Electroencephalogram findings in 10 patients with post-stroke epilepsy:A retrospective study

BACKGROUND Post-stroke epilepsy is a common and easily overlooked complication of acute cerebrovascular disease.Long-term seizures can seriously affect the prognosis and quality of life of patients.Electroencephalogram(EEG)is the simplest way to diagnose epilepsy,and plays an important role in predicting seizures and guiding medication.AIM To explore the EEG characteristics of patients with post-stroke epilepsy and improve the detection rate of inter-seizure epileptiform discharges.METHODS From January 2017 to June 2020,10 patients with post-stroke epilepsy in our hospital were included.The clinical,imaging,and EEG characteristics were collected.The stroke location,seizure type,and ictal and interictal EEG manifestations of the patients with post-stroke epilepsy were then retrospectively analyzed.RESULTS In all 10 patients,epileptiform waves occurred in the side opposite to the stroke lesion during the interictal stage;these manifested as sharp wave,sharp-wave complex,or spike discharges in the anterior head lead of the side opposite to the lesion.CONCLUSION In EEG,epileptiform waves can occur in the side opposite to the stroke lesion in patients with post-stroke epilepsy. 展开更多

Objective To investigate the impact of CYP2C19 gene polymorphism on clopidogrel reactivity and its association with longterm clinical outcome in patients with coronary heart disease(CHD)undergoing percutaneous coronary intervention(PCI).Methods In total,675 patients were enrolled.Based on the platelet inhibition rate,patients were categorized into two groups:clopidogrel low responsiveness(CLR)and normal clopidogrel responsiveness(NCR).The CLR group was divided into ticagrelor and clopidogrel group based on the antiplatelet drugs used in the follow-up treatment.Patients were classified into three groups(normal metabolizer,intermediate metabolizer,and poor metabolizer)based on the CYP2C19 genotype.We aimed to evaluate the impact of CYP2C19 gene polymorphism on clopidogrel reactivity.The cumulative rates of 12-month all-cause deaths,major adverse cardiovascular events(MACCEs),and bleeding events were calculated.Results CLR was observed in 44.4%of the overall population.Significant differences were observed in the platelet inhibition rate of clopidogrel among the three metabolic genotypes(P<0.05).At the 12-month follow-up,13 patients(1.9%)died and 96 patients(14.2%)experienced MACCEs.Patients with CLR(9.6%vs.11.7%vs.22.1%,P<0.05)or poor metabolizer(10.7%vs.16.4%vs.22.6%,P=0.026)experienced a higher rate of MACCEs.A MACCEs risk score between zero and two was calculated.The highest incidence of MACCEs significantly increased with the 2-positive results,and the area under the curve(AUC)was 0.712(95%CI:0.650-0.774,P<0.05).There was no significant difference between the group with a score of one and the occurrence of MACCEs(P>0.05).Conclusions Low response to clopidogrel in CHD patients is correlated with CYP2C19 gene polymorphism.CYP2C19 genotyping combined with platelet reactivity is an independent predictor of 12-months MACCEs in patients with clopidogrel treatment after PCI,which is better than either test alone. 展开更多

Ischemic accumulation of succinate causes cerebral damage by excess production of reactive oxygen species. However, it is unknown whether ischemic accumulation of succinate affects neural stem cell proliferation. In this study, we established a rat model of cerebral ischemia/reperfusion injury by occlusion of the middle cerebral artery. We found that succinate levels increased in serum and brain tissue(cortex and hippocampus) after ischemia/reperfusion injury. Oxygen-glucose deprivation and reoxygenation stimulated primary neural stem cells to produce abundant succinate. Succinate can be converted into diethyl succinate in cells. Exogenous diethyl succinate inhibited the proliferation of mouse-derived C17.2 neural stem cells and increased the infarct volume in the rat model of cerebral ischemia/reperfusion injury. Exogenous diethyl succinate also increased the succinylation of the Rho family GTPase Cdc42 but repressed Cdc42 GTPase activity in C17.2 cells. Increasing Cdc42 succinylation by knockdown of the desuccinylase Sirt5 also inhibited Cdc42 GTPase activity in C17.2 cells. Our findings suggest that ischemic accumulation of succinate decreases Cdc42 GTPase activity by induction of Cdc42 succinylation, which inhibits the proliferation of neural stem cells and aggravates cerebral ischemia/reperfusion injury. 展开更多

