In the present prospective study we investigated the clinical characteristics and emergency management of children with severe encephalitis associated with NPE caused by enterovirus 71.METHODS: The study was conducted in 2 pediatric intensive care units (PICUs) over a 2-month period. Clinical records were reviewed of critically ill children with severe encephalitis associated with NPE caused by EV71 who were admitted to PICUs during the period of May to June 2008 in Fuyang.RESULTS: We reviewed the complete records of 36 children, of whom 23 (63.9%) were male and 13 (36.1%) female. Their age ranged from 4 to 48 months, with an average of 15.8 months. All children except one were under 3 years of age. The overall mortality in these children was 19.4%. The average duration of critical life threatening signs and symptoms was 2.1 days (12 hours-5 days). Nervous system diseases included brainstem encephalitis in 27 children (75%), brainstem encephalitis associated with myelitis in 6 children (16.7%), and general encephalitis in 3 chidren (8.3%), respectively. In 12 patients of NPE (33.3%) pink or bloody bubble sputum and asymmetric pulmonary edema or hemorrhage was the primary manifestation but no typical exanthema was observed. Five children died of acute onset of NPE and / or pulmonary hemorrhage with rapid progression of cardiopulmonary failure within hours after admission. Therapeutic management consisted of mechanical ventilation and administration of mannitol, methylprednisolone, intravenous immunoglobulin (IVIG) and vasoactive drugs, associated with the need of fluid volume resuscitation in 9 (25%) of the 36 children.CONCLUSION: In children less than 3 years of age found to be affected by severe EV71 encephalitis associated with NPE, one fifth may die. The major organ systems infected by severe EV71 include the central nervous system, the respiratory system, and the cardiovascular system. Early diagnosis and evaluation, respiratory support, treatment of intracranial hypertension, and mainttenance of function of the cardiovascular system are the most important therapeutic measures.展开更多
AIM:To evaluate the prognosis of patients with acute fatty liver of pregnancy(AFLP)6 mo or longer after discharge.METHODS:The records of pregnant patients diagnosed with AFLP at Beijing Ditan Hospital over a 16-year p...AIM:To evaluate the prognosis of patients with acute fatty liver of pregnancy(AFLP)6 mo or longer after discharge.METHODS:The records of pregnant patients diagnosed with AFLP at Beijing Ditan Hospital over a 16-year period were reviewed in November 2012.Patients weremonitored using abdominal ultrasound,liver and kidney functions,and routine blood examination.RESULTS:A total of 42 patients were diagnosed with AFLP during the study period,and 25 were followed.The mean follow-up duration was 54.5 mo(range:6.5-181 mo).All patients were in good physical condition,but one patient had gestational diabetes.The renal and liver functions normalized in all patients after recovery,including in those with pre-existing liver or kidney failure.The ultrasound findings were normal in12 patients,an increasingly coarsened echo-pattern and increased echogenicity of the liver in 10 patients,and mild to moderate fatty liver infiltration in 3 patients.Cirrhosis or liver nodules were not observed in any patient.CONCLUSION:Acute liver failure and acute renal failure in AFLP patients is reversible.Patients do not require any specific long-term follow-up after recovery from AFLP if their liver function tests have normalized and they remain well.展开更多

