BACKGROUND Endoscopic full-thickness resection(EFTR)of gastric submucosal tumors(SMTs)is safe and effective;however,postoperative wound management is equally important.Literature on suturing following EFTR for large(...BACKGROUND Endoscopic full-thickness resection(EFTR)of gastric submucosal tumors(SMTs)is safe and effective;however,postoperative wound management is equally important.Literature on suturing following EFTR for large(≥3 cm)SMTs is scarce and limited.AIM To evaluate the efficacy and clinical value of double-nylon purse-string suture in closing postoperative wounds following EFTR of large(≥3 cm)SMTs.METHODS We retrospectively analyzed the data of 85 patients with gastric SMTs in the fundus of the stomach or in the lesser curvature of the gastric body whose wounds were treated with double-nylon purse-string sutures after successful tumor resection at the Endoscopy Center of Renmin Hospital of Wuhan University.The operative,postoperative,and follow-up conditions of the patients were evaluated.RESULTS All tumors were completely resected using EFTR.36(42.35%)patients had tumors located in the fundus of the stomach,and 49(57.65%)had tumors located in the body of the stomach.All patients underwent suturing with double-nylon sutures after EFTR without laparoscopic assistance or further surgical treatment.Postoperative fever and stomach pain were reported in 13(15.29%)and 14(16.47%)patients,respectively.No serious adverse events occurred during the intraoperative or postoperative periods.A postoperative review of all patients revealed no residual or recurrent lesions.CONCLUSION Double-nylon purse-string sutures can be used to successfully close wounds that cannot be completely closed with a single nylon suture,especially for large(≥3 cm)EFTR wounds in SMTs.展开更多
BACKGROUND Burkholderia pseudomallei(B.pseudomallei)is a short,straight,medium-sized Gramnegative bacterium that mostly exists alone,without a capsule or spores,has more than three flagella at one end,and actively mov...BACKGROUND Burkholderia pseudomallei(B.pseudomallei)is a short,straight,medium-sized Gramnegative bacterium that mostly exists alone,without a capsule or spores,has more than three flagella at one end,and actively moves.B.pseudomallei confers high morbidity and mortality,with frequent granulocytopenia in B.pseudomallei sepsisrelated deaths.However,mortality may be related to hemophagocytic lymphohistiocytosis(HLH)secondary to B.pseudomallei infection.CASE SUMMARY A 12-year-old female was referred from a local hospital to the pediatric intensive care unit with suspected septic shock and fever,cough,dyspnea,and malaise.After admission,supportive symptomatic treatments including fluid resuscitation,anti-infective therapy,mechanical ventilation,and a vasoactive drug maintenance cycle were carefully initiated.The patient became unconscious,her blood pressure could not be maintained even under the exposure of vasoactive drugs,and she experienced cardiorespiratory arrest.The patient died due to ineffective high-quality in-hospital cardiopulmonary resuscitation.A subsequent bone marrow smear examination revealed extensive phagocytosis,and the blood culture was positive for B.pseudomallei.Family history revealed a sibling death from B.pseudomallei sepsis 5 years earlier.CONCLUSION The higher mortality rate in patients with B.pseudomallei sepsis may be related to secondary HLH after infection,wherein multiorgan dysfunction syndrome may be directly related to infection or immune damage caused by secondary HLH.Patients with B.pseudomallei can be asymptomatic and can become an infective source.展开更多

Confocal microscopy,Western blotting, dimethylthiahzolyl-2,5-diphenyl-tetrazolium bromide,Cell Counting Kit-8 assays and flow cytometry were performed to explore the possible mechanism.RESULTS: Among the 160 HCC patients enrolled in this study,130 patients survived 2 years (81.25%),with a survival rate of 86.8% in AFP < 2 0 μg/L group,88.9% in AFP 20-250 μg/L group,and 69.6% in AFP > 250 μg/L group, demonstrating a higher mortality rate in HCC patients with higher AFP levels. Surgical treatment was benef icial only in patients with low AFP levels.The mortality rate of HCC patients with high AFP levels who were treated surgically was apparently higher than those treated with conservative management.The results of immunohistochemistry showed that AFP and AFP receptor were merely expressed in tissues of HCC patients with positive serum AFP.Consistently,in vitro analysis showed that AFP and AFPS were expressed in HepG2 but not in HLE cells. AFP showed a capability to promote cell growth,and this was more apparent in HepG2 cells,in which the proliferation was increased by 3.5 folds. Cell cycle analysis showed that the percent-age of HepG2 cells in S phase after exposure to AFP was modestly increased.CONCLUSION:HCC patients with higher AFP levels show a higher mortality rate,which appears to be attributable to the growth promoting properties of AFP.展开更多
AIM:To describe clinical findings and complications from cases of traumatic lens subluxation/dislocation in patients with secondary glaucoma,and discuss the multiple treating methods of operation combined with primary...AIM:To describe clinical findings and complications from cases of traumatic lens subluxation/dislocation in patients with secondary glaucoma,and discuss the multiple treating methods of operation combined with primary intraocular lens(IOL)implantation.METHODS:Non-comparativeretrospectiveobservational case series.Participants:30 cases(30 eyes)of lens subluxation/dislocation in patients with secondary glaucoma were investigated which accepted the surgical treatment by author in the Ophthalmology of Xi'an No.4Hospital from 2007 to 2011.According to the different situations of lens subluxation/dislocation,various surgical procedures were performed such as crystalline lens phacoemulsification,crystalline lens phacoemulsification combined anterior vitrectomy,intracapsular cataract extraction combined anterior vitrectomy,lensectomy combined anterior vitrectomy though peripheral transparent cornea incision,pars plana lensectomy combined pars plana vitrectomy,and intravitreal cavity crystalline lens phacofragmentation combined pars plana vitrectomy.And whether to implement trabeculectomy depended on the different situations of secondary glaucoma.The posterior chamber intraocular lenses(PC-IOLs)were implanted in the capsular-bag or trassclerally sutured in the sulus decided by whether the capsular were present.Main outcome measures:visual acuity,intraocular pressure,the situation of intraocular lens and complications after the operations.RESULTS:The follow-up time was 11-36mo(21.4±7.13).Postoperative visual acuity of all eyes were improved;28 cases maintained IOP below 21 mm Hg;2cases had slightly IOL subluxation,4 cases had slightlytilted lens optical area;1 case had postoperative choroidal detachment;4 cases had postoperative corneal edema more than 1wk,but eventually recovered transparent;2 cases had mild postoperative vitreous hemorrhage,and absorbed 4wk later.There was no postoperative retinal detachment,IOL dislocation,and endophthalmitis.CONCLUSION:To take early treatment of traumatic lens subluxation/dislocation in patients with secondary glaucoma by individual surgical plan based on the different eye conditions would be safe and effective,which can effectively control the intraocular pressure and restore some vision.展开更多