AIM:To investigate the affecting factors of parapapillary gamma and delta zones and other fundus morphological features in high myopia.METHODS:Seventy high myopia patients were included in this retrospective observational study and 47 patients were female.Patients were divided into three groups:no posterior staphyloma(no PS),PS with myopic traction maculopathy(PS with MTM),and PS without MTM using 3-dimensional magnetic resonance imaging and optical coherence tomography.MTM patients were fur ther classified into three types[epiretinal membrane,macular hole,and macular retinoschisis(MRS)].Diameters of the gamma and delta zones were measured among other morphometric variables using fundus photographs.RESULTS:Of the 70 individuals(127 eyes),the mean age was 57.46±13.56y.In univariate analysis,morphological features changed most dramatically in PS with MTM patients,who had the largest gamma zone diameters,the largest disk-fovea distance(DFD)and disk-fovea angle,and the smallest angle kappa and vertical distance of temporal arterial arcade.However,their horizontal delta zone diameter was smaller than in the patients with PS yet without MTM.In multivariate analysis,with axial length(AL)and age adjusted,the horizontal diameter in the delta zone of the PS without MTM group was still significantly larger than in the PS with MTM group(P=0.024).Comparing the three subtypes of MTM patients,the diameters of the gamma zone and DFD in MRS group were the largest.CONCLUSION:The characteristics of the gamma and delta zones change inconsistently in different stages of high myopia.These changes may be associated with anatomical changes caused by local traction.Factors such as PS,AL and age play an important role.These findings may provide a hint about the pathogenesis of traction in high myopia. 展开更多

BACKGROUND Remote ischemic conditioning(RIC)is used to protect against myocardial injury.However,there is no adequate evidence for comprehensive RIC in elderly patients with ST-segment elevation myocardial infarction(STEMI).This study aimed to test whether comprehensive RIC,started pre-primary percutaneous coronary intervention(PPCI)and repeated daily on 1-30 days post-PPCI,can improve myocardial salvage index(SI),left ventricular ejection fraction(LVEF),Kansas City Cardiomyopathy Questionnaire Clinical Summary Score(KCCQ-CSS)and 6-min walk test distance(6MWD)in elderly patients with acute STEMI during 12 months follow-up.METHODS 328 consenting elderly patients were randomized to receive standard PPCI plus comprehensive RIC(the treatment group)or standard PPCI(the control group).SI at 5-7 days after PPCI,LVEF,left ventricular end-diastolic volume index(LVEDVI),left ventricular end-systolic volume index(LVESVI),KCCQ-CSS,6MWD and adverse events rates were measured and assessed.RESULTS SI was significantly higher in the treatment group[interquartile range(IQR):0.38-0.66,P=0.037].There were no significant differences in major adverse events at 12 months.Although the differences of LVEDVI,LVESVI and LVEF between the treatment group and the control group did not reach statistical significance at 6 months and 12 months,LVEF tended to be higher,LVEDVI tended to be lower in the treatment group.The KCCQ-CSS was significantly higher in the treatment group at 1 month(IQR:46.5-87,P=0.001)and 12 months(IQR:55-93,P=0.008).There was significant difference in 6MWD between the treatment group and the control group(IQR:258-360 vs.IQR:250-345,P=0.002)at 1 month and(IQR:360-445 vs.IQR:345-432,P=0.035)at 12 months.A modest correlation was found between SI and LVEF(r=0.452,P<0.01),KCCQ-CSS(r=0.440,P<0.01)and 6MWD(r=0.384,P<0.01)respectively at 12 months.CONCLUSIONS The comprehensive RIC can improve SI,KCCQ-CSS and 6MWD.It may be an adjunctive therapy to PPCI in elderly patients with STEMI. 展开更多