BACKGROUND Colorectal cancer is one of the most common cancers globally.In China,its prevalence ranks fourth and fifth among females and males,respectively.Presently,treatment of rectal cancer follows a multidisciplin...BACKGROUND Colorectal cancer is one of the most common cancers globally.In China,its prevalence ranks fourth and fifth among females and males,respectively.Presently,treatment of rectal cancer follows a multidisciplinary comprehensive treatment approach involving surgery,radiotherapy,chemotherapy,and targeted therapy.With deepening theoretical and molecular research on colorectal cancer,randomized controlled trials(RCTs)on colorectal cancer have made significant progress.However,many RCTs have shortfalls.AIM To investigate the RCTs of global colorectal cancer spanning from 2008 to 2018.To provide suggestions for conducting Chinese RCTs of colorectal cancer.METHODS PubMed and Web of Science databases were searched to obtain RCTs of colorectal cancer carried out between January 1,2008,and January 1,2018.The bibliometric method was used for statistical analysis of the publication years,countries/regions,authors,institutions,source journals,quoted times,key words,and authors.RESULTS Colorectal cancer RCTs showed an upward trend between 2008 to 2018;the top 10 research institutions in the included literature were from the United States,the United Kingdom,and other countries with a high incidence of colorectal cancer.Most of the related research journals are sponsored by European and American countries.The 15 most cited studies involved international multicenter clinical research,having few participants from Chinese research institutions.Network visualization using key words showed that RCTs on colorectal cancer focus on screening,disease-free survival,drug treatment,surgical methods,clinical trials,quality of life,and prognosis.The result of the coauthorship network analysis showed that Chinese researchers are less involved in international exchanges compared to those from leading publication countries.CONCLUSION High-quality RCTs are increasingly favored by leading international journals.However,there is still a large gap in clinical research between China and leading countries.Researchers should implement standardized and accurate clinical trials,strengthen international multicenter cooperation,and emphasize quality control.展开更多
BACKGROUND This case-control study compared the short-term clinical efficacy of natural orifice specimen extraction surgery(NOSES) using a prolapsing technique and the conventional laparoscopic-assisted approach for l...BACKGROUND This case-control study compared the short-term clinical efficacy of natural orifice specimen extraction surgery(NOSES) using a prolapsing technique and the conventional laparoscopic-assisted approach for low rectal cancer.AIM To further explore the application value of the transanal placement of the anvil and to evaluate the short-term efficacy of NOSES for resecting specimens of low rectal cancer, as well as to provide a theoretical basis for its extensive clinical application.METHODS From June 2015 to June 2018, 108 consecutive laparoscopic-assisted low rectal cancer resections were performed at our center. Among them, 26 specimens were resected transanally using a prolapsing technique(NOSES), and 82 specimens were resected through a conventional abdominal wall small incision(LAP). A propensity score matching method was used to select 26 pairs of matched patients, and their perioperative data were analyzed.RESULTS The baseline data were comparable between the two matched groups. All 52 patients underwent the surgery successfully. The operative time, blood loss,number of harvested lymph nodes, postoperative complication rate,circumferential margin involvement, postoperative follow-up data, and postoperative anal function were not statistically significant. The NOSES group had shorter time to gastrointestinal function recovery(2.6 ± 1.0 d vs 3.4 ± 0.9 d, P= 0.006), shorter postoperative hospital stay(7.1 ± 1.7 d vs 8.3 ± 1.1 d, P = 0.003),lower pain score(day 1: 2.7 ± 1.8 vs 4.6 ± 1.9, day 3: 2.0 ± 1.1 vs 4.1 ± 1.2, day 5: 1.7± 0.9 vs 3.3 ± 1.0, P < 0.001), a lower rate of additional analgesic use(11.5% vs61.5%, P = 0.001), and a higher satisfaction rate in terms of the aesthetic appearance of the abdominal wall after surgery(100% vs 23.1%, P < 0.001).CONCLUSION NOSES for low rectal cancer can achieve satisfactory short-term efficacy and has advantages in reducing postoperative pain, shortening the length of postoperative hospital stay, and improving patients' satisfaction in terms of a more aesthetic appearance of the abdominal wall.展开更多

Methods The clinical features and preliminary epidemiologic findings among 30 patients with confirmed H1N1 virus infection who developed into acute respiratory failure for ventilatory support were investigated. Results A total of 30 patients(37.43 ± 18.80 years old) with 2009 influenza A(H1N1) related acute respiratory distress syndrome(ARDS) received treatment with mechanical ventilation, 15 cases of whom were male and 17 cases died of ARDS. Fatal cases were significantly associated with an APACHE Ⅱ score(P = 0.016), but not with PaO 2 /FIO 2(P = 0.912) and chest radiograph(P = 0.333). The most common complication was acute renal failure(n = 9). Five patients received extracorporeal membrane oxygenation(ECMO), 3 of whom died and the others survived. The major causes of death were multiple organ dysfunction syndrome(MODS)(39%), intractable respiratory failure(27%) and sepsis(20%). Conclusions Most patients with respiratory failure due to influenza A(H1N1) virus infection were young, with a high mortality, particularly associated with APACHE Ⅱ score, secondary infection of lung or type 2 diabetes mellitus.展开更多

To promote diagnostic awarenes...Background Hand, foot, and mouth disease (HFMD) is a common infectious disease in childhood caused by an enterovirus (EV), and which is principally seen in children under 5 years of age. To promote diagnostic awareness and effective treat-ments, to further standardize and strengthen the clinical management and to reduce the mortality of HFMD, the guidelines for diagnosis and treatment have been developed. Methods National Health Commission of China assembled an expert committee for a revision of the guidelines. The com-mittee included 33 members who are specialized in diagnosis and treatment of HFMD. Results Early recognition of severe cases is utmost important in diagnosis and treatment of patients with HFMD. The key to diagnosis and treatment of severe cases lies in the timely and accurate recognition of stages 2 and 3 of HFMD, in order to stop progression to stage 4. Clinicians should particularly pay attention to those EV-A71 cases in children aged less than 3 years, and those with disease duration less than 3 days. The following indicators should alert the clinician of possible deterioration and impending critical disease: (1) persistent hyperthermia;(2) involvement of nervous system;(3) worsening respiratory rate and rhythm;(4) circulatory dysfunction;(5) elevated peripheral WBC count;(6) elevated blood glucose and (7) elevated blood lactic acid. For treatment, most mild cases can be treated as outpatients. Patients should be isolated to avoid cross-infection. Intense treatment modalities should be given for those severe cases. Conclusion The guidelines can provide systematic guidance on the diagnosis and management of HFMD.展开更多

