patients with human immunodeficiency virus(HIV)infection,the prognosis and pathological features of CRC in HIV-positive patients require examination.AIM To compare the differences in oncological features,surgical safety,and prognosis between patients with and without HIV infection who have CRC at the same tumor stage and site.METHODS In this retrospective study,we collected data from HIV-positive and-negative patients who underwent radical resection for CRC.Using random stratified sampling,24 HIV-positive and 363 HIV-negative patients with colorectal adenocarcinoma after radical resection were selected.Using propensity score matching,we selected 72 patients,matched 1:2(HIV-positive:negative=24:48).Differences in basic characteristics,HIV acquisition,perioperative serological indicators,surgical safety,oncological features,and long-term prognosis were compared between the two groups.RESULTS Fewer patients with HIV infection underwent chemotherapy compared to patients without.HIV-positive patients had fewer preoperative and postoperative leukocytes,fewer preoperative lymphocytes,lower carcinoembryonic antigen levels,more intraoperative blood loss,more metastatic lymph nodes,higher node stage,higher tumor node metastasis stage,shorter overall survival,and shorter progression-free survival compared to patients who were HIV-negative.CONCLUSION Compared with CRC patients who are HIV-negative,patients with HIV infection have more metastatic lymph nodes and worse long-term survival after surgery.Standard treatment options for HIV-positive patients with CRC should be explored.展开更多

METHODS: Telomeric repeated amplification protocol (TRAP)-enzyme-linked immunosorbent assay (ELISA) was performed to measure the telomerase activity in 60 patients with gastric cancer and 50 with peptic ulcer. PLC analysis of the 60 patients with gastric cancer was used for comparison. The proliferating cell nuclear antigen (PCNA) in gastric carcinoma was immunohistochemically examined. RESULTS: The telomerase activity and PLC positive rate in peritoneal lavage fluid from patients with gastric cancer was 41.7/ (25/60), and 25.0/ (15/60), respectively. The positive rate of telomerase activity was significantly higher than that of PLC in the group of pT4 (15/16 vs 9/16, P < 0.05), P1-3 (13/13 vs 9/13, P < 0.05) and diffuse type (22/42 vs 13/42, P < 0.05). The patients with positive telomerase activity, peritoneal metastasis, and serosal invasion had signifi cantly higher levels of average PCNA proliferation index (PI), (55.00 ± 6.59 vs 27.43 ± 7.72, 57.26 ± 10.18 vs 29.15 ± 8.31, and 49.82 ± 6.74 vs 24.65 ± 7.33, respectively, P < 0.05).CONCLUSION: The TRAP assay for telomerase activity is a useful adjunct for cytologic method in the diagnosis of peritoneal micrometastasis and well related to higher proliferating activity of gastric cancer. The results of this study also suggest a promising future therapeutic strategy for treating peritoneal dissemination based on telomerase inhibition.展开更多

In the long term, it is necessary to increase the development efficiency of block matrix, surfactant-aided imbibition is a potential way. The current work aimed to explain comprehensively how surfactants can enhance the imbibition rate. Laboratory experiments were performed to investigate the effects of wettability, interfacial tension(IFT), and relative permeability as the key parameters underlying surfactant solution imbibition. Two different types of surfactants, sodium dodecyl sulfate and polyethylene glycol octylphenol ether, at varied concentrations were tested on reservoir rocks. Experimental results showed that the oil recovery rate increased with increased wettability alteration and IFT and decreased residual oil saturation. A mechanistic simulator developed in previous studies was used to perform parametric analysis after successful laboratory-scale validation. Results were proven by parametric studies. This study,which examined the mechanism and factors influencing surfactant solution imbibition, can improve understanding of surfactant-aided imbibition and surfactant screening.展开更多

A class of systems with linear models in the form of fault related parameters is investigated, with model uncertainties and parameter variations taken into account explicitly and treated as bounded errors. An ellipsoid bounding set-membership identification algorithm is proposed to propagate bounded uncertainties rigorously and the guaranteed feasible set of faults parameters enveloping true parameter values is given. Faults arised from abrupt parameter variations can be detected and isolated on-line by consistency check between predicted and observed parameter sets obtained in the identification procedure. The proposed approach provides the improved robustness with its ability to distinguish real faults from model uncertainties, which comes with the inherent guaranteed robustness of the set-membership framework. Efforts are also made in this work to balance between conservativeness and computation complexity of the overall algorithm. Simulation results for the mobile robot with several slipping faults scenarios demonstrate the correctness of the proposed approach for faults detection and isolation(FDI).展开更多

