The transformation of aldose to ketose or common sugars into rare saccharides,including rare ketoses and aldoses,is of great value and interest to the food industry and for saccharidic biomass utilization,medicine,and...The transformation of aldose to ketose or common sugars into rare saccharides,including rare ketoses and aldoses,is of great value and interest to the food industry and for saccharidic biomass utilization,medicine,and the synthesis of drugs.Nowadays,high-fructose corn syrup(HFCS)is industrially produced in more than 10 million tons annually using immobilized glucose isomerase.Some low-calorie saccharides such as tagatose and psicose,which are becoming popular sweeteners,have also been produced on a pilot scale in order to replace sucrose and HFCS.However,current catalysts and catalytic processes are still difficult to utilize in biomass conversion and also have strong substrate dependence in producing high-value,rare sugars.Considering the specific reaction properties of saccharides and catalysts,since the pioneering discovery by Fischer,various catalysts and catalytic systems have been discovered or developed in attempts to extend the reaction pathways,improve the reaction efficiency,and to potentially produce commercial products.In this review,we trace the history of sugar isomerization/epimerization reactions and summarize the important breakthroughs for each reaction as well as the difficulties that remain unresolved to date.展开更多

Here, we systematically apply a light-sheet illumination, a time-gated detection, and a deep-learning algorithm to yield high-contrast high-resolution volumetric images. To achieve a large Fo V(field of view) and minimize the scattering effect, we generate a light sheet as thin as 100.5 μm with a Rayleigh length of 8 mm to yield an axial resolution of 220 μm. To further suppress the background, we time-gate to only detect long lifetime luminescence achieving a high contrast of up to 0.45 Icontrast. To enhance the resolution, we develop an algorithm based on profile protrusions detection and a deep neural network and distinguish vasculature from a low-contrast area of 0.07 Icontrast to resolve the 100μm small vessels. The system can rapidly scan a volume of view of 75 × 55 × 20 mm3and collect 750 images within 6mins. By adding a scattering-based modality to acquire the 3D surface profile of the mice skin, we reveal the whole volumetric vasculature network with clear depth resolution within more than 1 mm from the skin. High-contrast large-scale 3D animal imaging helps us expand a new dimension in NIR-Ⅱ imaging.展开更多
Objective:To develop a novel diagnostic modality to identify and diagnose stroke in daily life scenarios for improving the therapeutic effects and prognoses of patients.Methods:In this study,16 stroke patients and 24 ...Objective:To develop a novel diagnostic modality to identify and diagnose stroke in daily life scenarios for improving the therapeutic effects and prognoses of patients.Methods:In this study,16 stroke patients and 24 age-matched healthy participants as controls were recruited for comparative analysis.Leveraging a portable eye-tracking device and integrating traditional Chinese medicine theory with modern color psychology principles,we recorded the eye movement signals and calculated eye movement features.Meanwhile,the stroke recognition models based on eye movement features were further trained by using random forest(RF),k-nearest neighbors(KNN),decision tree(DT),gradient boosting classifier(GBC),XGBoost,and CatBoost.Results:The stroke group and the healthy group showed significant differences in some eye movement features(P<.05).The models trained based on eye movement characteristics had good performances in recognizing stroke individuals,with accuracies ranging from 77.40%to 88.45%.Under the red stimulus,the eye movement model trained by RF became the best machine learning model with a recall of 84.65%,a precision of 86.48%,a F1 score of 85.47%.Among the six algorithms,RF and CatBoost performed better in classification.Conclusion:This study pioneers the application of traditional Chinese medicine's five-color stimuli to visual observation tasks.On the basis of the combined design,the eye-movement models can accurately identify stroke,and the developed high-performance models may be used in daily life scenarios.展开更多
Fibre can enhance the mechanical properties of cement-based composites,but fibre also degrades their workability.However,the quantitative effects of fiber content and length-diameter ratio on alkali-activated material...Fibre can enhance the mechanical properties of cement-based composites,but fibre also degrades their workability.However,the quantitative effects of fiber content and length-diameter ratio on alkali-activated materials are still unclear.Various aspect ratio,volume fraction of polyvinyl alcohol fibre(PVAF),and various water-binder ratio were employed to prepare a total of 26 groups of fibre reinforced alkali-activated composite(FRAAC).The influence of PVAF fibre factor(product of fiber volume fraction and length-diameter ratio)on flowability,compactness,strength,and crack fractal dimension of FRAAC was researched.The influence of water-binder ratio on the plastic viscosity of FRAAC was more significant than that on the yield stress.When fibre factor was lower than critical value of 150,the influence of fibres could be overlooked.The reason was that the space between fibres was distant,so the flowability of FRAAC was not impacted by PVAF.At this time,fibres were challenging to restrict the cracks in matrix and increase their mechanical properties.When fibre factor was higher than critical value 150 and lower than density packing value 450,the flexural strength,compressive strength and crack fractal dimension of FRAAC were considerably enhanced,and the FRAAC could still flow easily under dead weight.When fibre factor were above 450,noteworthy fibre balling considerably decreased the flowability,leading to poor solidity and reduced compressive strength.Hence,the ideal content of PVAF in alkali activated composite is between 150/(l/d)and 450/(l/d).展开更多
Anion exchange membrane(AEM)electrolysis is a promising membrane-based green hydrogen production technology.However,AEM electrolysis still remains in its infancy,and the performance of AEM electrolyzers is far behind ...Anion exchange membrane(AEM)electrolysis is a promising membrane-based green hydrogen production technology.However,AEM electrolysis still remains in its infancy,and the performance of AEM electrolyzers is far behind that of well-developed alkaline and proton exchange membrane electrolyzers.Therefore,breaking through the technical barriers of AEM electrolyzers is critical.On the basis of the analysis of the electrochemical performance tested in a single cell,electrochemical impedance spectroscopy,and the number of active sites,we evaluated the main technical factors that affect AEM electrolyzers.These factors included catalyst layer manufacturing(e.g.,catalyst,carbon black,and anionic ionomer)loadings,membrane electrode assembly,and testing conditions(e.g.,the KOH concentration in the electrolyte,electrolyte feeding mode,and operating temperature).The underlying mechanisms of the effects of these factors on AEM electrolyzer performance were also revealed.The irreversible voltage loss in the AEM electrolyzer was concluded to be mainly associated with the kinetics of the electrode reaction and the transport of electrons,ions,and gas-phase products involved in electrolysis.Based on the study results,the performance and stability of AEM electrolyzers were significantly improved.展开更多

