BACKGROUND Serum protein induced by vitamin K absence or antagonist-Ⅱ(PIVKA-Ⅱ) is a promising biomarker for hepatocellular carcinoma(HCC) surveillance.AIM To identify the contributing factors related to the abnormal elevation of PIVKA-Ⅱ level and assess their potential influence on the performance of PIVKA-Ⅱ in detecting HCC.METHODS This study retrospectively enrolled in 784 chronic liver disease(CLD) patients and 267 HCC patients in Mengchao Hepatobiliary Hospital of Fujian Medical University from April 2016 to December 2019. Logistic regression and the area under the receiver operating characteristic curve(AUC) were used to evaluate the influencing factors and diagnostic performance of PIVKA-Ⅱ for HCC, respectively.RESULTS Elevated PIVKA-Ⅱ levels were independently positively associated with alcohol-related liver disease, serum alkaline phosphatase(ALP), and total bilirubin(TBIL) for CLD patients and aspartate aminotransferase(AST) and tumor size for HCC patients(all P < 0.05). Serum PIVKA-Ⅱ were significantly lower in patients with viral etiology, ALP ≤ 1 × upper limit of normal(ULN), TBIL ≤ 1 × ULN, and AST ≤ 1 × ULN than in those with nonviral disease and abnormal ALP, TBIL, or AST(all P < 0.05), but the differences disappeared in patients with early-stage HCC. For patients with TBIL ≤ 1 × ULN, the AUC of PIVKA-Ⅱ was significantly higher compared to that in patients with TBIL > 1 × ULN(0.817 vs 0.669, P = 0.015), while the difference between ALP ≤ 1 × ULN and ALP > 1 × ULN was not statistically significant(0.783 vs 0.729, P = 0.398). These trends were then more prominently perceived in subgroups of patients with viral etiology and HBV alone.CONCLUSION Serum PIVKA-Ⅱ has better performance in detecting HCC at an early stage for CLD patients with normal serum TBIL. 展开更多

Objective: To study the clinical relations of portal hy-pertensive gastropathy (PHG) of hepatitis B cirrho-sis to other factors.Methods: Three groups of subjects were studied pro-spectively at our hospital from March 2000 to March2001: 159 hepatitis B cirrhotic patients with portalhypertension, 114 hepatitis B cirrhotic patients with-out portal hypertension, and 97 control subjects.Free portal vein pressure (FPP) was measured dur-ing surgery. Liver function was assessed by Pugh’smodification of Child’s criteria. The area of livercollagen fibrin was studied using color image analysissystem. Esophageal varices were identified by Dagra-di grading. Gastric varices were identified accordingto Northern Italian Endoscopic Council (NIEC)grading. Hypersplenism was assessed with the reduc-tion of WBC, HGB and PLT. Hepatitis B virus inthe gastric mucosa was detected by immunizing histo-chemistry. Helicobacter pylori (H. pylori) organismswere identified by rapid urease testing and/or exami-nation of the stained biopsy specimens (haematoxylinand eosin). To analyze the correlation between theseendoscopic signs at the gastric level and other fac-tors.Results: The differences of FPP among the threegroups (patients with grade Ⅰ, Ⅱ, and Ⅲ gastropa-thy) were not significant. There was no correlationbetween Child-Pugh classification grading and the se-verity of gastropathy (P=0. 153). The differences ofthe area of liver collagen fibrin among the threegrade gastropathy were not statistically significant (P=0. 801). There was a significant difference in theprevalence of severe PHG among grade Ⅰ, Ⅱ, Ⅲ,Ⅳ and Ⅴ esophageal varices (P【0. 001). PHG waspresent in a similar percentage of patients with gas-tric varices compared with those without gastric vari-ces (P=0. 209). There was a significant difference inthe severity between PHG and hypersplenism (P=0. 003). Seven patients with PHG had no microscopicevidence of hepatitis B virus infection in the gastricwall. There was no correlation between Child-Pughclassification grading and infection of H. pylori (P=0. 7491).Conclusions: The most important element causingPHG is the increased portal pressure as a prerequi-site. In addition, other factors may contribute to thedevelopment of PHG. PHG often occurs in patientswith the presence of esophageal varices. There is amarked correlation between the severity of PHG andhypersplenism. Hepatitis B virus and H. pylori infec-tion are unlikely to be involved in the pathogenesis ofPHG. The development of PHG is less influenced ei-ther by the severity of liver disease (Child-Pughgrade) and cirrhosis or by the presence or non pre-sence of gastric varices. 展开更多

