肉芽肿性小叶性乳腺炎(granulomatous lobular mastitis,GLM)是一种临床少见疾病,发病率逐年上升,但病因及发病机制尚不明确。其临床表现多样,组织病理学特征性表现为以乳腺小叶单位为中心的非干酪样肉芽肿病变。治疗手段包括随访观察... 肉芽肿性小叶性乳腺炎(granulomatous lobular mastitis,GLM)是一种临床少见疾病,发病率逐年上升,但病因及发病机制尚不明确。其临床表现多样,组织病理学特征性表现为以乳腺小叶单位为中心的非干酪样肉芽肿病变。治疗手段包括随访观察、药物治疗以及手术,临床上常多种治疗方式联合应用。GLM病程长,易反复发作,迁延不愈,破坏乳房外形,严重影响患者身心健康。在目前的临床实践中,其诊断容易混淆,最佳治疗方式尚未统一。本文对GLM的病因、发病机制、临床表现、诊断以及治疗等内容进行阐述,并对其进行临床分型旨为临床诊疗提供参考。 展开更多

Granulomatous lobular mastitis(GLM) is a rare and chronic benign inflammatory disease of the breast. Granulomatous lobular mastitis(GLM) is a rare and chronic benign inflammatory disease of the breast. Difficulties exist in the management of GLM for many front-line surgeons and medical specialists who care for patients with inflammatory disorders of the breast. This consensus is summarized to establish evidence-based recommendations for the management of GLM. Literature was reviewed using PubMed from January 1, 1971 to July 31, 2020. Sixty-six international experienced multidisciplinary experts from 11 countries or regions were invited to review the evidence.Levels of evidence were determined using the American College of Physicians grading system, and recommendations were discussed until consensus. Experts discussed and concluded 30 recommendations on historical definitions,etiology and predisposing factors, diagnosis criteria, treatment, clinical stages, relapse and recurrence of GLM. GLM was recommended as a widely accepted definition. In addition, this consensus introduced a new clinical stages and management algorithm for GLM to provide individual treatment strategies. In conclusion, diagnosis of GLM depends on a combination of history, clinical manifestations, imaging examinations, laboratory examinations and pathology.The approach to treatment of GLM should be applied according to the different clinical stage of GLM. This evidencebased consensus would be valuable to assist front-line surgeons and medical specialists in the optimal management of GLM. 展开更多

Clearance not only affects the startup torque,rotation precision and stiffness of bearing,but also affects the load distribution,load-carrying capacity and life of bearing.A computational model in which the clearance of bearing is first included is presented for determining the contact force distribution and static load-carrying capacity of a double row four-point contact ball bearing which is subjected to the combined radial,axial and overturning moment loadings.The relation between the negative axial clearance and the contact force distribution is analyzed.The static load-carrying capacity curves are established,and the effects of the changes in negative axial clearance,curvature radius coefficient of raceway groove and initial contact angle on the static load-carrying capacity are analyzed.The results show that,with the increase in the absolute value of negative clearance,the maximum contact load decreases first and then increases.The clearance values in the range of 0.2 mme0 mm have little effect on the static load-carrying capacity of bearing.With the increase in the curvature radius coefficient of raceway groove and the decrease in the initial contact angle,the static load capacity of bearing decreases. 展开更多

