BACKGROUND Acute leg compartment syndrome is a well-known orthopedic emergency associated with potentially devastating consequences if not treated immediately.Multiple compartments are usually involved with a clear hi...BACKGROUND Acute leg compartment syndrome is a well-known orthopedic emergency associated with potentially devastating consequences if not treated immediately.Multiple compartments are usually involved with a clear history of trauma and classic symptoms and signs.However,isolated lateral leg compartment syndrome is relatively rare and is often misdiagnosed due to the atypical presentation of no trauma and the lack of pathognomonic signs.CASE SUMMARY A 31-year-old male patient presented to our emergency room with excruciating left calf pain and inability to mobilize one-day after participating in a football match despite no clear history of preceding trauma.The patient went to another hospital before presenting to us where he was diagnosed to have a soft tissue injury and was discharged home on simple analgesics.On clinical examination,the left leg showed a tense lateral compartment with severe tenderness.The pain was aggravated by dorsiflexion and ankle inversion.Neurovascular examination of the limb was normal.We suspected a compartment syndrome but as the presentation was atypical and an magnetic resonance imaging(MRI)was readily available in our institution,we immediately performed an MRI and this confirmed a large hematoma in the lateral compartment with a possible partial proximal peroneus longus muscle tear.The patient was taken immediately for an emergency open fasciotomy.The patient is now 18 mo postoperatively having recovered completely and engages fully in sports with no restrictions.CONCLUSION Atypical presentation due to the lack of pathognomonic signs makes the diagnosis of isolated lateral leg compartment syndrome difficult.Pain on passive inversion and dorsiflexion and weak active eversion may be suggested as sensitive signs.展开更多

Whether these developmental changes rela...Previous research has shown that smooth muscle of the stomach undergoes developmental changes in the intracellular regulatory mechanism responsible for the contractile process. Whether these developmental changes relate to differences in the expression and/or activity of the key enzymes regulating smooth muscle contraction has not been previously evaluated. Therefore, we aimed to examine the expression and activation of the small monomeric G protein "Rho A" and Rho kinase(ROCK) as well as their correlation with the contraction of gastric smooth muscle cells(GSMCs) in newborn vs. adult rats. Freshly isolated single GSMCs from Sprague-Dawley rats at 1 week(newborn) and 3 months(adult) of age were used in the study. Protein and m RNA expression levels of both ROCK2 and total Rho A were higher in adult compared to newborn rats. Moreover, acetylcholine(ACh)-induced contractions of GSMCs in adult rats were significantly higher than that in newborn animals. Meanwhile,ROCK and Rho activation was higher in adult stomach cells compared to newborn ones. Pretreatment of GSMCs with Y-27632, the ROCK inhibitor, significantly reduced ACh-induced contraction in both groups of cells and greatly abolished contractile differences. In conclusion, our results indicate that Rho A/ROCK pathway and contraction of stomach muscle cells are under developmental regulation.展开更多
Carbonate rocks contain prolific hydrocarbon reserves all over the world,particularly in the Middle East.For exploration and production strategies,it is essential to understand carbonate reservoirs in terms of their i...Carbonate rocks contain prolific hydrocarbon reserves all over the world,particularly in the Middle East.For exploration and production strategies,it is essential to understand carbonate reservoirs in terms of their internal characteristics,depositional environment,relative age,diagenetic processes and impact on petrophysical properties.This study has been performed on exposed Cretaceous,Paleocene and Miocene marine carbonate sedimentary sequences in two localities(Maghdoucheh and Qennarit)near the city of Sidon(Southern Lebanon).It represents the first comprehensive study that takes into consideration the carbonate reservoir facies,diagenetic history and reservoir quality in the area.Rocks at Maghdoucheh are mainly dominated by limestone beds showing sedimentary structures and erosive bases alternating with microfossil-rich silty marls,related to a shallowing upward sequence in a restricted marine platform environment.Shells of benthic foraminifera and mollusks dominate the fossil assemblage extracted from the studied rocks.The microfossil and nannofossil assemblage detected in the Maghdoucheh sections indicates a middle Miocene age.Rocks at Qennarit are composed of mudstone/wackestone limestone beds rich in planktonic foraminifera and nannofossils related to open marine conditions.Based on the nannofossil content,rocks from Qennarit 1 and 2 are Paleocene and Cretaceous in age,respectively.Four main types of microfacies have been identified,i.e.(1)microbioclastic peloidal calcisiltite,(2)pelagic lime mudstone and wackestone with planktonic microfossils,(3)grainstone/packstone with abundant foraminifera and(4)fenestral bindstones,mudstones and packstones with porostromate microstructures.The porosity-permeability(poro-perm)analysis of representative samples reveals moderate to good porosity but very low permeability.This is mainly due to the presence of large moldic pores that are isolated in nature.The diagenetic features are dominated by micritization and dissolution(both fabric selective and non-fabric selective).Among all diagenetic features,dissolution in both localities contributes to porosity enhancement,while micritization,cementation,compaction and the filled fractures have negative impacts on permeability;hence the overall reservoir quality.展开更多
