sTREM-1 as promising prognostic biomarker for acute-on-chronic liver failure and mortality in patients with acute decompensation of cirrhosis

BACKGROUND Acute decompensation(AD)of cirrhosis is associated with high short-term mortality,mainly due to the development of acute-on-chronic liver failure(ACLF).Thus,there is a need for biomarkers for early and accurate identification of AD patients with high risk of development of ACLF and mortality.Soluble triggering receptor expressed on myeloid cells-1(sTREM-1)is released from activated innate immune cells and correlated with various inflammatory processes.AIM To explore the prognostic value of sTREM-1 in patients with AD of cirrhosis.METHODS A multicenter prospective cohort of 442 patients with cirrhosis hospitalized for AD was divided into a study cohort(n=309)and validation cohort(n=133).Demographic and clinical data were collected,and serum sTREM-1 was measured at admission.All enrolled patients were followed-up for at least 1 year.RESULTS In patients with AD and cirrhosis,serum sTREM-1 was an independent prognosis predictor for 1-year survival and correlated with liver,coagulation,cerebral and kidney failure.A new prognostic model of AD(P-AD)incorporating sTREM-1,blood urea nitrogen(BUN),total bilirubin(TBil),international normalized ratio(INR)and hepatic encephalopathy grades was established and performed better than the model for end-stage liver disease(MELD),MELD-sodium(MELD-Na),chronic liver failure-consortium(CLIF-C)ACLF and CLIF-C AD scores.Additionally,sTREM-1 was increased in ACLF and predicted the development of ACLF during first 28-d follow-up.The ACLF risk score incorporating serum sTREM-1,BUN,INR,TBil and aspartate aminotransferase levels was established and significantly superior to MELD,MELD-Na,CLIF-C ACLF,CLIF-C AD and P-AD in predicting risk of ACLF development.CONCLUSION Serum sTREM-1 is a promising prognostic biomarker for ACLF development and mortality in patients with AD of cirrhosis. 展开更多

Perovskite nanocrystals(PNCs)have recently become promising optoelectronic materials due to their excellent photophysical properties.However,the highly dynamic binding state between ligands and the surface of PNCs has severely restricted their luminescent properties and stabilities.In this work,1,3-bisbenzyl-2-oxoimidazolidine-4,5-dicarboxylic acid(cycle acid,CA)is introduced as both an etchant and a ligand upon post-synthetic surface treatment of PNCs.By removing the imperfect octahedrons[Pb X_(6)]^(4-)and passivating the surface defects synergistically,this treatment improves photoluminescence quantum yields from 76%to 95%and enhances the stability of PNCs against polar solvent,moisture,heat,and illumination.Meanwhile,CA can effectively and instantly recover the luminescence emission for aged PNCs.As a result,the CA-Cs Pb Br_(3)PNCs and CA-Cs Pb IxBr_(3-x)PNCs are applied as color-converting layers on a blue LED chip for warm white light-emitting diodes(WLEDs)with a color coordinate of(0.41,0.40).Importantly,the CA-based WLED device exhibits superior stability in operational conditions. 展开更多

BACKGROUND The extensive availability of ultrasound(US)technology has increased its use for point-of-care applications in many health care settings.During anaesthesia and surgery,acute respiratory failure or pulmonary oedema are common lifethreatening events that,if not recognized and treated appropriately,result in a high mortality rate.CASE SUMMARY We report a patient under anaesthesia whose lung US examination showed multiple vertical artefacts(B-lines)in the lung tissue,indicating pulmonary oedema.The respiratory state improved with the resolution of the pulmonary oedema after our treatment.CONCLUSION We believe that US of the lungs may be a useful tool for dynamic respiratory monitoring at the bedside during anaesthesia. 展开更多

目的了解并评估DTP药房患者管理服务现状与效果。方法本研究是一项基于院外DTP药房日常运营数据的真实世界研究,采用回顾性队列研究设计方法,利用全国79家DTP药房的销售及随访数据,建立了一个使用PD-1/L1类药物的肺癌患者队列。使用Cox... 目的了解并评估DTP药房患者管理服务现状与效果。方法本研究是一项基于院外DTP药房日常运营数据的真实世界研究,采用回顾性队列研究设计方法,利用全国79家DTP药房的销售及随访数据,建立了一个使用PD-1/L1类药物的肺癌患者队列。使用Cox回归分析方法探索购药时长和随访时长的影响因素。结果12226名患者中,76.4%为男性,中位年龄为62岁,其中低随访率组1902人(15.56%)、中随访率组1448人(11.84%)、高随访率组8876人(72.60%)。三组在中位购药时长与中位随访时长方面差异有统计学意义。Cox回归分析显示,与更长的购药时长相关的影响因素包括参加慈善赠药项目(HR=0.873,P<0.001)、随访中曾经出现不良事件记录(HR=0.761,P<0.001)、随访接通率大于70%(HR=0.790,P<0.001);与更长的随访时长相关的影响因素包括参与慈善赠药项目(HR=0.535,P<0.001)、随访中曾经出现不良事件记录(HR=0.689,P<0.001)、末次随访中出现不良事件记录(HR=0.763,P<0.001)、随访接通率30%~70%(HR=0.688,P<0.001)和大于70%(HR=0.579,P<0.001)。结论DTP药房开展的患者管理服务对于延长肺癌患者PD-1/L1类药物的治疗时间和随访时间具有积极作用。 展开更多