Since this disease was...Combined hepatocellular cholangiocarcinoma(CHC) accounts for 0.4%-14.2% of primary liver cancer cases and possesses pathological features of both hepatocellular carcinoma and cholangiocarcinoma. Since this disease was first described and classified in 1949, the classification of CHC has continuously evolved. The latest definition and classification of CHC by the World Health Organization is based on the speculation that CHC arises from hepatic progenitor cells. However, there is no evidence demonstrating the common origin of different components of CHC. Furthermore, the definition of CHC subtypes is still ambiguous and the identification of CHC subtype when a single tumor contains many components has remained unresolved. In addition, there is no summary on the newly recognized histopathology features or the contribution of CHC components to prognosis and outcome of this disease. 目的:观察区域折射型多焦点人工晶状体(MIOL)SBL-3植入术后视觉质量。方法:回顾性对照研究南昌大学第二附属医院2019-09/2020-07收治的57例68眼白内障患者。其中植入区域折射型MIOL SBL-3者33例36眼(SBL-3组),植入非球面单焦点人工晶状...目的:观察区域折射型多焦点人工晶状体(MIOL)SBL-3植入术后视觉质量。方法:回顾性对照研究南昌大学第二附属医院2019-09/2020-07收治的57例68眼白内障患者。其中植入区域折射型MIOL SBL-3者33例36眼(SBL-3组),植入非球面单焦点人工晶状体(SIOL)ZCB00者24例32眼(ZCB00组),对比两组术后3mo裸眼和矫正远、中、近视力、离焦曲线、OPDIII客观视觉质量、QoV问卷调查、患者满意度及术后脱镜率。结果:术后3mo SBL-3组和ZCB00组裸眼和矫正远视力LogMAR值(0.13±0.09 vs 0.10±0.08,0.06±0.06 vs 0.08±0.08,均P>0.05)无显著差异,裸眼远视力均较术前有显著改善(P<0.05)。SBL-3组裸眼及最佳矫正远视力下的中、近视力明显优于ZCB00组(0.10±0.14 vs 0.27±0.10,0.05±0.16 vs 0.35±0.17,0.11±0.14 vs 0.26±0.11,0.03±0.17 vs 0.35±0.17,均P<0.05)。在所评估的各个空间频率上患者对比敏感度的比较中ZCB00组均优于SBL-3组(P<0.05)。SBL-3组总像差、总高阶像差、彗差、三叶草差均大于ZCB00组且有显著差异(P<0.05),SBL-3组平均SR值小于ZCB00组(P<0.05)。术后3mo SBL-3组出现眩光1例,光晕1例,视远模糊4例。SBL-3组和ZCB00组患者表示非常满意者分别占82%、88%。两组间满意度比较无显著差异(P>0.05),脱镜率(94%vs 67%)比较有显著差异(P<0.05),且SBL-3组脱镜率更高。结论:新型区域折射型MIOL SBL-3提供了良好的全程视力,术后不良视觉症状少,满意度和脱镜率高。展开更多
AIM To explore the effect of hydrogen sulfide(H2S)on restraint water-immersion stress(RWIS)-induced gastric lesions in rats and the influence of adenosine triphosphate(ATP)-sensitive potassium(KATP)channels and nuclea...AIM To explore the effect of hydrogen sulfide(H2S)on restraint water-immersion stress(RWIS)-induced gastric lesions in rats and the influence of adenosine triphosphate(ATP)-sensitive potassium(KATP)channels and nuclear factor kappa-light-chain-enhancer of activated B cells(NF-κB)pathway on such an effect.METHODS Male Wistar rats were randomly divided into a control group,a physiological saline(PS)group,a sodium hydrosulfide(Na HS)group,a glibenclamide(Gl)group,Gl plus Na HS group,a pyrrolidine dithiocarbamate(PDTC)group,and a PDTC plus Na HS group.Gastric mucosal injury was induced by RWIS for 3 h in rats,and gastric mucosal damage was analyzed after that.The PS,Na HS(100μmol/kg body weight),Gl(100μmol/kg body weight),Gl(100μmol/kg or 150μmol/kg body weight)plus Na HS(100μmol/kg body weight),PDTC(100μmol/kg body weight),and PDTC(100μmol/kg body weight)plus Na HS(100μmol/kg bodyweight)were respectively injected intravenously before RWIS.RESULTS RWIS induced serious gastric lesions in the rats in the PS pretreatment group.The pretreatment of Na HS(a H2S donor)significantly reduced the damage induced by RWIS.The gastric protective effect of the Na HS during RWIS was attenuated by PDTC,an NF-κB inhibitor,and also by glibenclamide,an ATP-sensitive potassium channel blocker,in a dose-dependent manner.CONCLUSION These results suggest that exogenous H2S plays a protective role against RWIS injury in rats,possibly through modulation of KATP channel opening and the NF-κB dependent pathway.展开更多
When the operation speed of the high-speed train increases and the weight of the carbody becomes lighter,not only does the sensitivity of the wheel/rail contact get higher,but also the vibration frequency range of the...When the operation speed of the high-speed train increases and the weight of the carbody becomes lighter,not only does the sensitivity of the wheel/rail contact get higher,but also the vibration frequency range of the vehicle system gets enlarged and more frequencies are transmitted from the wheelset to the carbody.It is important to investigate the vibration characteristics and the dynamic frequency transmission from the wheel/rail interface to the carbody of the high-speed electric multi-uint(EMU).An elastic highspeed vehicle dynamics model is established in which the carbody,bogieframes,and wheelsets are all dealt with as flexible body.A rigid high-speed vehicle dynamics model is set up to compare with the simulation results of the elastic model.In the rigid vehicle model,the carbody,bogieframes and wheelsets are treated as rigid component while the suspension and structure parameters are the same as used in the elastic model.The dynamic characteristic of the elastic high speed vehicle is investigated in time and frequency domains and the di ff erence of the acceleration,frequency distribution and transmission of the two types of models are presented.The results show that the spectrum power density of the vehicle decreases from the wheelset to the carbody and the acceleration transmission ratio is approximately from 1%to 10%for each suspension system.The frequency of the wheelset rotation is evident in the vibration of the flexible model and is transmitted from the wheelset to the bogieframe and to thecarbody.The results of the flexible model are more reasonable than that of the rigid model.A field test data of the high speed train are presented to verify the simulation results.It shows that the simulation results are coincident with the field test data.展开更多

