Objective:This study aimed to explore the risk factors and outcomes of hypokalemia during the recovery period from anesthesia in the gynecological population.Methods:This retrospective cohort study included 208 patien...Objective:This study aimed to explore the risk factors and outcomes of hypokalemia during the recovery period from anesthesia in the gynecological population.Methods:This retrospective cohort study included 208 patients who underwent gynecological surgery at our institution between January 2021 and March 2022.Data were collected for each patient,including demographics,disease status,surgical data,and clinical information.Preoperative bowel preparation,postoperative gastrointestinal function,and electrolyte levels were compared between the two groups using propensity score matching(PSM).Results:The incidence of hypokalemia(serum potassium level<3.5 mmol/L)during the recovery period from anesthesia was approximately 43.75%.After PSM,oral laxative use(96.4%vs.82.4%,P=0.005),the number of general enemas(P=0.014),and the rate of≥2 general enemas(92.9%vs.77.8%,P=0.004)were identified as risk factors for hypokalemia,which was accompanied by decreased PaCO_(2) and hypocalcemia.There were no significant differences in postoperative gastrointestinal outcomes,such as the time to first flatus or feces,the I-FEED score(a scoring system was created to evaluate impaired postoperative gastrointestinal function),or postoperative recovery outcomes,between the hypokalemia group and the normal serum potassium group.Conclusion:Hypokalemia during postanesthesia recovery period occurred in 43.75%of gynecological patients,which resulted from preoperative mechanical bowel preparation;however,it did not directly affect clinical outcomes,including postoperative gastrointestinal function,postoperative complications,and length of hospital stay.展开更多
Diabetes is an urgent health issue characterized by ethnic and regional variations,and is inseparable from the differ-ent dietary habits.It is worthy to note that the incidence of diabetes in Bai nationality has been ...Diabetes is an urgent health issue characterized by ethnic and regional variations,and is inseparable from the differ-ent dietary habits.It is worthy to note that the incidence of diabetes in Bai nationality has been reported to be much lower than Han in China.As a daily vegetable of Bai,the phytochemical and antidiabetic study of Ottelia acuminata var.acuminata had not been carried out.In this study,41 metabolites with diverse diarylheptanoid(six new ones,Otteacumienes A-F),flavone,sesquiterpenoid,coumarin,lignan,polyacetylene,and alkaloid skeletons were charac-terized from O.acuminata var.acuminata.Among them,the racemic nature of 3 was characterized by chiral resolution and calculated ECD methods.The biological study revealed diarylheptanoids showed significantα-glucosidase inhibi-tory activities with 5 as the most effective one(60-fold stronger than acarbose).Molecular docking studies indicated that these structures have different binding cavities with acarbose.This study demonstrated that O.acuminata var.acuminata might correlated with the low incidence diabetes of Bai and the diarylheptanoids may have potential therapeutic value for diabetes mellitus.展开更多
AIM:To investigate the published papers of ophthalmology in past ten years and explore the development of ophthalmology.METHORDS:The data of this study retrieved from Science Citation Index Expanded and downloaded o...AIM:To investigate the published papers of ophthalmology in past ten years and explore the development of ophthalmology.METHORDS:The data of this study retrieved from Science Citation Index Expanded and downloaded online in November 2017,including all the papers with publication year from 2007-2016 were analyzed.The papers were based on the Web of Science category and the journals were based on the Journal Citation Report category.RESULTS:The number of ophthalmology papers increased from 7450 to 9089 during 2007 to 2017.The average rate increased 2.2% annually.USA accounts for one thirds of the total and two thirds of the highly cited papers.In Asia,China,Japan and South Korea were in Top 10 by the number of ophthalmology papers.UK,Germany,Japan and Australia also had great impact in global ophthalmology.The hot spots included endothelial growth factor,optical coherence tomography and open-angle glaucoma.CONCLUSION:USA is in the leading position in global ophthalmology.Part of Asian countries play an important role in the development of ophthalmology,but the impact needs to be improved.展开更多
