目的:比较不同剂量地塞米松对罗哌卡因臂丛神经阻滞效果的影响。方法:采取双盲对照原则,选取择期上肢骨折手术患者100例,随机分成5组,A组0.4%罗哌卡因复合地塞米松0.2 mg/kg混合液4 0m L,静脉注射生理盐水1 m L;B组0.4%罗哌卡因复合地...目的:比较不同剂量地塞米松对罗哌卡因臂丛神经阻滞效果的影响。方法:采取双盲对照原则,选取择期上肢骨折手术患者100例,随机分成5组,A组0.4%罗哌卡因复合地塞米松0.2 mg/kg混合液4 0m L,静脉注射生理盐水1 m L;B组0.4%罗哌卡因复合地塞米松0.15 mg/kg混合液40 m L,静脉注射生理盐水1 m L;C组0.4%罗哌卡因复合地塞米松0.1 mg/kg混合液40 m L,静脉注射生理盐水1 m L;D组0.4%罗哌卡因40 m L,静脉注射地塞米松1 m L(5 mg);E组0.4%罗哌卡因40 m L,静脉注射生理盐水1 m L。观察记录加入不同剂量的地塞米松在0.4%罗哌卡因臂丛腋路神经阻滞中的起效时间,维持时间,记录临床麻醉满意度,记录5组不同时间点的平均动脉压(MAP),心率(HR)及术后24 h不良反应(如嗜睡、恶心、呕吐,皮肤瘙痒、呼吸抑制等)。结果:A、B、C组感觉神经起效时间分别为(3±1)min、(3±1)min、(4±5)min,均明显低于D组、E组(P<0.05);A、B、C组运动神经起效时间分别为(11±2)min、(11±2)min、(13±8)min,均明显低于D组、E组(P<0.05);A、B、C组运动神经维持时间分别(8±9)h、(8±8)h、(7±5)h,均明显高于D组、E组(P<0.05);A、B、C组患者麻醉效果满意者分别为16、16、15,明显高于D组、E组(P<0.05)。5组患者不同时间点平均动脉压、心率差异均无统计学意义(P>0.05);5组患者术后24小时均无恶心、呕吐、嗜睡、神经过敏等不良反应,E组有1例运动神经未阻滞。结论:0.15 mg/kg地塞米松复合0.4%罗哌卡因进行腋路臂丛神经阻滞,起效时间短,维持时间长,可达到满意阻滞效果。展开更多

Influences of working gas pressure and pulse width of the bias voltage on pr...Diamond-like carbon (DLC) films were synthesized by plasma immersion ion implantation and deposition (PIIID) on 9Cr18 bearing steel surface. Influences of working gas pressure and pulse width of the bias voltage on properties of the thin film were investigated. The chemical compositions of the as-deposited films were characterized by Raman spectroscopy. The micro-hardness, friction and wear behavior, corrosion resistance of the samples were evaluated, respectively. Compared with uncoated substrates, micro-hardness results reveal that the maximum is increased by 88.7%. In addition, the friction coefficient decreases to about 0.1, and the corrosion resistance of treated coupons surface are improved significantly.展开更多