MicroRNAs(miRNAs) are a type of small, endogenous RNAs that regulate gene expression post-transcriptionally and influence signaling networks responsible for several cellular processes. In this study, mi R-103-3 p was transfected into neural stem cells derived from embryonic hippocampal neural stem cells. The results showed that mi R-103-3 p suppressed neural stem cell proliferation and differentiation, and promoted apoptosis. In addition, mi R-103-3 p negatively regulated Nud E neurodevelopment protein 1-like 1(Ndel1) expression by binding to the 3′ untranslated region of Ndel1. Transduction of neural stem cells with a lentiviral vector overexpressing Ndel1 significantly increased cell proliferation and differentiation, decreased neural stem cell apoptosis, and decreased protein expression levels of Wnt3 a, β-catenin, phosphor-GSK-3β, LEF1, c-myc, c-Jun, and cyclin D1, all members of the Wnt/β-catenin signaling pathway. These findings suggest that Ndel1 is a novel mi R-103-3 p target and that mi R-103-3 p acts by suppressing neural stem cell proliferation and promoting apoptosis and differentiation. Design and simulation results of a novel multifunctional electronic calibration kit based on microelectromechanical system(MEMS)single-pole double-throw(SPDT)switches are presented in this paper.The short-open-load-th...Design and simulation results of a novel multifunctional electronic calibration kit based on microelectromechanical system(MEMS)single-pole double-throw(SPDT)switches are presented in this paper.The short-open-load-through(SOLT)calibration states can be completed simultaneously by using the MEMS electronic calibration,and the electronic calibrator can be reused 10^(6) times.The simulation results show that this novel electronic calibration can be used in a frequency range of 0.1 GHz–20 GHz,the return loss is less than 0.18 dB and 0.035 dB in short-circuit and open-circuit states,respectively,and the insertion loss in through(thru)state is less than 0.27 dB.On the other hand,the size of this novel calibration kit is only 6 mm×2.8 mm×0.8 mm.Our results demonstrate that the calibrator with integrated radiofrequency microelectromechanical system(RF MEMS)switches can not only provide reduced size,loss,and calibration cost compared with traditional calibration kit but also improves the calibration accuracy and efficiency.It has great potential applications in millimeter-wave measurement and testing technologies,such as device testing,vector network analyzers,and RF probe stations.展开更多
The Dayin’gezhuang gold deposit is located in the central part of the Zhaoping Fault metallogenic belt in the Jiaodong gold province-the world’s third-largest gold metallogenic area.It is a typical successful case o...The Dayin’gezhuang gold deposit is located in the central part of the Zhaoping Fault metallogenic belt in the Jiaodong gold province-the world’s third-largest gold metallogenic area.It is a typical successful case of prospecting at a depth of 500-2000 m in recent years,with cumulative proven gold resources exceeding 180 t.The main orebodies(No.1 and No.2 orebody)generally have a pitch direction of NNE and a plunge direction of NEE.As the ore-controlling fault,the Zhaoping Fault is a shovel-shaped stepped fault,with its dip angle presenting stepped high-to-low transitions at the elevation of -2000-0 m.The gold mineralization enrichment area is mainly distributed in the step parts where the fault plane changes from steeply to gently,forming a stepped metallogenic pattern from shallow to deep.It can be concluded from previous studies that the gold mineralization of the Dayin’gezhuang gold deposit occurred at about 120 Ma.The ore-forming fluids were H_(2)O-CO_(2)-NaCl-type hydrothermal solutions with a medium-low temperature and medium-low salinity.The H-O isotopic characteristics indicate that the fluids in the early ore-forming stage were possibly magmatic water or mantle water and that meteoric water gradually entered the ore-forming fluids in the late ore-forming stage.The S and Pb isotopes indicate that the ore-forming materials mainly originate from the lower crust and contain a small quantity of mantle-derived components.The comprehensive analysis shows that the Dayin’gezhuang gold deposit was formed by thermal uplifting-extensional tectonism.The strong crust-mantle interactions,large-scale magmatism,and the material exchange arising from the transformation from adakitic granites to arc granites and from the ancient lower crust to the juvenile lower crust during the Early Cretaceous provided abundant fluids and material sources for mineralization.Moreover,the detachment faults formed by the rapid magmatic uplift and the extensional tectonism created favorable temperature and pressure conditions and space for fluid accumulation and gold precipitation and mineralization.展开更多
AIM: To explore the performance in diabetic retinopathy(DR) screening of artificial intelligence(AI) system by evaluating the image quality of a handheld Optomed Aurora fundus camera in comparison to traditional table...AIM: To explore the performance in diabetic retinopathy(DR) screening of artificial intelligence(AI) system by evaluating the image quality of a handheld Optomed Aurora fundus camera in comparison to traditional tabletop fundus cameras and the diagnostic accuracy of DR of the two modalities. METHODS: Overall, 630 eyes were included from three centers and screened by a handheld camera(Aurora, Optomed, Oulu, Finland) and a table-top camera. Image quality was graded by three masked and experienced ophthalmologists. The diagnostic accuracy of the handheld camera and AI system was evaluated in assessing DR lesions and referable DR.RESULTS: Under nonmydriasis status, the handheld fundus camera had better image quality in centration, clarity, and visible range(1.47, 1.48, and 1.40) than conventional tabletop cameras(1.30, 1.28, and 1.18;P<0.001). Detection of retinal hemorrhage, hard exudation,and macular edema were comparable between the two modalities, in principle, with the area under the curve of the handheld fundus camera slightly lower. The sensitivity and specificity for the detection of referable DR with the handheld camera were 82.1%(95%CI: 72.1%-92.2%) and 97.4%(95%CI: 95.4%-99.5%), respectively. The performance of AI detection of DR using the Phoebus Algorithm was satisfactory;however, Phoebus showed a high sensitivity(88.2%, 95%CI: 79.4%-97.1%) and low specificity(40.7%, 95%CI: 34.1%-47.2%) when detecting referable DR.CONCLUSION: The handheld Aurora fundus camera combined with autonomous AI system is well-suited in DR screening without mydriasis because of its high sensitivity of DR detection as well as its image quality, but its specificity needs to be improved with better modeling of the data. Use of this new system is safe and effective in the detection of referable DR in real world practice.展开更多