The treatment of iron-bearing dusts and sludges by the rotary hearth furnace process has the advantage of sufficient utilization of valuable metals and a high impurity removal rate,but the lower strength of the metall...The treatment of iron-bearing dusts and sludges by the rotary hearth furnace process has the advantage of sufficient utilization of valuable metals and a high impurity removal rate,but the lower strength of the metallized product needs to be addressed.The effects of quaternary basicity R4(w(CaO+MgO)/w(SiO_(2)+Al_(2)O_(3)))on the reduction behavior and physical and chemical properties of metallized pellets,including phase composition,compressive strength,microstructure and soft melting area,were investigated with FactSage thermodynamic software and experiments.The strength of metallized pellets depended on the gangue composition,such as CaO,MgO,Al_(2)O_(3) and SiO_(2),due to the altered chemical composition,physical phase composition,microscopic morphology and stability of the slag phase.The reduction of carbon-bearing pellets was significantly promoted by suitable basicity.The lower basicity(R_(4)<1.4)facilitated the formation of low melting point iron-containing compounds from SiO_(2) and Al_(2)O_(3) with FeO,resulting in increased liquid phase generation,but lower metallization rate,due to the hindered precipitation and growth of iron grains.Interestingly,the higher basicity(R_(4)>1.8)also increased the amount of liquid phase and improved the strength of the pellets,due to the granular iron crystals bonded into sheets.Notably,the main component of the liquid phase in high-basicity conditions was calcium ferrite.Although the additional amount of liquid phase was beneficial to the strength of the metallized pellets,calcium disilicate was formed at R_(4)=1.6,resulting in a reduction in the compressive strength of the pellets to 1521.9 N/pellet.展开更多

Background:Human infections with zoonotic coronaviruses(CoVs),including severe acute respiratory syndrome(SARS)-CoV and Middle East respiratory syndrome(MERS)-CoV,have raised great public health concern globally.Here,...Background:Human infections with zoonotic coronaviruses(CoVs),including severe acute respiratory syndrome(SARS)-CoV and Middle East respiratory syndrome(MERS)-CoV,have raised great public health concern globally.Here,we report a novel batorigin CoV causing severe and fatal pneumonia in humans.Methods:We collected clinical data and bronchoalveolar lavage(BAL)specimens from five patients with severe pneumonia from Wuhan Jinyintan Hospital,Hubei province,China.Nucleic acids of the BAL were extracted and subjected to next-generation sequencing.Virus isolation was carried out,and maximum-likelihood phylogenetic trees were constructed.Results:Five patients hospitalized from December 18 to December 29,2019 presented with fever,cough,and dyspnea accompanied by complications of acute respiratory distress syndrome.Chest radiography revealed diffuse opacities and consolidation.One of these patients died.Sequence results revealed the presence of a previously unknownβ-CoV strain in all five patients,with 99.8%to 99.9%nucleotide identities among the isolates.These isolates showed 79.0%nucleotide identity with the sequence of SARS-CoV(GenBank NC_004718)and 51.8%identity with the sequence of MERS-CoV(GenBank NC_019843).The virus is phylogenetically closest to a bat SARS-like CoV(SL-ZC45,GenBank MG772933)with 87.6%to 87.7%nucleotide identity,but is in a separate clade.Moreover,these viruses have a single intact open reading frame gene 8,as a further indicator of bat-origin CoVs.However,the amino acid sequence of the tentative receptor-binding domain resembles that of SARS-CoV,indicating that these viruses might use the same receptor.Conclusion:A novel bat-borne CoV was identified that is associated with severe and fatal respiratory disease in humans.展开更多

Background Previous studies showed that soluble IL-2Rαis an important marker of cellular immune activation and might be a marker of treatment efficacy for children with brucellosis.However,data regarding adult patien...Background Previous studies showed that soluble IL-2Rαis an important marker of cellular immune activation and might be a marker of treatment efficacy for children with brucellosis.However,data regarding adult patients with brucellosis were unknown.The aim of study was to explore the potential role of serum sIL-2Rαevaluating treatment responses in adult patients with brucellosis,and T cell immune status was also examined.Methods During January 2016–April 2017,30 patients with acute brucellosis from the Third People’s Hospital of Linfen in Shanxi Province and Beijing Di Tan Hospital,and 28 healthy controls were included in this study.Peripheral blood samples were collected before and after six weeks of antibiotic treatment.Serum sIL-2Rαlevels were measured by enzyme-linked immunosorbent assay,and the percentage of Th1,Th2,Tc1,Tc2,and Tregs was detected by flow cytometry after intracellular staining for cytokines(interferon-γand interleukin-4)and Foxp3 in T lymphocytes from peripheral blood.The obtained data were analyzed with Wilcoxon ranked sum tests for paired values,Mann-Whitney U-tests for comparisons between patients and healthy controls,and Spearman rank tests for correlation analyses.Results Serum sIL-2Rαlevels were significantly higher in patients than in controls(P=0.001).A significant decline was observed in patients after the cessation of treatment(P<0.001)and return to normal(P>0.05).Th1,Tc1,Th2,and Tc2 cell frequencies were higher in patients than in healthy subjects(P<0.05),while the Th1/Th2 and Tc1/Tc2 ratios were significantly lower(P=0.0305 and 0.0005,respectively)and returned to normal levels after treatment.In patients with acute brucellosis,serum sIL-2Rαlevels were negatively correlated with the Th1/Th2 ratio(r=−0.478,P=0.028),Tc1/Tc2 ratio(r=−0.677,P=0.001),and Tc1 percentage(r=−0.516,P=0.017).Serum sIL-2Rαand Tc2 percentages were positively correlated(r=0.442,P=0.045).Conclusions Based on the correlations with Th1/Th2 and Tc1/Tc2 ratios,serum sIL-2Rαlevels may reflect the immune response status.sIL-2Rαmay be a marker for therapeutic efficacy in acute brucellosis.展开更多

