BACKGROUND Lingguizhugan(LGZG)decoction is a widely used classic Chinese medicine formula that was recently shown to improve high-fat diet(HFD)-induced insulin resistance(IR)in animal studies.AIM To assess the therape...BACKGROUND Lingguizhugan(LGZG)decoction is a widely used classic Chinese medicine formula that was recently shown to improve high-fat diet(HFD)-induced insulin resistance(IR)in animal studies.AIM To assess the therapeutic effect of LGZG decoction on HFD-induced IR and explore the potential underlying mechanism.METHODS To establish an IR rat model,a 12-wk HFD was administered,followed by a 4-wk treatment with LGZG.The determination of IR status was achieved through the use of biochemical tests and oral glucose tolerance tests.Using a targeted metabolomics platform to analyze changes in serum metabolites,quantitative real-time PCR(qRT-PCR)was used to assess the gene expression of the ribosomal protein S6 kinase beta 1(S6K1).RESULTS In IR rats,LGZG decreased body weight and indices of hepatic steatosis.It effectively controlled blood glucose and food intake while protecting islet cells.Metabolite analysis revealed significant differences between the HFD and HFDLGZG groups.LGZG intervention reduced branched-chain amino acid levels.Levels of IR-related metabolites such as tryptophan,alanine,taurine,and asparagine decreased significantly.IR may be linked to amino acids due to the contemporaneous increase in S6K1 expression,as shown by qRT-PCR.CONCLUSIONS Our study strongly suggests that LGZG decoction reduces HFD-induced IR.LGZG may activate S6K1 via metabolic pathways.These findings lay the groundwork for the potential of LGZG as an IR treatment.展开更多

The present study aimed to investigate the durability and microstructure evolution of road base materials(RBM)prepared from red mud and flue gas desulfurization fly ash.The durability testing showed that the strength ...The present study aimed to investigate the durability and microstructure evolution of road base materials(RBM)prepared from red mud and flue gas desulfurization fly ash.The durability testing showed that the strength of RBM with the blast furnace slag addition of 1wt%,3wt%and 5wt%reached 3.81,4.87,and 5.84 MPa after 5 freezing–thawing(F–T)cycles and reached 5.21,5.75,and 6.98 MPa after 20 weting–drying(W–D)cycles,respectively.The results also indicated that hydration products were continuously formed even during W–D and F–T exposures,resulting in an increase of the strength and durability of RBM.The observed increase of macropores(>1μm)after F–T and W–D exposures suggested that the mechanism of RBM deterioration is pore enlargement due to cracks that develop inside their matrix.Moreover,the F–T exposure showed a greater negative effect on the durability of RBM compared to the W–D exposure.The leaching tests showed that sodium and heavy metals were solidified below the minimum requirement,which indicates that these wastes are suitable for use as a natural material replacement in road base construction.展开更多
Red mud is a type of highly alkaline waste residue produced in the process of alumina smelting by the Bayer process.Based on the idea of medium calcium content,solid wastes such as red mud and fly ash were used to pre...Red mud is a type of highly alkaline waste residue produced in the process of alumina smelting by the Bayer process.Based on the idea of medium calcium content,solid wastes such as red mud and fly ash were used to prepare non-burnt bricks;and the mass ratio of CaO/SiO2 was selected in the range of 0.88–1.42.Mechanical properties and durability were investigated with a compressive strength test.X-ray diffractometry(XRD),scanning electron microscope(SEM),and Fourier transform infrared spectroscopy(FTIR)techniques were used to characterize the hydration characteristic.The environmental performance was analyzed by Inductively Coupled Plasma Mass Spectrometry(ICP).The results indicated that the mechanical properties and the durability were optimal when the mass ratio of CaO/SiO2 was 1.23.The hydration products were mostly C–S–H gel,ettringite,Na4Ca(Si10All6)O32·12H2O and Ca3Al2(SiO4)(OH)8.They were responsible for the strength development,and the CaO/SiO2 mass ratio of 1.23 had the best polymerized structure.The results of an environmental performance test showed that the heavy metals in the raw materials were well-solidified in the brick.Therefore,this paper provides an effective solution for use of solid wastes in building material.展开更多
AIM To compare the outcomes of retrograde intrarenal surgery(RIRS) and miniaturized percutaneous nephrolithotomy(mini-PCNL) in treating lower pole(LP) renal stones with a diameter of 1.5-2.5 cm.METHODS A total of 216 ...AIM To compare the outcomes of retrograde intrarenal surgery(RIRS) and miniaturized percutaneous nephrolithotomy(mini-PCNL) in treating lower pole(LP) renal stones with a diameter of 1.5-2.5 cm.METHODS A total of 216 patients who underwent mini-PCNL(n = 103) or RIRS n = 113) for LP stones with a diameter of 1.5-2.5 cm were enrolled between December 2015 and April 2017 at the Urology Department of Ningbo Urology and Nephrology Hospital.RESULTS Significant differences were found in the hospital stay(9.39 ± 4.01 vs 14.08 ± 5.26, P < 0.0001) and hospitalization costs(2624.5 ± 513.36 vs 3255.2 ± 976.5, P < 0.0001) between the RIRS and mini-PCNL groups. The mean operation time was not significantly different between the RIRS group(56.48 ± 24.77) and the mini-PCNL group(60.04 ± 30.38, P = 0.345). The stone-free rates at the first postoperative day(RIRS vs mini-PCNL: 90.2% vs 93.2%, P = 0.822) and the second month postoperatively(RIRS vs mini-PCNL: 93.8% vs 95.1%, P = 0.986) were not significantly different.CONCLUSION RIRS and mini-PCNL are both safe and effective methods for treating LP stones with a diameter of 1.5-2.5 cm. RIRS can be considered as an alternative to PCNL for the treatment for LP stones of 1.5-2.5 cm.展开更多