Objective:Regarding frozen-thawed embryo transfer(FET),there is limited consensus on whether extending embryo culture from the cleavage stage to the blastocyst stage affects perinatal outcomes.This study aimed to comp...Objective:Regarding frozen-thawed embryo transfer(FET),there is limited consensus on whether extending embryo culture from the cleavage stage to the blastocyst stage affects perinatal outcomes.This study aimed to compare perinatal outcomes of singletons between blastocyst-stage embryo transfer(BT)and cleavage-stage embryo transfer(CT)in FET.Methods:A total of 9408 FET cycles that met the inclusion criteria were included in this retrospective cohort study between 2019 and 2022.Blastocyst-stage embryo transfers were performed in the BT group,and cleavage-stage embryo transfers were performed in the CT group.Multivariate logistic regression analyses were performed,as well as propensity score matching(PSM)to adjust for confounders.Results:After PSM,a higher risk of pre-term birth(PTB;odds ratio[OR]:1.23,95%confidence interval[CI]:1.00-1.50,P=0.048)and being large for gestational age(LGA;OR:1.16,95%CI:1.00-1.35,P=0.050)was observed in the BT group compared to that in the CT group.After stratified PSM,in the subgroup under 35 years of age,only an increased risk of LGA was observed in the BT group compared to the CT group.Perinatal outcomes in the double-embryo transfer subgroup were similar to those in the unstratified group.However,in the subgroup beyond 35 years of age and the single embryo transfer subgroup,perinatal outcomes were not statistically different between the BT and CT groups(P>0.05).Conclusions:In FET,prolonged embryo culture to the blastocyst stage increased the risk of PTB and LGA in single fetuses.However,stratified analysis based on age and the number of transferred embryos yielded different results,necessitating further mechanistic studies.展开更多
Objective:To investigate the potential relationships among the ovarian response prediction index(ORPI),follicle-oocyte index(FOI),and clinical pregnancy rate(CPR)in women undergoing their first in vitro fertilization/...Objective:To investigate the potential relationships among the ovarian response prediction index(ORPI),follicle-oocyte index(FOI),and clinical pregnancy rate(CPR)in women undergoing their first in vitro fertilization/intracytoplasmic sperm injectionembryo transfer(IVF/ICSI-ET)fresh cycle transfer.Methods:In this retrospective cohort study,we included 12,218 women who underwent their first IVF/ICSI-ET cycle between December 2014 and January 2021.The primary and secondary outcomes of our study were CPR and cumulative live birth rate(CLBR),respectively.The data were divided into three groups according to the ORPI and FOI tertiles.Multivariate logistic regression analyses,stratification analyses,interaction,restricted cubic splines,and receiver operating characteristic(ROC)curves were constructed to identify the relationships among ORPI,FOI,and CPR.Results:A statistically significant increase in CPR was detected from the lowest to the highest tertile group(ORPI:48.12%,54.07%,and 53.47%,P<0.001;FOI:49.99%,52.95%,and 52.71%,P=0.012).A higher CLBR was observed in the high group(ORPI:38.63%,44.62%,and 44.19%,P<0.001;FOI:41.02%,43.78%,and 42.59%,P=0.039).Multivariate logistic regression analysis revealed no statistically significant differences between ORPI,FOI,and neither CPR(odds ratio[OR][95%confidence interval{CI}],0.99[0.97–1.00]vs.[1.02{0.84–1.24}])nor CLBR(OR[95%CI],0.99[0.97–1.01]vs.0.99[0.81–1.20]).No significant association was found among FOI,ORPI,and CPR,even in the subgroups.Restricted cubic spline analyses indicated the existence of a non-linear relationship across the entire range of FOI and ORPI.The ORPI and FOI variables had poor predictive ability(AUC<0.60)for CPR.Conclusions:Both ORPI and FOI are not reliable predictors of clinical pregnancy or live birth outcomes in fresh ETs.Clinicians and researchers should avoid using FOI and ORPI to assess pregnancy outcomes after fresh ET because of their limited relevance and predictive value.展开更多