Yet, the spectrogram alone does not take into account a substantial amount of time-frequency information. In this study, we present an approach for exploring the benefits of deep scalogram representations, extracted in segments from an audio stream. The approach presented firstly transforms the segmented acoustic scenes into bump and morse scalograms, as well as spectrograms; secondly, the spectrograms or scalograms are sent into pre-trained convolutional neural networks; thirdly,the features extracted from a subsequent fully connected layer are fed into(bidirectional) gated recurrent neural networks, which are followed by a single highway layer and a softmax layer;finally, predictions from these three systems are fused by a margin sampling value strategy. We then evaluate the proposed approach using the acoustic scene classification data set of 2017 IEEE AASP Challenge on Detection and Classification of Acoustic Scenes and Events(DCASE). On the evaluation set, an accuracy of 64.0 % from bidirectional gated recurrent neural networks is obtained when fusing the spectrogram and the bump scalogram, which is an improvement on the 61.0 % baseline result provided by the DCASE 2017 organisers. This result shows that extracted bump scalograms are capable of improving the classification accuracy,when fusing with a spectrogram-based system.展开更多
Hepatocellular carcinoma(HCC) is one of the most deadly and frequent cancers worldwide,although great advancement in the treatment of this malignancy have been made within the past few decades.It continues to be a maj...Hepatocellular carcinoma(HCC) is one of the most deadly and frequent cancers worldwide,although great advancement in the treatment of this malignancy have been made within the past few decades.It continues to be a major health issue due to an increasing incidence and a poor prognosis.The majority of patients have their HCC diagnosed at an intermediate or advanced stage in theUSA or China.Curative therapy such as surgical resection or liver transplantation is not considered anoption of treatment at these stages.Transarterial chemoembolization(TACE),the most widely used locoregional therapeutic approach,used to be the mainstay of treatment for cases with unresectable cancer entities.However,for those patients with hypovascular tumors or impaired liver function reserve,TACE is a suboptimal treatment option.For example,embolization does not result in complete coverage of a hypovascular tumor,and may rather promotes postoperative tumor recurrence,or leave residual tumor,in these TACE-resistance patients.In addition,TACE carries a higher risk of hepatic decompensation in patients with poor liver function or reserve.Non-vascular interventional locoregional therapies for HCC include radiofrequency ablation(RFA),microwave ablation(MWA),high-intensity focused ultrasound(HIFU),laser-induced thermotherapy(LITT),cryosurgical ablation(CSA),irreversible Electroporation(IRE),percutaneous ethanol injection(PEI),and brachytherapy.Recent advancements in these techniques have significantly improved the treatment efficacy of HCC and expanded the population of patients who qualify for treatment.This review embraces the current status of imaging-guided locoregional non-intravascular interventional treatments for HCCs,with a primary focus on the clinical evaluation and assessment of the efficacy of combined therapies using these interventional techniques.展开更多
AIM: To compare the effect of transarterial chemoembolization(TACE) plus GRGDSP(Gly-Arg-Gly-Asp-SerPro, integrin-inhibitor) loaded nanoparticles with TACE alone or TACE + GRGDSP in a rat model of liver tumor. METHODS:...AIM: To compare the effect of transarterial chemoembolization(TACE) plus GRGDSP(Gly-Arg-Gly-Asp-SerPro, integrin-inhibitor) loaded nanoparticles with TACE alone or TACE + GRGDSP in a rat model of liver tumor. METHODS: Morris hepatoma 3924 A tumors were implanted in the livers of 30 ACI rats. The ACI rats were divided randomly into three groups(10 animals each). Tumor volume before treatment(V1) was examined by magnetic resonance imaging(MRI), and then, after laparotomy and placement of a PE-10 catheter into the hepatic artery, the following interventional protocols were performed: TACE(mitomycin C + lipiodol + degradable starch microspheres) + GRGDSP loaded nanoparticles for group A; TACE + GRGDSP for group B(control group 1); TACE alone for group C(control group 2). Tumor volume(V2) was assessed by MRI and the mean ratio of the post-treatment to pretreatment tumor volumes(V2/V1) was calculated. Immunohistochemical analysis was performed to assess the quantification of matrix metalloprotein 9(MMP-9) and vascular endothelial growth factor(VEGF) positive tumor cells in each treatment group.RESULTS: The mean tumor growth ratios(V2/V1) were 1.3649 ± 0.1194 in group A, 2.0770 ± 0.1595 in group B, and 3.2148 ± 0.1075 in group C. Compared with groups B and C, group A showed a significant reduction in tumor volume. Lower expression of MMP-9 and VEGF in hepatocellular carcinoma was observed in group A than in groups B and C. The angiogenesis of tumor was evaluated using anti-VEGF antibodies, and the metastasis of tumor was assessed using antiMMP-9 antibody. MMP-9 and VEGF were expressed in all specimens. The immunoexpression of these proteins was confirmed by the presence of red cytoplasmic staining in tumor cells. Lower expression of MMP-9 and VEGF in hepatocellular carcinoma was observed in group A than in groups B and C.CONCLUSION: Transarterial administration of integrin inhibitor loaded nanoparticles combined with TACE evidently retards tumor growth and intrahepatic metastases compared with TACE alone or TACE plus integrin inhibitor in an animal model of hepatocellular carcinoma.展开更多