Percutaneous ethanol injection(PEI)has been used for treating hepatocellular carcinoma(HCC)for decades, while the recurrence after PEI treatment remains a major limitation. Recent evidence mounted that cancer cells could survive from chemical induced apoptosis, suggesting a potential route through which cancer relapse may occur. This study focuses on the consequence of HepG2 recovery from ethanol-induced apoptotic event.METHODS: The model of HepG2 recovery from ethanolinduced apoptotic event was established by live cell imaging,BrdU assay and Western blotting. MTT assay, wound healing assay and invasion assay were used to investigate the behavior of HepG2 after recovery.RESULTS: HepG2 cells could recover from ethanol-induced apoptosis. These cells changed their behaviors such as drug resistance, mobility and invasiveness. On average, the recovered HepG2 cell clones were found to be 46% more resistant to ethanol and 84% higher in mobility. The recovered clones became 58.2% more sensitive to 5-fluorouracil.CONCLUSIONS: HepG2 cells can recover from ethanolinduced apoptotic event. These cells became more resistant to ethanol and more invasive. Although the recovered cell clones were more resistant to ethanol, they became more sensitive to5-fluorouracil treatment.展开更多
Objective:To perform whole-genome sequencing and phylogenetic analysis of the local endemic strain of Rodent Torque teno virus (RoTTV), RoTTV3-HMU1, found in Rattus norvegicus, Haikou City, Hainan Province, and establ...Objective:To perform whole-genome sequencing and phylogenetic analysis of the local endemic strain of Rodent Torque teno virus (RoTTV), RoTTV3-HMU1, found in Rattus norvegicus, Haikou City, Hainan Province, and establish a SYBR Green I based real-time PCR detection assay for RoTTV3.Methods: Based on the high-throughput genome sequencing analysis, specific primers were designed and the whole genome sequence was amplified by PCR and Sanger sequencing. Specific detection primers were designed based on the conserved sequences of RoTTV3. The recombinant plasmid contained the whole genome of RoTTV3-HMU1 was constructed as a standard control. The experimental conditions were optimized and the real-time PCR detection assay of RoTTV3 was established.Results: The genomic sequence of RoTTV carried by Rattus norvegicus in Haikou City was successfully sequenced. Phylogenetic analysis indicated that the virus belongs to the RoTTV3 genotype. In this experiment, the real-time PCR detection method of RoTTV3 was established. The standard curve generated had a wide dynamic range from 1×(102-108) copies/μL, with a linear correlation (R2=1.000). The melting curve analysis using SYBR Green showed only one specific melting peak and no primer-dimmers represented. The detection limit was 100 copies/reaction.Discussion: This study was the first report of the RoTTV in Hainan Islands, and its phylogenetic analysis was of great significance to the origin and evolution of RoTTV. The RoTTV3 real-time PCR detection method established in this experiment has a high sensitivity and good specificity, which lays a technical foundation for the epidemiological investigation of RoTTV3.展开更多

