This editorial is stimulated by the article by Alqifari et al published in the World Journal of Diabetes(2024).Alqifari et al focus on practical advice for the clinical use of glucagon-like-peptide-1(GLP-1)receptor agonists(GLP-1RAs)in the management of type 2 diabetes and this editorial provides complementary information.We initially give a brief historical perspective of the development of GLP-1RAs stimulated by recognition of the‘incretin effect’,the substantially greater insulin increase to enteral when compared to euglycaemic intravenous glucose,and the identification of the incretin hormones,GIP and GLP-1.In addition to stimulating insulin,GLP-1 reduces postprandial glucose levels by slowing gastric emptying.GLP-1RAs were developed because native GLP-1 has a very short plasma half-life.The majority of current GLP-1RAs are administered by subcutaneous injection once a week.They are potent in glucose lowering without leading to hypoglycaemia,stimulate weight loss in obese individuals and lead to cardiovascular and renal protection.The landscape in relation to GLP-1RAs is broadening rapidly,with different formulations and their combination with other peptides to facilitate both glucose lowering and weight loss.There is a need for more information relating to the effects of GLP-1RAs to induce gastrointestinal symptoms and slow gastric emptying which is likely to allow their use to become more effective and personalised. 展开更多

Diabetes and associated complications represent major global public health issues which are associated with impaired quality of life and premature death.Although some diabetic complications have decreased in the developed world,the majority are still prevalent,with an increasing trend in the developing world.Currently used therapies are mainly‘glucocentric’,focusing on the optimization of glycemic control to prevent,delay or manage diabetes-associated complications-other common comorbidities,such as dyslipidemia and hypertension are often underestimated.Although a number of novel therapeutic approaches have been reported recently,some of them have not received comparable attention in relation to either further studies or potential clinical implementation.This editorial briefly discusses some recent therapeutic approaches to the prevention and management of diabetes and its associated complications,as well as potential directions for future research and development in this area. 展开更多

Uncontrolled hyperglycemia or poorly managed disease increases the propensityfor a number of diabetes-related complications targeting major organs includingthe heart, eyes, and kidney. Although the mechanisms by which diabetes inducescardiovascular diseases include oxidative stress and inflammation, when insulinresistance remains the key to the pathogenesis, as implicated in the two reviews inthis issue. This editorial mainly comments on the potential preventive applicationof glycyrrhetinic acid (or 18β-GA) in relation to diabetic nephropathy. The therapeuticor preventive effects of 18β-GA, as a hydrolytic product of glycy-rrhizicacid that is a component of licorice, have been appreciated in other disorders, buthave received much less attention in relation to diabetic complications. A study inthis issue has identified 18β-GA as a therapeutic for preventing diabeticnephropathy and provides evidence to support efficacy in cultured human renaltubule cells in vitro. Although it represents a pilot study, the observations supporta new therapeutic approach that warrants further ex-ploration. 展开更多

AIM: To study the possible causes of sorbitol (S)-based diarrhea and its mechanism of reduction by rice gruel (RG) in cecectomized rats. METHODS: S was dissolved either in distilled water or in RG (50 g/L) and ingested as a single oral dose (1.2 g/kg body mass, containing 0.5 g/L phenol red as a re-covery marker) by S (control) and S + RG groups (n = 7), respectively. This dose is over the laxative dose for hu-mans. Animals were sacrificed exactly 1 h after dose in-gestion, without any access to drinking water. The whole gastro-intestinal tract was divided into seven segments and sampled to analyze the S and marker remaining in its contents.RESULTS: Gastric-emptying and intestinal transit were comparatively slower in the S + RG group. Also, the S absorption index in the 3rd and last quarter of the small intestine (24.85 ± 18.88% vs 0.0 ± 0.0% and 39.09 ± 32.75% vs 0.0 ± 0.0%, respectively, P < 0.05) was sig-nificantly higher in the S + RG group than in the control group. The S absorption index and the intestinal fluid volume are inversely related to each other. CONCLUSION: The intestinal mal-absorption of S is the main reason for S-based osmotic diarrhea. Where RG en-hanced the absorption of S through passive diffusion, the degree of diarrhea was reduced in cecectomized rats. 展开更多