Objective:Few data are available on the epidemiology of HPV and cervical cancer among Chinese women younger than 25 years old.This study aimed to estimate the HPV infection rate and the prevalence of cervical intraepi...Objective:Few data are available on the epidemiology of HPV and cervical cancer among Chinese women younger than 25 years old.This study aimed to estimate the HPV infection rate and the prevalence of cervical intraepithelial neoplasia(CIN) in women aged 18-25,as well as their knowledge of and attitudes towards HPV vaccination.Methods:A population-based cervical cancer screening study was conducted on women aged 18-25 in Jiangsu province in 2008.Participants provided socio-demographic,reproductive and behavioral information and completed a survey about their knowledge of and attitudes towards HPV vaccination.Women then underwent a gynecologic exam to provide two cervical exfoliated cell samples for high risk HPV DNA testing and liquid-based cytology(LBC) as well as visual inspection with acetic acid(VIA).Women testing positive for any test were referred to colposcopy and biopsy.The gold standard for diagnosis of cervical lesions was directed or random biopsies.Results:Within the sample of 316 women,3.4% of them were diagnosed with CIN grade 2 or worse lesions and 17.1% were found to be positive for HPV DNA.Among these young women,extra-marital sexual behavior of them(OR=2.0,95%CI:1.1-3.8) or their husbands(OR=2.6,95%:1.4-4.7) were associated with an increased risk of HPV positivity.Although overall HPV awareness was low,after a brief educational intervention,98.4% reported they would electively receive HPV vaccination and would also recommend that their daughters be vaccinated.However,most urban and rural women reported their ideal maximum out-of-pocket contribution for HPV vaccination to be less than 500 RMB and 50-100 RMB,respectively.Conclusion:Our study indicates cervical disease burden is relatively high among sampled Chinese women aged 18-25.Appropriate educational interventions for female adolescents and strategies to subsidize vaccine costs are definitely needed to ensure the effectiveness of vaccination campaigns in China.展开更多

Considerations of combining type-specific prevalence and attribution proportion to high-grade cervical intraepithelial neoplasia are informative to more precise and effective region-specific cervical cancer prevention and control programs. The aim of the current study was to determine the genotype distribution of HPV and attribution to cervical precancerous lesions among women from rural areas in North China.Methods: A total of 9,526 women participated in the cervical cancer screening project in rural China. The samples of women who tested positive for HPV were retested with a polymerase chain reaction(PCR)-based HPV genotyping test. The attribution proportion of specific high-risk human papillomavirus(HR-HPV) types for different grades of cervical lesions was calculated by using the type contribution weighting method.Results: A total of 22.2%(2,112/9,526) of women were HR-HPV positive and HPV52(21.7%) was the most common HR-HPV genotype, followed by HPV58(18.2%), HPV53(18.2%) and HPV16(16.2%). The top three genotypes detected in HR-HPV-positive cervical intraepithelial neoplasia(CIN)1 were HPV16(36.7%), HPV58(20.4%), HPV56(15.3%). Among CIN2+, the most frequent genotypes were HPV16(75.6%), HPV52(17.8%),HPV58(16.7%). HPV16, 56, 58, 53, 52, 59, 68, and 18 combined were attributed to 84.17% of all CIN1 lesions,and HPV16, 58, and 52 combined were attributed to 86.98% of all CIN2+ lesions.Conclusions: The prevalence of HR-HPV infection among women from rural areas in North China was high and HPV16, HPV58, HPV52 had paramount attributable fraction in CIN2+. Type-specific HPV prevalence and attribution proportion to cervical precancerous lesions should be taken into consideration in the development of vaccines and strategy for screening in this population.展开更多