METHODS: Eighty-six elderly patients with mild/m...AIM: To investigate stepwise sedation for elderly patients with mild/moderate chronic obstructive pulmonary disease (COPD) during upper gastrointestinal (GI) endoscopy. METHODS: Eighty-six elderly patients with mild/moderate COPD and 82 elderly patients without COPD scheduled for upper GI endoscopy were randomly assigned to receive one of the following two sedation methods: stepwise sedation involving three-stage administration of propofol combined with midazolam [COPD with stepwise sedation (group Cs), and nonCOPD with stepwise sedation (group Ns)] or continuous sedation involving continuous administration of propofol combined with midazolam [COPD with continuous sedation (group Cc), and non-COPD with continuous sedation (group Nc)]. Saturation of peripheral oxygen (SpO 2 ), blood pressure, and pulse rate were monitored, and patient discomfort, adverse events, drugs dosage, and recovery time were recorded. RESULTS: All endoscopies were completed successfully. The occurrences of hypoxemia in groups Cs, Cc, Ns, and Nc were 4 (9.3%), 12 (27.9%), 3 (7.3%), and 5 (12.2%), respectively. The occurrence of hypoxemia in group Cs was significantly lower than that in group Cc (P < 0.05). The average decreases in value of SpO 2 , systolic blood pressure, and diastolic blood pressure in group Cs were significantly lower than those in group Cc. Additionally, propofol dosage and overall rate of adverse events in group Cs were lower than those in group Cc. Finally, the recovery time in group Cs was significantly shorter than that in group Cc, and that in group Ns was significantly shorter than that in group Nc (P < 0.001). CONCLUSION: The stepwise sedation method is effective and safer than the continuous sedation method for elderly patients with mild/moderate COPD during upper GI endoscopy.展开更多

Notoginsenoside R1, ginsenoside Rg1, Re, Rb1 and Rd are the major active constituents of Sanqi. The purpose of the study was to investigate the pharmacokinetic behavior of the five active constituents from total saponin from Sanqi when it was used in the blood stasis animals or in combination with Gegen. The concentrations of the five active constituents in rat plasma were determined by an ultra-HPLC-ESI-MS/MS method. The main pharmacokinetic parameters were calculated and statistically analyzed using the unpaired student&#39;s t-test. It was found that the pharmacokinetic parameters of notoginsenoside R1, ginsenoside Rg1 and Rb1 represented a statistically significant difference (Po0.05) between the normal rats and the blood stasis rats after administration of total saponin from Sanqi (TSFS). And there were statistically significant differences (Po0.05) in the pharmacokinetic parameters of all the five constituents between administration of TSFS alone and combined with total flavonoid from Gegen (TFFG) in blood stasis rats. It suggested that the pharmacokinetic behavior of the active constituents from TSFS could be changed when it was used in blood stasis animals or in combination with TFFG.展开更多