METHODS: A total of 48 consecutive patients undergo- ing abdominal operations were randomized to receive either subcutaneous rhGH (0.15 IU/kg) or placebo (men- struum) injections daily for 7 d after surgery. The two groups had similar nutritional intake. Blood samples for serum fibronectin, albumin, prealbumin, transferrin and the total lymphocyte count, as well as glucose levels were collected to study the rhGH effect. Basal laboratory evaluation, and nutritional status were estimated on d 1 before as baseline and d 3 and 10 after operation using standard laboratory techniques. Nitrogen balance was measured from d 3 to 9 after operation. RESULTS: The cumulative nitrogen balance was signifi- cantly improved in rhGH group compared with the pla- cebo group (11.37 ± 16.82 vs -9.11 ± 17.52, P = 0.0003). Serum fibronectin was also significantly higher in the rhGH group than in the placebo group (104.77 ± 19.94 vs 93.03 ± 16.03, P < 0.05), whereas changes in serum albumin, prealbumin, transferrin and total lymphocyte counts were not statistically significant. Mean blood glu- cose levels were significantly higher in the rhGH group from d 3 to 6 after operation. CONCLUSION: If blood glucose can be controlled, low- dose growth hormone together with hypocaloric nutrition is effective on improving positive nitrogen balance and protein conservation and safe is in postoperative patients.展开更多
For electrochemical CO_(2) reduction to HCOOH,an ongoing challenge is to design energy efficient electrocatalysts that can deliver a high HCOOH current density(JHCOOH)at a low overpotential.Indium oxide is good HCOOH ...For electrochemical CO_(2) reduction to HCOOH,an ongoing challenge is to design energy efficient electrocatalysts that can deliver a high HCOOH current density(JHCOOH)at a low overpotential.Indium oxide is good HCOOH production catalyst but with low con-ductivity.In this work,we report a unique corn design of In_(2)O_(3-x)@C nanocatalyst,wherein In_(2)O_(3-x)nanocube as the fine grains dispersed uniformly on the carbon nanorod cob,resulting in the enhanced conductivity.Excellent performance is achieved with 84%Faradaic efficiency(FE)and 11 mA cm^(−2)JHCOOH at a low potential of−0.4 V versus RHE.At the current density of 100 mA cm^(−2),the applied potential remained stable for more than 120 h with the FE above 90%.Density functional theory calculations reveal that the abundant oxygen vacancy in In_(2)O_(3-x) has exposed more In^(3+) sites with activated electroactivity,which facilitates the formation of HCOO*intermediate.Operando X-ray absorp-tion spectroscopy also confirms In^(3+) as the active site and the key intermediate of HCOO*during the process of CO_(2) reduction to HCOOH.展开更多

