It starts from integrating the basic theoretical knowledge of medicine and clinical practice knowledge, improving students’ bilingual level of medical genetics, cultivating students’ literature retrieval ability, and promoting early clinical, multi-clinical and repeated clinical consciousness for medical students. Multiple sclerosis is characterized by demyelination and neuronal loss caused by inflammatory cell activation and infiltration into the central nervous system.Macrophage polarization plays an important role in the pat...Multiple sclerosis is characterized by demyelination and neuronal loss caused by inflammatory cell activation and infiltration into the central nervous system.Macrophage polarization plays an important role in the pathogenesis of experimental autoimmune encephalomyelitis,a traditional experimental model of multiple sclerosis.This study investigated the effect of Fasudil on macrophages and examined the therapeutic potential of Fasudil-modified macrophages in experimental autoimmune encephalomyelitis.We found that Fasudil induced the conversion of macrophages from the pro-inflammatory M1 type to the anti-inflammatory M2 type,as shown by reduced expression of inducible nitric oxide synthase/nitric oxide,interleukin-12,and CD16/32 and increased expression of arginase-1,interleukin-10,CD14,and CD206,which was linked to inhibition of Rho kinase activity,decreased expression of toll-like receptors,nuclear factor-κB,and components of the mitogen-activated protein kinase signaling pathway,and generation of the pro-inflammatory cytokines tumor necrosis factor-α,interleukin-1β,and interleukin-6.Crucially,Fasudil-modified macrophages effectively decreased the impact of experimental autoimmune encephalomyelitis,resulting in later onset of disease,lower symptom scores,less weight loss,and reduced demyelination compared with unmodified macrophages.In addition,Fasudil-modified macrophages decreased interleukin-17 expression on CD4^(+)T cells and CD16/32,inducible nitric oxide synthase,and interleukin-12 expression on F4/80^(+)macrophages,as well as increasing interleukin-10 expression on CD4^(+)T cells and arginase-1,CD206,and interleukin-10 expression on F4/80^(+)macrophages,which improved immune regulation and reduced inflammation.These findings suggest that Fasudil-modified macrophages may help treat experimental autoimmune encephalomyelitis by inducing M2 macrophage polarization and inhibiting the inflammatory response,thereby providing new insight into cell immunotherapy for multiple sclerosis.展开更多