To further explore the role of rituximab when added to the CHOP-like regimen in the treatment of immunohistochemically defined non-germinal center B-cell subtype (non-GCB) diffuse large B-celllymphoma (DLBCL), 159 new...To further explore the role of rituximab when added to the CHOP-like regimen in the treatment of immunohistochemically defined non-germinal center B-cell subtype (non-GCB) diffuse large B-celllymphoma (DLBCL), 159 newly diagnosed DLBCL patients were studied retrospectively based on the immunohistochemical evaluation of CD10, Bcl-6, MUM-1, and Bcl-2. Altogether, 110 patients underwent the CHOP-like regimen, and rituximab was added for the other 49 patients. Cox regression analysis showed that compared with the CHOP-like regimen, the rituximab-based regimen (R-CHOP regimen)significantly decreased the risk of disease relapse and progression in CD10-negative patients (P=0.001),Bcl-6-negative patients (P=0.01), and MUM-1-positive patients (P=0.003). The risk of disease relapse in patients with non-GCB subtype (P=0.002) also decreased. In contrast, patients with the opposite immunohistochemical marker expression profile and GCB subtype did not benefit from treatment with the R-CHOP regimen. In addition, non-GCB subtype patients had a significantly higher expression rate of Bcl-2 than GCB subtype patients (P=0.042). Although univariate analysis found that both Bcl-2-positive and-negative patients had significantly higher event-free survival rates with the R-CHOP regimen, only Bcl-2 positivity (P=0.004) maintained significance in the Cox regression analysis. We conclude that the addition of rituximab can significantly improve the prognosis of patients with non-GCB subtype DLBCL, which is closely related to the expression of CD10, Bcl-6, MUM-1, and Bcl-2.展开更多
Objective: Care HPV is a human papillomavirus(HPV) DNA test for low-resource settings(LRS).This study assesses optimum triage strategies for care HPV-positive women in LRS.Methods: A total of 2,530 Chinese women were ...Objective: Care HPV is a human papillomavirus(HPV) DNA test for low-resource settings(LRS).This study assesses optimum triage strategies for care HPV-positive women in LRS.Methods: A total of 2,530 Chinese women were concurrently screened for cervical cancer with visual inspection with acetic acid(VIA),liquid-based cytology and HPV testing by physician-and self-collected care HPV,and physician-collected Hybrid Capture 2(HC2).Screen-positive women were referred to colposcopy with biopsy and endocervical curettage as necessary.HPV-positivity was defined as ≥1.0 relative light units/cutoff(RLU/CO) for both care HPV and HC2.Primary physician-HC2,physician-care HPV and self-care HPV and in sequential screening with cytology,VIA,or increased HPV test-positivity performance,stratified by age,were assessed for cervical intraepithelial neoplasia(CIN) grade 2/3 or worse(CIN2/3+) detection.Results: The sensitivities and specificities of primary HPV testing for CIN2+ were: 83.8%,88.1% for physiciancare HPV; 72.1%,88.2% for self-care HPV; and 97.1%,86.0% for HC2.Physician-care HPV test-positive women with VIA triage had a sensitivity of 30.9% for CIN2+ versus 80.9% with cytology triage.Self-care HPV testpositive women with VIA triage was 26.5% versus 66.2% with cytology triage.The sensitivity of HC2 test-positive women with VIA triage was 38.2% versus 92.6% with cytology triage.The sensitivity of physician-care HPV testing for CIN2+ decreased from 83.8% at ≥1.0 RLU/CO to 72.1% at ≥10.00 RLU/CO,while the sensitivity of selfcare HPV testing decreased from 72.1% at ≥1.0 RLU/CO to 32.4% at ≥10.00 RLU/CO; similar trends were seen with age-stratification.Conclusions: VIA and cytology triage improved specificity for CIN2/3 than no triage.Sensitivity with VIA triage was unsuitable for a mass-screening program.VIA provider training might improve this strategy.Cytology triage could be feasible where a high-quality cytology program exists.Triage of HPV test-positive women by increased test positivity cutoff adds another LRS triage option.展开更多