METHODS Approximately 25 specimens at...AIM To clarify the mechanisms of connexin 32 (Cx32) downregulation by potential transcriptional factors (TFs) in Helicobacter pylori (H. pylori)-associated gastric carcinogenesis. METHODS Approximately 25 specimens at each developmental stage of gastric carcinogenesis [non-atrophic gastritis, chronic atrophic gastritis, intestinal metaplasia, dysplasia and gastric carcinoma (GC)] with H. pylori infection [H. pylori (+)] and 25 normal gastric mucosa (NGM) without H. pylori infection [H. pylori (-)] were collected. After transcriptional factor array analysis, the Cx32 and PBX1 expression levels of H. pylori-infected tissues from the developmental stages of GC and NGM with no H. pylori infection were measured by real-time polymerase chain reaction (RT-PCR) and Western blot analysis. Regarding H. pylori-infected animal models, the Cx32 and PBX1 mRNA expression levels and correlation between the gastric mucosa from 10 Mongolian gerbils with long-term H. pylori colonization and 10 controls were analyzed. PBX1 and Cx32 mRNA and protein levels were further studied under the H. pylori-infected condition as well as PBX1 overexpression and knockdown conditions in vitro. RESULTS Incremental PBX1 was first detected by TF microarray in H. pylori-related gastric carcinogenesis. The identical trend of PBX1 and Cx32 expression was confirmed in the developmental stages of H. pylori-related clinical specimens. The negative correlation of PBX1 and Cx32 was confirmed in H. pylori-infected Mongolian gerbils. Furthermore, decreased PBX1 expression was detected in the normal gastric epithelial cell line GES-1 with H. pylori infection. Enforced overexpression or RNAi-mediated knockdown of PBX1 contributed to the diminished or restored Cx32 expression in GES-1 and the gastric carcinoma cell line BGC823, respectively. Finally, dual-luciferase reporter assay in HEK293T cells showed that Cx32 promoter activity decreased by 30% after PBX1 vector co-transfection, indicating PBX1 as a transcriptional downregulator of Cx32 by directly binding to its promoters. ONCLUSION PBX1 is one of the determinants in the Cx32 promoter targeting site, preventing further damage of gap junction protein in H. pylori-associated gastric carcinogenesis.展开更多
BACKGROUND Helicobacter pylori(H.pylori)infection is closely associated with the etiology of a variety of gastric diseases.The effective eradication of H.pylori infection has been shown to reduce the incidence of gast...BACKGROUND Helicobacter pylori(H.pylori)infection is closely associated with the etiology of a variety of gastric diseases.The effective eradication of H.pylori infection has been shown to reduce the incidence of gastric carcinoma.However,the rate of H.pylori eradication has significantly declined due to its increasing resistance to antibiotics,especially to clarithromycin.Therefore,the detection of clarithromycin resistance is necessary prior to the treatment of H.pylori.Although many studies have been conducted on the use of polymerase chain reaction(PCR)-based tests to detect clarithromycin resistance in stool samples,no accurate data on the feasibility of these tests are available.Here,we performed a meta-analysis to assess the feasibility of these noninvasive tests.AIM To evaluate the reliability of PCR-based tests for detecting H.pylori clarithromycin resistance in stool samples.METHODS We searched PubMed,Medline,Embase,and other databases for articles that evaluated the value of the PCR analysis of stool samples for detecting the resistance of H.pylori to clarithromycin.We collected cross-sectional studies that met the inclusion criteria.Diagnostic accuracy measures were pooled using a random-effects model.The risk of bias was assessed using the Quality Assessment of Diagnostic Accuracy Studies-2 tool.Subgroup analysis was also conducted according to PCR type,purification technique,reference standard,mutation site,sample weight,number of patients,and age group,and the clinical utility of diagnostic tests was evaluated using the Likelihood Ratio Scatter Graph.RESULTS Out of the 1818 identified studies,only 11 met the eligibility criteria,with a total of 592 patients assessed.A meta-analysis of the random-effect model showed that PCR-based analysis of stool samples had high diagnostic accuracy for detecting clarithromycin resistance in patients infected with H.pylori.The combined sensitivity was 0.91[95%confidence interval(CI):0.83-0.95],Q=30.34,and I2=67.04,and the combined specificity was 0.97(95%CI:0.62-1.00),Q=279.54,and I2=96.42.The likelihood ratio for a positive test was 33.25(95%CI:1.69-652.77),and that for a negative test was 0.10(95%CI:0.05-0.18),with an area under the curve of 0.94.The diagnostic odds ratio was 347.68(95%CI:17.29-6991.26).There was significant statistical heterogeneity,and the sub-analyses showed significant differences in the number of patients,sample weight,purification methods,PCR types,mutation points,and reference standards.The included studies showed no risk of publication bias.CONCLUSION PCR-based tests on stool samples have high diagnostic accuracy for detecting H.pylori clarithromycin resistance.展开更多

An isothermal calorimeter was used to characterize the hydration heat evolution. The results show that the hydration of RGC is controlled by the processes of nucleation and crystal growth(NG), interaction at phase boundaries(I), and diffusion(D) in order, and the pozzolanic reactions of slag and compound-activated red mud-coal gangue are mainly controlled by the I process. Slag accelerates the clinker hydration during NG process, whereas the compound-activated red mud-coal gangue retards the hydration of RGC and the time required for I process increases with increasing dosage of red mud-coal gangue in RGC.展开更多

Methods : A total of 36 male monkeys were randomly divided into four groups and fed human diets with high sugar, high fat, double high sugar and fat, and a normal diet. The preclinical diabetes phase was determined by monitoring the metabolic characteristic indices and the results of oral glucose tolerance tests( OGTT). The mRNA expression of 45 diabetes-related genes in peripheral blood leukocytes was analyzed using real-time PCR. Results : A total of 22, 25, and 21 genes were significantly up-regulated( P < 0.05) and 5, 7, and 5 genes were significantly down-regulated( P < 0.05) in the above three induced groups, respectively, compared with the control group. Of the 45 tested genes, the expression profiles of 21 genes were consistent. Most of the expression levels in the double high sugar-and-fat individuals were slightly lower than those in the high glucose and high fat groups, although the expression patterns of the three groups were essentially similar. Conclusion : The different high energy diets all induced diabetes and shared some phenotypic properties with human T2 DM. Most of the expression patterns of the related genes were identical. The gene expression profiles could be used as references for the study of early diagnostic indicators and T2 DM pathogenesis.展开更多