文摘BACKGROUND Endoscopic full-thickness resection(EFTR)of gastric submucosal tumors(SMTs)is safe and effective;however,postoperative wound management is equally important.Literature on suturing following EFTR for large(≥3 cm)SMTs is scarce and limited.AIM To evaluate the efficacy and clinical value of double-nylon purse-string suture in closing postoperative wounds following EFTR of large(≥3 cm)SMTs.METHODS We retrospectively analyzed the data of 85 patients with gastric SMTs in the fundus of the stomach or in the lesser curvature of the gastric body whose wounds were treated with double-nylon purse-string sutures after successful tumor resection at the Endoscopy Center of Renmin Hospital of Wuhan University.The operative,postoperative,and follow-up conditions of the patients were evaluated.RESULTS All tumors were completely resected using EFTR.36(42.35%)patients had tumors located in the fundus of the stomach,and 49(57.65%)had tumors located in the body of the stomach.All patients underwent suturing with double-nylon sutures after EFTR without laparoscopic assistance or further surgical treatment.Postoperative fever and stomach pain were reported in 13(15.29%)and 14(16.47%)patients,respectively.No serious adverse events occurred during the intraoperative or postoperative periods.A postoperative review of all patients revealed no residual or recurrent lesions.CONCLUSION Double-nylon purse-string sutures can be used to successfully close wounds that cannot be completely closed with a single nylon suture,especially for large(≥3 cm)EFTR wounds in SMTs.
文摘BACKGROUND Burkholderia pseudomallei(B.pseudomallei)is a short,straight,medium-sized Gramnegative bacterium that mostly exists alone,without a capsule or spores,has more than three flagella at one end,and actively moves.B.pseudomallei confers high morbidity and mortality,with frequent granulocytopenia in B.pseudomallei sepsisrelated deaths.However,mortality may be related to hemophagocytic lymphohistiocytosis(HLH)secondary to B.pseudomallei infection.CASE SUMMARY A 12-year-old female was referred from a local hospital to the pediatric intensive care unit with suspected septic shock and fever,cough,dyspnea,and malaise.After admission,supportive symptomatic treatments including fluid resuscitation,anti-infective therapy,mechanical ventilation,and a vasoactive drug maintenance cycle were carefully initiated.The patient became unconscious,her blood pressure could not be maintained even under the exposure of vasoactive drugs,and she experienced cardiorespiratory arrest.The patient died due to ineffective high-quality in-hospital cardiopulmonary resuscitation.A subsequent bone marrow smear examination revealed extensive phagocytosis,and the blood culture was positive for B.pseudomallei.Family history revealed a sibling death from B.pseudomallei sepsis 5 years earlier.CONCLUSION The higher mortality rate in patients with B.pseudomallei sepsis may be related to secondary HLH after infection,wherein multiorgan dysfunction syndrome may be directly related to infection or immune damage caused by secondary HLH.Patients with B.pseudomallei can be asymptomatic and can become an infective source.

文摘Background:According to previous studies on professional English course teaching,the evaluation of course teaching was positive,but the vast majorities focus on medical English literature reading,professional English vocabulary,and professional English translation.As an alternative,the course design based on academic learning needs under the outcome-oriented education/model emphasizes the improvement of students'comprehensive ability in oral expression,literature reading,writing,and academic communication.Objectives:The objective of this study was to analyze nursing postgraduates'opinions on learning the outcome-oriented academic English course.Methods:This is a cross-sectional descriptive study.A total of 150 first-year nursing postgraduates enrolled in the“Academic Professional English for Nursing Postgraduates”course.After completing the course learning,students scanned QR codes generated by the online questionnaire and completed it anonymously within 48 h.Results:The students who participated in this course strongly believed that it“helped them strengthen their English speakability”(4.8 points),“made them more confident to participate in international academic conferences and exchanges in the future”(4.8 points),and“helped them apply English more in the nursing professional field in the future”(4.7 points).Conclusions:The implementation of outcome-oriented course teaching helps students to understand the research of foreign scholars and effectively express their own research content with English as a tool.It motivates them to continuously use English for professional and academic communication.