Many of these networks require elevated bridges. This results in a bridge-borne noise source, which occurs in addition to the main noise source(i.e., wheel–rail interactions). Bridge-borne noise is attracting increasing attention because of its low-frequency noise characteristics. This review paper first analyzes the space distribution, spectral characteristics, and sound pressure levels of noise radiated by all-concrete, steel–concrete composite, and all-steel bridges, mainly according to experimental studies. Second, this paper reviews existing theoretical prediction models of noise emanating from bridges: the semianalytical method, the Rayleigh integral method, the boundary element method, and statistical energy analysis. Several case studies are reviewed, and their results are discussed. Finally, according to the results of the current review, the main factors affecting bridgeborne noise are analyzed, several noise reduction measures are proposed for different types of bridges, and their effectiveness is demonstrated.展开更多
Objective:The accuracy of colposcopy-guided biopsy is key to the success of colposcopic triage in cervical cancer screening programs.However,there is no widely adopted biopsy guideline up to date.Our study aimed to de...Objective:The accuracy of colposcopy-guided biopsy is key to the success of colposcopic triage in cervical cancer screening programs.However,there is no widely adopted biopsy guideline up to date.Our study aimed to determine whether multi-quadrants biopsy improves the yield of cervical lesions.Methods:Eleven population-based cervical cancer screening studies were conducted in China.Cytology,highrisk human papillomavirus(hrHPV)testing and visual inspection were performed for primary screening.Females positive on one or more tests were referred for colposcopy and biopsy.The proportion of detected cervical intraepithelial neoplasia(CIN)2+and yields by quadrant lesion-targeted biopsy or 4-quadrant random biopsy were compared.Results:Among 4,923 females included,1,606 had quadrant lesion-targeted biopsy,and 3,317 had 4-quadrant random biopsy.The cumulative CIN2+yield increased from 0.10 for only one quadrant-targeted biopsy to 0.21,0.34,and 0.58 for at most two,three and four quadrants targeted biopsies.Among hrHPV positive females with high-grade squamous intraepithelial lesion(HSIL)+cytology,the cumulative CIN2+yield of a second targeted biopsy in another quadrant was significantly increased(P<0.05).Among hrHPV-negative females,the yield of 4-quadrant random biopsies was 0.005,and the yield by lesion-targeted biopsies was 0.017.For hrHPV positive females who had 4-quadrant random biopsy,the additional CIN2+yield for HSIL+,low-grade squamous intraepithelial lesion(LSIL)cytology,or abnormal visual inspection via acetic acid and Lugol’s iodine(VIA/VILI)were 0.46,0.11,0.14.Conclusions:A 4-quadrant random biopsy is recommended only for hrHPV positive females with HSIL cytology,and is acceptable if hrHPV positive with LSIL cytology or with abnormal VIA/VILI.Our findings add evidences for an objective and practical biopsy standard to guide colposcopy in cervical cancer screening programs in low-and middle-income countries.展开更多
BACKGROUND Tissue resident memory T(TRM)cells have been reported to play a significant role in the pathogenesis and relapse of chronic eczema.AIM To compare the efficacy and safety of the intralesional injection of 5-...BACKGROUND Tissue resident memory T(TRM)cells have been reported to play a significant role in the pathogenesis and relapse of chronic eczema.AIM To compare the efficacy and safety of the intralesional injection of 5-fluorouracil(5-FU)and triamcinolone(TA)with those associated with TA alone for the treatment of chronic eczema.METHODS A total of 168 patients were randomized to 5-FU+TA or TA groups and received a one-time intralesional injection of 5-FU+TA or TA only.Biopsies were collected before and 2 wk after treatment for evaluation of histopathological changes.All patients were followed up monthly for up to 1 year.RESULTS No serious adverse event was observed in either group.Although the mean atopic dermatitis severity index scores and effective rates were comparable between the two groups after 2 wk of treatment,the relapse rate was significantly lower in the 5-FU+TA group than in the TA group.Histological examination showed significantly fewer CD8^(+)and CD103^(+)T cells but not CD4^(+)T cells in the 5-FU+TA group.CONCLUSION One-time intralesional injection of 5-FU+TA is effective and safe for chronic eczema treatment and can further reduce the retention of T_(RM) cells in the lesional skin and the relapse rate of chronic eczema.展开更多
AIM To determine the tools needed and problems encountered during the transition of inflammatory bowel disease(IBD) patients from pediatric to adult gastroenterologists(GIs) in Québec, Canada.METHODS We conducted...AIM To determine the tools needed and problems encountered during the transition of inflammatory bowel disease(IBD) patients from pediatric to adult gastroenterologists(GIs) in Québec, Canada.METHODS We conducted a needs assessment survey of Quebec health care professionals(HCPs). The survey was handed out to 136 Québec HCPs at a local conference in 2013. Additionally, it was emailed to any other HCPs in Quebec involved in caring for IBD patients. The completed surveys were compiled to derive descriptive data. Further specific subgroup analysis was then conducted. RESULTS Among the conference attendees and individuals emailed 77(28.2%) completed the questionnaire. Respondents included adult GIs(61.3%), pediatric GIs(20.8%) and IBD nurses(18.3%). The majority of respondents believed that a standardized structure is important for a successful transition. Adult and pediatric GIs equally felt that patients were inadequately prepared for the transition(P = 0.6). There were significant differences between adult and pediatric GIs when it came to resource availability(55.6% vs 90.9%, P = 0.002) and perceived need of a formal transition clinic(21.7% vs 68.8%, P = 0.0006). Both transition program and medical summaries were identified as the most valuable tools to improve transition. CONCLUSION As described in previous studies, our survey reinforces the importance of a transition program, education for young adult IBD patients and the need to improve communication between adult and pediatric GIs.展开更多
Owning to good mechanical properties,steel–concrete composite(SCC)and prestressed concrete(PC)box girders are the types of elevated structures used most in urban rail transit.However,their vibro-acoustic differences ...Owning to good mechanical properties,steel–concrete composite(SCC)and prestressed concrete(PC)box girders are the types of elevated structures used most in urban rail transit.However,their vibro-acoustic differences are yet to be explored in depth,while structure-radiated noise is becoming a main concern in noise-sensitive environments.In this work,numerical simulation is used to investigate the vibration and noise characteristics of both types of box girders induced by running trains,and the numerical procedure is verified with data measured from a PC box girder.The mechanism of vibration transmission and vibro-acoustic comparisons between SCC and PC box girders are investigated in detail,revealing that more vibration and noise arise from SCC box girders.The vibration differences between them are around 7.7 dB(A)at the bottom plate,19.3 dB(A)at the web,and 6.7 dB(A)at the flange,while for structure-radiated noise,the difference is around 5.9 dB(A).Then,potential vibroacoustic control strategies for SCC box girders are discussed.As the vibro-acoustic responses of two types of girders are dominated by the force transmitted to the bridge deck,track isolation is better than structural enhancement.It is shown that using a floating track slab can make the vibration and noise of an SCC box girder lower than those of a PC box girder.However,structural enhancement for the SCC box girder is extremely limited in effects.The six proposed structural enhancement measures reduce vibration by only 1.1–3.6 dB(A) and noise by up to1.5 dB(A).展开更多

文摘The transformation of aldose to ketose or common sugars into rare saccharides,including rare ketoses and aldoses,is of great value and interest to the food industry and for saccharidic biomass utilization,medicine,and the synthesis of drugs.Nowadays,high-fructose corn syrup(HFCS)is industrially produced in more than 10 million tons annually using immobilized glucose isomerase.Some low-calorie saccharides such as tagatose and psicose,which are becoming popular sweeteners,have also been produced on a pilot scale in order to replace sucrose and HFCS.However,current catalysts and catalytic processes are still difficult to utilize in biomass conversion and also have strong substrate dependence in producing high-value,rare sugars.Considering the specific reaction properties of saccharides and catalysts,since the pioneering discovery by Fischer,various catalysts and catalytic systems have been discovered or developed in attempts to extend the reaction pathways,improve the reaction efficiency,and to potentially produce commercial products.In this review,we trace the history of sugar isomerization/epimerization reactions and summarize the important breakthroughs for each reaction as well as the difficulties that remain unresolved to date.

