Therefore, it is an important component of comprehensive hemophilia care.Unfortunately, most hemophilic families in Shanxi province, China do not continue home therapy. Objective: To analyze the reasons that hemophilic families do not continue home therapy and to provide a foundation for formulating targeted interventions by the Hemophilia Treatment Center(HTC) in Shanxi Province. Methods: A qualitative phenomenological approach using purposeful sampling of 17 hemophilic families, two physicians, and one nurse from 11 branches across 11 cities in Shanxi Province. Interviews were recorded, transcribed verbatim and analyzed using qualitative content analysis. Results: Four themes were identified:(1) home therapy was generally positively evaluated;(2) concerns about the safety of home therapy;(3) an imperfect social medical security system; and(4) inadequate continuous nursing of hemophilia. Conclusions: Home therapy for hemophilia remains in a preliminary stage in Shanxi Province, and inadequate family and social support systems have obstructed the implementation of home therapy. It is necessary to develop a home therapy model suitable for hemophilic families in Shanxi Province as soon as possible. Such a model would improve the community and home care medical systems for hemophilia and guarantee home therapy throughout the entire medical care system, thus enabling more hemophilic families to carry out home therapy. Achieving these goals requires urgent research and attention from medical staff.展开更多

Methods: Puerperae who underwent cesarean section and got postoperative incision infection in Maternal and Child Health Care in Lintong between January 2014 and December 2016 were selected as the infection group of the research, and puerperae who underwent cesarean section and were without postoperative incision infection in Maternal and Child Health Care in Lintong during the same period were selected as the control group of the research. The contents of inflammatory factors and stress hormones in serum as well as the contents of immune cells and the levels of erythrocyte immunity indexes in peripheral blood were measured. Results:TNF-α, IL-1, IL-6, IL-8, Cor, NE, INS and AT-II contents in serum of infection group were significantly higher than those of control group;CD3+CD4+T cell and CD16+CD25+NK cell contents as well as DTER, ATER, NTER and FEER levels in peripheral blood were lower than those of control group whereas CD3+CD8+T cell content and FEIR level were higher than those of control group. TNF-α, IL-1, IL-6 and IL-8 contents in serum of infection group were negatively correlated with CD3+CD4+T cell and CD16+CD25+NK cell contents as well as DTER, ATER, NTER and FEER levels in peripheral blood, and positively correlated with CD3+CD8+T cell content and FEIR level in peripheral blood as well as Cor, NE, INS and AT-II contents in serum. Conclusion: The abnormal activation of inflammatory response and the abnormal secretion of inflammatory factors are closely related to the immune response disorder and stress response activation.展开更多

This study examined an array of factors, including waist circumference (WC) and folate deficiency, which may mediate the association of BMI with serum ALT concentration in Chinese hypertensive adults without known hepatic diseases. Methods:A multicenter, cross-sectional study was carried out. A total of 378 patients with mild or moderate hypertension and without known hepatic diseases were recruited from five hospitals in Harbin, Shanghai, Beijing, Xi'an, and Nanjing. Results:Of the 360 hypertensive patients with complete data in our final analysis, 13.6% had high ALT concentrations (>40 IU/L). Factors including BMI, WC, triglyceride level, and folate concentration were associated with ALT concentration in univariate analysis. Consistently higher prevalence rates of elevated ALT were observed in subjects with lower folate concentrations (≥12 vs. <12 nmol/L, 9.9% vs. 17.8%, P=0.03), with higher BMI (≥28 vs. <28 kg/m2, 21.5% vs. 11.4%, P=0.02) or higher WC (≥90 vs. <90 cm, 18.5% vs. 10.0%, P=0.02). However, in multivariate analysis, the association between BMI and ALT concentration disappeared (P=0.802 in males and 0.369 in females), while WC in females (P<0.001) and folate concentration (P=0.036 in males and 0.044 in females) remained as significant predictors for ALT concentration. Conclusions:This multicenter study demonstrated that WC and low folate concentration were important factors underlying the association between BMI and ALT concentrations in Chinese hypertensive adults without known hepatic diseases.展开更多