Objective:This study aims to explore the prevalence,features,and risk factors of lower respiratory tract infections(LRTIs)in the intensive care unit(ICU)of a newly established hospital in Zunyi City.The goal is to dev...Objective:This study aims to explore the prevalence,features,and risk factors of lower respiratory tract infections(LRTIs)in the intensive care unit(ICU)of a newly established hospital in Zunyi City.The goal is to devise strategies for preventing LRTIs in the ICU of new hospitals,thereby mitigating the incidence of nosocomial LRTIs in ICU patients.Methods:A case-control study was conducted from March 2019 to December 2022 to investigate the incidence rate of LRTIs in the ICU of a newly constructed hospital in Zunyi City.Patients with LRTIs constituted the case group,while those without LRTIs constituted the control group,where a 1:1 matching principle was adhered to.A single-factor chi-square(χ2)test was employed to analyze the risk factors,with independent risk factors being explored using a multivariate logistic regression analysis.Results:A total of 169 strains of pathogenic bacteria were isolated,comprising 66.28%gram-negative bacteria,17.75%gram-positive bacteria,and 15.97%fungi.The most prevalent pathogens included Acinetobacter baumannii(43.20%),Candida albicans(10.65%),and Pseudomonas aeruginosa(8.88%).Of the 82 strains infected by multidrug-resistant bacteria in patients with LRTIs,81.7%were carbapenem-resistant Acinetobacter baumannii,9.8%were multidrug-resistant Pseudomonas aeruginosa,and 6.1%were carbapenem-resistant Escherichia coli.Identified risk factors included smoking history,total hospitalization days,ICU stay length,hypoproteinemia,indwelling gastric tube,intubation type,duration of mechanical ventilation,usage of antibacterial drugs,and administration of protein drugs(P<0.05).Multivariate logistic regression analysis demonstrated that these factors were independent risk factors for nosocomial LRTIs in ICU patients(P<0.05).Conclusion:ICU patients in our hospital were mainly infected by carbapenem-resistant Acinetobacter baumannii.To prevent LRTIs in patients,tailored preventive measures should be developed and the rational use of antibacterial drugs should be promoted.展开更多
The robotic liver resection(RLR)has been increasingly applied in recent years and its benefits shown in some aspects owing to the technical advancement of robotic surgical system,however,controversies still exist.Base...The robotic liver resection(RLR)has been increasingly applied in recent years and its benefits shown in some aspects owing to the technical advancement of robotic surgical system,however,controversies still exist.Based on the foundation of the previous consensus statement,this new consensus document aimed to update clinical recommendations and provide guidance to improve the outcomes of RLR clinical practice.The guideline steering group and guideline expert group were formed by 29 international experts of liver surgery and evidence-based medicine(EBM).Relevant literature was reviewed and analyzed by the evidence evaluation group.According to the WHO Handbook for Guideline Development,the Guidance Principles of Development and Amendment of the Guidelines for Clinical Diagnosis and Treatment in China 2022,a total of 14 recommendations were generated.Among them were 8 recommendations formulated by the GRADE method,and the remaining 6 recommendations were formulated based on literature review and experts’opinion due to insufficient EBM results.This international experts consensus guideline offered guidance for the safe and effective clinical practice and the research direction of RLR in future.展开更多
The ^(232)Th(n,f)cross section is very important in basic nuclear physics and applications based on the Th/U fuel cycle.Using the time-of-flight method and a multi-cell fast-fission ionization chamber,a novel measurem...The ^(232)Th(n,f)cross section is very important in basic nuclear physics and applications based on the Th/U fuel cycle.Using the time-of-flight method and a multi-cell fast-fission ionization chamber,a novel measurement of the^(232)Th(n,f)cross sec-tion relative to^(235)U in the 1–200 MeV range was performed at the China Spallation Neutron Source Back-n white neutron source(Back-n).The fission event-neutron energy spectra of^(232)Th and^(235)U fission cells were measured in the single-bunch mode.Corrected 232Th/235U fission cross-sectional ratios were obtained,and the measurement uncertainties were 2.5–3.7%for energies in the 2–20 MeV range and 3.6–6.2%for energies in the 20–200 MeV range.The^(232)Th(n,f)cross section was obtained by introducing the standard cross section of^(235)U(n,f).The results were compared with those of previous theoreti-cal calculations,measurements,and evaluations.The measured 232Th fission cross section agreed with the main evaluation results in terms of the experimental uncertainty,and 232Th fission resonances were observed in the 1–3 MeV range.The present results provide^(232)Th(n,f)cross-sectional data for the evaluation and design of Th/U cycle nuclear systems.展开更多
To reduce the experimental uncertainty in the 235 U resonance energy region and improve the detection efficiency for neutron total cross section measurements compared with those obtained with the neutron total cross s...To reduce the experimental uncertainty in the 235 U resonance energy region and improve the detection efficiency for neutron total cross section measurements compared with those obtained with the neutron total cross section spectrometer(NTOX), a dedicated lithium-containing scintillation detector has been developed on the Back-n beam line at the China Spallation Neutron Source. The Fast Scintillator-based Neutron Total Cross Section(FAST) spectrometer has been designed based on a Cs2Li La Br6(CLLB) scintillator considering the γ-ray flash and neutron environment on the Back-n beam line. The response of the CLLB scintillator to neutrons and γ-rays was evaluated with different 6Li/7 Li abundance ratios using Geant4. The neutron-γdiscrimination performance of the CLLB has been simulated considering different scintillation parameters, physical designs,and light readout modes. A cubic 6Li-enriched( > 90%) CLLB scintillator, which has a thickness of 4-9 mm and side length of no less than 50 mm to cover the Φ 50 mm neutron beam at the spectrometer position, has been proposed coupling to a side readout SiPM array to construct the FAST spectrometer. The developed simulation techniques for neutron-γ discrimination performance could provide technical support for other neutron-induced reaction measurements on the Back-n beam line.展开更多
Background: Robotic pancreaticoduodenectomy(RPD) has been reported to be safe and feasible for patients with pancreatic ductal adenocarcinoma(PDAC) of the pancreatic head. This study aimed to analyze the surgical outc...Background: Robotic pancreaticoduodenectomy(RPD) has been reported to be safe and feasible for patients with pancreatic ductal adenocarcinoma(PDAC) of the pancreatic head. This study aimed to analyze the surgical outcomes and risk factors for poor long-term prognosis of these patients. Methods: Data from patients who underwent RPD for PDAC of pancreatic head were retrospectively analyzed. Multivariate Cox regression analysis was used to seek the independent prognostic factors for overall survival(OS), and an online nomogram calculator was developed based on the independent prognostic factors. Results: Of the 273 patients who met the inclusion criteria, the median operative time was 280.0 minutes, the estimated blood loss was 100.0 m L, the median OS was 23.6 months, and the median recurrence-free survival(RFS) was 14.4 months. Multivariate analysis showed that preoperative carbohydrate antigen 19-9(CA19-9) [hazard ratio(HR) = 2.607, 95% confidence interval(CI): 1.560-4.354, P < 0.001], lymph node metastasis(HR = 1.429, 95% CI: 1.005-2.034, P = 0.047), tumor moderately(HR = 3.190, 95% CI: 1.813-5.614, P < 0.001) or poorly differentiated(HR = 5.114, 95% CI: 2.839-9.212, P < 0.001), and Clavien-Dindo grade ≥ Ⅲ(HR = 1.657, 95% CI: 1.079-2.546, P = 0.021) were independent prognostic factors for OS. The concordance index(C-index) of the nomogram constructed based on the above four independent prognostic factors was 0.685(95% CI: 0.640-0.729), which was significantly higher than that of the AJCC staging(8th edition): 0.541(95% CI: 0.493-0.589)( P < 0.001). Conclusions: This large-scale study indicated that RPD was feasible for PDAC of pancreatic head. Preoperative CA19-9, lymph node metastasis, tumor poorly differentiated, and Clavien-Dindo grade ≥ Ⅲ were independent prognostic factors for OS. The online nomogram calculator could predict the OS of these patients in a simple and convenient manner.展开更多