Objective:To investigate the antilithiatic effect of the whole Leea macrophylla(L macrophylla) Roxb(Leeaeeae) ethanol extract in ediylene glycol-induced urolithiasis model of rats.Methods: Forty two seven weeks old male wistar albino rats were randomly divided into two major groups namely:preventive(n=18) and therapeutic(n=24).Preventive group was further subdivided into 3 groups of 6 rats namely:preventive control(PC),preventive lithiatic control(PLC) and preventive lithiatic L.macrophylla(PLLM).Similarly,therapeutic group was also subdivided into 4 groups of 6 rats namely:therapeutic control(TC),therapeutic lithiatic control(TLC).therapeutic lithitatic L macrophylla(TLLM) and therapeutic lithiatic cystone(TLCYS).The lithiasis was induced by 0.75%(v/v) ethylene glycol in the drinking water of all groups except the PC and TC groups.The urinary ionic parameters such as calcium,inorganic phosphate,oxalate,magnesium & creatinine and renal morphology were altered by ediylene glycol,which were partially recovered by 14 d preventive and almost fully recovered by 28 d therapeutic intervention trials with L macrophylla extract(500 mg/kg BW daily).Results:Significant difference on recovery was observed between preventive and dierapeutic interventional trials.Anti-urolithiatic effect of cystone was significantly(P【0.001) higher than extracts.L macrophylla extract was found nontoxic in the acute toxicity test.Conclusions:The results of this study demonstrated very promising antiurolithiatic effect of L.macrophylla extract with preventive and therapeutic treatments in this experimental condition. 展开更多

The 2021 online editorial board meeting of the World Journal of Diabetes(WJD)was held on November 9,2021.Jin-Lei Wang,General Manager on behalf of the Baishideng Publishing Group,and Professor Islam,one of the Editors-in-Chiefs(EiCs)of the WJD,organized the meeting.Three EiCs and 18 Baishideng Publishing Group staff attended the meeting.The meeting goal was to brief the EiCs on the journal’s performance,discuss the issues of concern of the EiCs,and gather ideas for the journal’s development in 2022.As of November 8,the WJD had received 287 manuscripts since the year’s start,among which 122 met the criteria for publication.These numbers represent an increase of 117.4%for submissions and 110.3%for publications compared to those in 2020.However,how to effectively control the academic quality of manuscripts and attract highquality original article submissions remain a challenge.The EiCs provided feedback and suggestions centered on three topics:(1)Who should and how to control the academic quality of the manuscripts;(2)How the EiCs perform their responsibilities;and(3)The distinctive and shared responsibilities of the publisher and the EiCs. 展开更多

The aim of the study was to develop protein enriched instant fish soup mix and analysis of the quality of various conditions. The study used a mechanical dryer for maintaining a constant air flow and temperature at its drying and dehydration stage. Powder was made after drying. An appropriate composition was used as an indicator. Various compositions like 75%, 65%, 55% and 45% fish powder were used as formulation 0, formulation 1, formulation 2, and formulation 3 respectively. Then these were reconstituted by boiled water for soup mix preparation. The result indicated that mixer 2 was the best composition. The finding suggested that 55% fish powder mix is the best composition for protein enriched instant soup mix. It will help to produce the instant soup mix and an alternative source of various costly animal and plant protein as a value-added material. 展开更多

Reproductive stage frost poses a major constraint for wheat production in countries such as Australia.However,little progress has been made in identifying key genes to overcome the constraint.In the present study,a severe frost event hit two large-scale field trials consisting of six doubled haploid(DH)wheat populations at reproductive stage(young microspore stage)in Western Australia,leading to the identification of 30 robust frost QTL on 17 chromosomes.The major 18 QTL with the phenotype variation over 9.5%were located on 13 chromosomes including 2 A,2 B,2 D,3 A,4 A,4 B,4 D,5 A,5 D,6 D,7 A,7 B and7 D.Most frost QTL were closely linked to the QTL of anthesis,maturity,Zadok stages as well as linked to anthesis related genes.Out of those,six QTL were repetitively detected on the homologous regions on 2 B,4 B,4 D,5 A,5 D,7 A in more than two populations.Results showed that the frost damage is associated with alleles of Vrn-A1 a,Vrn-D1 a,Rht-B1 b,Rht-D1 b,and the high copy number of Ppd-B1.However,anthesis QTL and anthesis related genes of Vrn-B1 a and Ta FT3-1 B on chromosomes 5 B and 1 B did not lead to frost damage,indicating that these early-flowering phenotype related genes are compatible with frost tolerance and thus can be utilised in breeding.Our results also indicate that wild-type alleles Rht-B1 a and Rht-D1 a can be used when breeding for frost-tolerant varieties without delaying flowering time. 展开更多