Objective: Targeting protein for Xenopus kinesin-like protein 2(TPX2) is a nuclear proliferation-related protein that plays a critical role in the formation of mitotic spindle. High expression of TPX2 has been observed in several types of tumors. However, the role of TPX2 in hepatocellular carcinoma(HCC) remains unclear. Our study aimed to investigate the effect of TPX2 on HCC cell invasion.Methods: The immortalized normal human liver cell line L02 and six HCC cell lines including SMMC-7721, BEL-7402, Huh-7, HepG2, Hep3 B and SKHep1 were subjected to qRT-PCR and western blot for TPX2 mRNA and protein, respectively. Furthermore, TPX2 small interfering RNA(siRNA) was used to knock down TPX2 expression in SMMC-7721 and HepG2 cells. Cell proliferation and invasion were determined by MTT and transwell assays. Otherwise, expression of p-AKT, MMP2 and MMP9 were evaluated by western blot in SMMC-7721 cells.Results: The expression of TPX2 in HCC cell lines was markedly higher than that in normal human liver cell line. TPX2 knockdown using a specific TPX2-siRNA reduced the number of invaded cells and inhibited cell proliferation in SMMC-7721 and HepG2 cells. Furthermore, TPX2 knockdown resulted in inactivation of AKT signaling and down-regulation of MMP2 and MMP9 expression in SMMC-7721 cells.Conclusions: Our study identified that TPX2 might contribute to tumor cell invasion through activating AKT signaling and subsequently increasing MMP2 and MMP9 in HCC. 展开更多

目的:观察丝裂霉素C在超声乳化联合小梁切除术中对角膜内皮细胞的影响。方法:选取本院收治的58例58眼青光眼合并白内障患者,随机分为两组,每组各29例29眼。两组患者均予以超声乳化联合小梁切除术,对照组术中不使用抗代谢药物,观察组以0.... 目的:观察丝裂霉素C在超声乳化联合小梁切除术中对角膜内皮细胞的影响。方法:选取本院收治的58例58眼青光眼合并白内障患者,随机分为两组,每组各29例29眼。两组患者均予以超声乳化联合小梁切除术,对照组术中不使用抗代谢药物,观察组以0.2g/L丝裂霉素浸润2min。比较两组患者手术前后视力与眼压、滤过泡形态与并发症,并采用角膜地形图测量平均中央角膜厚度,采用非接触型角膜内皮镜测定角膜内皮细胞密度、六角形细胞比率和变异系数。结果:术前,两组患者视力差异无统计学意义(P>0.05);术后,两组患者视力较术前有所提高,但组间差异无统计学意义(P>0.05)。术前,两组患者眼压差异无统计学意义(P>0.05);术后,两组患者眼压均有所降低,且观察组低于对照组,组间和组内差异均有统计学意义(P<0.05)。术后,两组患者间功能型滤过泡形成率差异无统计学意义(P>0.05);观察组并发症发生率低于对照组,组间差异有统计学意义(P<0.05)。术后,两组患者的平均中央角膜厚度、内皮细胞密度、六角形细胞比率及变异系数均无明显变化,且手术前后的组间差异也无统计学意义(P>0.05)。结论:超声乳化联合小梁切除术中应用丝裂霉素C利于稳定眼压,减少术后并发症,且并不增加角膜内皮细胞损伤风险。 展开更多

As one of the three payloads for the Advanced Space-based Solar Observatory(ASO-S)mission,the Lyman-alpha(Lyα)Solar Telescope(LST)is composed of three instruments:a Solar Corona Imager(SCI),a LyαSolar Disk Imager(SDI)and a full-disk White-light Solar Telescope(WST).When working in-orbit,LST will simultaneously perform high-resolution imaging observations of all regions from the solar disk to the inner corona up to 2.5 R⊙(R⊙stands for the mean solar radius)with a spatial resolution of 4.8′′and 1.2′′for coronal and disk observations,respectively,and a temporal resolution of 30–120 s and 1–120 s for coronal and disk observations,respectively.The maximum exposure time can be up to20 s due to precise pointing and image stabilization function.Among the three telescopes of LST,SCI is a dual-waveband coronagraph simultaneously and independently observing the inner corona in the HI Lyα(121.6±10 nm)line and white light(WL)(700±40 nm)wavebands by using a narrowband Lyαbeam splitter and has a field of view(FOV)from 1.1 to 2.5 R⊙.The stray-light suppression level can attain<10^-6 B⊙(B⊙is the mean brightness of the solar disk)at 1.1 R⊙and≤5×10^-8 B⊙at 2.5 R⊙.SDI and WST are solar disk imagers working in the Lyαline and 360.0 nm wavebands,respectively,which adopt an off-axis two-mirror reflective structure with an FOV up to 1.2 R⊙,covering the inner coronal edge area and relating to coronal imaging.We present the up-to-date design for the LST payload. 展开更多

The process of development and calibration for the first Moon-based extreme ultraviolet(EUV) camera to observe Earth's plasmasphere is introduced and the design, test and calibration results are presented. The EUV camera is composed of a multilayer film mirror, a thin film filter, a photon-counting imaging detector, a mechanism that can adjust the direction in two dimensions, a protective cover, an electronic unit and a thermal control unit. The center wavelength of the EUV camera is 30.2 nm with a bandwidth of 4.6 nm. The field of view is 14.7° with an angular resolution of 0.08°, and the sensitivity of the camera is 0.11 count s-1Rayleigh-1. The geometric calibration, the absolute photometric calibration and the relative photometric calibration are carried out under different temperatures before launch to obtain a matrix that can correct geometric distortion and a matrix for relative photometric correction,which are used for in-orbit correction of the images to ensure their accuracy. 展开更多