Glucocorticoid?induced transcript 1 gene (GLCCI1) is reported to be associated with GCs efficiency in asthmatics, while its exact mechanism remains unknown. Methods: A total of 30 asthmatic patients received fluticasone propionate for 12 weeks. Forced expiratory volume in 1 s (FEV1) and GLCCI1 expression were detected. Asthma model was constructed in wild?type and GLCCI1 knockout (GLCCI1?/?) mice. Glucocorticoid receptor (GR) and mitogen?activated protein kinase phosphatase 1 (MKP?1) expression were detected by polymerase chain reaction and Western blotting (WB). The phosphorylation of p38 mitogen?activated protein kinase (MAPK) was also detected by WB. Results: In asthmatic patients, the change of FEV1 was well positively correlated with change of GLCCI1 expression (r = 0.430, P = 0.022). In animal experiment, GR and MKP?1 mRNA levels were significantly decreased in asthmatic mice than in control mice (wild?type: GR: 0.769 vs. 1.000, P = 0.022; MKP?1: 0.493 vs. 1.000, P < 0.001. GLCCI1?/?: GR: 0.629 vs. 1.645, P < 0.001; MKP?1: 0.377 vs. 2.146, P < 0.001). Hydroprednisone treatment significantly increased GR and MKP?1 mRNA expression levels than in asthmatic groups; however, GLCCI1?/?.asthmatic mice had less improvement (wild?type: GR: 1.517 vs. 0.769, P = 0.023; MKP?1: 1.036 vs. 0.493, P = 0.003. GLCCI1?/?: GR: 0.846 vs. 0.629, P = 0.116; MKP?1: 0.475 vs. 0.377, P = 0.388). In this paper,based on the finite volume method,a high-precision hydrodynamic model coupled with the habitat suitability model is established,and the computational efficiency of the coupled model is improved by a grap...In this paper,based on the finite volume method,a high-precision hydrodynamic model coupled with the habitat suitability model is established,and the computational efficiency of the coupled model is improved by a graphics processing unit(GPU)-accelerated technology.The coupled model is used to solve the problem of the non-conservation of mass that may be caused by the nearshore hydrodynamic model in the processing of wetting and drying,while avoiding the unphysical high velocities at the wetting and drying boundaries.The coupled model is applied to simulate the high-precision hydrodynamic process of the Liao River estuary(LRE)and the hydrological habitat suitability of the estuarine vegetation(Suaeda heteroptera)growing in the LRE.The simulated values of the hydrological variables(the water level,the water depth,the current velocity and direction)are highly consistent with the measured values.The root mean square errors(RMSE)of the hydrological variables are 0.10m,0.12m/s and 17.24°,respectively.Furthermore,the simulated combined suitability index(CSI)distribution of Suaeda heteroptera(S.heteroptera)matches with the distribution of S.heteroptera obtained from the high-resolution remote sensing satellite images during the same time period.The ratio of the simulated weighted usable area(WUA)of S.heteroptera to the area obtained from the remote sensing satellite images during the same period is 81.9%.This study reveals the phenomenon that the distribution of S.heteroptera in the LRE is highly correlated with the high-precision hydrodynamic processes,and provides a scientific basis and a valuable reference for the conservation and the restoration of the estuarine vegetation.展开更多

A microcalorimetric study on molecular recognition of p-sulfonatocalix[4]arene derivatives at selfassembled interface in comparison with in bulk water was performed,inspired by the dramatic change in physicochemical c...A microcalorimetric study on molecular recognition of p-sulfonatocalix[4]arene derivatives at selfassembled interface in comparison with in bulk water was performed,inspired by the dramatic change in physicochemical characteristics from bulk water to interface.A total of six cationic molecules were screened as model guests,including ammonium(NH_4~+),guanidinium(Gdm~+).N,N'-dimethyl-1,4-diazabicyclo[2.2.2]octane(DMDABCO^(2+)),tropylium(Tpm~+),N-methyl pyridinium(N-mPY*) and methyl viologen(MV^(2+)).The complexation with NH_4~+.Gdm~+ and DMDABCO2* is pronouncedly enhanced when the recognition process moved from bulk water to interface,whereas the complexation stabilities with Tpm~+,N-mPY~+ and MV2* increase slightly or even decrease to some extent.A more interesting phenomenon arises from the NH_4~+/Gdm~+ pair that the thermodynamic origin at interface differs definitely from each other although with similar association constants.The results were discussed in terms of differential driving forces,electrostatic,hydrogen bond as well as π-stacking interactions,originating from the unique physicochemical features of interfaces,mainly the polarity and dielectric constant.展开更多

