高原人群CYP2C9和VKORC1基因多态性及其与华法林用药剂量的相关性研究 被引量:3

目的探讨高原人群基于中国人群华法林用药剂量计算公式(PRC模型)和国际华法林遗传药理学协会推荐的亚裔人群华法林剂量计算公式(IWPC模型)两种模型预测剂量的准确性及其临床应用价值。方法回顾性分析曲靖市第一人民医院2016年10月—2020... 目的探讨高原人群基于中国人群华法林用药剂量计算公式(PRC模型)和国际华法林遗传药理学协会推荐的亚裔人群华法林剂量计算公式(IWPC模型)两种模型预测剂量的准确性及其临床应用价值。方法回顾性分析曲靖市第一人民医院2016年10月—2020年1月375例行华法林代谢基因多态性检测患者的临床资料。通过qRT-PCR检测CYP2C9和VKORC1基因多态性,记录患者基本信息和临床用药情况,采用两种模型计算预测剂量并分析其与维持剂量[国际标准化比值(INR)稳定维持在2.0~3.0范围内时所服用的华法林剂量]的相关性,评估两模型预测的准确性。结果在实际治疗过程中患者是否选择服用华法林进行抗凝,通常与患者的性别、身高、吸烟史、是否合并房颤、是否注射低分子肝素钙无关,而与患者年龄、体重、体表面积(BSA)、初INR、是否置换主动脉瓣膜、是否服用阿司匹林、利伐沙班或氯吡格雷相关。375例患者CYP2C9和VKORC1基因频率符合Hardy-Weinberg遗传平衡定律,其中CYP2C9基因*1/*1(AA)、*1/*3(AC)及*3/*3(CC)基因型频率分别为93.07%(349/375)、6.93%(26/375)和0.00%(0/375),VKORC1-1639基因AA、AG和GG基因型频率分别为82.66%(310/375)、16.27%(61/375)和1.07%(4/375)。无论是使用PRC还是IWPC模型,除CYP2C9*1/*1&VKORC1AA(n=289)组与CYP2C9*1/*3&VKORC1AG(n=5)组预测剂量的差异无统计学意义(P>0.05)外,其他所有基因型组预测剂量两两比较均有差异(P<0.05)。在收集到维持剂量的174例患者中,CYP2C9*1/*1&VKORC1 AG和CYP2C9*1/*1&VKORC1 GG组维持剂量分别为[(3.41±1.01)mg,n=30]和[(4.75±0.35)mg,n=2],均高于CYP2C9*1/*1&VKORC1 AA组[(2.59±0.73)mg,n=136](P<0.05);CYP2C9*1/*3&VKORC1 AA组维持剂量为[(2.00±0.53)mg,n=5],低于其他基因型组合(P<0.05)。PRC和IWPC模型预测准确性分别为72.99%(127/174)和62.64%(109/174);Pearson相关系数(r_(1)=0.546,r_(2)=0.567);决定系数(R_(1)^(2)=0.298,R_(2)^(2)=0.322)。两个模型预测剂量间无差异(r=0.839,P>0.05)。结论携带CYP2C9*3等位基因的患者对华法林更敏感,所需华法林剂量较低,而携带VKORC1-1639 G等位基因的患者需要更高剂量的华法林才可以获得有效的抗凝疗效。基因组学预测剂量可为临床使用华法林剂量提供依据,提高抗凝治疗的安全性和有效性。 展开更多

Analyzing and predicting the learning behavior data of students in blended teaching can provide reference basis for teaching.Aiming at weak generalization ability of existing algorithm models in performance prediction,a BP neural network is introduced to classify and predict the grades of students in the blended teaching.L2 regularization term is added to construct the BP neural network model in order to reduce the risk of overfitting.Combined with Pearson coefficient,effective feature data are selected as the validation dataset of the model by mining the data of Chao-Xing platform.The performance of common machine learning algorithms and the BP neural network are compared on the dataset.Experiments show that BP neural network model has stronger generalizability than common machine learning models.The BP neural network with L2 regularization has better fitting ability than the original BP neural network model.It achieves better performance with improved accuracy. 展开更多

Objective:To study the effect of sevoflurane pretreatment before one-lung ventilation on pulmonary compliance, oxygenation function and systemic inflammatory response.Methods:104 patients who accepted one-lung ventilation operation in Mianyang Central Hospital between April 2014 and December 2015 were selected and randomly divided into the sevoflurane group who accepted sevoflurane pretreatment combined with routine anesthesia and the control group who accepted routine anesthesia, and the pulmonary compliance and oxygenation function parameters during one-lung ventilation as well as the contents of inflammatory mediators in serum and the contents of inflammation regulation molecules in alveolar lavage fluid were determined.Results: At T1, T2 and T3, Cdyn and PaO2/FiO2 levels of sevoflurane group were significantly higher than those of control group, and serum ICAM-1, IL-6, IL-8 and TNF-α contents were significantly lower than those of control group while IL-10 contents were significantly higher than those of control group;after operation, AQP-1, p38MAPK, TLR4 and NF-κB contents in collapsed alveolar lavage fluid of sevoflurane group were significantly lower than those of control group.Conclusion: Sevoflurane pretreatment before one-lung ventilation can improve the pulmonary compliance and oxygenation function, and inhibit the activation of inflammatory response. 展开更多