The targeted construction of donor-acceptor(D-A)materials featuring efficient photothermal(PT)conversion properties has been an attractive but challenging goal.Herein,a new series of coordination polymers(CPs)featurin...The targeted construction of donor-acceptor(D-A)materials featuring efficient photothermal(PT)conversion properties has been an attractive but challenging goal.Herein,a new series of coordination polymers(CPs)featuring different PT performances were constructed with flexible linker 1,1′-Ferrocenedicarboxylic acid(FCA)and 2,4,6-tri(pyridin-4-yl)-1,3,5-triazine(TPT)as acceptor and donor molecules including coronene and tetrathiafulvalene(TTF).Based on the flexible configurations of FCA ligand and the directing of donor dependent D-A interactions,the structures of TF-1 to TF-3 featuring distinctive dimension were obtained based on Cd(Ⅱ)ions for investigating the PT property in configurational perspectives.A systematic investigation of the PT properties of the CPs was performed.Notably,TF-3 exhibits the finest PT conversion effect under the irradiation of560 nm laser,while TF-2 shows optimal PT conversion under 808 nm laser irradiation required for biological PT therapy,illustrating the correlation between the structural and component features of the CPs and their PT performances.Furthermore,Zn(Ⅱ)as bio-friendly ion was utilized to construct hypotoxic TF-4 that reveals similar structure to that of TF-2 for potential application.Polydimethylsiloxane(PDMS)patches doped with TF-4 exhibits considerable NIR PT conversion effect under808 nm,represented by the nearly 80℃temperature increased in 120 s for TF-4@PDMS patch(1.2 wt%)under 0.9 W cm^(-2)irradiation,the results of which herein indicate the potential of D-A CPs as versatile platform for the modulation of PT materials.展开更多
文摘BACKGROUND: Hand-foot-mouth disease has become a major public health issue in children in China. In the present prospective study we investigated the clinical characteristics and emergency management of children with severe encephalitis associated with NPE caused by enterovirus 71.METHODS: The study was conducted in 2 pediatric intensive care units (PICUs) over a 2-month period. Clinical records were reviewed of critically ill children with severe encephalitis associated with NPE caused by EV71 who were admitted to PICUs during the period of May to June 2008 in Fuyang.RESULTS: We reviewed the complete records of 36 children, of whom 23 (63.9%) were male and 13 (36.1%) female. Their age ranged from 4 to 48 months, with an average of 15.8 months. All children except one were under 3 years of age. The overall mortality in these children was 19.4%. The average duration of critical life threatening signs and symptoms was 2.1 days (12 hours-5 days). Nervous system diseases included brainstem encephalitis in 27 children (75%), brainstem encephalitis associated with myelitis in 6 children (16.7%), and general encephalitis in 3 chidren (8.3%), respectively. In 12 patients of NPE (33.3%) pink or bloody bubble sputum and asymmetric pulmonary edema or hemorrhage was the primary manifestation but no typical exanthema was observed. Five children died of acute onset of NPE and / or pulmonary hemorrhage with rapid progression of cardiopulmonary failure within hours after admission. Therapeutic management consisted of mechanical ventilation and administration of mannitol, methylprednisolone, intravenous immunoglobulin (IVIG) and vasoactive drugs, associated with the need of fluid volume resuscitation in 9 (25%) of the 36 children.CONCLUSION: In children less than 3 years of age found to be affected by severe EV71 encephalitis associated with NPE, one fifth may die. The major organ systems infected by severe EV71 include the central nervous system, the respiratory system, and the cardiovascular system. Early diagnosis and evaluation, respiratory support, treatment of intracranial hypertension, and mainttenance of function of the cardiovascular system are the most important therapeutic measures.
文摘AIM:To evaluate the prognosis of patients with acute fatty liver of pregnancy(AFLP)6 mo or longer after discharge.METHODS:The records of pregnant patients diagnosed with AFLP at Beijing Ditan Hospital over a 16-year period were reviewed in November 2012.Patients weremonitored using abdominal ultrasound,liver and kidney functions,and routine blood examination.RESULTS:A total of 42 patients were diagnosed with AFLP during the study period,and 25 were followed.The mean follow-up duration was 54.5 mo(range:6.5-181 mo).All patients were in good physical condition,but one patient had gestational diabetes.The renal and liver functions normalized in all patients after recovery,including in those with pre-existing liver or kidney failure.The ultrasound findings were normal in12 patients,an increasingly coarsened echo-pattern and increased echogenicity of the liver in 10 patients,and mild to moderate fatty liver infiltration in 3 patients.Cirrhosis or liver nodules were not observed in any patient.CONCLUSION:Acute liver failure and acute renal failure in AFLP patients is reversible.Patients do not require any specific long-term follow-up after recovery from AFLP if their liver function tests have normalized and they remain well.