METHODS: Totally, 90 patients(90 eyes) were included. They were divided into high myopia group and control group. Ocular parameters, including axial length(AL), mean keratometry(Km), anterior chamber depth(ACD), and white to white(WTW), were obtained from the OA-2000 and IOLMaster 500. RESULTS: For the control group, we applied BlandAltman graphs to assess the 95% limits of agreement(LoA) for most parameters including AL, ACD, Km, and WTW(-0.24 to 0.29 mm,-0.22 to 0.45 mm,-0.39 to 0.31 D, and-0.90 to 0.86 mm, respectively). In high myopia patients, AL, ACD, Km values had wider 95% LoA(-0.34 to 0.32 mm,-0.36 to 0.34 mm,-0.57 to 0.47 D, respectively), except WTW(-0.80 to 0.68 mm). Differences were not statistically significant between these two instruments(P>0.05). CONCLUSION: Most parameters obtained by the OA-2000 and IOLMaster 500 are comparable, including the AL, ACD, and K values. Among them, the agreement of the high myopia patients is poor compared to the patients without high myopia. 展开更多

AIM: To evaluate the effect of internal limiting membrane(ILM) peeling with indocyanine green(ICG), brilliant blue G(BBG), triamcinolone acetonide(TA), trypan blue(TB), or without dye for the treatment of idiopathic macular hole(IMH). METHODS: A search was conducted using Pub Med, EMBASE, and CENTRAL(Cochrane Central Register of Controlled Trials) for related studies published before October 2018. RESULTS: A total of 29 studies and 2514 eyes were included in this network Meta-analysis. For IMH closure, the rank from the best to the worse treatment was: BBG, TB, TA, ICG, and no dye. There was a significant difference in postoperative IMH closure rate between BBG and no dye. The rank of the best to the worse treatment to improve visual acuity was: BBG, TB, no dye, TA, and ICG. The improvement rate of visual acuity after using BBG was significantly higher than ICG. The improvement rate of visual acuity was more favorable with TB than ICG, TA, and no dye. CONCLUSION: BBG can contribute to better anatomical and functional outcomes compared to other dyes for ILM peeling in patients with IMH. The results show that the best treatment of ILM peeling with dyes is BBG. 展开更多

The mutation in CNKSR2 leads to a broad spectrum of phenotypic variability and manifests as an X-linked intellectual disability. However, we reported that the male patient in this study not only had intellectual disability but also epileptic seizures. In addition, there were progressive language impairment, attention deficit hype-ractivity disorder and autism. Electroencephalograms showed continuous spike-and-wave during sleep. Genetic testing revealed a de novo mutation of the CNKSR2 gene(c.2185C >T, p.Arg729Ter) in the child that was not detected in the parents. Therefore, the child was diagnosed with X-linked epilepsy aphasia syndrome. Deletion of the CNKSR2 gene has been rarely reported in epilepsy aphasia syndrome, but no de novo mutation has been found in this gene. This report not only adds to the spectrum of epilepsy aphasia syndrome but also helps clinicians in diagnosis and genetic counseling. 展开更多

AIM:To demonstrate the differences of retinal and choroidal structure changes in internal carotid artery(ICA) patients of China by enhanced-depth imaging optical coherence tomography(EDI-OCT).METHODS:A retrospective review was conducted of 46 patients with a diagnosis of ICA stenosis greater than 65% on only one side(the opposite ICA Stenosis had less than 40% stenosis) from June 2015 through June 2016.All of the patients were combined with amaurosis fugax,but without any abnormality with other ocular examination.Thickness and volume of choroid and retina were manually measured by EDI-OCT.Differences were compared between ICA stenosis eyes and fellow eyes.RESULTS:There were no significant differences in the retinal thickness,macular retinal nerve fiber layer(m RNFL) thickness,inner thickness,or outer retinal thickness between the ICA Stenosis group and the control group(P=0.834,0.187,0.552,and 0.903,respectively).The mean central choroidal thickness of the ICA Stenosis group was significantly lower than that of the control group(239.70±23.76 μm vs 257.46±22.13 μm,P<0.001).The percentage of ICA stenosis was significantly associated with the central choroidal thickness,central retinal thickness,foveal center choroidal volume,and foveal center retinal volume(r=0.854,0.678,0.729,and 0.785,respectively;P<0.001).There were no significant differences in the retinal and choroidal volume values in the 4 inner and 4 outer sectors between the two groups.CONCLUSION:The choroidal thickness in severe ICA stenosis eyes is lower than in fellow eyes.The choroidal thinning may occur before the retinal changes in patients with ocular ischemic syndrome.Evaluations of choroidal thickness may be useful to choose the optimal therapeutic schedule for ICA patients. 展开更多

