恶性中心气道狭窄(malignant central airway obstruction,MCAO)是指因原发或转移的恶性肿瘤侵及气管、隆突、左右主支气管或右中间支气管引起的管腔狭窄[1];患者会出现不同程度的呼吸困难,严重者可导致窒息、甚至危及生命。按照中心气...恶性中心气道狭窄(malignant central airway obstruction,MCAO)是指因原发或转移的恶性肿瘤侵及气管、隆突、左右主支气管或右中间支气管引起的管腔狭窄[1];患者会出现不同程度的呼吸困难,严重者可导致窒息、甚至危及生命。按照中心气道病变位置将MCAO分为管内型、管壁型、管外型和混合型[2]。管外型和混合型MCAO的瘤体组织可向管腔外生长并侵及纵隔结构;由于上腔静脉具有管壁薄、内部血流压力低的特点,因此一旦上纵隔受到肿瘤侵犯,就容易导致上腔静脉受压,出现头痛、头晕、颜面部水肿和呼吸困难等上腔静脉综合征(superior vena cava syndrome,SVCS)表现。对于MCAO,支气管镜下气道消瘤和/或气道支架置入术可以快速缓解呼吸困难[3,4];对于恶性肿瘤所致SVCS则可通过血管腔内支架置入术来缓解症状[5]。然而,晚期肺癌同时侵及中心气道和上腔静脉导致MCAO合并SVCS的病例罕见报道,其治疗方法亦无统一共识。现报道我科气道支架和上腔静脉支架联合置入治疗晚期非小细胞肺癌导致中心气道狭窄和上腔静脉综合征1例,效果满意,以供临床借鉴。展开更多
Green building is an essential component of sustainable development,but previous studies have offered little in the way of systematic reviews of green building development from the perspective of project management.Th...Green building is an essential component of sustainable development,but previous studies have offered little in the way of systematic reviews of green building development from the perspective of project management.This research presents the current research trends in this discipline by analyzing the publications in ten major international journals from 2007 to 2016.The analysis was conducted on papers that have reported relevant investigations during that 10-year period.The analyzed research topics were found to group into five themes:green building management in general,the benefits and barriers to green building development,green building performance,stakeholder behavior with regard to green buildings,and green building strategies.Future directions for research relating to green building are suggested for the areas stakeholder management,policies and incentives,communication platform development,and retrofitting of existing buildings.Green building development will continue to be an important research area,and more comprehensive studies on green building management can help to promote further progress in this field.展开更多
Effective waste management is an important aspect of green building development.However,a number of studies find that construction waste minimization actions are not satisfactorily performed in practice.Thus,it is of ...Effective waste management is an important aspect of green building development.However,a number of studies find that construction waste minimization actions are not satisfactorily performed in practice.Thus,it is of significance to identify the driving factors that influence construction practitioners’waste minimization behavior.This paper aims to explore the driving factors by comparing the affecting factors and construction waste minimization behavior between Hong Kong and Shenzhen.Firstly,through a comprehensive literature review,potential affecting factors were categorized into five groups:background information variables,construction waste minimization intentions,economic viability,governmental supervision,and project constraints.Based on the identified factors,a questionnaire was initially designed and then refined through a focus group meeting with three experienced experts.A total of 166 questionnaires were collected and non-parametric tests(Mann-Whitney U Test and Kruskal-Wallis Test)were conducted to investigate the underlying driving factors.Results revealed that background information variables play insignificant roles in construction waste minimization.Compared with other affecting factors(i.e.,waste minimization intentions,governmental supervision,project constraints),economic viability is the most important driving factor that determines construction waste minimization.Results suggested that local governments should make policies that increase practitioners’profit,rather than just increasing their intentions or regulating waste management behavior.展开更多