文摘Objective:To study the influence of targeted inhibition of Notch1 gene on the killing effects of Paclitaxel on triple-negative breast cancer cells.Methods:The triple-negative [estrogen receptor(ER)/progesterone receptor(PR)/human epidermal growth factor receptor 2(Her2)] breast cancer cell line MDA-MB-231 and ER/PR/HER-2-positive breast cancer cell line MCF-7 were cultured,transfected with Notch1-si RNA-overexpression plasmid and blank plasmid,and treated with different concentrations of paclitaxel,and then the cell proliferation activity and apoptosis rate as well as the m RNA expression of Caspase-3,Caspase-9 and Bcl-2 were determined.Results:Paclitaxel could decrease the MDA-MB-231 and MCF-7 cell proliferation activity as well as Bcl-2 mRNA expression,and increase MDA-MB-231 and MCF-7 cell apoptosis rate as well as Caspase-3 and Caspase-9 mRNA expression in dosedependent manners;with the same dose of paclitaxel treatment,the inhibitory effects on MDAMB-231 cell proliferation activity and Bcl-2 m RNA expression as well as the promoting effects on MDA-MB-231 cell apoptosis and mR NA expression of Caspase-3 and Caspase-9 were weaker than those on MCF-7 cell;after 0.5 μM paclitaxel combined with Notch1-siRNA treatment,MDA-MB-231 cell proliferation activity and Bcl-2 mRNA expression were significantly lower than those after 0.5 μM paclitaxel combined with control plasmid treatment while cell apoptosis rate and mR NA expression of Caspase-3 and Caspase-9 were higher than those after 0.5 μM paclitaxel combined with control plasmid treatment.Conclusions:Targeted inhibition of Notch1 gene may enhance the killing effects of paclitaxel on triple-negative breast cancer cells by up-regulating the expression of Caspase-3 and Caspase-9 and inhibiting the expression of Bcl-2.

文摘INTRODUCTION A 56-year-old Chinese female patient with A (H1N1) influenza pneumonia accompanied by acute disseminated encephalomyelitis (ADEM) of the Central Nervous System (CNS) is described in this article. The patient had typical clinical manifestation, and the diagnosis was reached after MRI and other examinations. From this case, we can conclude that the virus of A (H1N1) influenza can infect CNS, and we should pay more attention to patients of A (H1N1) influenza pneumonia with neurological complications.

文摘BACKGROUND Colorectal cancer is one of the most common cancers globally.In China,its prevalence ranks fourth and fifth among females and males,respectively.Presently,treatment of rectal cancer follows a multidisciplinary comprehensive treatment approach involving surgery,radiotherapy,chemotherapy,and targeted therapy.With deepening theoretical and molecular research on colorectal cancer,randomized controlled trials(RCTs)on colorectal cancer have made significant progress.However,many RCTs have shortfalls.AIM To investigate the RCTs of global colorectal cancer spanning from 2008 to 2018.To provide suggestions for conducting Chinese RCTs of colorectal cancer.METHODS PubMed and Web of Science databases were searched to obtain RCTs of colorectal cancer carried out between January 1,2008,and January 1,2018.The bibliometric method was used for statistical analysis of the publication years,countries/regions,authors,institutions,source journals,quoted times,key words,and authors.RESULTS Colorectal cancer RCTs showed an upward trend between 2008 to 2018;the top 10 research institutions in the included literature were from the United States,the United Kingdom,and other countries with a high incidence of colorectal cancer.Most of the related research journals are sponsored by European and American countries.The 15 most cited studies involved international multicenter clinical research,having few participants from Chinese research institutions.Network visualization using key words showed that RCTs on colorectal cancer focus on screening,disease-free survival,drug treatment,surgical methods,clinical trials,quality of life,and prognosis.The result of the coauthorship network analysis showed that Chinese researchers are less involved in international exchanges compared to those from leading publication countries.CONCLUSION High-quality RCTs are increasingly favored by leading international journals.However,there is still a large gap in clinical research between China and leading countries.Researchers should implement standardized and accurate clinical trials,strengthen international multicenter cooperation,and emphasize quality control.

文摘BACKGROUND This case-control study compared the short-term clinical efficacy of natural orifice specimen extraction surgery(NOSES) using a prolapsing technique and the conventional laparoscopic-assisted approach for low rectal cancer.AIM To further explore the application value of the transanal placement of the anvil and to evaluate the short-term efficacy of NOSES for resecting specimens of low rectal cancer, as well as to provide a theoretical basis for its extensive clinical application.METHODS From June 2015 to June 2018, 108 consecutive laparoscopic-assisted low rectal cancer resections were performed at our center. Among them, 26 specimens were resected transanally using a prolapsing technique(NOSES), and 82 specimens were resected through a conventional abdominal wall small incision(LAP). A propensity score matching method was used to select 26 pairs of matched patients, and their perioperative data were analyzed.RESULTS The baseline data were comparable between the two matched groups. All 52 patients underwent the surgery successfully. The operative time, blood loss,number of harvested lymph nodes, postoperative complication rate,circumferential margin involvement, postoperative follow-up data, and postoperative anal function were not statistically significant. The NOSES group had shorter time to gastrointestinal function recovery(2.6 ± 1.0 d vs 3.4 ± 0.9 d, P= 0.006), shorter postoperative hospital stay(7.1 ± 1.7 d vs 8.3 ± 1.1 d, P = 0.003),lower pain score(day 1: 2.7 ± 1.8 vs 4.6 ± 1.9, day 3: 2.0 ± 1.1 vs 4.1 ± 1.2, day 5: 1.7± 0.9 vs 3.3 ± 1.0, P < 0.001), a lower rate of additional analgesic use(11.5% vs61.5%, P = 0.001), and a higher satisfaction rate in terms of the aesthetic appearance of the abdominal wall after surgery(100% vs 23.1%, P < 0.001).CONCLUSION NOSES for low rectal cancer can achieve satisfactory short-term efficacy and has advantages in reducing postoperative pain, shortening the length of postoperative hospital stay, and improving patients' satisfaction in terms of a more aesthetic appearance of the abdominal wall.