BACKGROUND Bacterial infection is an important cause of cholelithiasis or gallstones and interferes with its treatment.There is no consensus on bile microbial culture profiles in previous studies,and identified microbial spectrum and drug resistance is helpful for targeted preventive and therapeutic drugs in the perioperative period.AIM To analyze the bile microbial spectrum of patients with cholelithiasis and the drug susceptibility patterns in order to establish an empirical antibiotic treatment for cholelithiasis-associated infection.METHODS A retrospective single-center study was conducted on patients diagnosed with cholelithiasis between May 2013 and December 2018.RESULTS This study included 185 patients,of whom 163(88.1%)were diagnosed with gallstones and 22(11.9%)were diagnosed with gallstones and common bile duct stones(CBDSs).Bile culture in 38 cases(20.5%)was positive.The presence of CBDSs(OR=5.4,95%CI:1.3-21.9,P=0.03)and longer operation time(>80 min)(OR=4.3,95%CI:1.4-13.1,P=0.01)were identified as independent risk factors for positive bile culture.Gram-negative bacteria were detected in 28 positive bile specimens,and Escherichia coli(E.coli)(19/28)and Klebsiella pneumoniae(5/28)were the most frequently identified species.Gram-positive bacteria were present in 10 specimens.The resistance rate to cephalosporin in E.coli was above 42%and varied across generations.All the isolated E.coli strains were sensitive to carbapenems,with the exception of one imipenem-resistant strain.K.pneumoniae showed a similar resistance spectrum to E.coli.Enterococcus spp.was largely sensitive to glycopeptides and penicillin,except for a few strains of E.faecium.CONCLUSION The presence of common bile duct stones and longer operation time were identified as independent risk factors for positive bile culture in patients with cholelithiasis.The most commonly detected bacterium was E.coli.The combination ofβ-lactam antibiotics andβ-lactamase inhibitors prescribed perioperatively appears to be effective against bile pathogens and is recommended.Additionally,regular monitoring of emerging resistance patterns is required in the future. 展开更多

The mechanisms of magnetoreception have been proposed as the magnetitebased, the chemical radical-pair and biocompass model, in which magnetite particles, the cryptochrome (Cry) or iron-sulfur cluster assembly 1 (IscA1) may be involved. However, little is known about the association among the molecules. Here we investigated the molecular characterization and the mRNA expression of IscA1 in different developmental stages, tissues and magnetic fields in the migratory brown planthopper (BPH), Nilaparvata lugens. NlIscA1 contains an open reading frame of 390 bp, encoding amino acids of 129, with the predicted molecular weight of 14.0 kDa and the isoelectric point of 9.10. Well-conserved Fe-S cluster binding sites were observed in the predicted protein. Phylogenetic analysis demonstrated NlIscA1 to be clustered into the insect's IscA1. NlIscA1 showed up-regulated mRNA expression during the period of migration. The mRNA expression of NlIscA1 could be detected in all the three tissues of head, thorax and abdomen, with the highest expression level in the abdomen. For the macropterous migratory Nilaparvata lugens, mRNA expression of NlIscA1 and N. lugens cryptochromel (Nlcry1) were up-regulated under the magnetic fields of 5 Gauss and 10 Gauss in strength (vs. local geomagnetic field), while N. lugens cryptochrome 2 (Nlcry2) remained stable. For the brachyterous non-migratory Nilaparvata lugens, no significant changes were found in mRNA expression of NlIscA1, Nlcry1 and Nlcry2 among different magnetic fields. These findings preliminarily reveal that the expression of NlIscA1 and Nlcry1 exhibited coordinated responses to the magnetic field. It suggests some potential associations among the putative magneto-sensitive molecules of cryptochrome and iron-sulfur cluster assembly. 展开更多