The reaction performance of silica-alumina-based materials with different molar ratios of Si/Al, which were thermally activated, was also investigated. With the increase in calcining temperature, the coordination of Al in metakaolin becomes four, five, and six firstly, and then transforms completely to four and six. It is indicated by identical coupled plasma optical emission spectroscopy (ICP) and NMR that, the reaction performance of monomeric silicate anions is better than that of polymeric silicate anions which are primarily cross-linked in the alkali solution. Moreover, it also shows that the thermal activation temperature, cooling method, and the molar ratio of Na/Ca have remarkable effects on the reaction performance.展开更多

IgG4-related disease(IgG4-RD)is an increasingly recognized pathological entity that tends to involve multiple organs with an elevated level of serum IgG4,which is easily misdiagnosed owing to sharing common clinical f...IgG4-related disease(IgG4-RD)is an increasingly recognized pathological entity that tends to involve multiple organs with an elevated level of serum IgG4,which is easily misdiagnosed owing to sharing common clinical features with a variety of other diseases.Here,we report an interesting IgG4-RD case of a woman with progressive multi-organ involvement for over 19 years,started with swollen eyelids,dry eye and mouth,and polydipsia and hydruria.Imaging diagnosis revealed diffuse enlargement of the parotid glands,enlargement of the head of the pancreas,pulmonary infection and interstitial lung.Serological tests showed a remarkable elevation of the serum IgG4,and cytological analysis further revealed a large amount of lymphoplasmacytic infiltration into the focal lobule,and IgG 4-positive cell infiltration in bladder mucosa.Therapeutically,the patient responded well to steroid therapy,and thus,she was diagnosed as IgG 4-RD suspicious.This report highlights the importance of an early diagnosis in this autoimmune disease and suggests that patients with a clinically unclear cause of inflammation,swelling and refractory glands,rhinitis,pancreatitis,hypophysitis,and/or interstitial pneumonia should be considered for IgG 4-RD.The plasma IgG4 level and lymphoplasmacytic infiltration may be useful indexes for screening,and a low dose of steroid maintaining therapy may offer benefits for patients with IgG4-RD.展开更多

A micro-electro-mechanical systems-based (MEMS-based) high-resistance silicon C-band microstrip antenna array has been designed for the intelligent ammunition. The center frequency is 4.5 GHz. A cavity has been designed in substrate to reduce the dielectric constant of silicon and high-resistance silicon has been used as the material of substrate to improve the gain of antenna. It is very easy to be manufactured by using MEMS technology because of the improved structure of the antenna. The results show that the gain of the antenna is 8 dB and voltage standing wave ratio (VSWR) is less than 2 by the analysis and simulation in high freauencv structure simulator (HFSS).展开更多
BACKGROUND Despite the overload of publications on Crohn’s disease(CD),no comprehensive analysis of biologic therapy for CD has been reported.AIM To determine knowledge gaps and identify areas of interest of biologic...BACKGROUND Despite the overload of publications on Crohn’s disease(CD),no comprehensive analysis of biologic therapy for CD has been reported.AIM To determine knowledge gaps and identify areas of interest of biologic therapy for CD.METHODS The top 100 highest-cited original articles were identified from January 1991 to December 2020 in the Clarivate Analytics Web of Science Core Collection database.We conducted a bibliometric analysis of biologic therapy for CD based on total citations,summarized the bibliographic information of the articles related to CD biologic therapy,and explored the research hotspots.RESULTS The top 100 highest-cited original articles were identified with total citations ranging from 307 to 2978.The 2000s(Period II,n=66)yielded the most influential original articles and saw the most dramatic growth.Among the top 10 countries,including 8 European countries and 2 North American countries,the United States(n=37)and Belgium(n=20)contributed the most publications.Among the top 10 institutions,the University Hospital Gasthuisberg in Belgium(n=23),the University of Chicago in the United States(n=20),and the Mayo Clinic in the United States(n=17)published the most papers.Regarding authors,Rutgeerts P in Belgium(n=32),Sandborn WJ in the United States(n=23),and Feagan BG in Canada(n=18)published the highest number of studies.The cooperation relationships between the United States and Europe were most frequent.Gastroenterology(impact factor=22.682)published the most articles on biologic therapy for CD(n=32)with 17654 total citations.Anti-tumor necrosis factor biologics and monoclonal antibodies were the most studied topics.CONCLUSION The bibliometric analysis emphasized the key contributions to the development of the specialized field.These data would provide useful research insights into biologic therapy for CD for clinicians and researchers.展开更多