文摘Fluorescence imaging through the second near-infrared window(NIR-Ⅱ,1000–1700 nm) allows in-depth imaging.However, current imaging systems use wide-field illumination and can only provide low-contrast 2D information, without depth resolution. Here, we systematically apply a light-sheet illumination, a time-gated detection, and a deep-learning algorithm to yield high-contrast high-resolution volumetric images. To achieve a large Fo V(field of view) and minimize the scattering effect, we generate a light sheet as thin as 100.5 μm with a Rayleigh length of 8 mm to yield an axial resolution of 220 μm. To further suppress the background, we time-gate to only detect long lifetime luminescence achieving a high contrast of up to 0.45 Icontrast. To enhance the resolution, we develop an algorithm based on profile protrusions detection and a deep neural network and distinguish vasculature from a low-contrast area of 0.07 Icontrast to resolve the 100μm small vessels. The system can rapidly scan a volume of view of 75 × 55 × 20 mm3and collect 750 images within 6mins. By adding a scattering-based modality to acquire the 3D surface profile of the mice skin, we reveal the whole volumetric vasculature network with clear depth resolution within more than 1 mm from the skin. High-contrast large-scale 3D animal imaging helps us expand a new dimension in NIR-Ⅱ imaging.

文摘Objective:To develop a novel diagnostic modality to identify and diagnose stroke in daily life scenarios for improving the therapeutic effects and prognoses of patients.Methods:In this study,16 stroke patients and 24 age-matched healthy participants as controls were recruited for comparative analysis.Leveraging a portable eye-tracking device and integrating traditional Chinese medicine theory with modern color psychology principles,we recorded the eye movement signals and calculated eye movement features.Meanwhile,the stroke recognition models based on eye movement features were further trained by using random forest(RF),k-nearest neighbors(KNN),decision tree(DT),gradient boosting classifier(GBC),XGBoost,and CatBoost.Results:The stroke group and the healthy group showed significant differences in some eye movement features(P<.05).The models trained based on eye movement characteristics had good performances in recognizing stroke individuals,with accuracies ranging from 77.40%to 88.45%.Under the red stimulus,the eye movement model trained by RF became the best machine learning model with a recall of 84.65%,a precision of 86.48%,a F1 score of 85.47%.Among the six algorithms,RF and CatBoost performed better in classification.Conclusion:This study pioneers the application of traditional Chinese medicine's five-color stimuli to visual observation tasks.On the basis of the combined design,the eye-movement models can accurately identify stroke,and the developed high-performance models may be used in daily life scenarios.

文摘Fibre can enhance the mechanical properties of cement-based composites,but fibre also degrades their workability.However,the quantitative effects of fiber content and length-diameter ratio on alkali-activated materials are still unclear.Various aspect ratio,volume fraction of polyvinyl alcohol fibre(PVAF),and various water-binder ratio were employed to prepare a total of 26 groups of fibre reinforced alkali-activated composite(FRAAC).The influence of PVAF fibre factor(product of fiber volume fraction and length-diameter ratio)on flowability,compactness,strength,and crack fractal dimension of FRAAC was researched.The influence of water-binder ratio on the plastic viscosity of FRAAC was more significant than that on the yield stress.When fibre factor was lower than critical value of 150,the influence of fibres could be overlooked.The reason was that the space between fibres was distant,so the flowability of FRAAC was not impacted by PVAF.At this time,fibres were challenging to restrict the cracks in matrix and increase their mechanical properties.When fibre factor was higher than critical value 150 and lower than density packing value 450,the flexural strength,compressive strength and crack fractal dimension of FRAAC were considerably enhanced,and the FRAAC could still flow easily under dead weight.When fibre factor were above 450,noteworthy fibre balling considerably decreased the flowability,leading to poor solidity and reduced compressive strength.Hence,the ideal content of PVAF in alkali activated composite is between 150/(l/d)and 450/(l/d).

文摘Anion exchange membrane(AEM)electrolysis is a promising membrane-based green hydrogen production technology.However,AEM electrolysis still remains in its infancy,and the performance of AEM electrolyzers is far behind that of well-developed alkaline and proton exchange membrane electrolyzers.Therefore,breaking through the technical barriers of AEM electrolyzers is critical.On the basis of the analysis of the electrochemical performance tested in a single cell,electrochemical impedance spectroscopy,and the number of active sites,we evaluated the main technical factors that affect AEM electrolyzers.These factors included catalyst layer manufacturing(e.g.,catalyst,carbon black,and anionic ionomer)loadings,membrane electrode assembly,and testing conditions(e.g.,the KOH concentration in the electrolyte,electrolyte feeding mode,and operating temperature).The underlying mechanisms of the effects of these factors on AEM electrolyzer performance were also revealed.The irreversible voltage loss in the AEM electrolyzer was concluded to be mainly associated with the kinetics of the electrode reaction and the transport of electrons,ions,and gas-phase products involved in electrolysis.Based on the study results,the performance and stability of AEM electrolyzers were significantly improved.

