Effects of Lactobacillus paracasei N1115 on gut microbial imbalance and liver function in patients with hepatitis B-related cirrhosis

BACKGROUND Hepatitis B cirrhosis(HBC)is a chronic disease characterized by irreversible diffuse liver damage and aggravated by intestinal microbial imbalance and metabolic dysfunction.Although the relationship between certain single probiotics and HBC has been explored,the impact of the complex ready-to-eat Lactobacillus paracasei N1115(LP N1115)supplement on patients with HBC has not been determined.AIM To compare the changes in the microbiota,inflammatory factor levels,and liver function before and after probiotic treatment in HBC patients.METHODS This study included 160 HBC patients diagnosed at the General Hospital of Ningxia Medical University between October 2018 and December 2020.Patients were randomly divided into an intervention group that received LP N1115 supplementation and routine treatment and a control group that received routine treatment only.Fecal samples were collected at the onset and conclusion of the 12-wk intervention period.The structure of the intestinal microbiota and the levels of serological indicators,such as liver function and inflammatory factors,were assessed.RESULTS Following LP N1115 intervention,the intestinal microbial diversity significantly increased in the intervention group(P<0.05),and the structure of the intestinal microbiota was characterized by an increase in the proportions of probiotic microbes and a reduction in harmful bacteria.Additionally,the intervention group demonstrated notable improvements in liver function indices and significantly lower levels of inflammatory factors(P<0.05).CONCLUSION LP N1115 is a promising treatment for ameliorating intestinal microbial imbalance in HBC patients by modulating the structure of the intestinal microbiota,improving liver function,and reducing inflammatory factor levels. 展开更多

AIM To investigate the value of the gamma-glutamyltraspeptidase(GGT)-to-platelet(PLT) ratio(GPR) in the diagnosis of hepatic fibrosis in patients with chronic hepatitis B(CHB). METHODS We included 390 untreated CHB patients in this study. The GPR, aspartate aminotransferase(AST)-to-PLT ratio index(APRI), and fibrosis-4(FIB-4) of all patients were analysed to determine if these parameter were correlated with age, gender, medical history, liver function [total bilirubin(TBil), alanine aminotransferase(ALT), and AST], GGT, PLT count, or hepatic fibrosis stage. The GPR, APRI, and FIB-4, as well as the combination of the GPR and APRI or the GPR and FIB-4 were assessed in different cirrhosis stages using receiver operating characteristic(ROC) curve analysis to evaluate their value in diagnosing hepatic fibrosis in CHB patients. RESULTS The GPR, APRI, and FIB-4 were not correlated withCHB patients' age, gender, or disease duration(P > 0.05), but all of these parameters were positively correlated with serum ALT, AST, GGT, and PLT count(P < 0.01). Additionally, the GPR, APRI, and FIB-4 were positively correlated with hepatic fibrosis(P < 0.01); the areas under the ROC curve for the GPR in F1, F2, F3, and F4 stages were 0.723, 0.741, 0.826, and 0.833, respectively, which were significantly higher than the respective values for the FIB-4 and APRI(F1: 0.581, 0.612; F2: 0.706, 0.711; F3: 0.73, 0.751; and F4: 0.799, 0.778). The respective diagnostic cut-off points for each stage were 0.402, 0.448, 0.548, and 0.833, respectively. The diagnostic sensitivity and specificity were, respectively, 88.8% and 87.5% in F1, 72.7% and 89.7% in F2, 81.3% and 98.6% in F3, and 80% and 97.4% in F4 when the GPR and APRI were connected in parallel; 86.6% and 90.2%, 78.4% and 96%, 78.6% and 97.4%, and 73.2% and 97.9%, respectively, when the GPR and APRI were connected in series; 80.2% and 89%, 65% and 89%, 70.3% and 98.5%, and 78.8% and 96.8%, respectively, when the GPR and FIB-4 were connected in parallel; and 83.6% and 87.9%, 76.8% and 96.6%, 72.7% and 98%, and 74.4% and 97.7%, respectively, when the GPR and FIB-4 were connected in series.CONCLUSION The GPR, as a serum diagnostic index of liver fibrosis, is more accurate, sensitive, and easy to use than the FIB-4 and APRI, and the GPR can significantly improve the sensitivity and specificity of hepatic fibrosis diagnosis in CHB when combined with the FIB-4 or APRI. 展开更多