文摘BACKGROUND Lingguizhugan(LGZG)decoction is a widely used classic Chinese medicine formula that was recently shown to improve high-fat diet(HFD)-induced insulin resistance(IR)in animal studies.AIM To assess the therapeutic effect of LGZG decoction on HFD-induced IR and explore the potential underlying mechanism.METHODS To establish an IR rat model,a 12-wk HFD was administered,followed by a 4-wk treatment with LGZG.The determination of IR status was achieved through the use of biochemical tests and oral glucose tolerance tests.Using a targeted metabolomics platform to analyze changes in serum metabolites,quantitative real-time PCR(qRT-PCR)was used to assess the gene expression of the ribosomal protein S6 kinase beta 1(S6K1).RESULTS In IR rats,LGZG decreased body weight and indices of hepatic steatosis.It effectively controlled blood glucose and food intake while protecting islet cells.Metabolite analysis revealed significant differences between the HFD and HFDLGZG groups.LGZG intervention reduced branched-chain amino acid levels.Levels of IR-related metabolites such as tryptophan,alanine,taurine,and asparagine decreased significantly.IR may be linked to amino acids due to the contemporaneous increase in S6K1 expression,as shown by qRT-PCR.CONCLUSIONS Our study strongly suggests that LGZG decoction reduces HFD-induced IR.LGZG may activate S6K1 via metabolic pathways.These findings lay the groundwork for the potential of LGZG as an IR treatment.

文摘Helicobacter pylori (H. pylori) is a Gram-negative bacterium with a number of virulence factors, such as cytotoxin-associated gene A, vacuolating cytotoxin A, its pathogenicity island, and lipopolysaccharide, which cause gastrointestinal diseases. Connexins function in gap junctional homeostasis, and their downregulation is closely related to gastric carcinogenesis. Investigations into H. pylori infection and the fine-tuning of connexins in cells or tissues have been reported in previous studies. Therefore, in this review, the potential mechanisms of H. pylori-induced gastric cancer through connexins are summarized in detail.

文摘The present study aimed to investigate the durability and microstructure evolution of road base materials(RBM)prepared from red mud and flue gas desulfurization fly ash.The durability testing showed that the strength of RBM with the blast furnace slag addition of 1wt%,3wt%and 5wt%reached 3.81,4.87,and 5.84 MPa after 5 freezing–thawing(F–T)cycles and reached 5.21,5.75,and 6.98 MPa after 20 weting–drying(W–D)cycles,respectively.The results also indicated that hydration products were continuously formed even during W–D and F–T exposures,resulting in an increase of the strength and durability of RBM.The observed increase of macropores(>1μm)after F–T and W–D exposures suggested that the mechanism of RBM deterioration is pore enlargement due to cracks that develop inside their matrix.Moreover,the F–T exposure showed a greater negative effect on the durability of RBM compared to the W–D exposure.The leaching tests showed that sodium and heavy metals were solidified below the minimum requirement,which indicates that these wastes are suitable for use as a natural material replacement in road base construction.

文摘Red mud is a type of highly alkaline waste residue produced in the process of alumina smelting by the Bayer process.Based on the idea of medium calcium content,solid wastes such as red mud and fly ash were used to prepare non-burnt bricks;and the mass ratio of CaO/SiO2 was selected in the range of 0.88–1.42.Mechanical properties and durability were investigated with a compressive strength test.X-ray diffractometry(XRD),scanning electron microscope(SEM),and Fourier transform infrared spectroscopy(FTIR)techniques were used to characterize the hydration characteristic.The environmental performance was analyzed by Inductively Coupled Plasma Mass Spectrometry(ICP).The results indicated that the mechanical properties and the durability were optimal when the mass ratio of CaO/SiO2 was 1.23.The hydration products were mostly C–S–H gel,ettringite,Na4Ca(Si10All6)O32·12H2O and Ca3Al2(SiO4)(OH)8.They were responsible for the strength development,and the CaO/SiO2 mass ratio of 1.23 had the best polymerized structure.The results of an environmental performance test showed that the heavy metals in the raw materials were well-solidified in the brick.Therefore,this paper provides an effective solution for use of solid wastes in building material.

文摘AIM To compare the outcomes of retrograde intrarenal surgery(RIRS) and miniaturized percutaneous nephrolithotomy(mini-PCNL) in treating lower pole(LP) renal stones with a diameter of 1.5-2.5 cm.METHODS A total of 216 patients who underwent mini-PCNL(n = 103) or RIRS n = 113) for LP stones with a diameter of 1.5-2.5 cm were enrolled between December 2015 and April 2017 at the Urology Department of Ningbo Urology and Nephrology Hospital.RESULTS Significant differences were found in the hospital stay(9.39 ± 4.01 vs 14.08 ± 5.26, P < 0.0001) and hospitalization costs(2624.5 ± 513.36 vs 3255.2 ± 976.5, P < 0.0001) between the RIRS and mini-PCNL groups. The mean operation time was not significantly different between the RIRS group(56.48 ± 24.77) and the mini-PCNL group(60.04 ± 30.38, P = 0.345). The stone-free rates at the first postoperative day(RIRS vs mini-PCNL: 90.2% vs 93.2%, P = 0.822) and the second month postoperatively(RIRS vs mini-PCNL: 93.8% vs 95.1%, P = 0.986) were not significantly different.CONCLUSION RIRS and mini-PCNL are both safe and effective methods for treating LP stones with a diameter of 1.5-2.5 cm. RIRS can be considered as an alternative to PCNL for the treatment for LP stones of 1.5-2.5 cm.