文摘AIM:To investigate the biological role of alpha fetoprotein (AFP) and its clinical signif icance in carcinogenesis of hepatocellular carcinoma (HCC).METHODS:Clinical analysis of HCC patients and im-munohistochemical examination were conducted to evaluate the relationship between serum AFP level and patient mortality. Confocal microscopy,Western blotting, dimethylthiahzolyl-2,5-diphenyl-tetrazolium bromide,Cell Counting Kit-8 assays and flow cytometry were performed to explore the possible mechanism.RESULTS: Among the 160 HCC patients enrolled in this study,130 patients survived 2 years (81.25%),with a survival rate of 86.8% in AFP < 2 0 μg/L group,88.9% in AFP 20-250 μg/L group,and 69.6% in AFP > 250 μg/L group, demonstrating a higher mortality rate in HCC patients with higher AFP levels. Surgical treatment was benef icial only in patients with low AFP levels.The mortality rate of HCC patients with high AFP levels who were treated surgically was apparently higher than those treated with conservative management.The results of immunohistochemistry showed that AFP and AFP receptor were merely expressed in tissues of HCC patients with positive serum AFP.Consistently,in vitro analysis showed that AFP and AFPS were expressed in HepG2 but not in HLE cells. AFP showed a capability to promote cell growth,and this was more apparent in HepG2 cells,in which the proliferation was increased by 3.5 folds. Cell cycle analysis showed that the percent-age of HepG2 cells in S phase after exposure to AFP was modestly increased.CONCLUSION:HCC patients with higher AFP levels show a higher mortality rate,which appears to be attributable to the growth promoting properties of AFP.
文摘AIM:To describe clinical findings and complications from cases of traumatic lens subluxation/dislocation in patients with secondary glaucoma,and discuss the multiple treating methods of operation combined with primary intraocular lens(IOL)implantation.METHODS:Non-comparativeretrospectiveobservational case series.Participants:30 cases(30 eyes)of lens subluxation/dislocation in patients with secondary glaucoma were investigated which accepted the surgical treatment by author in the Ophthalmology of Xi'an No.4Hospital from 2007 to 2011.According to the different situations of lens subluxation/dislocation,various surgical procedures were performed such as crystalline lens phacoemulsification,crystalline lens phacoemulsification combined anterior vitrectomy,intracapsular cataract extraction combined anterior vitrectomy,lensectomy combined anterior vitrectomy though peripheral transparent cornea incision,pars plana lensectomy combined pars plana vitrectomy,and intravitreal cavity crystalline lens phacofragmentation combined pars plana vitrectomy.And whether to implement trabeculectomy depended on the different situations of secondary glaucoma.The posterior chamber intraocular lenses(PC-IOLs)were implanted in the capsular-bag or trassclerally sutured in the sulus decided by whether the capsular were present.Main outcome measures:visual acuity,intraocular pressure,the situation of intraocular lens and complications after the operations.RESULTS:The follow-up time was 11-36mo(21.4±7.13).Postoperative visual acuity of all eyes were improved;28 cases maintained IOP below 21 mm Hg;2cases had slightly IOL subluxation,4 cases had slightlytilted lens optical area;1 case had postoperative choroidal detachment;4 cases had postoperative corneal edema more than 1wk,but eventually recovered transparent;2 cases had mild postoperative vitreous hemorrhage,and absorbed 4wk later.There was no postoperative retinal detachment,IOL dislocation,and endophthalmitis.CONCLUSION:To take early treatment of traumatic lens subluxation/dislocation in patients with secondary glaucoma by individual surgical plan based on the different eye conditions would be safe and effective,which can effectively control the intraocular pressure and restore some vision.
文摘Combined hepatocellular cholangiocarcinoma(CHC) accounts for 0.4%-14.2% of primary liver cancer cases and possesses pathological features of both hepatocellular carcinoma and cholangiocarcinoma. Since this disease was first described and classified in 1949, the classification of CHC has continuously evolved. The latest definition and classification of CHC by the World Health Organization is based on the speculation that CHC arises from hepatic progenitor cells. However, there is no evidence demonstrating the common origin of different components of CHC. Furthermore, the definition of CHC subtypes is still ambiguous and the identification of CHC subtype when a single tumor contains many components has remained unresolved. In addition, there is no summary on the newly recognized histopathology features or the contribution of CHC components to prognosis and outcome of this disease. Here we provide a review of the current literature to address these questions.

文摘MicroRNAs refer to a class of endogenous,short non-coding RNAs that mediate numerous biological functions.MicroRNAs regulate various physiological and pathological activities of peripheral nerves,including peripheral nerve repair and regeneration.Previously,using a rat sciatic nerve injury model,we identified many functionally annotated novel microRNAs,including miR-sc14.Here,we used real-time reverse transcription-polymerase chain reaction to examine miR-sc14 expression in rat sciatic nerve stumps.Our results show that miRsc14 is noticeably altered following sciatic nerve injury,being up-regulated at 1 day and diminished at 7 days.EdU and transwell chamber assay results showed that miR-sc14 mimic promoted proliferation and migration of Schwann cells,while miR-sc14 inhiThe study was approved by the Jiangsu Provincial Laboratory Animal Management Committee,China on March 4,2015(approval No.20150304-004).bitor suppressed their proliferation and migration.Additionally,bioinformatic analysis examined potential target genes of miR-sc14,and found that fibroblast growth factor receptor 2 might be a potential target gene.Specifically,our results show changes of miR-sc14 expression in the sciatic nerve of rats at different time points after nerve injury.Appropriately,up-regulation of miR-sc14 promoted proliferation and migration of Schwann cells.Consequently,miR-sc14 may be an intervention target to promote repair of peripheral nerve injury.The study was approved by the Jiangsu Provincial Laboratory Animal Management Committee,China on March 4,2015(approval No.20150304-004).
文摘目的:观察区域折射型多焦点人工晶状体(MIOL)SBL-3植入术后视觉质量。方法:回顾性对照研究南昌大学第二附属医院2019-09/2020-07收治的57例68眼白内障患者。其中植入区域折射型MIOL SBL-3者33例36眼(SBL-3组),植入非球面单焦点人工晶状体(SIOL)ZCB00者24例32眼(ZCB00组),对比两组术后3mo裸眼和矫正远、中、近视力、离焦曲线、OPDIII客观视觉质量、QoV问卷调查、患者满意度及术后脱镜率。结果:术后3mo SBL-3组和ZCB00组裸眼和矫正远视力LogMAR值(0.13±0.09 vs 0.10±0.08,0.06±0.06 vs 0.08±0.08,均P>0.05)无显著差异,裸眼远视力均较术前有显著改善(P<0.05)。SBL-3组裸眼及最佳矫正远视力下的中、近视力明显优于ZCB00组(0.10±0.14 vs 0.27±0.10,0.05±0.16 vs 0.35±0.17,0.11±0.14 vs 0.26±0.11,0.03±0.17 vs 0.35±0.17,均P<0.05)。在所评估的各个空间频率上患者对比敏感度的比较中ZCB00组均优于SBL-3组(P<0.05)。SBL-3组总像差、总高阶像差、彗差、三叶草差均大于ZCB00组且有显著差异(P<0.05),SBL-3组平均SR值小于ZCB00组(P<0.05)。术后3mo SBL-3组出现眩光1例,光晕1例,视远模糊4例。SBL-3组和ZCB00组患者表示非常满意者分别占82%、88%。两组间满意度比较无显著差异(P>0.05),脱镜率(94%vs 67%)比较有显著差异(P<0.05),且SBL-3组脱镜率更高。结论:新型区域折射型MIOL SBL-3提供了良好的全程视力,术后不良视觉症状少,满意度和脱镜率高。