文摘In this paper,based on the finite volume method,a high-precision hydrodynamic model coupled with the habitat suitability model is established,and the computational efficiency of the coupled model is improved by a graphics processing unit(GPU)-accelerated technology.The coupled model is used to solve the problem of the non-conservation of mass that may be caused by the nearshore hydrodynamic model in the processing of wetting and drying,while avoiding the unphysical high velocities at the wetting and drying boundaries.The coupled model is applied to simulate the high-precision hydrodynamic process of the Liao River estuary(LRE)and the hydrological habitat suitability of the estuarine vegetation(Suaeda heteroptera)growing in the LRE.The simulated values of the hydrological variables(the water level,the water depth,the current velocity and direction)are highly consistent with the measured values.The root mean square errors(RMSE)of the hydrological variables are 0.10m,0.12m/s and 17.24°,respectively.Furthermore,the simulated combined suitability index(CSI)distribution of Suaeda heteroptera(S.heteroptera)matches with the distribution of S.heteroptera obtained from the high-resolution remote sensing satellite images during the same time period.The ratio of the simulated weighted usable area(WUA)of S.heteroptera to the area obtained from the remote sensing satellite images during the same period is 81.9%.This study reveals the phenomenon that the distribution of S.heteroptera in the LRE is highly correlated with the high-precision hydrodynamic processes,and provides a scientific basis and a valuable reference for the conservation and the restoration of the estuarine vegetation.

文摘A supramolecular nanoparticle was fabricated based on the aggregation of amphiphilic p- sulfonatocalixarene induced by selenocystamine dihydrochloride (Se-Cys). The application of Se-Cys remarkably decreases the critical aggregation concentration of sulfonatocalixarene, and the resultant spherical nanoparticle was investigated by fluorescence spectroscopy, dynamic laser scattering, and transmission electron microscopy. Owing to the property of Se-Cys, the nanoparticles showed the redox- responsive disassembly behaviors with the addition of H2O2 and GSH.

文摘Insulin fibrillation poses a variety of problems in biomedical and biotechnological applications of insulin.Inhibiting insulin fibrillation is highly on demand to address those problems.We herein demonstrate the capability of amphiphilic sulfonatocalixarene to inhibit insulin fibrillation.The amphiphilic assembly of p-sulfonatocalix[4]arene tetra dodecyl ether exhibits much better efficiency on inhibiting insulin fibrillation,with respect to p-sulfonatocalix[4]arene and sodium dodecyl benzenesulphonate.The pronounced inhibition effect results from both the preorganized scaffold of calixarene and the amphiphilic assembly.

文摘A microcalorimetric study on molecular recognition of p-sulfonatocalix[4]arene derivatives at selfassembled interface in comparison with in bulk water was performed,inspired by the dramatic change in physicochemical characteristics from bulk water to interface.A total of six cationic molecules were screened as model guests,including ammonium(NH_4~+),guanidinium(Gdm~+).N,N'-dimethyl-1,4-diazabicyclo[2.2.2]octane(DMDABCO^(2+)),tropylium(Tpm~+),N-methyl pyridinium(N-mPY*) and methyl viologen(MV^(2+)).The complexation with NH_4~+.Gdm~+ and DMDABCO2* is pronouncedly enhanced when the recognition process moved from bulk water to interface,whereas the complexation stabilities with Tpm~+,N-mPY~+ and MV2* increase slightly or even decrease to some extent.A more interesting phenomenon arises from the NH_4~+/Gdm~+ pair that the thermodynamic origin at interface differs definitely from each other although with similar association constants.The results were discussed in terms of differential driving forces,electrostatic,hydrogen bond as well as π-stacking interactions,originating from the unique physicochemical features of interfaces,mainly the polarity and dielectric constant.