文摘AIM: To investigate stepwise sedation for elderly patients with mild/moderate chronic obstructive pulmonary disease (COPD) during upper gastrointestinal (GI) endoscopy. METHODS: Eighty-six elderly patients with mild/moderate COPD and 82 elderly patients without COPD scheduled for upper GI endoscopy were randomly assigned to receive one of the following two sedation methods: stepwise sedation involving three-stage administration of propofol combined with midazolam [COPD with stepwise sedation (group Cs), and nonCOPD with stepwise sedation (group Ns)] or continuous sedation involving continuous administration of propofol combined with midazolam [COPD with continuous sedation (group Cc), and non-COPD with continuous sedation (group Nc)]. Saturation of peripheral oxygen (SpO 2 ), blood pressure, and pulse rate were monitored, and patient discomfort, adverse events, drugs dosage, and recovery time were recorded. RESULTS: All endoscopies were completed successfully. The occurrences of hypoxemia in groups Cs, Cc, Ns, and Nc were 4 (9.3%), 12 (27.9%), 3 (7.3%), and 5 (12.2%), respectively. The occurrence of hypoxemia in group Cs was significantly lower than that in group Cc (P < 0.05). The average decreases in value of SpO 2 , systolic blood pressure, and diastolic blood pressure in group Cs were significantly lower than those in group Cc. Additionally, propofol dosage and overall rate of adverse events in group Cs were lower than those in group Cc. Finally, the recovery time in group Cs was significantly shorter than that in group Cc, and that in group Ns was significantly shorter than that in group Nc (P < 0.001). CONCLUSION: The stepwise sedation method is effective and safer than the continuous sedation method for elderly patients with mild/moderate COPD during upper GI endoscopy.

文摘Sanqi is a popular traditional Chinese medicine and commonly used for promoting blood circulation and removing blood stasis. Notoginsenoside R1, ginsenoside Rg1, Re, Rb1 and Rd are the major active constituents of Sanqi. The purpose of the study was to investigate the pharmacokinetic behavior of the five active constituents from total saponin from Sanqi when it was used in the blood stasis animals or in combination with Gegen. The concentrations of the five active constituents in rat plasma were determined by an ultra-HPLC-ESI-MS/MS method. The main pharmacokinetic parameters were calculated and statistically analyzed using the unpaired student&#39;s t-test. It was found that the pharmacokinetic parameters of notoginsenoside R1, ginsenoside Rg1 and Rb1 represented a statistically significant difference (Po0.05) between the normal rats and the blood stasis rats after administration of total saponin from Sanqi (TSFS). And there were statistically significant differences (Po0.05) in the pharmacokinetic parameters of all the five constituents between administration of TSFS alone and combined with total flavonoid from Gegen (TFFG) in blood stasis rats. It suggested that the pharmacokinetic behavior of the active constituents from TSFS could be changed when it was used in blood stasis animals or in combination with TFFG.

文摘AIM To clarify the mechanisms of connexin 32 (Cx32) downregulation by potential transcriptional factors (TFs) in Helicobacter pylori (H. pylori)-associated gastric carcinogenesis. METHODS Approximately 25 specimens at each developmental stage of gastric carcinogenesis [non-atrophic gastritis, chronic atrophic gastritis, intestinal metaplasia, dysplasia and gastric carcinoma (GC)] with H. pylori infection [H. pylori (+)] and 25 normal gastric mucosa (NGM) without H. pylori infection [H. pylori (-)] were collected. After transcriptional factor array analysis, the Cx32 and PBX1 expression levels of H. pylori-infected tissues from the developmental stages of GC and NGM with no H. pylori infection were measured by real-time polymerase chain reaction (RT-PCR) and Western blot analysis. Regarding H. pylori-infected animal models, the Cx32 and PBX1 mRNA expression levels and correlation between the gastric mucosa from 10 Mongolian gerbils with long-term H. pylori colonization and 10 controls were analyzed. PBX1 and Cx32 mRNA and protein levels were further studied under the H. pylori-infected condition as well as PBX1 overexpression and knockdown conditions in vitro. RESULTS Incremental PBX1 was first detected by TF microarray in H. pylori-related gastric carcinogenesis. The identical trend of PBX1 and Cx32 expression was confirmed in the developmental stages of H. pylori-related clinical specimens. The negative correlation of PBX1 and Cx32 was confirmed in H. pylori-infected Mongolian gerbils. Furthermore, decreased PBX1 expression was detected in the normal gastric epithelial cell line GES-1 with H. pylori infection. Enforced overexpression or RNAi-mediated knockdown of PBX1 contributed to the diminished or restored Cx32 expression in GES-1 and the gastric carcinoma cell line BGC823, respectively. Finally, dual-luciferase reporter assay in HEK293T cells showed that Cx32 promoter activity decreased by 30% after PBX1 vector co-transfection, indicating PBX1 as a transcriptional downregulator of Cx32 by directly binding to its promoters. ONCLUSION PBX1 is one of the determinants in the Cx32 promoter targeting site, preventing further damage of gap junction protein in H. pylori-associated gastric carcinogenesis.