文摘AIM To explore the effect of hydrogen sulfide(H2S)on restraint water-immersion stress(RWIS)-induced gastric lesions in rats and the influence of adenosine triphosphate(ATP)-sensitive potassium(KATP)channels and nuclear factor kappa-light-chain-enhancer of activated B cells(NF-κB)pathway on such an effect.METHODS Male Wistar rats were randomly divided into a control group,a physiological saline(PS)group,a sodium hydrosulfide(Na HS)group,a glibenclamide(Gl)group,Gl plus Na HS group,a pyrrolidine dithiocarbamate(PDTC)group,and a PDTC plus Na HS group.Gastric mucosal injury was induced by RWIS for 3 h in rats,and gastric mucosal damage was analyzed after that.The PS,Na HS(100μmol/kg body weight),Gl(100μmol/kg body weight),Gl(100μmol/kg or 150μmol/kg body weight)plus Na HS(100μmol/kg body weight),PDTC(100μmol/kg body weight),and PDTC(100μmol/kg body weight)plus Na HS(100μmol/kg bodyweight)were respectively injected intravenously before RWIS.RESULTS RWIS induced serious gastric lesions in the rats in the PS pretreatment group.The pretreatment of Na HS(a H2S donor)significantly reduced the damage induced by RWIS.The gastric protective effect of the Na HS during RWIS was attenuated by PDTC,an NF-κB inhibitor,and also by glibenclamide,an ATP-sensitive potassium channel blocker,in a dose-dependent manner.CONCLUSION These results suggest that exogenous H2S plays a protective role against RWIS injury in rats,possibly through modulation of KATP channel opening and the NF-κB dependent pathway.

文摘When the operation speed of the high-speed train increases and the weight of the carbody becomes lighter,not only does the sensitivity of the wheel/rail contact get higher,but also the vibration frequency range of the vehicle system gets enlarged and more frequencies are transmitted from the wheelset to the carbody.It is important to investigate the vibration characteristics and the dynamic frequency transmission from the wheel/rail interface to the carbody of the high-speed electric multi-uint(EMU).An elastic highspeed vehicle dynamics model is established in which the carbody,bogieframes,and wheelsets are all dealt with as flexible body.A rigid high-speed vehicle dynamics model is set up to compare with the simulation results of the elastic model.In the rigid vehicle model,the carbody,bogieframes and wheelsets are treated as rigid component while the suspension and structure parameters are the same as used in the elastic model.The dynamic characteristic of the elastic high speed vehicle is investigated in time and frequency domains and the di ff erence of the acceleration,frequency distribution and transmission of the two types of models are presented.The results show that the spectrum power density of the vehicle decreases from the wheelset to the carbody and the acceleration transmission ratio is approximately from 1%to 10%for each suspension system.The frequency of the wheelset rotation is evident in the vibration of the flexible model and is transmitted from the wheelset to the bogieframe and to thecarbody.The results of the flexible model are more reasonable than that of the rigid model.A field test data of the high speed train are presented to verify the simulation results.It shows that the simulation results are coincident with the field test data.

文摘The regulation of adult neural stem cells(NSCs) is critical for lifelong neurogenesis. MicroRNAs(miRNAs) are a type of small, endogenous RNAs that regulate gene expression post-transcriptionally and influence signaling networks responsible for several cellular processes. In this study, mi R-103-3 p was transfected into neural stem cells derived from embryonic hippocampal neural stem cells. The results showed that mi R-103-3 p suppressed neural stem cell proliferation and differentiation, and promoted apoptosis. In addition, mi R-103-3 p negatively regulated Nud E neurodevelopment protein 1-like 1(Ndel1) expression by binding to the 3′ untranslated region of Ndel1. Transduction of neural stem cells with a lentiviral vector overexpressing Ndel1 significantly increased cell proliferation and differentiation, decreased neural stem cell apoptosis, and decreased protein expression levels of Wnt3 a, β-catenin, phosphor-GSK-3β, LEF1, c-myc, c-Jun, and cyclin D1, all members of the Wnt/β-catenin signaling pathway. These findings suggest that Ndel1 is a novel mi R-103-3 p target and that mi R-103-3 p acts by suppressing neural stem cell proliferation and promoting apoptosis and differentiation. 文摘Design and simulation results of a novel multifunctional electronic calibration kit based on microelectromechanical system(MEMS)single-pole double-throw(SPDT)switches are presented in this paper.The short-open-load-through(SOLT)calibration states can be completed simultaneously by using the MEMS electronic calibration,and the electronic calibrator can be reused 10^(6) times.The simulation results show that this novel electronic calibration can be used in a frequency range of 0.1 GHz–20 GHz,the return loss is less than 0.18 dB and 0.035 dB in short-circuit and open-circuit states,respectively,and the insertion loss in through(thru)state is less than 0.27 dB.On the other hand,the size of this novel calibration kit is only 6 mm×2.8 mm×0.8 mm.Our results demonstrate that the calibrator with integrated radiofrequency microelectromechanical system(RF MEMS)switches can not only provide reduced size,loss,and calibration cost compared with traditional calibration kit but also improves the calibration accuracy and efficiency.It has great potential applications in millimeter-wave measurement and testing technologies,such as device testing,vector network analyzers,and RF probe stations.