However, so far, little is known about the solution structures of these electrolytes. Here,we compare the solvation phenomenon of sodium tetraphenylborate(NaBPh_(4)) salt dissolved in organic solvents of propylene carbonate(PC), 1,2-dimethoxyethane(DME), acetonitrile(ACN) and tetrahydrofuran(THF). Small-angle X-ray scattering(SAXS) reveals a unique two-peak structural feature in this saltconcentrated PC electrolyte, while solutions using other solvents only have one scattering peak.Molecular dynamics(MD) simulations further reveal that there are anion-based clusters in addition to the short-range charge ordering in the concentrated NaBPh4/PC electrolyte. Raman spectroscopy confirms the existence of considerable contact ion pairs(CIPs). This work emphasizes the importance of global and local structural analysis, which will provide valuable clues for understanding the structureperformance relationship of electrolytes.展开更多

Small nutrients detection is challenging due to the high diversity and broad dynamic range of molecules in food samples, and a further issue is to track low abundance toxins. Herein, we developed a novel plasmonic matrix-assisted laser desorption/ionization mass spectrometry(MALDI MS)approach to detect small nutrients and toxins in complex biological emulsion samples. Silver nanoshells(SiO_2@-Ag) with optimized structures were used as matrices andachieved direct analysis of ~ 6 n L of human breast milk without any enrichment or separation. We performed identification and quantitation of small nutrients and toxins with limit-of-detection down to 0.4 pmol(for melamine) and reaction time shortened to minutes, which is superior to the conventional biochemical method currently in use. The developed approach contributes to the near-future application of MALDI MS in a broad field and personalized design of plasmonic materials for real-case bio-analysis.展开更多

Faults arised from abrupt parameter variations can be detected and isolated on-line by consistency check between predicted and observed parameter sets obtained in the identification procedure. The proposed approach provides the improved robustness with its ability to distinguish real faults from model uncertainties, which comes with the inherent guaranteed robustness of the set-membership framework. Efforts are also made in this work to balance between conservativeness and computation complexity of the overall algorithm. Simulation results for the mobile robot with several slipping faults scenarios demonstrate the correctness of the proposed approach for faults detection and isolation(FDI).

文摘Spectrogram representations of acoustic scenes have achieved competitive performance for acoustic scene classification. Yet, the spectrogram alone does not take into account a substantial amount of time-frequency information. In this study, we present an approach for exploring the benefits of deep scalogram representations, extracted in segments from an audio stream. The approach presented firstly transforms the segmented acoustic scenes into bump and morse scalograms, as well as spectrograms; secondly, the spectrograms or scalograms are sent into pre-trained convolutional neural networks; thirdly,the features extracted from a subsequent fully connected layer are fed into(bidirectional) gated recurrent neural networks, which are followed by a single highway layer and a softmax layer;finally, predictions from these three systems are fused by a margin sampling value strategy. We then evaluate the proposed approach using the acoustic scene classification data set of 2017 IEEE AASP Challenge on Detection and Classification of Acoustic Scenes and Events(DCASE). On the evaluation set, an accuracy of 64.0 % from bidirectional gated recurrent neural networks is obtained when fusing the spectrogram and the bump scalogram, which is an improvement on the 61.0 % baseline result provided by the DCASE 2017 organisers. This result shows that extracted bump scalograms are capable of improving the classification accuracy,when fusing with a spectrogram-based system.

