新入职护士转型冲击潜在剖面分析及其与离职意愿的关系

目的:探讨新入职护士转型冲击潜在类别特征及不同潜在类别转型冲击与离职意愿之间的关系。方法:2023年5-9月,选取四川省5所三级公立医院的513名新入职护士进行问卷调查,运用潜在剖面分析识别转型冲击潜在类别;运用多元logistic回归分析... 目的:探讨新入职护士转型冲击潜在类别特征及不同潜在类别转型冲击与离职意愿之间的关系。方法:2023年5-9月,选取四川省5所三级公立医院的513名新入职护士进行问卷调查,运用潜在剖面分析识别转型冲击潜在类别;运用多元logistic回归分析其转型冲击的影响因素,运用二元logistic回归分析不同潜在类别转型冲击与离职意愿之间的关系。结果:新入职护士可分为低稳定冲击组(17.9%)、转型冲击常规组(59.1%)和高转型冲击组(23.0%);入职工作时间、每月夜班数、每周工作时间是转型冲击的影响因素;转型冲击常规组、高转型冲击组新入职护士离职意愿分别是低稳定冲击组的1.92倍(P<0.01)和2.76倍(P<0.001)。结论:新入职护士转型冲击存在异质性,应注重不同潜在类别转型冲击与离职意愿的关系,实施针对性的干预措施,从而降低其离职意愿水平。 展开更多

椎间盘退变伴氧化应激关键生物标志物:生物信息学和机器学习算法的识别

背景:氧化应激与椎间盘退变的发生发展息息相关,但其发病机制和有效治疗方法仍不明确。目的:运用生物信息学及3种机器学习算法识别与椎间盘退变伴氧化应激相关的关键基因及免疫浸润分析,并进行实验验证。方法:从GEO数据库获得椎间盘退... 背景:氧化应激与椎间盘退变的发生发展息息相关,但其发病机制和有效治疗方法仍不明确。目的:运用生物信息学及3种机器学习算法识别与椎间盘退变伴氧化应激相关的关键基因及免疫浸润分析,并进行实验验证。方法:从GEO数据库获得椎间盘退变基因表达谱以及从GeneCards数据库获得氧化应激相关基因,对椎间盘退变数据集进行差异分析及加权基因共表达网络(WGCNA)分析,两者取交集并与氧化应激相关基因取交集得到候选hub基因,对候选hub基因进行GO和KEGG分析;运用机器学习(LASSO回归、SVM-RFE及随机森林)筛选最佳特征基因并进行受试者特征曲线验证,同时行相关免疫浸润分析。收集2023年7-11月就诊于山西医科大学第二医院的颈椎病患者的椎间盘样本作为椎间盘退变组,颈椎脊髓损伤患者的椎间盘样本作为对照组,采用qPCR方法验证特征基因在椎间盘退变组织中的相对表达量。结果与结论:(1)经过差异基因分析获取424个差异表达基因,WGCNA分析得到5087个基因,同时获得氧化应激基因1399个,进而得到23个候选hub基因;(2)GO分析结果显示,主要参与细菌防御反应、细菌来源分子反应等生物过程;涉及分泌颗粒腔、细胞质囊泡腔等细胞组成;涉及内肽酶活性和硫化合物结合等分子功能;(3)KEGG分析结果显示,候选hub基因与中性粒细胞胞外诱捕网形成、肾素-血管紧张素系统通路等信号通路有关;(4)运用3种机器学习和ROC验证后得到关键基因HSPA6和PKD1;(5)免疫浸润分析显示HSPA6与活化树突状细胞(r=0.88,P<0.001)、活化CD4^(+)T细胞(r=-0.72,P<0.01)等密切相关,同时PKD1与效应型记忆CD8^(+)T细胞(r=0.55,P<0.05)、活化树突状细胞(r=-0.56,P<0.05)等密切相关;(6)q PCR实验结果表明椎间盘退变组中HSPA6基因低于对照组(P<0.0001),而PKD1基因高于对照组(P<0.0001);(7)结果表明运用生物信息学及机器学习算法证实HSPA6和PKD1可作为椎间盘退变伴氧化应激的生物标志物,可能通过干预HSPA6和PKD1来改善椎间盘退变。 展开更多