文摘Optical Character Recognition(OCR)refers to a technology that uses image processing technology and character recognition algorithms to identify characters on an image.This paper is a deep study on the recognition effect of OCR based on Artificial Intelligence(AI)algorithms,in which the different AI algorithms for OCR analysis are classified and reviewed.Firstly,the mechanisms and characteristics of artificial neural network-based OCR are summarized.Secondly,this paper explores machine learning-based OCR,and draws the conclusion that the algorithms available for this form of OCR are still in their infancy,with low generalization and fixed recognition errors,albeit with better recognition effect and higher recognition accuracy.Finally,this paper explores several of the latest algorithms such as deep learning and pattern recognition algorithms.This paper concludes that OCR requires algorithms with higher recognition accuracy.
文摘Objective:Few data are available on the epidemiology of HPV and cervical cancer among Chinese women younger than 25 years old.This study aimed to estimate the HPV infection rate and the prevalence of cervical intraepithelial neoplasia(CIN) in women aged 18-25,as well as their knowledge of and attitudes towards HPV vaccination.Methods:A population-based cervical cancer screening study was conducted on women aged 18-25 in Jiangsu province in 2008.Participants provided socio-demographic,reproductive and behavioral information and completed a survey about their knowledge of and attitudes towards HPV vaccination.Women then underwent a gynecologic exam to provide two cervical exfoliated cell samples for high risk HPV DNA testing and liquid-based cytology(LBC) as well as visual inspection with acetic acid(VIA).Women testing positive for any test were referred to colposcopy and biopsy.The gold standard for diagnosis of cervical lesions was directed or random biopsies.Results:Within the sample of 316 women,3.4% of them were diagnosed with CIN grade 2 or worse lesions and 17.1% were found to be positive for HPV DNA.Among these young women,extra-marital sexual behavior of them(OR=2.0,95%CI:1.1-3.8) or their husbands(OR=2.6,95%:1.4-4.7) were associated with an increased risk of HPV positivity.Although overall HPV awareness was low,after a brief educational intervention,98.4% reported they would electively receive HPV vaccination and would also recommend that their daughters be vaccinated.However,most urban and rural women reported their ideal maximum out-of-pocket contribution for HPV vaccination to be less than 500 RMB and 50-100 RMB,respectively.Conclusion:Our study indicates cervical disease burden is relatively high among sampled Chinese women aged 18-25.Appropriate educational interventions for female adolescents and strategies to subsidize vaccine costs are definitely needed to ensure the effectiveness of vaccination campaigns in China.

文摘Objective: Precise prevention is more desired for cervical cancer due to the huge population, high prevalence of human papillomavirus(HPV) infection in China and the vision of screen-and-treat strategies in low-and middleincome countries(LMICs). Considerations of combining type-specific prevalence and attribution proportion to high-grade cervical intraepithelial neoplasia are informative to more precise and effective region-specific cervical cancer prevention and control programs. The aim of the current study was to determine the genotype distribution of HPV and attribution to cervical precancerous lesions among women from rural areas in North China.Methods: A total of 9,526 women participated in the cervical cancer screening project in rural China. The samples of women who tested positive for HPV were retested with a polymerase chain reaction(PCR)-based HPV genotyping test. The attribution proportion of specific high-risk human papillomavirus(HR-HPV) types for different grades of cervical lesions was calculated by using the type contribution weighting method.Results: A total of 22.2%(2,112/9,526) of women were HR-HPV positive and HPV52(21.7%) was the most common HR-HPV genotype, followed by HPV58(18.2%), HPV53(18.2%) and HPV16(16.2%). The top three genotypes detected in HR-HPV-positive cervical intraepithelial neoplasia(CIN)1 were HPV16(36.7%), HPV58(20.4%), HPV56(15.3%). Among CIN2+, the most frequent genotypes were HPV16(75.6%), HPV52(17.8%),HPV58(16.7%). HPV16, 56, 58, 53, 52, 59, 68, and 18 combined were attributed to 84.17% of all CIN1 lesions,and HPV16, 58, and 52 combined were attributed to 86.98% of all CIN2+ lesions.Conclusions: The prevalence of HR-HPV infection among women from rural areas in North China was high and HPV16, HPV58, HPV52 had paramount attributable fraction in CIN2+. Type-specific HPV prevalence and attribution proportion to cervical precancerous lesions should be taken into consideration in the development of vaccines and strategy for screening in this population.
文摘目的:探讨细胞学、高危型人乳头瘤病毒(high risk human papillomavirus,hrHPV)分型对于阴道镜结果正常或低级别鳞状上皮内病变(low-grade squamous intraepithelial lesion,LSIL)妇女的风险预测作用。方法:基于1999年6月在山西省建立的宫颈癌筛查队列,以2005年随访时阴道镜结果为正常或低度病变的596例妇女为研究对象,于2010年和2014年进行随访。分析hrHPV阴性组、hrHPV阳性组、HPV16/18阳性组、细胞学LSIL以下组和细胞学LSIL及以上组发生宫颈上皮内瘤样病变2级及以上(cervi cal intraepithelial neoplasia grade 2 or worse,CIN2+)的瞬时、5年和9年累积风险和相对危险度。结果:细胞学LSIL以下组发生CIN2+的瞬时、5年和9年累积风险分别为0.2%、2.8%和4.2%,细胞学LSIL及以上组相应的风险分别为14.7%(RR=73.8,95%CI为9.7~561.5)、40.0%(RR=16.0,95%CI为8.2~31.1)和51.4%(RR=15.0,95%CI为8.3~27.0)。hrHPV阴性组发生CIN2+的瞬时风险、5年和9年累积风险较低,分别为0.6%、2.7%和3.8%,hrHPV阳性和HPV16/18阳性组发生CIN2+的风险逐渐升高,其中HPV16/18阳性组的相应风险分别为13.2%(RR=23.4,95%CI为5.1~106.9)、36.9%(RR=15.4,95%CI为6.9~34.3)和42.6%(RR=14.1,95%CI为6.8~29.2)。结论:阴道镜结果正常或LSIL妇女,若细胞学结果为LSIL及以上或HPV16/18阳性,未来进展为高度宫颈癌前病变的风险较高,细胞学和HPV16/18分型可用于该人群的临床分流管理。
文摘To further explore the role of rituximab when added to the CHOP-like regimen in the treatment of immunohistochemically defined non-germinal center B-cell subtype (non-GCB) diffuse large B-celllymphoma (DLBCL), 159 newly diagnosed DLBCL patients were studied retrospectively based on the immunohistochemical evaluation of CD10, Bcl-6, MUM-1, and Bcl-2. Altogether, 110 patients underwent the CHOP-like regimen, and rituximab was added for the other 49 patients. Cox regression analysis showed that compared with the CHOP-like regimen, the rituximab-based regimen (R-CHOP regimen)significantly decreased the risk of disease relapse and progression in CD10-negative patients (P=0.001),Bcl-6-negative patients (P=0.01), and MUM-1-positive patients (P=0.003). The risk of disease relapse in patients with non-GCB subtype (P=0.002) also decreased. In contrast, patients with the opposite immunohistochemical marker expression profile and GCB subtype did not benefit from treatment with the R-CHOP regimen. In addition, non-GCB subtype patients had a significantly higher expression rate of Bcl-2 than GCB subtype patients (P=0.042). Although univariate analysis found that both Bcl-2-positive and-negative patients had significantly higher event-free survival rates with the R-CHOP regimen, only Bcl-2 positivity (P=0.004) maintained significance in the Cox regression analysis. We conclude that the addition of rituximab can significantly improve the prognosis of patients with non-GCB subtype DLBCL, which is closely related to the expression of CD10, Bcl-6, MUM-1, and Bcl-2.

