It's urgently necessary to develop a guideline of acupuncture for Bell's palsy based on principles of evidence-based medicine to optimize acupuncture treating,standardize outcomes evaluating and to improve the quality of acupuncture for patients with Bell's palsy under general circumstances.Objective: To improve the accuracy of diagnosing and managing Bell's palsy, optimize acupuncture treating and outcomes evaluating for patients with Bell's palsy, and to improve the quality of acupuncture for patients with Bell's palsy in most instances.Methods: This guideline was developed using an explicit and transparent a priori protocol based on supporting evidences and experts' consensus. The guideline developing Group followed the protocol through all stages of the development process: proposed clinical questions,searched clinical evidences, evaluated levels of evidences, developed recommendations, peer reviewed and consummated, and finally formed the draft of this guideline.Results:(1)The guideline development group made a Grade A recommendation that ①With a course of Bell's palsy within 3 months, the patients with mild facial palsy may be treated with any one of acupuncture, western drugs, or acupuncture combing with western drugs,whereas the patients with severe facial palsy may be treated with acupuncture or acupuncture combing with western drugs. With a course of more than 3 months, acupuncture is more suitable.②Acupuncture should be applied as early as possible for Bell's palsy.③The principle of selecting acupoints for Bell's palsy is to select local points, points of corresponding meridians and those according to differentiation. Generally,the points of yangming meridians are the main ones. ④The various methods of acupuncture and moxibustion are adopted for Bell's palsy,including filiform needling, moxibustion, electro-acupuncture, etc. Two or more methods are usually used together in clinical practice.(2) The development group formed expert consensus on the principles of acupuncture treatment for Bell' palsy. Lyapunov functionals are constructed to establish the global asymptotic stability of the uninfected and infected steady states. It is shown that if the basic reproduction number R0 is less than unity, then the uninfected steady state is globally asymptotically stable. The explicit expression of the basic reproduction number is obtained, the local and global asymptotical stability of the disease-free equilibrium is proved under certain conditions. It is shown that the model exhibits the phenomenon of backward bifurcation where the stable disease-free equilibrium coexists with a stable endemic equilibrium. Further, it is proved that the unique endemic equilibrium is globally asymptotically stable under certain conditions.展开更多

The relations between the Human Immunodeficiency Virus-1 (HIV-1) and the human immune system are astonishingly multifaceted, where the critical role for cytotoxic T lymphocytes (CTLs) in the suppression of viral r...The relations between the Human Immunodeficiency Virus-1 (HIV-1) and the human immune system are astonishingly multifaceted, where the critical role for cytotoxic T lymphocytes (CTLs) in the suppression of viral replication in HIV-1 infected individuals cannot be ignored. In this research paper, we have proposed a mathematical model incorporating half saturation constant through the CTL mediated killing process and also in that sense, it has been infiltrated in the generation process of CTL through infected cells. To make the model more realistic, a time lag is introduced in the generation term of CTL population. Also an optimal control theory paradigm is used in our mathematical model to suppress the viral production. From our entire analysis, we have found threshold condition of half saturation constant and treatment schedule so that we can handle the situation of Acquired Immunodeficiency Syndrome (AIDS) patients in a better way. Our analysis reveals that, if the half saturation constant is around 47 mm^-3 in the saturation process and the drug therapy is to be used around 76 days, then we can get adequate results for better treatment of a HIV-1 patient. Based on numerical results, we observed that in a highly unstable situation, administration of chemotherapy at a high dose can stabilize the system.展开更多

DCs play a dual role by enhancing both HIV infection progression, as well as antiviral immune response. To explore the implications of these dual roles, we have formulated our mathematical model and analyzed the model by both analytical and numerical approaches. By using an impulsive differential equation, we have studied the effect of DC-based vaccination. Analytically we have determined the threshold value of drug dosage and dosing interval for optimum levels of injection. We have also investigated the effect of perfect adherence of drug dose on the immune cell count in extreme cases and observed that, systematic drug dose of the immune cells leads to its maximum level.展开更多

It is assumed that strain one can mutate into strain two. Also latent-stage progression age and mutation are incorporated into the model. Stability of equilibria (including the disease free equilibrium, dominant equilibria and the coexistence equilibrium) is investigated and it is found that they are locally stable under suitable and biological feasible constraints. Results indicate that the competition exclusion and coexistence of the two strains are possible depending on the mutation. Numerical simulations are also performed to illustrate these results.展开更多