文摘BACKGROUND Helicobacter pylori(H.pylori)infection is closely associated with the etiology of a variety of gastric diseases.The effective eradication of H.pylori infection has been shown to reduce the incidence of gastric carcinoma.However,the rate of H.pylori eradication has significantly declined due to its increasing resistance to antibiotics,especially to clarithromycin.Therefore,the detection of clarithromycin resistance is necessary prior to the treatment of H.pylori.Although many studies have been conducted on the use of polymerase chain reaction(PCR)-based tests to detect clarithromycin resistance in stool samples,no accurate data on the feasibility of these tests are available.Here,we performed a meta-analysis to assess the feasibility of these noninvasive tests.AIM To evaluate the reliability of PCR-based tests for detecting H.pylori clarithromycin resistance in stool samples.METHODS We searched PubMed,Medline,Embase,and other databases for articles that evaluated the value of the PCR analysis of stool samples for detecting the resistance of H.pylori to clarithromycin.We collected cross-sectional studies that met the inclusion criteria.Diagnostic accuracy measures were pooled using a random-effects model.The risk of bias was assessed using the Quality Assessment of Diagnostic Accuracy Studies-2 tool.Subgroup analysis was also conducted according to PCR type,purification technique,reference standard,mutation site,sample weight,number of patients,and age group,and the clinical utility of diagnostic tests was evaluated using the Likelihood Ratio Scatter Graph.RESULTS Out of the 1818 identified studies,only 11 met the eligibility criteria,with a total of 592 patients assessed.A meta-analysis of the random-effect model showed that PCR-based analysis of stool samples had high diagnostic accuracy for detecting clarithromycin resistance in patients infected with H.pylori.The combined sensitivity was 0.91[95%confidence interval(CI):0.83-0.95],Q=30.34,and I2=67.04,and the combined specificity was 0.97(95%CI:0.62-1.00),Q=279.54,and I2=96.42.The likelihood ratio for a positive test was 33.25(95%CI:1.69-652.77),and that for a negative test was 0.10(95%CI:0.05-0.18),with an area under the curve of 0.94.The diagnostic odds ratio was 347.68(95%CI:17.29-6991.26).There was significant statistical heterogeneity,and the sub-analyses showed significant differences in the number of patients,sample weight,purification methods,PCR types,mutation points,and reference standards.The included studies showed no risk of publication bias.CONCLUSION PCR-based tests on stool samples have high diagnostic accuracy for detecting H.pylori clarithromycin resistance.

文摘To elucidate the intrinsic reaction mechanism of cementitious materials composed of red mud and coal gangue(RGC), the hydration kinetics of these cementitious materials at 20°C was investigated on the basis of the Krstulovi?-Dabi? model. An isothermal calorimeter was used to characterize the hydration heat evolution. The results show that the hydration of RGC is controlled by the processes of nucleation and crystal growth(NG), interaction at phase boundaries(I), and diffusion(D) in order, and the pozzolanic reactions of slag and compound-activated red mud-coal gangue are mainly controlled by the I process. Slag accelerates the clinker hydration during NG process, whereas the compound-activated red mud-coal gangue retards the hydration of RGC and the time required for I process increases with increasing dosage of red mud-coal gangue in RGC.

文摘Background : Cynomolgus disease models that are similar to the preclinical stage of human type 2 diabetes mellitus(T2 DM) were established by feeding middle-aged cynomolgus monkeys different high energy diets to study the differential expression of diabetes-related genes. Methods : A total of 36 male monkeys were randomly divided into four groups and fed human diets with high sugar, high fat, double high sugar and fat, and a normal diet. The preclinical diabetes phase was determined by monitoring the metabolic characteristic indices and the results of oral glucose tolerance tests( OGTT). The mRNA expression of 45 diabetes-related genes in peripheral blood leukocytes was analyzed using real-time PCR. Results : A total of 22, 25, and 21 genes were significantly up-regulated( P < 0.05) and 5, 7, and 5 genes were significantly down-regulated( P < 0.05) in the above three induced groups, respectively, compared with the control group. Of the 45 tested genes, the expression profiles of 21 genes were consistent. Most of the expression levels in the double high sugar-and-fat individuals were slightly lower than those in the high glucose and high fat groups, although the expression patterns of the three groups were essentially similar. Conclusion : The different high energy diets all induced diabetes and shared some phenotypic properties with human T2 DM. Most of the expression patterns of the related genes were identical. The gene expression profiles could be used as references for the study of early diagnostic indicators and T2 DM pathogenesis.

文摘A systematic study was conducted to comprehend the mechanism of thermal activation of silica-alumina materials by using ^29Si and ^27Al magnetic angle spinning nuclear magnetic resonance (MAS NMR) spectroscopy. The reaction performance of silica-alumina-based materials with different molar ratios of Si/Al, which were thermally activated, was also investigated. With the increase in calcining temperature, the coordination of Al in metakaolin becomes four, five, and six firstly, and then transforms completely to four and six. It is indicated by identical coupled plasma optical emission spectroscopy (ICP) and NMR that, the reaction performance of monomeric silicate anions is better than that of polymeric silicate anions which are primarily cross-linked in the alkali solution. Moreover, it also shows that the thermal activation temperature, cooling method, and the molar ratio of Na/Ca have remarkable effects on the reaction performance.