The study aimed to develop and validate a novel preoperative prognostic model to accurately predict the long-term survival of patients with PDAC.Methods: Patients with PDAC of pancreatic head from Chinese PLA General Hospital were included. The preoperative PDAC model with contour plots was developed using a non-linear model in the training cohort and then tested in the validation cohort.Results: Of 421 patients who met the inclusion criteria, 280 were in the training cohort and 141 in the validation cohort. Contour plots for preoperative PDAC model were established to visually predict the survival probabilities of these patients, based on preoperative carbohydrate antigen 19-9, preoperative fibrinogen to albumin ratio and pain symptoms. This model stratified patients into low-and high-risk groups with distinctly different long-term survival in the training cohort [median overall survival(OS)32.1 vs. 17.5 months;median recurrence-free survival(RFS) 19.3 vs. 10.0 months, both P < 0.001] and the validation cohort(median OS 28.3 vs. 19.0 months;median RFS 17.5 vs. 11.2 months, both P < 0.001).Time-dependent receiver operating characteristic and decision curve analyses revealed that the model provided higher diagnostic accuracy and superior net benefit compared to other staging systems.Conclusions: This study constructed and validated a novel preoperative prognostic model that can accurately and conveniently predict the long-term survival of patients with resectable PDAC of pancreatic head. Besides, the model can screen high-risk patients with poor prognosis, which may provide references for personal treatment strategies in the future.展开更多
Objective The prevalence of carbapenem-resistant Klebsiella pneumoniae(CR-KP)is a global public health problem.It is mainly caused by the plasmid-carried carbapenemase gene.Outer membrane vesicles(OMVs)contain toxins ...Objective The prevalence of carbapenem-resistant Klebsiella pneumoniae(CR-KP)is a global public health problem.It is mainly caused by the plasmid-carried carbapenemase gene.Outer membrane vesicles(OMVs)contain toxins and other factors involved in various biological processes,includingβ-lactamase and antibiotic-resistance genes.This study aimed to reveal the transmission mechanism of OMV-mediated drug resistance of Klebsiella(K.)pneumoniae.Methods We selected CR-KP producing K.pneumoniae carbapenemase-2(KPC-2)to study whether they can transfer resistance genes through OMVs.The OMVs of CR-KP were obtained by ultracentrifugation,and incubated with carbapenem-sensitive K.pneumoniae for 4 h.Finally,the carbapenem-sensitive K.pneumoniae was tested for the presence of bla_(KPC-2)resistance gene and its sensitivity to carbapenem antibiotics.Results The existence of OMVs was observed by the electron microscopy.The extracted OMVs had bla_(KPC-2)resistance gene.After incubation with OMVs,bla_(KPC-2)resistance gene was detected in sensitive K.pneumoniae,and it became resistant to imipenem and meropenem.Conclusion This study demonstrated that OMVs isolated from KPC-2-producing CR-KP could deliver bla_(KPC-2)to sensitive K.pneumoniae,allowing the bacteria to produce carbapenemase,which may provide a novel target for innovative therapies in combination with conventional antibiotics for treating carbapenem-resistant Enterobacteriaceae.展开更多
Objective Schizophrenia(SZ)is associated with cognitive impairment,and it is known that the activity of cAMP response element binding protein(CREB)decreases in the brain of SZ patients.The previous study conducted by ...Objective Schizophrenia(SZ)is associated with cognitive impairment,and it is known that the activity of cAMP response element binding protein(CREB)decreases in the brain of SZ patients.The previous study conducted by the investigators revealed that the upregulation of CREB improves the MK801-related SZ cognitive deficit.The present study further investigates the mechanism on how CREB deficiency is associated with SZ-related cognitive impairment.Methods MK-801 was used to induce SZ in rats.Western blotting and immunofluorescence were performed to investigate CREB and the CREB-related pathway implicated in MK801 rats.The long-term potentiation and behavioral tests were performed to assess the synaptic plasticity and cognitive impairment,respectively.Results The phosphorylation of CREB at Ser133 decreased in the hippocampus of SZ rats.Interestingly,among the upstream kinases of CREB,merely ERK1/2 was downregulated,while CaMKII and PKA remained unchanged in the brain of MK801-related SZ rats.The inhibition of ERK1/2 by PD98059 reduced the phosphorylation of CREB-Ser133,and induced synaptic dysfunction in primary hippocampal neurons.Conversely,the activation of CREB attenuated the ERK1/2 inhibitor-induced synaptic and cognitive impairment.Conclusion These present findings partially suggest that the deficiency of the ERK1/2-CREB pathway is involved in MK801-related SZ cognitive impairment.The activation of the ERK1/2-CREB pathway may be therapeutically useful for treating SZ cognitive deficits.展开更多
The transient multiphysics models were updated in CAMPUS to evaluate the accident-tolerant fuel performance under accident conditions.CAMPUS is a fuel performance code developed based on COMSOL.The simulated results o...The transient multiphysics models were updated in CAMPUS to evaluate the accident-tolerant fuel performance under accident conditions.CAMPUS is a fuel performance code developed based on COMSOL.The simulated results of the UO_(2)–Zircaloy fuel performance under accident conditions were compared with those of the FRAPTRAN code and the experimental data to verify the correctness of the updated CAMPUS.Subsequently,multiphysics models of the UO_(2)–BeO fuel and composite SiC coated with Cr(SiC_(f)/SiC-Cr)cladding were implemented in CAMPUS.Finally,the fuel performance of the three types of fuel cladding systems under Loss of Coolant Accident(LOCA)and Reactivity Insertion Accident(RIA)conditions was evaluated and compared,including the temperature distribution,stress distribution,pressure evolution,and cladding failure time.The results showed that the fuel temperature of the UO_(2) fuel under accident conditions without pre-irradiation was lower after being combined with SiC_(f)/SiC-Cr cladding.Moreover,the centerline and outer surface temperatures of the UO_(2)–BeO fuel combined with SiC_(f)/SiC-Cr cladding reduced further under accident conditions.The cladding temperature increased after the combination with the SiC_(f)/SiC-Cr cladding under accident conditions with pre-irradiation.In addition,the use of SiC_(f)/SiC-Cr cladding significantly reduced the cladding hoop strain and plenum pressure.展开更多