Knowing methane desorption characteristics is essential to define the contribution of adsorbed gas to gas well production.To evaluate the synthetic effect of a clay stabilizer solution on methane desorption kinetics and isotherms pertaining to Longmaxi shale,an experimental setup was designed based on the volumetric method.The objective was to conduct experiments on methane adsorption and desorption kinetics and isotherms before and after clay stabilizer treatments.The experimental data were a good fit for both the intraparticle diffusion model and the Freundlich isotherm model.We analyzed the effect of the clay stabilizer on desorption kinetics and isotherms.Results show that clay stabilizer can obviously improve the diffusion rate constant and reduce the methane adsorption amount.Moreover,we analyzed the desorption efficiency before and after treatment as well as the adsorbed methane content.The results show that a higher desorption efficiency after treatment can be observed when the pressure is higher than 6.84 MPa.Meanwhile,the adsorbed methane content before and after treatment all increase when the pressure decreases,and clay stabilizer can obviously promote the adsorbed methane to free gas when the pressure is lower than 19 MPa.This can also be applied to the optimization formulation of slickwater and the design of gas well production. 展开更多

The relation between serum phosphorus levels and long-term mortality in Chinese patients with ST-segment elevation myocardial infarction

Background Elevated serum phosphorus levels may be associated with adverse outcomes in cardiovascular disease. This study aimed to investigate the relation between serum phosphorus levels and risk of all-cause mortality in Chinese patients with ST-segment elevation myocardial infarction (STEMI) who had preserved renal function at baseline. Methods We enrolled patients with STEMI who had preserved renal function at baseline in Xuanwu Hospital from January 2011 to December 2016. Those patients were divided into four groups based on serum phosphorus levels. All-cause mortality rates were compared between groups. Mean duration of follow up was 54.6 months. We used Cox proportional-hazards models to examine the relation between serum phosphorus levels and all-cause mortality after adjustment for potential confounders. Results 1989 patients were involved and 211 patients (10.6%) died during follow-up. Based on serum phosphorus levels, patients were categorized into the following groups:< 2.50 mg/dL (n = 89), 2.51–3.50 mg/dL (n = 1066), 3.51–4.50 mg/dL (n = 672) and > 4.50 mg/dL (n = 162), respectively. The lowest mortality occurred in patients with serum phosphorus levels between 2.51–3.50 mg/dL, with a multivariable-adjusted hazard ratio of 1.19 (95% CI: 0.64–1.54), 1.37 (95% CI: 1.22–1.74), and 1.46 (95% CI: 1.35–1.83) in patients with serum phosphorus levels of < 2.50 mg/dL, 3.51–4.50 mg/dL and > 4.50 mg/dL, respectively. Conclusions Elevated serum phosphorus levels were associated with all-cause mortality in Chinese patients with STEMI who had preserved renal function at baseline. 展开更多