目的了解新入职护士转型冲击水平及影响机制,为降低新入职护士转型冲击提供参考。方法采用生涯适应力量表、反馈寻求行为量表、转型冲击量表、组织支持感量表对四川省6所三级甲等公立医院357名新入职护士进行调查,利用结构方程模型验证... 目的了解新入职护士转型冲击水平及影响机制,为降低新入职护士转型冲击提供参考。方法采用生涯适应力量表、反馈寻求行为量表、转型冲击量表、组织支持感量表对四川省6所三级甲等公立医院357名新入职护士进行调查,利用结构方程模型验证新入职护士转型冲击与生涯适应力、反馈寻求行为、组织支持的关系。结果新入职护士转型冲击总分为(68.769±19.953)分;新入职护士生涯适应力负向影响转型冲击;反馈寻求行为在生涯适应力与转型冲击间起部分中介作用,中介效应占总效应的42.01%;组织支持在生涯适应力与反馈寻求行为之间起正向调节作用,组织支持水平越高,生涯适应力对反馈寻求行为的正向作用越强,反馈寻求行为的中介作用也越强。结论新入职护士转型冲击水平中等,提升新入职护士生涯适应力水平,促进反馈寻求行为及提升组织支持水平可减少转型冲击带来的负面影响,促进新入职护士尽快适应临床岗位。 展开更多

中国成人非酒精性脂肪肝风险预测模型的系统评价

目的系统地检索和评价中国成人非酒精性脂肪性肝病(non-alcoholic fatty liver disease,NAFLD)风险预测模型的偏倚性和适用性。方法计算机检索在PubMed、Embase、The Cochrane Library、Web of Science、中国知网、万方、维普、中国生... 目的系统地检索和评价中国成人非酒精性脂肪性肝病(non-alcoholic fatty liver disease,NAFLD)风险预测模型的偏倚性和适用性。方法计算机检索在PubMed、Embase、The Cochrane Library、Web of Science、中国知网、万方、维普、中国生物医学等数据库发布的有关成人NAFLD风险预测模型的研究,检索时间为从建库至2023年9月1日。由2名研究者独立筛选文献,并根据预测模型研究数据提取表进行资料提取,应用预测模型研究偏倚风险评估工具评价纳入文献的偏倚风险和适用性。结果共纳入17项中国成人NAFLD风险预测模型研究,报告了25个风险预测模型。仅1项研究对模型进行内外部验证,其余均对模型进行内部验证。模型的受试者工作特征曲线下面积为0.718~0.945,预测因子主要包含人口学因素(年龄、性别等)、体格检查指标(BMI、腹围等)、实验室检查指标(TG、ALT等),除1项研究偏倚风险不清楚,其余均存在较高的偏倚风险。结论纳入的模型具有良好的预测效能,预测因子不尽相同但存在一定共性,存在高偏倚风险且缺乏外部验证,其临床价值有待进一步验证。 展开更多

目的采用meta分析咀嚼功能与认知能力下降风险的关系。方法检索PubMed、Embase、The Cochrane Library、Web of Science、CINAHL、中国知识资源综合数据库、万方医学网、维普资讯中文期刊服务平台、中国生物医学文献服务系统等数据库中... 目的采用meta分析咀嚼功能与认知能力下降风险的关系。方法检索PubMed、Embase、The Cochrane Library、Web of Science、CINAHL、中国知识资源综合数据库、万方医学网、维普资讯中文期刊服务平台、中国生物医学文献服务系统等数据库中咀嚼功能与认知能力下降风险相关性的文献。使用StataSE15.0软件进行meta分析。结果最终纳入11项研究,包括8项横断面研究和3项病例对照研究,共66108例参与者。咀嚼功能与认知能力下降或痴呆的发生风险存在相关性(优势比=0.52,95%可信区间0.22~0.82)。样本量小于或等于1000例,以及不同研究地区、发表年份、研究类型中咀嚼功能与认知能力下降或痴呆风险均存在相关性。结论咀嚼功能与认知能力存在相关性,良好的咀嚼功能是认知能力下降的保护因素。应继续进行大样本纵向研究,以进一步阐明可能的因果关系。 展开更多