To quantitatively evaluate severity of behavioral and psychological symptoms of dementia(BPSD)for vascular dementia(VD).Changes of 51 patients with VD in BPSD between the first and 24th week were assessed using the Neuropsychiatric Inventory(NPI)and the behavioral pathology in Alzheimer’s disease(BEHAVE-AD)rating scale,in detrended fluctuation analysis(DFA)represented by diurnal activity(DA),evening activity(EA),and nocturnal activity(NA),and the relationships were analyzed.The subscores of activity disturbances,diurnal rhythm disturbances,and anxieties and phobias in the BEHAVE-AD score,and that of agitation,irritability,and sleep disorder in the NPI score were significantly increased compared with the first week,as was for the changes for EA in the DFA value.A linear correlation was observed between the changes of activity disturbances plus anxieties and phobias,and those of DA,and between the development of diurnal rhythm and those of EA,the vehement and autism scores and those of DA,and the difference in sleep disorder scores and those of EA,respectively.Analysis of DA,NA,and EA may reflect the fluctuational degrees of VD-BPSD,can provide a useful assessment of VD-BPSD accompanied by clinical scores for VD. 展开更多

The geomagnetic field(GMF)is well documented for its essential role as a cue used in animal orientation or navigation.Recent evidence indicates that the absence of GMF(mimicked by the near-zero magnetic field,NZMF)can trigger stress-like responses such as reduced body weight,as we have previously shown in the brown planthopper,Nilaparvata lugens.In this study,we found that consistent with the significantly decreased body weight of newly emerged female(—14.67%)and male(—13.17%)adult N.lugens,the duration of the phloem ingestion feeding waveform was significantly reduced by 32.02%in 5th instar nymphs reared under the NZMF versus GMF.Interestingly,5th instar nymphs that exhibited reduced feeding had significantly higher glucose levels(+16.98%and+20.05%;24 h and 48 h after molting),which are associated with food aversion,and expression patterns of their appetite-related neuropeptide genes(neuropeptide F,dow regulated overall;short neuropeptide F,dowregulated overall;adipokinetic hormone up-regulated overall;and adipokinetic hormone receptor,down-regulated overall)were also altered under the absence of GMF in a manner consistent with diminishing appetite.Moreover,the expressions of the potential magnetosensor croptochromes(Crys)were found significantly altered under the absence of GMF,indicating the likely upstream signaling of the Cry-mediated magnetoreception mechanisms.These findings support the hypothesis that strong changes in GMF intensity can reduce adult body weight through affecting insect feeding behavior and underlying regulatory processes including appetite regulation.Our results highlight that GMF could be necessary for the maintenanee of energy homeostasis in insects. 展开更多

The geomagnetic field(GMF)is an environmental cue that provides directional information for animals.The intensity of GMF is varied over space and time.Variations in the GMF intensity afect the navigation of animals and their physiology.In this study,the phototaxis of the migratory insect rice planthopper Nilaparvata lugens(N.lugens)and frataxin in N.lugens(NI-fh),which is a mitochondrial protein required for cellular iron homeostasis and iron-sulfur cluster assembly,were investigated by using different intensities of magnetic field.From the results,individuals of N.lugens showed decreased phototaxis when reared and tested in a behavioral arena under a strong magnetic field.Besides the reduction in performance,an accompanying ffect of the strong magnetic field condition was a reduced level of Nl-fh-messenger RNA,and a NI-fh knockdown indeed impaired the phototactic behavior in a tested sample of insects.This leads to the conclusion that the expression of frataxin is dependent on the strength of the surrounding magnetic field and that functional frataxin facilitates phototactic behavior in N.lugens. 展开更多