Objective:To observe the effect of Jiuyi powder and its active components on the bacterial culture and macrophage phenotypic factors of the chronic refractory wound rat model,and to explore its mechanism of removing decay and promoting muscle growth.Methods:SD rats were divided into control group,Jiuyi powder group,Shengdan group,and calcined gypsum group,with 8 rats in each group.MRSA-infected skin lesions and wounds were used to build a model of chronic and difficult-to-heal wounds in rats.After the model was formed,the control group was treated with daily routine nitrofural disinfection and replaced with sterile gauze.On the basis of the control group,quantitative Jiuyi powder,Shengdan powder,and calcined gypsum powder were used for dressing change,once a day for 7 consecutive days.Before and after the last administration,collect rat wound secretions for bacterial culture,inducible monoxide nitrogen synthase content.At the same time,after the last administration,the rat wound tissue was excised for histopathology and immunofluorescence double staining to label macrophages and their M1 phenotype.Results:After the last dressing change,the wound healing of Jiuyi powder group was better than the other groups,and the wound healing rate of each group had significant difference(P<0.05).The histomology showed that the inflammation of Jiuyi powder group was controlled and had a healing trend.After the last drug change,the contents of TNF-α,IL-6 and iNOS in serum of all groups decreased,and the contents of IL-6,TNF-αand iNOS in serum of Jiuyi powder group decreased significantly before and after medication(P<0.05).There was statistical significance in serum IL-6 content between calcined gypsum group and Shengdan group before and after medication(P<0.05).In addition,the results showed that the contents of IL-6 and iNOS in serum of Jiuyi powder group were statistically different from those of the control group(P<0.05).Tissue immunofluorescence double staining showed that the positive rate of M1 macrophages in Jiuyi powder group and Shengdan group was significantly lower than that in control group(P<0.05).The MRSA negative conversion rate of Jiuyi powder group and Shengdan group was better than that of the control group and calcined gypsum group(P<0.05).Conclusion:Jiuyi powder can improve the inflammation of chronic refractory wounds,and has antibacterial,anti-corrosion and myogenic effects.Its mechanism may be related to the inhibition of macrophage M1 phenotype polarization. 展开更多

And they were divided into bleeding group 48 cases and hemorrhage transformation group 48 cases according to whether with hemorrhage occurs transformation. Then the lipid metabolism, atrial fibrillation, history of smoking and drinking, history of hypertension and diabetes, blood pressure, treatment time after onset and infarction area of two groups were compared, and the relationship between those factors and the disease were analyzed by the multi-factor Logistic regression analysis.Results: The atrial fibrillation, history of smoking and drinking of two groups had significant differences;The hospital fasting plasma glucose and LDL-C level of two groups had significant differences;the treatment time after onset and infarction area of two groups had significant differences;The multi-factor Logistic regression analysis showed that atrial fibrillation, blood glucose on admission, LDL-C and large area of infarction are the factors affecting the risk of bleeding in patients with acute cerebral infarction transformation.Conclusion:Atrial fibrillation, blood glucose on admission, LDL-C, treatment time after onset and large area of infarction belongs to the patients with acute cerebral infarction after the thrombolysis transformation of bleeding risk factors. 展开更多

The effect of rolling reduction and annealing process on the microstructure and corrosion behavior of Mg-9Li-1Zn(LZ91) alloy was investigated. The test alloy sheets were cold rolled with the reduction of 50% and 75%, respectively, and then were annealed at 200 ℃ for 1 h. The microstructure of test alloys was observed by OM and SEM while the phase composition was determined by XRD. The corrosion property was evaluated by electrochemical measurements and immersion tests. The results show that LZ91 alloy sheet consists of α-Mg, β-Li and precipitated Mg-Li-Zn compounds(MgLi2Zn and MgLiZn phases). Dynamic recrystallization grains appear in β-Li phase during annealing process, leading to grain refinement. The results indicate that the increasing rolling reduction and performing the annealing process can enhance the corrosion resistance of LZ91 alloy. The 75% cold-rolled and annealed LZ91 alloy shows the best corrosion resistance. 展开更多

目的:观察配戴夜戴型角膜塑形镜不同时间对睑板腺及睑缘蠕形螨检出率的影响。方法:病例对照研究。选取2019-06/12我院配戴夜戴型角膜塑形镜患者47例91眼,分为短期组(配戴时间<2a)及长期组(配戴时间≥2a),同时选取性别年龄及屈光度相... 目的:观察配戴夜戴型角膜塑形镜不同时间对睑板腺及睑缘蠕形螨检出率的影响。方法:病例对照研究。选取2019-06/12我院配戴夜戴型角膜塑形镜患者47例91眼,分为短期组(配戴时间<2a)及长期组(配戴时间≥2a),同时选取性别年龄及屈光度相匹配的配戴框架镜患者为对照组。三组均行泪膜破裂时间检查、睑板腺红外拍照及结膜眼红评分,并行睫毛采样。结果:短期组,长期组及对照组泪膜破裂时间分别为12.68±4.59、12.37±4.97、10.45±6.04s(P>0.05)。三组球结膜眼红评分分别为0.99±0.41、0.90±0.37、0.84±0.35分(P>0.05)。三组睑板腺评分均为1(0,1)(P>0.05)。三组蠕形螨检出率及检出量分别为42%,1(1,2)个;39%,2(1,2)个;33%,1(1,2)个,无明显差异(均P>0.05)。角膜点染的患者中,螨虫阳性5眼,螨虫阴性11眼(P>0.05);睑板腺评分0分的4眼,1分以上12眼(P>0.05);短期组6眼,长期组10眼(P>0.05)。结论:配戴夜戴型角膜塑形镜对患者睑板腺及睑缘蠕形螨检出率、检出数量未见明显影响,睑板腺评分及蠕形螨检出率与角膜点染无明显关系。 展开更多