文摘The Dayin’gezhuang gold deposit is located in the central part of the Zhaoping Fault metallogenic belt in the Jiaodong gold province-the world’s third-largest gold metallogenic area.It is a typical successful case of prospecting at a depth of 500-2000 m in recent years,with cumulative proven gold resources exceeding 180 t.The main orebodies(No.1 and No.2 orebody)generally have a pitch direction of NNE and a plunge direction of NEE.As the ore-controlling fault,the Zhaoping Fault is a shovel-shaped stepped fault,with its dip angle presenting stepped high-to-low transitions at the elevation of -2000-0 m.The gold mineralization enrichment area is mainly distributed in the step parts where the fault plane changes from steeply to gently,forming a stepped metallogenic pattern from shallow to deep.It can be concluded from previous studies that the gold mineralization of the Dayin’gezhuang gold deposit occurred at about 120 Ma.The ore-forming fluids were H_(2)O-CO_(2)-NaCl-type hydrothermal solutions with a medium-low temperature and medium-low salinity.The H-O isotopic characteristics indicate that the fluids in the early ore-forming stage were possibly magmatic water or mantle water and that meteoric water gradually entered the ore-forming fluids in the late ore-forming stage.The S and Pb isotopes indicate that the ore-forming materials mainly originate from the lower crust and contain a small quantity of mantle-derived components.The comprehensive analysis shows that the Dayin’gezhuang gold deposit was formed by thermal uplifting-extensional tectonism.The strong crust-mantle interactions,large-scale magmatism,and the material exchange arising from the transformation from adakitic granites to arc granites and from the ancient lower crust to the juvenile lower crust during the Early Cretaceous provided abundant fluids and material sources for mineralization.Moreover,the detachment faults formed by the rapid magmatic uplift and the extensional tectonism created favorable temperature and pressure conditions and space for fluid accumulation and gold precipitation and mineralization.

文摘AIM: To explore the performance in diabetic retinopathy(DR) screening of artificial intelligence(AI) system by evaluating the image quality of a handheld Optomed Aurora fundus camera in comparison to traditional tabletop fundus cameras and the diagnostic accuracy of DR of the two modalities. METHODS: Overall, 630 eyes were included from three centers and screened by a handheld camera(Aurora, Optomed, Oulu, Finland) and a table-top camera. Image quality was graded by three masked and experienced ophthalmologists. The diagnostic accuracy of the handheld camera and AI system was evaluated in assessing DR lesions and referable DR.RESULTS: Under nonmydriasis status, the handheld fundus camera had better image quality in centration, clarity, and visible range(1.47, 1.48, and 1.40) than conventional tabletop cameras(1.30, 1.28, and 1.18;P<0.001). Detection of retinal hemorrhage, hard exudation,and macular edema were comparable between the two modalities, in principle, with the area under the curve of the handheld fundus camera slightly lower. The sensitivity and specificity for the detection of referable DR with the handheld camera were 82.1%(95%CI: 72.1%-92.2%) and 97.4%(95%CI: 95.4%-99.5%), respectively. The performance of AI detection of DR using the Phoebus Algorithm was satisfactory;however, Phoebus showed a high sensitivity(88.2%, 95%CI: 79.4%-97.1%) and low specificity(40.7%, 95%CI: 34.1%-47.2%) when detecting referable DR.CONCLUSION: The handheld Aurora fundus camera combined with autonomous AI system is well-suited in DR screening without mydriasis because of its high sensitivity of DR detection as well as its image quality, but its specificity needs to be improved with better modeling of the data. Use of this new system is safe and effective in the detection of referable DR in real world practice.

文摘BACKGROUND: Cancer relapse, associated with increased drug resistance and rate of metastasis, often follows completion of chemotherapy but the cancer escape mechanisms are still incompletely understood. Percutaneous ethanol injection(PEI)has been used for treating hepatocellular carcinoma(HCC)for decades, while the recurrence after PEI treatment remains a major limitation. Recent evidence mounted that cancer cells could survive from chemical induced apoptosis, suggesting a potential route through which cancer relapse may occur. This study focuses on the consequence of HepG2 recovery from ethanol-induced apoptotic event.METHODS: The model of HepG2 recovery from ethanolinduced apoptotic event was established by live cell imaging,BrdU assay and Western blotting. MTT assay, wound healing assay and invasion assay were used to investigate the behavior of HepG2 after recovery.RESULTS: HepG2 cells could recover from ethanol-induced apoptosis. These cells changed their behaviors such as drug resistance, mobility and invasiveness. On average, the recovered HepG2 cell clones were found to be 46% more resistant to ethanol and 84% higher in mobility. The recovered clones became 58.2% more sensitive to 5-fluorouracil.CONCLUSIONS: HepG2 cells can recover from ethanolinduced apoptotic event. These cells became more resistant to ethanol and more invasive. Although the recovered cell clones were more resistant to ethanol, they became more sensitive to5-fluorouracil treatment.

文摘Objective:To perform whole-genome sequencing and phylogenetic analysis of the local endemic strain of Rodent Torque teno virus (RoTTV), RoTTV3-HMU1, found in Rattus norvegicus, Haikou City, Hainan Province, and establish a SYBR Green I based real-time PCR detection assay for RoTTV3.Methods: Based on the high-throughput genome sequencing analysis, specific primers were designed and the whole genome sequence was amplified by PCR and Sanger sequencing. Specific detection primers were designed based on the conserved sequences of RoTTV3. The recombinant plasmid contained the whole genome of RoTTV3-HMU1 was constructed as a standard control. The experimental conditions were optimized and the real-time PCR detection assay of RoTTV3 was established.Results: The genomic sequence of RoTTV carried by Rattus norvegicus in Haikou City was successfully sequenced. Phylogenetic analysis indicated that the virus belongs to the RoTTV3 genotype. In this experiment, the real-time PCR detection method of RoTTV3 was established. The standard curve generated had a wide dynamic range from 1×(102-108) copies/μL, with a linear correlation (R2=1.000). The melting curve analysis using SYBR Green showed only one specific melting peak and no primer-dimmers represented. The detection limit was 100 copies/reaction.Discussion: This study was the first report of the RoTTV in Hainan Islands, and its phylogenetic analysis was of great significance to the origin and evolution of RoTTV. The RoTTV3 real-time PCR detection method established in this experiment has a high sensitivity and good specificity, which lays a technical foundation for the epidemiological investigation of RoTTV3.