文摘Objective:Cytogenetic abnormalities have been proven to be the most valuable parameter for risk stratification of childhood acute lymphoblastic leukemia(ALL).However,studies on the prevalence of cytogenetic abnormalities and their correlation to clinical features in Chinese pediatric patients are limited,especially large-scale studies.Methods:We collected the cytogenetics and clinical data of 1541 children newly diagnosed with ALL between 2001 and 2014 in four Chinese hospitals,and retrospectively analyzed their clinical features,prognosis and risk factors associated with pediatric ALL.
文摘IgG4-related disease(IgG4-RD)is an increasingly recognized pathological entity that tends to involve multiple organs with an elevated level of serum IgG4,which is easily misdiagnosed owing to sharing common clinical features with a variety of other diseases.Here,we report an interesting IgG4-RD case of a woman with progressive multi-organ involvement for over 19 years,started with swollen eyelids,dry eye and mouth,and polydipsia and hydruria.Imaging diagnosis revealed diffuse enlargement of the parotid glands,enlargement of the head of the pancreas,pulmonary infection and interstitial lung.Serological tests showed a remarkable elevation of the serum IgG4,and cytological analysis further revealed a large amount of lymphoplasmacytic infiltration into the focal lobule,and IgG 4-positive cell infiltration in bladder mucosa.Therapeutically,the patient responded well to steroid therapy,and thus,she was diagnosed as IgG 4-RD suspicious.This report highlights the importance of an early diagnosis in this autoimmune disease and suggests that patients with a clinically unclear cause of inflammation,swelling and refractory glands,rhinitis,pancreatitis,hypophysitis,and/or interstitial pneumonia should be considered for IgG 4-RD.The plasma IgG4 level and lymphoplasmacytic infiltration may be useful indexes for screening,and a low dose of steroid maintaining therapy may offer benefits for patients with IgG4-RD.

文摘In recent years, microstrip antennas have been more widely applied in satellite communications, mobile phones, unmanned aerial vehicle (UAV), and weapons. A micro-electro-mechanical systems-based (MEMS-based) high-resistance silicon C-band microstrip antenna array has been designed for the intelligent ammunition. The center frequency is 4.5 GHz. A cavity has been designed in substrate to reduce the dielectric constant of silicon and high-resistance silicon has been used as the material of substrate to improve the gain of antenna. It is very easy to be manufactured by using MEMS technology because of the improved structure of the antenna. The results show that the gain of the antenna is 8 dB and voltage standing wave ratio (VSWR) is less than 2 by the analysis and simulation in high freauencv structure simulator (HFSS).

文摘BACKGROUND Despite the overload of publications on Crohn’s disease(CD),no comprehensive analysis of biologic therapy for CD has been reported.AIM To determine knowledge gaps and identify areas of interest of biologic therapy for CD.METHODS The top 100 highest-cited original articles were identified from January 1991 to December 2020 in the Clarivate Analytics Web of Science Core Collection database.We conducted a bibliometric analysis of biologic therapy for CD based on total citations,summarized the bibliographic information of the articles related to CD biologic therapy,and explored the research hotspots.RESULTS The top 100 highest-cited original articles were identified with total citations ranging from 307 to 2978.The 2000s(Period II,n=66)yielded the most influential original articles and saw the most dramatic growth.Among the top 10 countries,including 8 European countries and 2 North American countries,the United States(n=37)and Belgium(n=20)contributed the most publications.Among the top 10 institutions,the University Hospital Gasthuisberg in Belgium(n=23),the University of Chicago in the United States(n=20),and the Mayo Clinic in the United States(n=17)published the most papers.Regarding authors,Rutgeerts P in Belgium(n=32),Sandborn WJ in the United States(n=23),and Feagan BG in Canada(n=18)published the highest number of studies.The cooperation relationships between the United States and Europe were most frequent.Gastroenterology(impact factor=22.682)published the most articles on biologic therapy for CD(n=32)with 17654 total citations.Anti-tumor necrosis factor biologics and monoclonal antibodies were the most studied topics.CONCLUSION The bibliometric analysis emphasized the key contributions to the development of the specialized field.These data would provide useful research insights into biologic therapy for CD for clinicians and researchers.

文摘The internal quality inspection of the continuous casting billets is very important,and mis-inspection will seriously affect the subsequent production process.The UNet-VGG16 transfer learning model was used for semantic segmentation of the central shrinkage defects of the continuous casting billets.The automatic recognition accuracy of the central shrinkage defects of the continuous casting billets reaches more than 0.9.We use the minimum circumscribed rectangle to quantify the geometric dimensions such as length,width and area of the central shrinkage defects and use the threshold method to rate the central shrinkage defects of the continuous casting billets.The results show that all the testing images are rated correctly,and this method achieves the automatic recognition and intelligent analysis of the central shrinkage defects of the continuous casting billets.