文摘Hepatocellular carcinoma(HCC) is one of the most deadly and frequent cancers worldwide,although great advancement in the treatment of this malignancy have been made within the past few decades.It continues to be a major health issue due to an increasing incidence and a poor prognosis.The majority of patients have their HCC diagnosed at an intermediate or advanced stage in theUSA or China.Curative therapy such as surgical resection or liver transplantation is not considered anoption of treatment at these stages.Transarterial chemoembolization(TACE),the most widely used locoregional therapeutic approach,used to be the mainstay of treatment for cases with unresectable cancer entities.However,for those patients with hypovascular tumors or impaired liver function reserve,TACE is a suboptimal treatment option.For example,embolization does not result in complete coverage of a hypovascular tumor,and may rather promotes postoperative tumor recurrence,or leave residual tumor,in these TACE-resistance patients.In addition,TACE carries a higher risk of hepatic decompensation in patients with poor liver function or reserve.Non-vascular interventional locoregional therapies for HCC include radiofrequency ablation(RFA),microwave ablation(MWA),high-intensity focused ultrasound(HIFU),laser-induced thermotherapy(LITT),cryosurgical ablation(CSA),irreversible Electroporation(IRE),percutaneous ethanol injection(PEI),and brachytherapy.Recent advancements in these techniques have significantly improved the treatment efficacy of HCC and expanded the population of patients who qualify for treatment.This review embraces the current status of imaging-guided locoregional non-intravascular interventional treatments for HCCs,with a primary focus on the clinical evaluation and assessment of the efficacy of combined therapies using these interventional techniques.

文摘AIM: To compare the effect of transarterial chemoembolization(TACE) plus GRGDSP(Gly-Arg-Gly-Asp-SerPro, integrin-inhibitor) loaded nanoparticles with TACE alone or TACE + GRGDSP in a rat model of liver tumor. METHODS: Morris hepatoma 3924 A tumors were implanted in the livers of 30 ACI rats. The ACI rats were divided randomly into three groups(10 animals each). Tumor volume before treatment(V1) was examined by magnetic resonance imaging(MRI), and then, after laparotomy and placement of a PE-10 catheter into the hepatic artery, the following interventional protocols were performed: TACE(mitomycin C + lipiodol + degradable starch microspheres) + GRGDSP loaded nanoparticles for group A; TACE + GRGDSP for group B(control group 1); TACE alone for group C(control group 2). Tumor volume(V2) was assessed by MRI and the mean ratio of the post-treatment to pretreatment tumor volumes(V2/V1) was calculated. Immunohistochemical analysis was performed to assess the quantification of matrix metalloprotein 9(MMP-9) and vascular endothelial growth factor(VEGF) positive tumor cells in each treatment group.RESULTS: The mean tumor growth ratios(V2/V1) were 1.3649 ± 0.1194 in group A, 2.0770 ± 0.1595 in group B, and 3.2148 ± 0.1075 in group C. Compared with groups B and C, group A showed a significant reduction in tumor volume. Lower expression of MMP-9 and VEGF in hepatocellular carcinoma was observed in group A than in groups B and C. The angiogenesis of tumor was evaluated using anti-VEGF antibodies, and the metastasis of tumor was assessed using antiMMP-9 antibody. MMP-9 and VEGF were expressed in all specimens. The immunoexpression of these proteins was confirmed by the presence of red cytoplasmic staining in tumor cells. Lower expression of MMP-9 and VEGF in hepatocellular carcinoma was observed in group A than in groups B and C.CONCLUSION: Transarterial administration of integrin inhibitor loaded nanoparticles combined with TACE evidently retards tumor growth and intrahepatic metastases compared with TACE alone or TACE plus integrin inhibitor in an animal model of hepatocellular carcinoma.
文摘AIM: To investigate the effectiveness and safety of re- combinant human growth hormone (rhGH) in postopera- tive patients. METHODS: A total of 48 consecutive patients undergo- ing abdominal operations were randomized to receive either subcutaneous rhGH (0.15 IU/kg) or placebo (men- struum) injections daily for 7 d after surgery. The two groups had similar nutritional intake. Blood samples for serum fibronectin, albumin, prealbumin, transferrin and the total lymphocyte count, as well as glucose levels were collected to study the rhGH effect. Basal laboratory evaluation, and nutritional status were estimated on d 1 before as baseline and d 3 and 10 after operation using standard laboratory techniques. Nitrogen balance was measured from d 3 to 9 after operation. RESULTS: The cumulative nitrogen balance was signifi- cantly improved in rhGH group compared with the pla- cebo group (11.37 ± 16.82 vs -9.11 ± 17.52, P = 0.0003). Serum fibronectin was also significantly higher in the rhGH group than in the placebo group (104.77 ± 19.94 vs 93.03 ± 16.03, P < 0.05), whereas changes in serum albumin, prealbumin, transferrin and total lymphocyte counts were not statistically significant. Mean blood glu- cose levels were significantly higher in the rhGH group from d 3 to 6 after operation. CONCLUSION: If blood glucose can be controlled, low- dose growth hormone together with hypocaloric nutrition is effective on improving positive nitrogen balance and protein conservation and safe is in postoperative patients.