The main theoretical results are sufficient conditions for the permanence and extinction of the stochastic differential equation model, which are also extensions of some results in the existing literatures. Owing to the bipartite nature of honeycomb lattice,the electrons in graphene host valley degree of freedom,which gives rise to a rich set of unique physical phenomena including chiral tunneling,Klein paradox,and quant...Owing to the bipartite nature of honeycomb lattice,the electrons in graphene host valley degree of freedom,which gives rise to a rich set of unique physical phenomena including chiral tunneling,Klein paradox,and quantum Hall ferromagnetism.Atomic defects in graphene can efficiently break the local sublattice symmetry,and hence,have significant effects on the valley-based electronic behaviors.Here we demonstrate that an individual flower defect in graphene has the ability of valley filter at the atomic scale.With the combination of scanning tunneling microscopy and Landau level measurements,we observe two valley-polarized density-of-states peaks near the outside of the flower defects,implying the symmetry breaking of the K and K′valleys in graphene.Moreover,the electrons in the K valley can highly penetrate inside the flower defects.In contrast,the electrons in the K′valley cannot directly penetrate,instead,they should be assisted by the valley switch from the K′to K.Our results demonstrate that an individual flower defect in graphene can be regarded as a nanoscale valley filter,providing insight into the practical valleytronics.展开更多

The condition is challenging to manage, especially in patients with large or multiple shunts, through which rapid blood flow can cause ectopic embolism. Background Methamphetamine(METH)addiction causes a huge burden on society.The prefrontal cortex(PFC),associated with emotion and cognitive behaviours,is also involved in addiction neurocircuitry.Although bulk RNA sequ...Background Methamphetamine(METH)addiction causes a huge burden on society.The prefrontal cortex(PFC),associated with emotion and cognitive behaviours,is also involved in addiction neurocircuitry.Although bulk RNA sequencing has shown METH-induced gene alterations in the mouse PFC,the impact on different cell types remains unknown.Aims To clarify the effects of METH treatment on different cell types of the PFC and the potential pathways involved in METH-related disorders.Methods We performed single-nucleus RNA sequencing(snRNA-seq)to examine the transcriptomes of 20465 nuclei isolated from the PFC of chronic METH-treated and control mice.Main cell types and differentially expressed genes(DEGs)were identified and confirmed by RNA fluorescence in situ hybridization(FISH).Results Six main cell types were identified depending on the single-cell nucleus sequencing;of particular interest were the mature oligodendrocytes in the PFC.The DEGs of mature oligodendrocytes were enriched in the myelin sheath,adenosine triphosphate(ATP)metabolic process,mitochondrial function and components,and so on.The messenger RNA levels of Aldoc and Atp5l(FISH)and the protein level of the mitochondrial membrane pore subunit TOM40(immunofluorescence)decreased in the mature oligodendrocytes.Fast blue staining and transmission electron microscopy image indicated myelin damage,and the myelin thickness decreased in METH brains.Conclusions snRNA-seq reveals altered transcriptomes of different cell types in mouse PFC induced by chronic METH treatment,underscoring potential relationships with psychiatric disorders.展开更多