文摘Objective Patients with H1N1 virus infection were hospitalized and quarantined, and some of them developed into acute respiratory failure, and were transfered to the medical intensive care unit of Beijing Ditan Hospital, Capital Medical University in Beijing, China. Methods The clinical features and preliminary epidemiologic findings among 30 patients with confirmed H1N1 virus infection who developed into acute respiratory failure for ventilatory support were investigated. Results A total of 30 patients(37.43 ± 18.80 years old) with 2009 influenza A(H1N1) related acute respiratory distress syndrome(ARDS) received treatment with mechanical ventilation, 15 cases of whom were male and 17 cases died of ARDS. Fatal cases were significantly associated with an APACHE Ⅱ score(P = 0.016), but not with PaO 2 /FIO 2(P = 0.912) and chest radiograph(P = 0.333). The most common complication was acute renal failure(n = 9). Five patients received extracorporeal membrane oxygenation(ECMO), 3 of whom died and the others survived. The major causes of death were multiple organ dysfunction syndrome(MODS)(39%), intractable respiratory failure(27%) and sepsis(20%). Conclusions Most patients with respiratory failure due to influenza A(H1N1) virus infection were young, with a high mortality, particularly associated with APACHE Ⅱ score, secondary infection of lung or type 2 diabetes mellitus.
文摘To clone,express and purify C12orf49 recombinant protein.To prepare rabbit anti-C12orf49 protein polyclonal antibody and further elucidate its biological function.Methods PCR was applied to amplify the gene C12orf49 in vitro.pET-32a(+)-C12orf49,the recombinant protein prokaryotic expression vector,was transformed into E.coli.IPTG was used as an inductive agent to obtain C12orf49 recombinant protein,then the recombinant protein was analyzed with sodium dodecyl sulfatepolyacrylamide gel electrophoresis(SDS-PAGE)and Western blot.Specific polyclonal antibody was derived from rabbits that was immunized by recombinant protein.ELISA and Western blot were used to detect its titer and specificity,respectively.MTT cell proliferation experiment was carried out to observe the effect of protein on proliferation of HepG2 cells.Results The C12orf49 recombinant protein was expressed in a large quantity.Data of ELISA indicated that the titer of polyclonal antibody was higher than 1:1 280 000.And a good specificity of the antibody was confirmed by Western blot.C12orf49 recombinant protein might have an advanced effect on the proliferation of HepG2 cells.Conclusions Using C12orf49 recombinant protein,we can obtain the polyclonal antibody with great titer and good specificity.Human novel gene C12orf49 encoded protein could promote the proliferation of HepG2 cells.

文摘Background Hand, foot, and mouth disease (HFMD) is a common infectious disease in childhood caused by an enterovirus (EV), and which is principally seen in children under 5 years of age. To promote diagnostic awareness and effective treat-ments, to further standardize and strengthen the clinical management and to reduce the mortality of HFMD, the guidelines for diagnosis and treatment have been developed. Methods National Health Commission of China assembled an expert committee for a revision of the guidelines. The com-mittee included 33 members who are specialized in diagnosis and treatment of HFMD. Results Early recognition of severe cases is utmost important in diagnosis and treatment of patients with HFMD. The key to diagnosis and treatment of severe cases lies in the timely and accurate recognition of stages 2 and 3 of HFMD, in order to stop progression to stage 4. Clinicians should particularly pay attention to those EV-A71 cases in children aged less than 3 years, and those with disease duration less than 3 days. The following indicators should alert the clinician of possible deterioration and impending critical disease: (1) persistent hyperthermia;(2) involvement of nervous system;(3) worsening respiratory rate and rhythm;(4) circulatory dysfunction;(5) elevated peripheral WBC count;(6) elevated blood glucose and (7) elevated blood lactic acid. For treatment, most mild cases can be treated as outpatients. Patients should be isolated to avoid cross-infection. Intense treatment modalities should be given for those severe cases. Conclusion The guidelines can provide systematic guidance on the diagnosis and management of HFMD.