BACKGROUND:Remimazolam is a novel ultra-short-acting sedative,but its safety and adverse events(AEs)in high-risk patients in the intensive care unit(ICU)setting remain unknown.METHODS:This was a single-center,retrospective study that compared remimazolam to propofol and midazolam in patients undergoing upper gastrointestinal endoscopy.The primary outcome was the incidence of treatment-related AEs.The secondary outcomes were the time to extubation,the length of ICU stay,and the average cost of sedative per case.RESULTS:Of the 88 patients analyzed,47 were treated with remimazolam(mean dose,7.90±4.84mg),and 41 were treated with propofol(21.19±17.98 mg)or midazolam(3.08±2.17 mg).There was no statistically significant difference in the average duration of the endoscopic procedure(35.89±13.37 min vs.44.51±21.68 min,P=0.133)or the time to extubation(15.00±9.75 h vs.20.59±18.71 h,P=0.211)in the remimazolam group(group I)compared to the propofol or midazolam group(group II).ICU stays(5.40±2.93 d vs.4.63±3.31 d,P=0.072)and treatment-related AEs(48.61%vs.51.38%,P=0.056)were similar between groups.The average cost of sedative per case was significantly lower in the group I than in the group II(RMB 16.07±10.58 yuan vs.RMB 24.37±15.46 yuan,P=0.016).CONCLUSION:Remimazolam-based sedation was noninferior to the classic sedatives and had lower average cost per case,indicating that it may be used as a promising sedative for high-risk patients during endoscopic procedures in the ICU setting. 展开更多

为进一步保护祁连山生态环境及合理开发利用河流沿线的植被资源,该文基于MODIS数据的NDVI产品及DEM数据集,对祁连山南坡主要河流谷地2000—2018年植被生长季的NDVI时空分布特征进行了研究,并提取研究区的主要地形因子,分析其对河流谷地... 为进一步保护祁连山生态环境及合理开发利用河流沿线的植被资源,该文基于MODIS数据的NDVI产品及DEM数据集,对祁连山南坡主要河流谷地2000—2018年植被生长季的NDVI时空分布特征进行了研究,并提取研究区的主要地形因子,分析其对河流谷地植被生长季NDVI的影响。结果表明:随着河流两侧缓冲区距离逐渐增大,各年份的NDVI值呈现先增加后平稳再减少的分布特点;2000—2018年研究区河谷NDVI的变化趋势基本一致,整体表现出2010年出现了近20年的峰值;祁连山南坡河流谷地NDVI值,从斜坡(16°~25°)至急坡(41°~45°)增加的速率最大,说明该区间为NDVI值突变区间。2000—2018年祁连山南坡主要河流谷地的NDVI时空分布,可能受地形、气温和降水等自然因素影响较大,其受人为干扰因素影响较小,其中降水并非影响研究区河谷植被NDVI分布的主导因素,而气温可能是影响研究区河谷植被NDVI分布的主导因素。在地形因子中,存在较适合植被生长的特定坡度区间,且太阳辐射能量的增加有利于植被的生长。 展开更多

目的探讨醋酸奥曲肽微球治疗胃肠胰腺神经内分泌肿瘤(GEP-NENs)的疗效。方法选择2017年11月至2019年3月我科收治GEP-NENs患者70例,随机数字表法分为GEM组及OA+GEM组各35例。GEM组采用吉西他滨(GEM)治疗,OA+GEM组采用GEM联合醋酸奥曲肽(... 目的探讨醋酸奥曲肽微球治疗胃肠胰腺神经内分泌肿瘤(GEP-NENs)的疗效。方法选择2017年11月至2019年3月我科收治GEP-NENs患者70例,随机数字表法分为GEM组及OA+GEM组各35例。GEM组采用吉西他滨(GEM)治疗,OA+GEM组采用GEM联合醋酸奥曲肽(OA)微球治疗。比较两组近期、短期远期疗效、治疗前后肿瘤标志物水平、血清激素水平、治疗后KPS评分与QOL-BREF评分及安全性。结果治疗后,OA+GEM组近期、短期远期疗效均优于GEM组,治疗有效率、疾病控制率与复发率比较,差异有统计学意义(P<0.05);治疗后,两组神经元特异性烯醇化酶(NSE)、嗜铬粒蛋白A(CgA)及癌胚抗原(CEA)水平低于治疗前,且OA+GEM组低于GEM组;两组胰岛素水平与治疗前比较显著上升,胃泌素以及胰高血糖素较治疗前显著降低,且治疗后OA+GEM组胰岛素水平高于GEM组,胃泌素与胰高血糖素水平低于GEM组,差异有统计学意义(P<0.05);OA+GEM组治疗后KPS评分与QOL-BREF评分明显优于GEM组(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论OA微球治疗GEP-NENs可以明显改善患者肿瘤标志物水平,纠正患者体内紊乱胰岛素、胃泌素与胰高血糖素水平,治疗有效率高,安全性高。 展开更多