It starts from integrating the basic theoretical knowledge of medicine and clinical practice knowledge, improving students’ bilingual level of medical genetics, cultivating students’ literature retrieval ability, and promoting early clinical, multi-clinical and repeated clinical consciousness for medical students. 文摘Multiple sclerosis is characterized by demyelination and neuronal loss caused by inflammatory cell activation and infiltration into the central nervous system.Macrophage polarization plays an important role in the pathogenesis of experimental autoimmune encephalomyelitis,a traditional experimental model of multiple sclerosis.This study investigated the effect of Fasudil on macrophages and examined the therapeutic potential of Fasudil-modified macrophages in experimental autoimmune encephalomyelitis.We found that Fasudil induced the conversion of macrophages from the pro-inflammatory M1 type to the anti-inflammatory M2 type,as shown by reduced expression of inducible nitric oxide synthase/nitric oxide,interleukin-12,and CD16/32 and increased expression of arginase-1,interleukin-10,CD14,and CD206,which was linked to inhibition of Rho kinase activity,decreased expression of toll-like receptors,nuclear factor-κB,and components of the mitogen-activated protein kinase signaling pathway,and generation of the pro-inflammatory cytokines tumor necrosis factor-α,interleukin-1β,and interleukin-6.Crucially,Fasudil-modified macrophages effectively decreased the impact of experimental autoimmune encephalomyelitis,resulting in later onset of disease,lower symptom scores,less weight loss,and reduced demyelination compared with unmodified macrophages.In addition,Fasudil-modified macrophages decreased interleukin-17 expression on CD4^(+)T cells and CD16/32,inducible nitric oxide synthase,and interleukin-12 expression on F4/80^(+)macrophages,as well as increasing interleukin-10 expression on CD4^(+)T cells and arginase-1,CD206,and interleukin-10 expression on F4/80^(+)macrophages,which improved immune regulation and reduced inflammation.These findings suggest that Fasudil-modified macrophages may help treat experimental autoimmune encephalomyelitis by inducing M2 macrophage polarization and inhibiting the inflammatory response,thereby providing new insight into cell immunotherapy for multiple sclerosis.

文摘Nondestructive and noninvasive neutron assays are essential applications of neutron techniques.Neutron resonance transmission analysis(NRTA)is a powerful nondestructive method for investigating the elemental composition of an object.The back-streaming neutron line(Back-n)is a newly built time-of-flight facility at the China Spallation Neutron Source(CSNS)that provides neutrons in the eV to 300 MeV range.A feasibility study of the NRTA method for nuclide identification was conducted at the CSNS Back-n via two test experiments.The results demonstrate that it is feasible to identify different elements and isotopes in samples using the NRTA method at Back-n.This study reveals its potential future applications.

文摘Strong impact does serious harm to the military industries so it is necessary to choose reasonable cushioning material and design effective buffers to prevent the impact of equipment.Based on the capillary property entangled porous metallic wire materials(EPMWM),this paper designed a composite buffer which uses EPMWM and viscous fluid as cushioning materials under the low-speed impact of the recoil force device of weapon equipment(such as artillery,mortar,etc.).Combined with the capillary model,porosity,hydraulic diameter,maximum pore diameter and pore distribution were used to characterize the pore structure characteristics of EPMWM.The calculation model of the damping force of the composite buffer was established.The low-speed impact test of the composite buffer was conducted.The parameters of the buffer under low-speed impact were identified according to the model,and the nonlinear model of damping force was obtained.The test results show that the composite buffer with EPMWM and viscous fluid can absorb the impact energy from the recoil movement effectively,and provide a new method for the buffer design of weapon equipment(such as artillery,mortar,etc.).

文摘Objective:This study aims to explore the prevalence,features,and risk factors of lower respiratory tract infections(LRTIs)in the intensive care unit(ICU)of a newly established hospital in Zunyi City.The goal is to devise strategies for preventing LRTIs in the ICU of new hospitals,thereby mitigating the incidence of nosocomial LRTIs in ICU patients.Methods:A case-control study was conducted from March 2019 to December 2022 to investigate the incidence rate of LRTIs in the ICU of a newly constructed hospital in Zunyi City.Patients with LRTIs constituted the case group,while those without LRTIs constituted the control group,where a 1:1 matching principle was adhered to.A single-factor chi-square(χ2)test was employed to analyze the risk factors,with independent risk factors being explored using a multivariate logistic regression analysis.Results:A total of 169 strains of pathogenic bacteria were isolated,comprising 66.28%gram-negative bacteria,17.75%gram-positive bacteria,and 15.97%fungi.The most prevalent pathogens included Acinetobacter baumannii(43.20%),Candida albicans(10.65%),and Pseudomonas aeruginosa(8.88%).Of the 82 strains infected by multidrug-resistant bacteria in patients with LRTIs,81.7%were carbapenem-resistant Acinetobacter baumannii,9.8%were multidrug-resistant Pseudomonas aeruginosa,and 6.1%were carbapenem-resistant Escherichia coli.Identified risk factors included smoking history,total hospitalization days,ICU stay length,hypoproteinemia,indwelling gastric tube,intubation type,duration of mechanical ventilation,usage of antibacterial drugs,and administration of protein drugs(P<0.05).Multivariate logistic regression analysis demonstrated that these factors were independent risk factors for nosocomial LRTIs in ICU patients(P<0.05).Conclusion:ICU patients in our hospital were mainly infected by carbapenem-resistant Acinetobacter baumannii.To prevent LRTIs in patients,tailored preventive measures should be developed and the rational use of antibacterial drugs should be promoted.