文摘Objective: Care HPV is a human papillomavirus(HPV) DNA test for low-resource settings(LRS).This study assesses optimum triage strategies for care HPV-positive women in LRS.Methods: A total of 2,530 Chinese women were concurrently screened for cervical cancer with visual inspection with acetic acid(VIA),liquid-based cytology and HPV testing by physician-and self-collected care HPV,and physician-collected Hybrid Capture 2(HC2).Screen-positive women were referred to colposcopy with biopsy and endocervical curettage as necessary.HPV-positivity was defined as ≥1.0 relative light units/cutoff(RLU/CO) for both care HPV and HC2.Primary physician-HC2,physician-care HPV and self-care HPV and in sequential screening with cytology,VIA,or increased HPV test-positivity performance,stratified by age,were assessed for cervical intraepithelial neoplasia(CIN) grade 2/3 or worse(CIN2/3+) detection.Results: The sensitivities and specificities of primary HPV testing for CIN2+ were: 83.8%,88.1% for physiciancare HPV; 72.1%,88.2% for self-care HPV; and 97.1%,86.0% for HC2.Physician-care HPV test-positive women with VIA triage had a sensitivity of 30.9% for CIN2+ versus 80.9% with cytology triage.Self-care HPV testpositive women with VIA triage was 26.5% versus 66.2% with cytology triage.The sensitivity of HC2 test-positive women with VIA triage was 38.2% versus 92.6% with cytology triage.The sensitivity of physician-care HPV testing for CIN2+ decreased from 83.8% at ≥1.0 RLU/CO to 72.1% at ≥10.00 RLU/CO,while the sensitivity of selfcare HPV testing decreased from 72.1% at ≥1.0 RLU/CO to 32.4% at ≥10.00 RLU/CO; similar trends were seen with age-stratification.Conclusions: VIA and cytology triage improved specificity for CIN2/3 than no triage.Sensitivity with VIA triage was unsuitable for a mass-screening program.VIA provider training might improve this strategy.Cytology triage could be feasible where a high-quality cytology program exists.Triage of HPV test-positive women by increased test positivity cutoff adds another LRS triage option.

文摘In recent years, there has been rapid growth of Chinese rail transit networks. Many of these networks require elevated bridges. This results in a bridge-borne noise source, which occurs in addition to the main noise source(i.e., wheel–rail interactions). Bridge-borne noise is attracting increasing attention because of its low-frequency noise characteristics. This review paper first analyzes the space distribution, spectral characteristics, and sound pressure levels of noise radiated by all-concrete, steel–concrete composite, and all-steel bridges, mainly according to experimental studies. Second, this paper reviews existing theoretical prediction models of noise emanating from bridges: the semianalytical method, the Rayleigh integral method, the boundary element method, and statistical energy analysis. Several case studies are reviewed, and their results are discussed. Finally, according to the results of the current review, the main factors affecting bridgeborne noise are analyzed, several noise reduction measures are proposed for different types of bridges, and their effectiveness is demonstrated.