Each individual underwent a blood routine and a kidney function examination. Routine ophthalmologic examination included best-corrected visual acuity(BCVA), a slit-lamp examination to detect cataract type, determination of intraocular pressure, a corneal endothelial count, and fundus examination. All patients received phacoemulsification and an IOL implantation.·RESULTS: For the types of cataract in the three groups, transplantation group was significantly different from normal control group(P =0.04), the most kind is posterior subcapsular cataract(PSC) in transplantation group 33(64.7%), hemodialysis group had no significantly difference from normal control group(P =0.43), and the difference between transplantation group and hemodialysis group also had significantly difference(P=0.02). For postoperative BCVA in the three groups,transplantation group had significantly difference from normal control group(P =0.03), hemodialysis group was significantly different from normal control group(P =0.00), and the difference between transplantation group and hemodialysis group also had significantly difference(P =0.00). The multiple linear regression equation is Y =0.007 hemoglobin(Hb)-0.000233 serum creatinine(Cr),R2=0.898. Postoperative fundus examination showed that hemorrhage, exudation, and macular degeneration were greater in the hemodialysis group.·CONCLUSION: This study showed that the PSC wasmore in the renal transplantation patients. BCVA was better and fundus lesions were less frequent in the renal transplantation group than in the hemodialysis group after cataract surgery. The multiple linear regression was showed that the Hb was positively correlated with postoperative BCVA, while Cr was negatively correlated with postoperative BCVA. These results may act as indicators in predicting visual acuity for the renal transplantation and hemodialysis patients. 展开更多

AIM:To evaluate whether narrowing of internal carotid artery siphon(ICAS)may increase the risk of developing non-arteritic anterior ischaemic optic neuropathy(NAION).METHODS:Totally 30 consecutive patients who had unilateral NAION and 30 gender-matched control subjects were recruited in the present study.The diameter of ICAS of all the participants were measured using head-and-neck computed tomographic angiography(CTA).Color doppler flow imaging(CDI)was used to measure the haemodynamics parameters of ICAS and short posterior ciliary arteries(SPCAs)in all subjects.Comparison of parameters between the NAION patients and controls as well as between the two sides within the patients were performed.The correlation between the diameter of ICAS and NAION was analyzed.RESULTS:A comparison of parameters between the affected side of the NAION patients and the controls,including the diameter of ICAS,the resistance index(RI)of ICAS,the blood flow velocities of SPCAs and RI of SPCAs,showed significantly difference(P<0.01),while there was no significant difference in terms of the mean blood flow velocity(Vm)of ICAS;Similar results were found while comparing all the measurements of the affected and unaffected side of patients(P for RI of SPCAs<0.05).No marked difference was detected in nearly all parameters except for RI of ICAS and SPCAs between the unaffected side of the NAION patients and the controls(P<0.05).The diameter of ICAS were significantly positive correlated with both peak systolic velocity(PSV)of SPCAs and end diastolic velocity(EDV)of SPCAs in patients with NAION(r=0.514,P<0.01 and r=0.418,P<0.05,respectively).CONCLUSION:Narrowing of ICAS may increase the risk of developing NAION. 展开更多

Coarctation of the aorta(CoA)refers to the congenital coarctation near the isthmus,ductus arteriosus or ligamentum arteriosus,with an incidence of 5%–10%in children with congenital heart defects.[1]It can be isolated or coexisted with ventricular septal defects,subaortic stenosis,patent ductus arteriosus,and mitral aortic valve.[2]CoA has a poor prognosis.Without intervention,the median age of death with aortic coarctation is 38 years[3]and 75%death of those patients is at the age of 46 years.[4]Causes of death included congestive heart failure(26%),aortic rupture(21%),bacterial endocarditis(18%),and intracranial hemorrhage(12%).[5]Therefore,early diagnosis and optimal treatment are the key to improve the prognosis of this disease.The aim of this report is underlining the importance of early and accurate diagnose of CoA as a cause of systemic hypertension in young patients and also emphasizing the genetic factors of CoA in twins. 展开更多