文摘The robotic liver resection(RLR)has been increasingly applied in recent years and its benefits shown in some aspects owing to the technical advancement of robotic surgical system,however,controversies still exist.Based on the foundation of the previous consensus statement,this new consensus document aimed to update clinical recommendations and provide guidance to improve the outcomes of RLR clinical practice.The guideline steering group and guideline expert group were formed by 29 international experts of liver surgery and evidence-based medicine(EBM).Relevant literature was reviewed and analyzed by the evidence evaluation group.According to the WHO Handbook for Guideline Development,the Guidance Principles of Development and Amendment of the Guidelines for Clinical Diagnosis and Treatment in China 2022,a total of 14 recommendations were generated.Among them were 8 recommendations formulated by the GRADE method,and the remaining 6 recommendations were formulated based on literature review and experts’opinion due to insufficient EBM results.This international experts consensus guideline offered guidance for the safe and effective clinical practice and the research direction of RLR in future.

文摘The ^(232)Th(n,f)cross section is very important in basic nuclear physics and applications based on the Th/U fuel cycle.Using the time-of-flight method and a multi-cell fast-fission ionization chamber,a novel measurement of the^(232)Th(n,f)cross sec-tion relative to^(235)U in the 1–200 MeV range was performed at the China Spallation Neutron Source Back-n white neutron source(Back-n).The fission event-neutron energy spectra of^(232)Th and^(235)U fission cells were measured in the single-bunch mode.Corrected 232Th/235U fission cross-sectional ratios were obtained,and the measurement uncertainties were 2.5–3.7%for energies in the 2–20 MeV range and 3.6–6.2%for energies in the 20–200 MeV range.The^(232)Th(n,f)cross section was obtained by introducing the standard cross section of^(235)U(n,f).The results were compared with those of previous theoreti-cal calculations,measurements,and evaluations.The measured 232Th fission cross section agreed with the main evaluation results in terms of the experimental uncertainty,and 232Th fission resonances were observed in the 1–3 MeV range.The present results provide^(232)Th(n,f)cross-sectional data for the evaluation and design of Th/U cycle nuclear systems.

文摘To reduce the experimental uncertainty in the 235 U resonance energy region and improve the detection efficiency for neutron total cross section measurements compared with those obtained with the neutron total cross section spectrometer(NTOX), a dedicated lithium-containing scintillation detector has been developed on the Back-n beam line at the China Spallation Neutron Source. The Fast Scintillator-based Neutron Total Cross Section(FAST) spectrometer has been designed based on a Cs2Li La Br6(CLLB) scintillator considering the γ-ray flash and neutron environment on the Back-n beam line. The response of the CLLB scintillator to neutrons and γ-rays was evaluated with different 6Li/7 Li abundance ratios using Geant4. The neutron-γdiscrimination performance of the CLLB has been simulated considering different scintillation parameters, physical designs,and light readout modes. A cubic 6Li-enriched( > 90%) CLLB scintillator, which has a thickness of 4-9 mm and side length of no less than 50 mm to cover the Φ 50 mm neutron beam at the spectrometer position, has been proposed coupling to a side readout SiPM array to construct the FAST spectrometer. The developed simulation techniques for neutron-γ discrimination performance could provide technical support for other neutron-induced reaction measurements on the Back-n beam line.
文摘Background: Robotic pancreaticoduodenectomy(RPD) has been reported to be safe and feasible for patients with pancreatic ductal adenocarcinoma(PDAC) of the pancreatic head. This study aimed to analyze the surgical outcomes and risk factors for poor long-term prognosis of these patients. Methods: Data from patients who underwent RPD for PDAC of pancreatic head were retrospectively analyzed. Multivariate Cox regression analysis was used to seek the independent prognostic factors for overall survival(OS), and an online nomogram calculator was developed based on the independent prognostic factors. Results: Of the 273 patients who met the inclusion criteria, the median operative time was 280.0 minutes, the estimated blood loss was 100.0 m L, the median OS was 23.6 months, and the median recurrence-free survival(RFS) was 14.4 months. Multivariate analysis showed that preoperative carbohydrate antigen 19-9(CA19-9) [hazard ratio(HR) = 2.607, 95% confidence interval(CI): 1.560-4.354, P < 0.001], lymph node metastasis(HR = 1.429, 95% CI: 1.005-2.034, P = 0.047), tumor moderately(HR = 3.190, 95% CI: 1.813-5.614, P < 0.001) or poorly differentiated(HR = 5.114, 95% CI: 2.839-9.212, P < 0.001), and Clavien-Dindo grade ≥ Ⅲ(HR = 1.657, 95% CI: 1.079-2.546, P = 0.021) were independent prognostic factors for OS. The concordance index(C-index) of the nomogram constructed based on the above four independent prognostic factors was 0.685(95% CI: 0.640-0.729), which was significantly higher than that of the AJCC staging(8th edition): 0.541(95% CI: 0.493-0.589)( P < 0.001). Conclusions: This large-scale study indicated that RPD was feasible for PDAC of pancreatic head. Preoperative CA19-9, lymph node metastasis, tumor poorly differentiated, and Clavien-Dindo grade ≥ Ⅲ were independent prognostic factors for OS. The online nomogram calculator could predict the OS of these patients in a simple and convenient manner.