Luminal型与Basal-like型乳腺癌差异miRNAs的表达与预后分析 被引量:1

目的分析Luminal型和Basal-like型乳腺癌之间差异表达的microRNAs(miRNAs),探讨其表达水平与Luminal型和Basal-like型乳腺癌预后的关系。方法从基因表达综合数据库(GEO)中下载包含Luminal和Basal-like乳腺癌患者相关的miRNAs微阵列数据... 目的分析Luminal型和Basal-like型乳腺癌之间差异表达的microRNAs(miRNAs),探讨其表达水平与Luminal型和Basal-like型乳腺癌预后的关系。方法从基因表达综合数据库(GEO)中下载包含Luminal和Basal-like乳腺癌患者相关的miRNAs微阵列数据集GSE81000与GSE40267,利用GEO2R在线分析工具筛选Luminal和Basal-like乳腺癌之间差异表达的miRNAs。通过mirDIP数据库预测其靶基因,并采用实时荧光定量PCR(quantitative real-time PCR,qRT-PCR)在人乳腺癌细胞MCF-7和MDA-MB-468中验证miRNAs的差异表达。利用METABRIC数据库,采用KaplanMeier Plotter在线工具分析miR-199a-5p和miR-199b-5p表达水平与Luminal型和Basal-like型乳腺癌预后的关系。结果筛选出了35种在Luminal型和Basal-like型乳腺癌中差异表达的miRNAs。相较于Luminal型,18种miRNAs在Basal-like型中表达下调,17种表达上调。靶基因预测结果显示,35种差异表达miRNAs的潜在靶基因共4180个,其中最相关的靶基因有19个,对应的差异miRNAs为miR-199a-5p、miR-199b-5p。相较于Luminal型乳腺癌,miR-199a-5p和miR-199b-5p在Basal-like型乳腺癌中表达下调。qRT-PCR检测结果表明,miR-199a-5p和miR-199b-5p在MCF-7(Luminal型)和MDA-MB-468(Basal-like型)细胞中的表达水平与GEO2R分析结果趋势一致。生存分析结果表明,miR-199a-5p与miR-199b-5p低表达与Luminal A型乳腺癌患者的总生存率降低显著相关。结论共筛选出35个在Luminal型和Basal-like型乳腺癌中差异表达的miRNAs。其中,miR-199a-5p、miR-199b-5p与乳腺癌的预后相关,有望成为治疗的新靶点。 展开更多

Surface-sensitive measurements are crucial to many types of researches in condensed matter physics.However,it is difficult to obtain atomically flat surfaces of many single crystals by the commonly used mechanical cleavage.We demonstrate that the grind-polish-sputter-anneal method can be used to obtain atomically flat surfaces on topological materials.Three types of surface-sensitive measurements are performed on CoSi(001)surface with dramatically improved quality of data.This method extends the research area of surface-sensitive measurements to hard-to-cleave alloys,and can be applied to irregular single crystals with selective crystalline planes.It may become a routine process of preparing atomically flat surfaces for surface-sensitive technologies. 展开更多

Two-dimensional(2D)ferromagnetic materials have been exhibiting promising potential in applications,such as spintronics devices.To grow epitaxial magnetic films on silicon substrate,in the single-layer limit,is practically important but challenging.In this study,we realized the epitaxial growth of Mn Sn monolayer on Si(111)substrate,with an atomically thin Sn/Si(111)-231/2×231/2-buffer layer,and controlled the Mn Sn thickness with atomic-layer precision.We discovered the ferromagnetism in Mn Sn monolayer with the Curie temperature(Tc)of^54 K.As the Mn Sn film is grown to 4 monolayers,Tc increases accordingly to^235 K.The lattice of the epitaxial Mn Sn monolayer as well as the Sn/Si(111)-231/2×231/2 is perfectly compatible with silicon,and thus an sharp interface is formed between Mn Sn,Sn and Si.This system provides a new platform for exploring the 2D ferromagnetism,integrating magnetic monolayers into silicon-based technology,and engineering the spintronics heterostructures. 展开更多

Fault location for distribution feeders short circuit especially single-phase grounding fault is an important task in distribution system with non-effectively grounded neutral.Fault location mode for distribution feeders using fault generated current and voltage transient traveling waves was investigated.The characteristics of transient traveling waves resulted from each short circuit fault and their transmission disciplinarian in distribution feeders are analyzed.This paper proposed that double end travelling waves theory which measures arriving time of fault initiated surge at both ends of the monitored line is fit for distribution feeders but single end traveling waves theory not.According to different distribution feeders,on the basis of analyzing original traveling waves reflection rule in line terminal, Current-voltage mode,voltage-voltage mode and current-current mode for fault location based on traveling waves are proposed and aerial mode component of original traveling waves is used to realize fault location.Experimental test verify the feasibility and correctness of the proposed method. 展开更多