t Successful pregnancy in placental mammals substantially depends on the establishment of maternal immune tolerance to the semi-allogenic fetus.Disorders in this process are tightly associated with adverse pregnancy o... t Successful pregnancy in placental mammals substantially depends on the establishment of maternal immune tolerance to the semi-allogenic fetus.Disorders in this process are tightly associated with adverse pregnancy outcomes including recurrent miscarriage(RM).However,an indepth understanding of the systematic and decidual immune environment in RM remains largely lacking.In this study,we utilized single-cell RNA-sequencing(scRNA-seq)to comparably analyze the cellular and molecular signatures of decidual and peripheral leukocytes in normal and unexplained RM pregnancies at the early stage of gestation.Integrative analysis identifies 22 distinct cell clusters in total,and a dramatic difference in leukocyte subsets and molecular properties in RM cases is revealed.Specifically,the cytotoxic properties of CD8^(+)effector T cells,nature killer(NK),and mucosal-associated invariant T(MAIT)cells in peripheral blood indicates apparently enhanced pro-inflammatory status,and the population proportions and ligand–receptor interactions of the decidual leukocyte subsets demonstrate preferential immune activation in RM patients.The molecular features,spatial distribution,and the developmental trajectories of five decidual NK(dNK)subsets have been elaborately illustrated.In RM patients,a dNK subset that supports embryonic growth is diminished in proportion,while the ratio of another dNK subset with cyto toxic and immune-active signature is significantly increased.Notably,a unique pro-inflammatory CD56^(+)CD16^(+)dNK subset substantially accumulates in RM decidua.These findings reveal a comprehensive cellular and molecular atlas of decidual and peripheral leukocytes in human early pregnancy and provide an in-depth insight into the immune pathogenesis for early pregnancy loss. 展开更多

Split Hopkinson pressure bar test system was used to investigate the plastic deformation behavior and dynamic response character of a-type Ti–5Al–2.5Sn ELI and near a-type Ti–8Al–1Mo–1V titanium alloy when subjected to dynamic loading. In the present work, stress–strain curves at strain rate from 1.5 9 103to 5.0 9 103s-1were analyzed, and optical microscope(OM) was used to reveal adiabatic shearing behavior of recovered samples. Results show that both the two alloys manifest significant strain hardening effects. Critical damage strain rate of the two alloys is about 4.3 9 103s-1, under which the impact absorbs energy of Ti–5Al–2.5Sn ELI and Ti–8Al–1Mo–1V are 560 and 470 MJ m-3, respectively. Both of them fracture along the maximum shearing strength orientation, an angle of 45° to the compression axis. No adiabatic shear band(ASB) is found in Ti–5Al–2.5Sn ELI alloy, whereas several ASBs with different widths exist without regular direction in Ti–8Al–1Mo–1V alloy. 展开更多

Serving as the interface between the fetal and maternal environments during gestation,the placenta plays critical roles in the protection of the developing fetus and the maintenance of maternal health.The placenta is primarily derived from the embryonic trophectoderm which differentiates into various subtypes of trophoblast cells through villous and extravillous pathways.The interactions among trophoblasts and multiple decidual cells and immune cells at the maternal-fetal interface fundamentally form the functional units of the placenta,which are responsible for blood perfusion and maternal-fetal material exchange,immune tolerance,and the regulation of pregnancy adaptation.Defects in placental development and functional maintenance are in tight association with adverse pregnancy outcomes such as preeclampsia.In this article,we review recent advances on human trophoblast cell differentiation and the construction of placental functional units and discuss the placental and maternal factors that may contribute to the occurrence of preeclampsia. 展开更多

In this work,we study the local distinguishability of maximally entangled states(MESs).In particular,we are concerned with whether any fixed number of MESs can be locally distinguishable for sufficiently large dimensions.Fan and Tian et al.have already obtained two satisfactory results for the generalized Bell states(GBSs)and the qudit lattice states when applied to prime or prime power dimensions.We construct a general twist-teleportation scheme for any orthonormal basis with MESs that is inspired by the method used in[Phys.Rev.A 70,022304(2004)].Using this teleportation scheme,we obtain a sufficient and necessary condition for one-way distinguishable sets of MESs,which include the GBSs and the qudit lattice states as special cases.Moreover,we present a generalized version of the results in[Phys.Rev.A 92,042320(2015)]for the arbitrary dimensional case. 展开更多