文摘Background: The prognosis of patients with pancreatic ductal adenocarcinoma(PDAC) remains poor even after radical pancreaticoduodenectomy(PD). The study aimed to develop and validate a novel preoperative prognostic model to accurately predict the long-term survival of patients with PDAC.Methods: Patients with PDAC of pancreatic head from Chinese PLA General Hospital were included. The preoperative PDAC model with contour plots was developed using a non-linear model in the training cohort and then tested in the validation cohort.Results: Of 421 patients who met the inclusion criteria, 280 were in the training cohort and 141 in the validation cohort. Contour plots for preoperative PDAC model were established to visually predict the survival probabilities of these patients, based on preoperative carbohydrate antigen 19-9, preoperative fibrinogen to albumin ratio and pain symptoms. This model stratified patients into low-and high-risk groups with distinctly different long-term survival in the training cohort [median overall survival(OS)32.1 vs. 17.5 months;median recurrence-free survival(RFS) 19.3 vs. 10.0 months, both P < 0.001] and the validation cohort(median OS 28.3 vs. 19.0 months;median RFS 17.5 vs. 11.2 months, both P < 0.001).Time-dependent receiver operating characteristic and decision curve analyses revealed that the model provided higher diagnostic accuracy and superior net benefit compared to other staging systems.Conclusions: This study constructed and validated a novel preoperative prognostic model that can accurately and conveniently predict the long-term survival of patients with resectable PDAC of pancreatic head. Besides, the model can screen high-risk patients with poor prognosis, which may provide references for personal treatment strategies in the future.

文摘Objective The prevalence of carbapenem-resistant Klebsiella pneumoniae(CR-KP)is a global public health problem.It is mainly caused by the plasmid-carried carbapenemase gene.Outer membrane vesicles(OMVs)contain toxins and other factors involved in various biological processes,includingβ-lactamase and antibiotic-resistance genes.This study aimed to reveal the transmission mechanism of OMV-mediated drug resistance of Klebsiella(K.)pneumoniae.Methods We selected CR-KP producing K.pneumoniae carbapenemase-2(KPC-2)to study whether they can transfer resistance genes through OMVs.The OMVs of CR-KP were obtained by ultracentrifugation,and incubated with carbapenem-sensitive K.pneumoniae for 4 h.Finally,the carbapenem-sensitive K.pneumoniae was tested for the presence of bla_(KPC-2)resistance gene and its sensitivity to carbapenem antibiotics.Results The existence of OMVs was observed by the electron microscopy.The extracted OMVs had bla_(KPC-2)resistance gene.After incubation with OMVs,bla_(KPC-2)resistance gene was detected in sensitive K.pneumoniae,and it became resistant to imipenem and meropenem.Conclusion This study demonstrated that OMVs isolated from KPC-2-producing CR-KP could deliver bla_(KPC-2)to sensitive K.pneumoniae,allowing the bacteria to produce carbapenemase,which may provide a novel target for innovative therapies in combination with conventional antibiotics for treating carbapenem-resistant Enterobacteriaceae.

文摘Objective Schizophrenia(SZ)is associated with cognitive impairment,and it is known that the activity of cAMP response element binding protein(CREB)decreases in the brain of SZ patients.The previous study conducted by the investigators revealed that the upregulation of CREB improves the MK801-related SZ cognitive deficit.The present study further investigates the mechanism on how CREB deficiency is associated with SZ-related cognitive impairment.Methods MK-801 was used to induce SZ in rats.Western blotting and immunofluorescence were performed to investigate CREB and the CREB-related pathway implicated in MK801 rats.The long-term potentiation and behavioral tests were performed to assess the synaptic plasticity and cognitive impairment,respectively.Results The phosphorylation of CREB at Ser133 decreased in the hippocampus of SZ rats.Interestingly,among the upstream kinases of CREB,merely ERK1/2 was downregulated,while CaMKII and PKA remained unchanged in the brain of MK801-related SZ rats.The inhibition of ERK1/2 by PD98059 reduced the phosphorylation of CREB-Ser133,and induced synaptic dysfunction in primary hippocampal neurons.Conversely,the activation of CREB attenuated the ERK1/2 inhibitor-induced synaptic and cognitive impairment.Conclusion These present findings partially suggest that the deficiency of the ERK1/2-CREB pathway is involved in MK801-related SZ cognitive impairment.The activation of the ERK1/2-CREB pathway may be therapeutically useful for treating SZ cognitive deficits.

文摘The transient multiphysics models were updated in CAMPUS to evaluate the accident-tolerant fuel performance under accident conditions.CAMPUS is a fuel performance code developed based on COMSOL.The simulated results of the UO_(2)–Zircaloy fuel performance under accident conditions were compared with those of the FRAPTRAN code and the experimental data to verify the correctness of the updated CAMPUS.Subsequently,multiphysics models of the UO_(2)–BeO fuel and composite SiC coated with Cr(SiC_(f)/SiC-Cr)cladding were implemented in CAMPUS.Finally,the fuel performance of the three types of fuel cladding systems under Loss of Coolant Accident(LOCA)and Reactivity Insertion Accident(RIA)conditions was evaluated and compared,including the temperature distribution,stress distribution,pressure evolution,and cladding failure time.The results showed that the fuel temperature of the UO_(2) fuel under accident conditions without pre-irradiation was lower after being combined with SiC_(f)/SiC-Cr cladding.Moreover,the centerline and outer surface temperatures of the UO_(2)–BeO fuel combined with SiC_(f)/SiC-Cr cladding reduced further under accident conditions.The cladding temperature increased after the combination with the SiC_(f)/SiC-Cr cladding under accident conditions with pre-irradiation.In addition,the use of SiC_(f)/SiC-Cr cladding significantly reduced the cladding hoop strain and plenum pressure.