文摘For electrochemical CO_(2) reduction to HCOOH,an ongoing challenge is to design energy efficient electrocatalysts that can deliver a high HCOOH current density(JHCOOH)at a low overpotential.Indium oxide is good HCOOH production catalyst but with low con-ductivity.In this work,we report a unique corn design of In_(2)O_(3-x)@C nanocatalyst,wherein In_(2)O_(3-x)nanocube as the fine grains dispersed uniformly on the carbon nanorod cob,resulting in the enhanced conductivity.Excellent performance is achieved with 84%Faradaic efficiency(FE)and 11 mA cm^(−2)JHCOOH at a low potential of−0.4 V versus RHE.At the current density of 100 mA cm^(−2),the applied potential remained stable for more than 120 h with the FE above 90%.Density functional theory calculations reveal that the abundant oxygen vacancy in In_(2)O_(3-x) has exposed more In^(3+) sites with activated electroactivity,which facilitates the formation of HCOO*intermediate.Operando X-ray absorp-tion spectroscopy also confirms In^(3+) as the active site and the key intermediate of HCOO*during the process of CO_(2) reduction to HCOOH.

文摘This study investigated the effect of sealing treatment on the corrosion performance of plasma electrolytic oxidation(PEO)coated AZ91D Mg alloy with and without addition of corrosion inhibitor.The microstructure,phase composition and corrosion property of the sealed and unsealed coatings were evaluated by using scanning electron microscopy(SEM),energy dispersion spectroscopy(EDS),x-ray diffraction(XRD),x-ray photoelectron spectroscopy(XPS),polarization,and electrochemical impedance spectroscopy(EIS)tests.Electrochemical experiments and salt spray tests showed that,after sealing in phosphate solution containing corrosion inhibitor,the corrosion current density of PEO-coated AZ91D decreased more than 10-fold and the anti-corrosion time in a salt spray environment increased more than three-fold.The corrosion rate of the PEO coating slowed down due to the releasing and adsorbing of the corrosion inhibitors in the pores and cracks of the coating during the corrosion process.

文摘Fluorine-free electrolytes have attracted great attention because of its low-cost and environmental friendliness. However, so far, little is known about the solution structures of these electrolytes. Here,we compare the solvation phenomenon of sodium tetraphenylborate(NaBPh_(4)) salt dissolved in organic solvents of propylene carbonate(PC), 1,2-dimethoxyethane(DME), acetonitrile(ACN) and tetrahydrofuran(THF). Small-angle X-ray scattering(SAXS) reveals a unique two-peak structural feature in this saltconcentrated PC electrolyte, while solutions using other solvents only have one scattering peak.Molecular dynamics(MD) simulations further reveal that there are anion-based clusters in addition to the short-range charge ordering in the concentrated NaBPh4/PC electrolyte. Raman spectroscopy confirms the existence of considerable contact ion pairs(CIPs). This work emphasizes the importance of global and local structural analysis, which will provide valuable clues for understanding the structureperformance relationship of electrolytes.

文摘Nutriology relies on advanced analytical tools to study the molecular compositions of food and provide key information on sample quality/safety. Small nutrients detection is challenging due to the high diversity and broad dynamic range of molecules in food samples, and a further issue is to track low abundance toxins. Herein, we developed a novel plasmonic matrix-assisted laser desorption/ionization mass spectrometry(MALDI MS)approach to detect small nutrients and toxins in complex biological emulsion samples. Silver nanoshells(SiO_2@-Ag) with optimized structures were used as matrices andachieved direct analysis of ~ 6 n L of human breast milk without any enrichment or separation. We performed identification and quantitation of small nutrients and toxins with limit-of-detection down to 0.4 pmol(for melamine) and reaction time shortened to minutes, which is superior to the conventional biochemical method currently in use. The developed approach contributes to the near-future application of MALDI MS in a broad field and personalized design of plasmonic materials for real-case bio-analysis.
