Value of D-Dimers in patients with acute aortic dissection 被引量:1

Serum D-dimers were measured by Sta-Liat... Objective: To evaluatel the value of D-dimers in patients with acute aortic dissection (AAD). Methods: This study consisted of 16 patients with AAD and 27 non-AAD patients. Serum D-dimers were measured by Sta-Liatest D-D1 immunoturbidimetric assay. Results: D-dimer level was higher (P 〈 0.001) in patients with AAD(7.91 ± 5.52 μg/ml) than that in non- AAD group(1.57± 1.24 μg/ml). D-dimer was positive (〉0.4 μg/ml) in all patients with AAD and in 10 control group patients (37%). Among patients with acute AAD, D-dimers tended to be higher in Stanford A than in Stanford B (8.67 ± 4.31 μg/ml vs. 3.24±1.27 μg/ml, P 〈 0.01). D-dimer values tended to be higher in more extended disease(3.84 ± 1.65 μg/ml, 8.57 ± 3.58 μg/ml and 11.87 ± 5.69 μg/ml in thoracic aorta, thoracic and abdominal aorta, thoracic and abdominal aorta and iliacal arteries, respectively, P 〈 0.05 for both 8.57 ± 3.58 and 11.87 ± 5.69 vs. 3.84 ± 1.65 ). Including the control group into the analysis, we found a sensitivity of 100%, a negative predictive value of 100%, and a specificity of 66% and a positive predictive value of 64% for D-dimer in diagnosis of AAD in our patients with suspected AAD. Conclusion: D-dimer was elevated in patients with AAD. A negative D-dimer test result could be useful in excluding AAD. 展开更多

Public buildings such as libraries consume a vast amount of cooling energy for maintaining a comfortable and stable indoor environment in summer,especially in the hot-humid climate.This study used a case study approach to discuss the effect of low-energy strategies that can be applied to improve indoor thermal environment and cooling energy consumption of library buildings in hot and humid cities like Nanning City(a southern city,China).The use of cooling window shutters(a shutter with the effects of shading and evaporative cooling)and ceiling fans for generating airflow was considered as applicable energy-saving measures in this study,and a university library was selected as the study building in which the two energy-saving measures were employed.The SET*and annual cooling load before and after the adoption of the proposed measures were quantitatively investigated with a building energy consumption simulation software(DesignBuilder).Simulation results showed that the daytime SET*values can be reduced by 3.0℃and 4.5℃respectively on a typical summer day after the use of the cooling shutters and ceiling fans.Moreover,the cooling loads can also be decreased by 8.4%and 16.6%respectively.Particularly,the combination of these two measures enabled the daytime SET*value and annual cooling load lower by 7.0℃and 60.8%respectively. 展开更多

We propose a method to generate specially shaped high-order singular beams of pre-designed intensity distributions. Such a method does not a priori assume a phase formula, but rather relies on the 'cake-cutting and assembly' approach to achieve the azimuthal phase gradient for beam shaping, inspired by the orbital motion trajectory change of an artificial satellite. Based on our method, several typical vortex beams with desired intensity patterns are experimentally generated. As an example, we realize optical trapping and transportation of microorganisms with a triangle-shaped vortex beam, demonstrating the applicability of such unconventional vortex beams in optical trapping and manipulation. 展开更多

The discrepancy of indoleamine 2,3-dioxygenase 1(IDO1)function in atherosclerosis has been noted.Compared to the protective effect of IDO1 against established atherogenesis,the role of IDO1 in the developmental process of atherosclerosis is still unclear.Here,the expression patterns and activities of IDO1 and its isoenzyme tryptophan 2,3-dioxygenase(TDO)in aortas and blood samples of patients with atherosclerosis were investigated.IDO1 and TDO were colocalized with CD3-positive lymphocytes and CD68-positive macrophages in atherosclerotic lesions.The expression and activity of IDO1 and TDO increased with the grade of the histological classification in early atherosclerosis(grade I,II),but the increase did not continue in advanced atherosclerosis(grade III).Treatment of THP-1 macrophages(THP-M)with oxidized low-density lipoprotein(oxLDL)induced the expression of IDO1 via the PI3K/Akt/NF-κB pathway,indicating the potential function of IDO1 in foam cells.Before and after treatment with oxLDL on THP-M,IFN-γ-induced IDO1 exhibited different degrees of promotion on foaming,inflammatory factor production and cell apoptosis.Finally,we found that the IDO1 inhibitor 1-methyl-tryptophan could elevate the high-density lipoprotein cholesterol level in serum and reduce the area of the aortic atherosclerotic lesions in high-fat diet-fed ApoE−/−mice.Our study indicated that IDO1 played a complicated and unfixed role in the entire process of atherogenesis,despite the atheroprotective role in established atherosclerosis.IDO1 also had proatherosclerotic functions in the developmental stages of atherosclerosis.Modulation of IDO1 could be a good method for alleviating atherosclerosis. 展开更多

To the Editor: Left atrial appendage (LAA) is one of the major sources of atrial arrhythmias in children.[1] But atrial tachycardia (AT) with an epicardial LAA origin is uncommon and usually arises from the base.[2] Catheter ablation (CA) may be an important strategy for patients with atrial arrhythmia,[3] especially due to the LAA firing trigger.[4] Due to a large number of trabeculations and true epicardial location, endocardial CA in LAA may fail or even cause fatal tamponade.[2,5] The epicardial CA may be an alternative approach.[6] Because the LAA has a very thin wall and may be prone to perforation, caution should be taken when LAA ablation is performed,[4] even during an epicardial ablation. There may be recurrence of AT, and surgical left atrial appendectomy (SAE) may be required. 展开更多