NiFe layered double hydroxides(NiFe LDHs)have been intensively developed for the oxygen evolution reaction(OER)in alkaline media;however,their unsatisfactory hydrogen evolution reaction(HER)performance limits their practical application in overall water splitting.Herein,a simple and efficient one-step electrodeposition method is used to accomplish in situ growth of NiFe LDHsNiFe alloy gradient hybrid coatings on a carbon cloth(CC).Within the binder-free electrode,NiFe LDHs nanosheets with a low-crystalline nature exhibit highly active bifunctional OER/HER activities,and the NiFe alloy acts as a stable electron highway and strong skeleton bridge between NiFe LDHs and the CC.When the electrodes are simultaneously employed as the cathode and anode for overall water splitting,they require low cell potentials of 1.441 V at10 mA·cm^(-2)and 1.703 V at 100 mA·cm^(-2),respectively,and they demonstrate outstanding stability at a current density greater than 100 mA·cm^(-2)for more than 100 h.This is one of the best bifunctional OER and HER catalysts for overall water splitting.Both lattice defects and surface reconstructions crucially contribute to the bifunctional OER/HER activities of NiFe LDHs.This simple and scalable synthesis approach presents an intriguing paradigm for industrial production,and the fabricated electrode has potential application in high-current-density water splitting. 展开更多

Transcription factors(TFs)control an array of expressed genes.However,the specifics of how a gene is expressed in time and space as controlled by a TF remain largely unknown.Here,in TRPC6-regulated proline oxidase 1(POX)transcription in human glioma,we report that OIP5-AS1,a long noncoding RNA,determines the specificity of p53-driven POX expression.The OIP5-AS1/p53 complex via its 24 nucleotides binds to the POX promoter and is necessary for POX expression but not for p21 transcription.An O-site in the POX promoter to which OIP5-AS1 binds was identified that is required for OIP5-AS1/p53 binding and POX transcription.Blocking OIP5-AS1 binding to the O-site inhibits POX transcription and promotes glioma development.Thus,the OIP5-AS1/O-site module decides p53-controlled POX expression as regulated by TRPC6 and affects glioma development. 展开更多

We present a detailed study of dynamically generating an M2 brane from super-gravitons(or DO branes)in a pp-wave background possessing maximal spacetime SUSY.We have three kinds of dynamical solutions depending on the excess energy which appears as an order parameter signalling a critical phenomenon about the solutions.As the excess energy is below a critical value,we have two branches of the solution,one can have its size zero while the other cannot for each given excess energy.However there can be an instanton tunnelling between the two.Once the excess energy is above the critical value,we have a single solution whose dynamical behavior is basically independent of the background chosen and whose size can be zero at some instant.A by product of this study is that the size of particles or extended objects can grow once there is a non-zero excess energy even without the presence of a background flux,therefore lending support to the spacetime uncertainty principle. 展开更多

From December 25,2019 to January 31,2020,33 cases of the coronavirus disease 2019(COVID-19)were identified in the Department of Respiratory and Critical Care Medicine of Zhongnan Hospital of Wuhan University,China,yet none of the affiliated HCWs was infected.Here we analyzed the infection control measures used in three different departments in the Zhongnan Hospital of Wuhan University and correlated the measures with the corresponding infection data of HCWs affiliated with these departments.We found that three infection control measures,namely the isolation of the presumed positive patients,the use of facemasks and intensified hand hygiene play important roles in preventing nosocomial transmission of COVID-19. 展开更多

We study the dynamical Myers effect by allowing the fuzzy(or the dynamical dielectric brane)coordinates to be time dependent.We find three novel kinds of the dynamical spherical dielectric branes depending on their respective excess energies.The first represents a dynamical spherical brane carrying a negative excess energy(having a lower bound)with its radius oscillating periodically between two given non-zero values.The second is the one with zero excess energy and whose time dependence can be expressed in terms of a simple function.This particular dynamical spherical configuration represents the dielectric brane creation and/or annihilation like a photon in the presence of a background creating an electron-position pair and then annihilating back to a photon.The third is the one carrying positive excess energy and the radius can also oscillate periodically between two non-zero values but,unlike the first kind,it passes zero twice for each cycle.Each of the above can also be interpreted as the time evolution of a semi-spherical D-brane–anti semi-spherical D-brane system. 展开更多