Novel coronavirus,SARS-CoV-2 is responsible for causing a pandemic that has affected individuals worldwide,over 192 million people and about 4.1 million people died so far.The spread is ongoing and the numbers are still increasing.Numerous therapeutic approaches have been explored and developed during this pandemic.Immunotherapy with virus-specific antibodies in convalescent plasma(CP)has shown potential benefits for various pathogenic diseases.In many instances,it is the only available and safe management option for the COVID-19 patients.Here we describe two confirmed cases of COVID-19 from two different geographical areas that were managed with standard treatment modalities initially.Both of the patients were presented with high-grade fever,dry cough,and sore throat.Lab reports showed increased values of D-dimer,serum ferritin,leukocyte count(LC),Lactate dehydrogenase(LDH),and C-reactive protein(CRP).Chest X-ray showed bilateral infiltration(multifocal and bilateral ground-glass opacities and consolidations with peripheral and basal predominance),consistent with the previous reports on COVID-19 infection.The patients received conservative treatment according to the hospital's protocol.The convalescent plasma(from recovered patients)infusion was the last treatment given to both patients.After the convalescent plasma transfusion,both patients showed a reduction of viral load,an increase of anti-SARS-CoV-2 IgG and IgM antibodies,reduction in lung infiltration,with no adverse events.However,further randomized controlled trials are needed to investigate the full scope of safety and efficacy(both short and long-term)of convalescent plasma therapy for COVID-19 and other related infections. 展开更多

Background:Besides maintaining health precautions,vaccination has been the only prevention from SARS-CoV-2,though no clinically proved 100%effective vaccine has been developed till date.At this stage,to withhold the debris of this pandemic-experts need to know the impact of the vaccine efficacy rates,the threshold level of vaccine effectiveness and how long this pandemic may extent with vaccines that have different efficacy rates.In this article,a mathematical model study has been done on the importance of vaccination and vaccine efficiency rate during an ongoing pandemic.Methods:We simulated a five compartment mathematical model to analyze the pandemic scenario in both California,and whole U.S.We considered four vaccines,Pfizer(95%),Moderna(94%),AstraZeneca(79%),and Johnson&Johnson(72%),which are being used rigorously to control the SARS-CoV-2 pandemic,in addition with two special cases:a vaccine with 100%efficacy rate and no vaccine under use.SARS-CoV-2 related data of California,and U.S.were used in this study.Findings:Both the infection and death rates are very high in California.Our model suggests that the pandemic situation in California will be under control in the last quartile of the year 2023 if vaccination program is continued with the Pfizer vaccine.During this time,six waves may happen from the beginning of the immunization where the case fatality and recovery rates will be 1.697%and 98.30%,respectively.However,according to the considered model,this period might be extended to the mid of 2024 when vaccines with lower efficacy rates are used.On the other hand,the daily cases and deaths in the U.S.will be under control at the end of 2026 with multiple waves.Although the number of susceptible people will fall down to none in the beginning of 2027,there is less chance to stop the vaccination program if vaccinated with a vaccine other than a 100%effective vaccine or Pfizer,and at that case vaccination program must run till the mid of 2028.According to this study,the unconfirmed-infectious and infected cases will be under control at the end of 2027 and at the mid of 2028,respectively.Interpretation:The more effective a vaccine is,the less people suffer from this malign infection.Vaccines which are less than 90%effective do not have notable contribution to control the pandemic besides hard immunity.Furthermore,specific groups of people are getting prioritized initially,mass vaccination and quick responses are required to control the spread of this disease. 展开更多