BACKGROUND Metabolic syndrome(MetS)is related to poor cognitive function.However,the results of previous studies were inconsistent,and whether the ApoEε4 allele modifies the association remains unclear.AIM To elucidate the relationships among MetS,ApoEε4,and cognitive dysfunction in an elderly population in China.METHODS One hundred elderly patients with MetS and 102 age-and gender-matched controls were included in the study.Baseline clinical characteristics and biochemical index for glucose and lipid metabolism were obtained.The distribution of ApoEε4 was assessed with PCR restriction fragment length polymorphism analysis.Cognitive function was evaluated by mini-mental status examination at the 1-year follow-up examination.RESULTS Compared with controls,MetS patients had worse cognitive function and decreased ability to participate in activities of daily life(P=0.001 and 0.046,respectively).Patients with cognitive dysfunction had higher prevalence of MetS(62.1%vs 36.4%,P<0.001)and were more likely to carry the ApoEε4 allele(22.3%vs 10.1%,P=0.019).Multivariate logistic regression analyses showed that diagnosis with MetS,severe insulin resistance,status as an ApoEε4 carrier,higher systolic blood pressure,and larger waist circumference were risk factors for cognitive dysfunction(P<0.05).Repeated-measures analysis of variance,performed with data collected at the 1-year follow-up,revealed continuous influences of MetS and ApoEε4 on the deterioration of cognitive function(time×team,P<0.001 for both).CONCLUSION Diagnosis of MetS and ApoEε4 carrier status were persistently associated with cognitive dysfunction among an elderly population in China. 展开更多

The as-rolled AZ31 Mg alloy sheets are welded by friction stir welding(FSW) in this study, followed by annealing and hot rolling with different reductions. The effects of welding speed and the rolling reduction on the microstructure, tensile properties, microhardness and fracture surfaces of the specimens are investigated. The results show that the microstructures of the FSW joint in different regions become more and more consistent as the hot rolling reduction increases. The mechanical properties of the FSW joints and the joint coefficient are improved with increasing rolling reduction; however,the hardness value in the stirred zone decreases firstly and then increases. When the rolling reduction is 50%, the tensile strength of FSW joint is close to that of base metal. The sample L_3 achieves the best comprehensive properties, with the yield strength, ultimate tensile strength and elongation of 162, 287.9 MPa, and 17.9%, respectively. 展开更多

Background and Aims:Studies have indicated that serum von Willebrand factor(vWF)has a positive correlation with hepatic venous pressure gradient.However,information on the value of vWF in the diagnosis of liver cirrhosis with portal hypertension has been lacking.The purpose of this meta-analysis was to assess the value of vWF in the diagnosis of liver cirrhosis with portal hypertension.Methods:Studies that analyzed the sensitivity,specificity,diagnostic odds ratio combined with likelihood ratios and test for heterogeneity of vWF in the diagnosis of liver cirrhosis with portal hypertension were found in the Cochrane Library,Ovid,VOS-SCI,CNKI,PubMed,Medline,EMBASE,CMB and Wanfang databases.In the end,the data was used to draw the summary receiver operating characteristic curve and to calculate the area under the curve.Results:Four studies involving 662 patients were analyzed.The results showed that serum vWF in liver cirrhosis with portal hypertension were significantly higher than in those without portal hypertension.Sensitivity combined was 0.823(95%CI:0.788,0.855).Specificity combined was 0.782(95%CI:0.708,0.845).+LR combined was 3.777(95%CI:2.794,5.107).-LR combined was 0.221(95%CI:0.180,0.272).Diagnostic odds ratio combined was 18.347(95%CI:11.725,28.708).The area under the curve was 0.8896.Conclusions:Serum vWF can be used as an effective and feasible method for noninvasive diagnosis of liver cirrhosis with portal hypertension.However,further studies are still needed to evaluate the severity of liver cirrhosis with portal hypertension. 展开更多