文摘The treatment of iron-bearing dusts and sludges by the rotary hearth furnace process has the advantage of sufficient utilization of valuable metals and a high impurity removal rate,but the lower strength of the metallized product needs to be addressed.The effects of quaternary basicity R4(w(CaO+MgO)/w(SiO_(2)+Al_(2)O_(3)))on the reduction behavior and physical and chemical properties of metallized pellets,including phase composition,compressive strength,microstructure and soft melting area,were investigated with FactSage thermodynamic software and experiments.The strength of metallized pellets depended on the gangue composition,such as CaO,MgO,Al_(2)O_(3) and SiO_(2),due to the altered chemical composition,physical phase composition,microscopic morphology and stability of the slag phase.The reduction of carbon-bearing pellets was significantly promoted by suitable basicity.The lower basicity(R_(4)<1.4)facilitated the formation of low melting point iron-containing compounds from SiO_(2) and Al_(2)O_(3) with FeO,resulting in increased liquid phase generation,but lower metallization rate,due to the hindered precipitation and growth of iron grains.Interestingly,the higher basicity(R_(4)>1.8)also increased the amount of liquid phase and improved the strength of the pellets,due to the granular iron crystals bonded into sheets.Notably,the main component of the liquid phase in high-basicity conditions was calcium ferrite.Although the additional amount of liquid phase was beneficial to the strength of the metallized pellets,calcium disilicate was formed at R_(4)=1.6,resulting in a reduction in the compressive strength of the pellets to 1521.9 N/pellet.

文摘Monkeypox is a zoonotic disease.Since the first human monkeypox case was detected in 1970,it has been prevalent in some countries in central and western Africa.Since May 2022,monkeypox cases have been reported in more than 96 non-endemic countries and regions worldwide.As of September 14,2022,there have been more than 58,200 human monkeypox cases,and there is community transmission.The cessation of smallpox vaccination in 1980,which had some cross-protection with monkeypox,resulted in a general lack of immunity to monkeypox,which caused global concern and vigilance.As of Sep-tember 14,2022,there are four monkeypox cases in China,including three in Taiwan province and one in Hong Kong city.Previous foreign studies have shown that children are vulnerable to monkeypox and are also at high risk for severe disease or complications.In order to improve pediatricians'understanding of monkeypox and achieve early detection,early diagno-sis,early treatment,and early disposal,we have organized national authoritative experts in pediatric infection,respiratory,dermatology,critical care medicine,infectious diseases,and public health and others to formulate this expert consensus,on the basis of the latest"Clinical management and infection prevention and control for monkeypox"released by The World Health Organization,the"guidelines for diagnosis and treatment of monkeypox(version 2022)"issued by National Health Commission of the People's Republic of China and other relevant documents.During the development of this consensus,multidisciplinary experts have repeatedly demonstrated the etiology,epidemiology,transmission,clinical manifestations,laboratory examinations,diagnosis,differential diagnosis,treatment,discharge criteria,prevention,disposal process,and key points of prevention and control of suspected and confirmed cases.