在全球气候变化背景下,乡村气候环境也受到了一定的影响,为了应对这些变化,本文研究了乡村人群对气候变化的感知情况,从而为定边县农户和政府更好地应对气候变化提供参考依据。通过实地调研和入户访谈的方式了解当地农户对近10年定边县... 在全球气候变化背景下,乡村气候环境也受到了一定的影响,为了应对这些变化,本文研究了乡村人群对气候变化的感知情况,从而为定边县农户和政府更好地应对气候变化提供参考依据。通过实地调研和入户访谈的方式了解当地农户对近10年定边县的气候变化感知情况,与当地2005~2014年的气象数据进行对比分析,实现定性与定量相结合,分析近10年来人们感知与科学观测的偏差以及不同年龄人群对气候变化感知强度与平均强度的偏差。结果表明:乡村人群对气候变化的感知大体上与科学监测的数据相符,但是在一些方面还存在着一定的偏差;只有某个年龄段人群的感知强度与平均感知强度接近,其余均存在较大偏差。 展开更多

Software-Defined Networking(SDN)adapts logically-centralized control by decoupling control plane from data plane and provides the efficient use of network resources.However,due to the limitation of traditional routing strategies relying on manual configuration,SDN may suffer from link congestion and inefficient bandwidth allocation among flows,which could degrade network performance significantly.In this paper,we propose EARS,an intelligence-driven experiential network architecture for automatic routing.EARS adapts deep reinforcement learning(DRL)to simulate the human methods of learning experiential knowledge,employs the closed-loop network control mechanism incorporating with network monitoring technologies to realize the interaction with network environment.The proposed EARS can learn to make better control decision from its own experience by interacting with network environment and optimize the network intelligently by adjusting services and resources offered based on network requirements and environmental conditions.Under the network architecture,we design the network utility function with throughput and delay awareness,differentiate flows based on their size characteristics,and design a DDPGbased automatic routing algorithm as DRL decision brain to find the near-optimal paths for mice and elephant flows.To validate the network architecture,we implement it on a real network environment.Extensive simulation results show that EARS significantly improve the network throughput and reduces the average packet delay in comparison with baseline schemes(e.g.OSPF,ECMP). 展开更多

AIM:To evaluate a novel biosensor-based microarray(BBM) assay for detecting rs12979860 and rs8099917 genotypes.METHODS:Four probes specific for rs8099917C/T or rs12979860G/T detection and three sets of quality control probes were designed,constructed and arrayed on an optical biosensor to develop a microarray assay.Two sets of primers were used in a one tube polymerase chain reaction(PCR) system to amplify two target fragments simultaneously.The biosensor microarray contained probes that had been sequenced to confirm that they included the rs8099917C/T or rs12979860G/T alleles of interest and could serve as the specific assay standards.In addition to rehybridization of four probes of known sequence,a total of 40 clinical samples collected from hepatitis C seropositive patients were also tested.The target fragments of all 40 samples were amplified in a 50 μL PCR system.Ten μL of each amplicon was tested by BBM assay,and another 40 μL was used for sequencing.The agreement of the results obtained by the two methods was tested statistically using the kappa coefficient.The sensitivity of the BBM assay was evaluated using serial dilutions of ten clinical blood samples containing 10 3-10 4 white cells/μL.RESULTS:As shown by polyacrylamide gel electrophoresis,two target segments of the interleukin 28Bassociated polymorphisms(SNPs) were successfully amplified in the one-tube PCR system.The lengths of the two amplified fragments were consistent with the known length of the target sequences,137 and 159 bps.After hybridization of the PCR amplicons with the probes located on the BBM array,the signals of each allele of both the rs8099917 SNPs and rs12979860 SNPs were observed simultaneously and were clearly visible by the unaided eye.The signals were distinct from each other,could be interpreted visually,and accurately recorded using an ordinary digital camera.To evaluate the specificity of the assay,both the plasmids and clinical samples were applied to the microarray.First,30 PCR amplicons of the various SNP alleles were hybridized on the BBM microarray.Full agreement between plasmids and the BBM assay was observed,with 30/30 correct matches(100%).The kappa value for the BBM assay with plasmids was 1.00(P < 0.05).For the 40 clinical blood samples,the BBM assay hybridization and direct sequencing results were compared for each amplicon.For patient blood samples,agreement was 28/28 for rs8099917T/T,9/11 for rs8099917T/G,1/1 for rs8099917G/G,24/24 for rs12979860C/C,11/14 for rs12979860C/T,and 2/2 for rs12979860T/T.Only five clinical samples of amplicon assay and direct sequencing results were discordant and heterozygotes:2/11 rs8099917T/G and 3/14 rs12979860C/T.The agreement of outcomes between BBM assay and direct sequencing for the detection of rs8099917 and rs12979860 was 95% and 92.5%,respectively;and the corresponding kappa values were 0.88 and 0.85(A kappa value > 0.75 was defined as substantial agreement).The BBM assay and sequencing had similar specificities for detection and identification of the two SNPs and their alleles.The sensitivity evaluation showed that the BBM assay could detect and identify SNP sequences present in blood samples containing as few as 10 2 white blood cells/μL.CONCLUSION:This biosensor microarray assay was highly specific,sensitive,rapid and easy to perform.It is compatible with clinical practice for detection of rs8099917 and rs12979860. 展开更多