文摘Objective:The accuracy of colposcopy-guided biopsy is key to the success of colposcopic triage in cervical cancer screening programs.However,there is no widely adopted biopsy guideline up to date.Our study aimed to determine whether multi-quadrants biopsy improves the yield of cervical lesions.Methods:Eleven population-based cervical cancer screening studies were conducted in China.Cytology,highrisk human papillomavirus(hrHPV)testing and visual inspection were performed for primary screening.Females positive on one or more tests were referred for colposcopy and biopsy.The proportion of detected cervical intraepithelial neoplasia(CIN)2+and yields by quadrant lesion-targeted biopsy or 4-quadrant random biopsy were compared.Results:Among 4,923 females included,1,606 had quadrant lesion-targeted biopsy,and 3,317 had 4-quadrant random biopsy.The cumulative CIN2+yield increased from 0.10 for only one quadrant-targeted biopsy to 0.21,0.34,and 0.58 for at most two,three and four quadrants targeted biopsies.Among hrHPV positive females with high-grade squamous intraepithelial lesion(HSIL)+cytology,the cumulative CIN2+yield of a second targeted biopsy in another quadrant was significantly increased(P<0.05).Among hrHPV-negative females,the yield of 4-quadrant random biopsies was 0.005,and the yield by lesion-targeted biopsies was 0.017.For hrHPV positive females who had 4-quadrant random biopsy,the additional CIN2+yield for HSIL+,low-grade squamous intraepithelial lesion(LSIL)cytology,or abnormal visual inspection via acetic acid and Lugol’s iodine(VIA/VILI)were 0.46,0.11,0.14.Conclusions:A 4-quadrant random biopsy is recommended only for hrHPV positive females with HSIL cytology,and is acceptable if hrHPV positive with LSIL cytology or with abnormal VIA/VILI.Our findings add evidences for an objective and practical biopsy standard to guide colposcopy in cervical cancer screening programs in low-and middle-income countries.

文摘BACKGROUND Tissue resident memory T(TRM)cells have been reported to play a significant role in the pathogenesis and relapse of chronic eczema.AIM To compare the efficacy and safety of the intralesional injection of 5-fluorouracil(5-FU)and triamcinolone(TA)with those associated with TA alone for the treatment of chronic eczema.METHODS A total of 168 patients were randomized to 5-FU+TA or TA groups and received a one-time intralesional injection of 5-FU+TA or TA only.Biopsies were collected before and 2 wk after treatment for evaluation of histopathological changes.All patients were followed up monthly for up to 1 year.RESULTS No serious adverse event was observed in either group.Although the mean atopic dermatitis severity index scores and effective rates were comparable between the two groups after 2 wk of treatment,the relapse rate was significantly lower in the 5-FU+TA group than in the TA group.Histological examination showed significantly fewer CD8^(+)and CD103^(+)T cells but not CD4^(+)T cells in the 5-FU+TA group.CONCLUSION One-time intralesional injection of 5-FU+TA is effective and safe for chronic eczema treatment and can further reduce the retention of T_(RM) cells in the lesional skin and the relapse rate of chronic eczema.
文摘AIM To determine the tools needed and problems encountered during the transition of inflammatory bowel disease(IBD) patients from pediatric to adult gastroenterologists(GIs) in Québec, Canada.METHODS We conducted a needs assessment survey of Quebec health care professionals(HCPs). The survey was handed out to 136 Québec HCPs at a local conference in 2013. Additionally, it was emailed to any other HCPs in Quebec involved in caring for IBD patients. The completed surveys were compiled to derive descriptive data. Further specific subgroup analysis was then conducted. RESULTS Among the conference attendees and individuals emailed 77(28.2%) completed the questionnaire. Respondents included adult GIs(61.3%), pediatric GIs(20.8%) and IBD nurses(18.3%). The majority of respondents believed that a standardized structure is important for a successful transition. Adult and pediatric GIs equally felt that patients were inadequately prepared for the transition(P = 0.6). There were significant differences between adult and pediatric GIs when it came to resource availability(55.6% vs 90.9%, P = 0.002) and perceived need of a formal transition clinic(21.7% vs 68.8%, P = 0.0006). Both transition program and medical summaries were identified as the most valuable tools to improve transition. CONCLUSION As described in previous studies, our survey reinforces the importance of a transition program, education for young adult IBD patients and the need to improve communication between adult and pediatric GIs.

文摘Owning to good mechanical properties,steel–concrete composite(SCC)and prestressed concrete(PC)box girders are the types of elevated structures used most in urban rail transit.However,their vibro-acoustic differences are yet to be explored in depth,while structure-radiated noise is becoming a main concern in noise-sensitive environments.In this work,numerical simulation is used to investigate the vibration and noise characteristics of both types of box girders induced by running trains,and the numerical procedure is verified with data measured from a PC box girder.The mechanism of vibration transmission and vibro-acoustic comparisons between SCC and PC box girders are investigated in detail,revealing that more vibration and noise arise from SCC box girders.The vibration differences between them are around 7.7 dB(A)at the bottom plate,19.3 dB(A)at the web,and 6.7 dB(A)at the flange,while for structure-radiated noise,the difference is around 5.9 dB(A).Then,potential vibroacoustic control strategies for SCC box girders are discussed.As the vibro-acoustic responses of two types of girders are dominated by the force transmitted to the bridge deck,track isolation is better than structural enhancement.It is shown that using a floating track slab can make the vibration and noise of an SCC box girder lower than those of a PC box girder.However,structural enhancement for the SCC box girder is extremely limited in effects.The six proposed structural enhancement measures reduce vibration by only 1.1–3.6 dB(A) and noise by up to1.5 dB(A).