文摘Background:Human infections with zoonotic coronaviruses(CoVs),including severe acute respiratory syndrome(SARS)-CoV and Middle East respiratory syndrome(MERS)-CoV,have raised great public health concern globally.Here,we report a novel batorigin CoV causing severe and fatal pneumonia in humans.Methods:We collected clinical data and bronchoalveolar lavage(BAL)specimens from five patients with severe pneumonia from Wuhan Jinyintan Hospital,Hubei province,China.Nucleic acids of the BAL were extracted and subjected to next-generation sequencing.Virus isolation was carried out,and maximum-likelihood phylogenetic trees were constructed.Results:Five patients hospitalized from December 18 to December 29,2019 presented with fever,cough,and dyspnea accompanied by complications of acute respiratory distress syndrome.Chest radiography revealed diffuse opacities and consolidation.One of these patients died.Sequence results revealed the presence of a previously unknownβ-CoV strain in all five patients,with 99.8%to 99.9%nucleotide identities among the isolates.These isolates showed 79.0%nucleotide identity with the sequence of SARS-CoV(GenBank NC_004718)and 51.8%identity with the sequence of MERS-CoV(GenBank NC_019843).The virus is phylogenetically closest to a bat SARS-like CoV(SL-ZC45,GenBank MG772933)with 87.6%to 87.7%nucleotide identity,but is in a separate clade.Moreover,these viruses have a single intact open reading frame gene 8,as a further indicator of bat-origin CoVs.However,the amino acid sequence of the tentative receptor-binding domain resembles that of SARS-CoV,indicating that these viruses might use the same receptor.Conclusion:A novel bat-borne CoV was identified that is associated with severe and fatal respiratory disease in humans.
文摘Background Herpangina is a common infectious disease in childhood caused by an enterovirus.This consensus is aiming to standardize and improve herpangina prevention and clinical diagnosis.Methods The Subspecialty Group of Infectious Diseases,the Society of Pediatric,Chinese Medical Association and Nation Medical Quality Control Center for Infectious Diseases gathered 20 experts to develop the consensus,who are specialized in diagnosis and treatment of herpangina.Results The main pathogenic serotypes of herpangina include Coxsackievirus-A,Enterovirus-A and Echovirus.Its diagnosis can be rendered on the basis of history of epidemiology,typical symptoms,characteristic pharyngeal damage and virological tests.The treatment is mainly symptomatic,and incorporates topical oral spray with antiviral drugs.The course of herpangina generally lasts 4-6 days with a good prognosis.Conclusion The consensus could provide advices and references for the diagnosis,treatment and management of herpangina in children.
文摘In the early February,2020,we called up an experts' committee with more than 30 Chinese experts from 11 national medical academic organizations to formulate the first edition of consensus statement on diagnosis,treatment and prevention of coronavirus disease 2019 (COVID-19) in children,which has been published in this journal.With accumulated experiences in the diagnosis and treatment of COVID-19 in children,we have updated the consensus statement and released the second edition recently.The current version in English is a condensed version of the second edition of consensus statement on diagnosis,treatment and prevention of COVID-19 in children.In the current version,diagnosis and treatement criteria have been optimized,and early identification of severe and critical cases is highlighted.The early warning indicators for severe pediatric cases have been summarized which is utmost important for clinical practice.This version of experts consensus will be valuable for better prevention,diagnosis and treatment of COVID-19 in children worldwide.
文摘Background Previous studies showed that soluble IL-2Rαis an important marker of cellular immune activation and might be a marker of treatment efficacy for children with brucellosis.However,data regarding adult patients with brucellosis were unknown.The aim of study was to explore the potential role of serum sIL-2Rαevaluating treatment responses in adult patients with brucellosis,and T cell immune status was also examined.Methods During January 2016–April 2017,30 patients with acute brucellosis from the Third People’s Hospital of Linfen in Shanxi Province and Beijing Di Tan Hospital,and 28 healthy controls were included in this study.Peripheral blood samples were collected before and after six weeks of antibiotic treatment.Serum sIL-2Rαlevels were measured by enzyme-linked immunosorbent assay,and the percentage of Th1,Th2,Tc1,Tc2,and Tregs was detected by flow cytometry after intracellular staining for cytokines(interferon-γand interleukin-4)and Foxp3 in T lymphocytes from peripheral blood.The obtained data were analyzed with Wilcoxon ranked sum tests for paired values,Mann-Whitney U-tests for comparisons between patients and healthy controls,and Spearman rank tests for correlation analyses.Results Serum sIL-2Rαlevels were significantly higher in patients than in controls(P=0.001).A significant decline was observed in patients after the cessation of treatment(P<0.001)and return to normal(P>0.05).Th1,Tc1,Th2,and Tc2 cell frequencies were higher in patients than in healthy subjects(P<0.05),while the Th1/Th2 and Tc1/Tc2 ratios were significantly lower(P=0.0305 and 0.0005,respectively)and returned to normal levels after treatment.In patients with acute brucellosis,serum sIL-2Rαlevels were negatively correlated with the Th1/Th2 ratio(r=−0.478,P=0.028),Tc1/Tc2 ratio(r=−0.677,P=0.001),and Tc1 percentage(r=−0.516,P=0.017).Serum sIL-2Rαand Tc2 percentages were positively correlated(r=0.442,P=0.045).Conclusions Based on the correlations with Th1/Th2 and Tc1/Tc2 ratios,serum sIL-2Rαlevels may reflect the immune response status.sIL-2Rαmay be a marker for therapeutic efficacy in acute brucellosis.
文摘Coronavirus disease 2019(COVID-19)is a public health disaster that has not been encountered for a hundred years.On January 12,2020,the World Health Organization(WHO)confirmed and named the coronavirus which caused unexplained pneumonia as 2019 novel coronavirus(2019-nCoV).Since then,the disease caused by this virus had been named as coronavirus disease 2019(COVID-19).On February 11,2020,the International Committee on Taxonomy of Viruses announced that the English name of the novel coronavirus was severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)[1].At present,SARS-CoV-2 infection is still rampant worldwide.As of September 10,2021,there were about 222 million confirmed cases of COVID-19 and more than 4.5 million deaths worldwide[2].According to the American Centers for Disease Control and Prevention,by July 29,2021,4.19 million pediatric COVID-19 cases had been reported in the United States。

文摘The targeted construction of donor-acceptor(D-A)materials featuring efficient photothermal(PT)conversion properties has been an attractive but challenging goal.Herein,a new series of coordination polymers(CPs)featuring different PT performances were constructed with flexible linker 1,1′-Ferrocenedicarboxylic acid(FCA)and 2,4,6-tri(pyridin-4-yl)-1,3,5-triazine(TPT)as acceptor and donor molecules including coronene and tetrathiafulvalene(TTF).Based on the flexible configurations of FCA ligand and the directing of donor dependent D-A interactions,the structures of TF-1 to TF-3 featuring distinctive dimension were obtained based on Cd(Ⅱ)ions for investigating the PT property in configurational perspectives.A systematic investigation of the PT properties of the CPs was performed.Notably,TF-3 exhibits the finest PT conversion effect under the irradiation of560 nm laser,while TF-2 shows optimal PT conversion under 808 nm laser irradiation required for biological PT therapy,illustrating the correlation between the structural and component features of the CPs and their PT performances.Furthermore,Zn(Ⅱ)as bio-friendly ion was utilized to construct hypotoxic TF-4 that reveals similar structure to that of TF-2 for potential application.Polydimethylsiloxane(PDMS)patches doped with TF-4 exhibits considerable NIR PT conversion effect under808 nm,represented by the nearly 80℃temperature increased in 120 s for TF-4@PDMS patch(1.2 wt%)under 0.9 W cm^(-2)irradiation,the results of which herein indicate the potential of D-A CPs as versatile platform for the modulation of PT materials.