Fourier transform infr... The 3D orthogonal woven basalt fiber reinforced polyimide (PI) composites were fabricated and characterized in this study. The PI film was firstly prepared to determine PI processing parameters. Fourier transform infrared (FTIR) analysis showed that 300°C was the suitable imidization temperature. Thermal gravimetric analysis (TGA) and differential scanning calorimetry (DSC) results showed relatively good thermal properties of the PI film. In the fabrication of composites, the multi-step impregnation method was applied. The bending properties of 3 mm-thick composite showed increasing trend in all and the second-time impregnated composite had much higher value than the first-time impregnated composite. Moreover, the bending fracture mode photos showed obvious creases except for the first-time impregnated materials, which agreed well with the bending property values. The dielectric constants for the composites were complex because they had not regular value following the mixing rule of the composites, which was mainly due to the interfacial polarization and other effects in the fabrication processing. 展开更多

We studied relations between natural seedling reproduction and above ground environment in a longleaf pine ecosystem. Forty-eight 0.05 ha circular plots were sampled under single-tree selection, group-tree selection and control stands in three main longleaf pine areas in south Alabama, USA. We measured six above-ground environment factors, viz. canopy closure, stand density, basal area, average tree height, understory cover and PAR under canopy. We employed forward, back-ward and stepwise selection regression to produce one model. Three main variables:canopy closure, stand density and basal area, were left in the model; light, PAR and understory cover were not incorporated into the model at the 0.10 significance level. Basal area was a positive pa-rameter, while canopy closure and stand density were negative parame-ters. Canopy closure was the main parameter in the model. The model proved to be meaningful, and has potential to provide useful guidance for future work. 展开更多

Reproductive health(RH) education and services of female migrants in China have become an important health issue.This research aimed to investigate the RH knowledge and utilization among married female migrants,and to explore the influencing factors from the perspectives of population and sociology.We conducted a cross-section survey in Shenzhen and Wuhan,China,using the purposive sampling method.A total of 1021 rural-to-urban married migrants were recruited,with 997 valid survey results obtained.A face-to-face structured questionnaire survey was used,with primary focus on knowledge of fertility,contraception,family planning policy and sexual transmitted diseases/acquired immunodeficiency syndrome(STD/AIDs),and RH service utilization.The results showed that the RH service utilization(38.0%) was at a low level in married migrants and the accessibility of RH service was poor.Females who migrated to(OR=0.32) Wuhan obtained fewer RH consultations than those in Shenzhen.The workers with high school education received additional RH consultations and checkup services than those with other background education,apart from the white collar workers who received extra RH consultations and checkup services than the blue collar workers(P〈0.05).We can draw a conclusion that the utilization of RH services in married female migrants remains at a low level in China.RH service utilization can be improved via the relevant health departments by enhancing the responsibility of maternal and health care in the community health service center. 展开更多