文摘The use of flocculants to collect/extract microorganisms is of great practical significance for the development of the application of microorganisms. In this paper, a high-dimensional nonlinear stochastic differential equation model is constructed to describe the continuous culture of microorganisms with multiple nutrients and the flocculation process of microorganisms. The study of the dynamics of this model can provide feasible control strategies for the collection/extraction of microorganisms. The main theoretical results are sufficient conditions for the permanence and extinction of the stochastic differential equation model, which are also extensions of some results in the existing literatures. In addition, through numerical simulations, we vividly demonstrate the statistical characteristics of the stochastic differential equation model.

文摘Pea(Pisum sativum L.)is an annual cool-season legume crop.Owing to its role in sustainable agriculture as both a rotation and a cash crop,its global market is expanding and increased production is urgently needed.For both technical and regulatory reasons,neither conventional nor transgenic breeding techniques can keep pace with the demand for increased production.In answer to this challenge,CRISPR/Cas9 genome editing technology has been gaining traction in plant biology and crop breeding in recent years.However,there are currently no reports of the successful application of the CRISPR/Cas9 genome editing technology in pea.We developed a transient transformation system of hairy roots,mediated by Agrobacterium rhizogenes strain K599,to validate the efficiency of a CRISPR/Cas9 system.Further optimization resulted in an efficient vector,PsU6.3-tRNA-PsPDS3-en35S-PsCas9.We used this optimized CRISPR/Cas9 system to edit the pea phytoene desaturase(PsPDS)gene,causing albinism,by Agrobacterium-mediated genetic transformation.This is the first report of successful generation of gene-edited pea plants by this route.

文摘Owing to the bipartite nature of honeycomb lattice,the electrons in graphene host valley degree of freedom,which gives rise to a rich set of unique physical phenomena including chiral tunneling,Klein paradox,and quantum Hall ferromagnetism.Atomic defects in graphene can efficiently break the local sublattice symmetry,and hence,have significant effects on the valley-based electronic behaviors.Here we demonstrate that an individual flower defect in graphene has the ability of valley filter at the atomic scale.With the combination of scanning tunneling microscopy and Landau level measurements,we observe two valley-polarized density-of-states peaks near the outside of the flower defects,implying the symmetry breaking of the K and K′valleys in graphene.Moreover,the electrons in the K valley can highly penetrate inside the flower defects.In contrast,the electrons in the K′valley cannot directly penetrate,instead,they should be assisted by the valley switch from the K′to K.Our results demonstrate that an individual flower defect in graphene can be regarded as a nanoscale valley filter,providing insight into the practical valleytronics.
文摘Congenital intrahepatic portosystemic venous shunt(CPSVS), a rare vascular malformation, has been described in both children and adults and can lead to severe neurophysiological complications. However, a standard therapeutic protocol for CPSVS has not been elucidated. With the advantage of minimally invasive techniques,transcatheter embolization has been used to treat CPSVS. The condition is challenging to manage, especially in patients with large or multiple shunts, through which rapid blood flow can cause ectopic embolism. Here, we describe a case of CPSVS with a large shunt that was successfully treated with balloon-occluded retrograde transvenous obliteration with interlocking detachable coils.

文摘Background Methamphetamine(METH)addiction causes a huge burden on society.The prefrontal cortex(PFC),associated with emotion and cognitive behaviours,is also involved in addiction neurocircuitry.Although bulk RNA sequencing has shown METH-induced gene alterations in the mouse PFC,the impact on different cell types remains unknown.Aims To clarify the effects of METH treatment on different cell types of the PFC and the potential pathways involved in METH-related disorders.Methods We performed single-nucleus RNA sequencing(snRNA-seq)to examine the transcriptomes of 20465 nuclei isolated from the PFC of chronic METH-treated and control mice.Main cell types and differentially expressed genes(DEGs)were identified and confirmed by RNA fluorescence in situ hybridization(FISH).Results Six main cell types were identified depending on the single-cell nucleus sequencing;of particular interest were the mature oligodendrocytes in the PFC.The DEGs of mature oligodendrocytes were enriched in the myelin sheath,adenosine triphosphate(ATP)metabolic process,mitochondrial function and components,and so on.The messenger RNA levels of Aldoc and Atp5l(FISH)and the protein level of the mitochondrial membrane pore subunit TOM40(immunofluorescence)decreased in the mature oligodendrocytes.Fast blue staining and transmission electron microscopy image indicated myelin damage,and the myelin thickness decreased in METH brains.Conclusions snRNA-seq reveals altered transcriptomes of different cell types in mouse PFC induced by chronic METH treatment,underscoring potential relationships with psychiatric disorders.
