BACKGROUND Deep endometriosis(DE)is the most aggressive subtype of endometriosis.The diagnosis may be challenging,and no biomarkers that can discriminate women with DE from those without DE have been developed.AIM To evaluate the role of blood hemostatic parameters and inflammatory indices in the prediction of DE.METHODS This case-control study was performed at the Women’s Hospital,Zhejiang University School of Medicine between January 2015 and December 2016.Women with DE and women with benign gynecologic disease(control group)eligible for gynecological surgery were enrolled.Routine plasma hemostatic parameters and inflammatory indices were obtained before surgery.Univariate and multivariate analysis were performed.Receiver operating characteristic(ROC)curves were generated,and areas under the curve(AUC)were calculated to assess the predictive values of the selected parameters.RESULTS A total of 126 women were enrolled,including 31 with DE and 95 controls.Plasma fibrinogen(Fg,P<0.01),international normalized ratio(P<0.05),and Creactive protein levels(P<0.01)were significantly higher in women with DE compared with controls.Plasma hemoglobin(HB)levels(P<0.05)and shortened thrombin time(P<0.05)were significantly lower in women with DE than in controls.Plasma Fg levels[adjusted OR(aOR)2.12,95%confidence interval(CI):1.31-3.75]and plasma HB levels(aOR 0.48,95%CI:0.29-0.78)were significantly associated with DE(both P<0.05).ROC analysis showed that the diagnostic value of Fg or HB alone for DE was limited.The AUC of the combination of both markers as a dual marker index was 0.773 with improved sensitivity(67.7%)and specificity(78.9%)at cutoffs of 3.09 g/L and 126 g/L,respectively.CONCLUSION The combination of Fg and HB was a reliable predictor of DE.A larger study is needed to confirm the findings. 展开更多

The interaction effect of grip strength and lung function (especially FVC) on cardiovascular diseases: a prospective cohort study in Jiangsu Province, China

OBJECTIVE Lung function and grip strength(GS)are associated with cardiovascular disease(CVD),but whether these risk factors interact to affect CVD is unknown.This study aimed to explore the interactions between lung function and GS with major CVD(defined as fatal/non-fatal myocardial infarction,stroke,and heart failure)incidence.METHODS We conducted a prospective cohort study on the Chinese population in Jiangsu Province.Cox proportional haz-ards models were used to explore the associations between GS,lung function,and major CVD incidence.RESULTS A total of 5967 participants were included in our study;among them,182 participants developed major CVD.Parti-cipants with low forced vital capacity(FVC)had a higher risk of major CVD(hazard ratio(HR)=1.45;95%confidence interval(CI):1.05-2.01;P<0.05)compared with normal FVC.The risk of major CVD incidence(HR=0.54;95%CI:0.35-0.83;P<0.01)was significantly lower in participants with high GS than in individuals with low GS.The interaction between FVC and GS for major CVD incidence(P=0.006)was statistically significant.Compared with normal FVC participants with high GS,low FVC parti-cipants with low GS had the highest risk of major CVD incidence(HR=2.50;95%CI:1.43-4.36;P<0.01).CONCLUSION Among people with low FVC,the risk of major CVD is lower with high GS.Participants with low FVC and low GS have the highest risk of major CVD.Therefore,more attention should be paid to the incidence of major CVD in individu-als with low FVC,especially those who have lower GS. 展开更多

Aim:To study a possible defect in spermatogenesis of Fragile X syndrome(FXS)patients.Methods:Two different polymerase chain reaction(PCR)based methods were used for the molecular diagnosis of FXS.Sperm collection was done mostly according to the laboratory manual of the World Health Organization.Results:We failed to collect sperm samples from five Fragile X subjects aged 18-60 years as a result of an unexpected erectile dysfunction(ED). Multiple examinations of the same subject at different times,and of different subjects from different provinces by different physicians,showed the same result consistently in all five subjects examined.Conclusion:Erectile reflex is an instinctive response in all healthy males.The absence of erection can be caused by hormonal,physical or neuronal malfunction.As hormonal profiles were reported to be generally normal in Fragile X men,we propose that an unknown physical factor or the neuronal circuit,or both,underlying the erection is compromised.The finding of ED in Fragile X patients may help better understand the clinical spectrum and pathogenesis of the disease.(Asian J Androl 2006 Jul;8:483-487) 展开更多

Background:Comprehensive management of diabetes should include management of its comorbid conditions,especially cardiovascular complications,which are the leading cause of morbidity and mortality among patients with diabetes.Dyslipidemia is a comorbid condition of diabetes and a risk factor for cardiovascular complications.Therefore,lipid level management is a key of managing patients with diabetes successfully.However,it is not clear that how well dyslipidemia is managed in patients with diabetes in local Chinese health-care communities.This study aimed to assess how well low-density lipoprotein cholesterol (LDL-C) was managed in Nanjing community hospitals,China.Methods:We reviewed clinical records of 7364 diabetic patients who were treated in eleven community hospitals in Nanjing from October 2005 to October 2014.Information regarding LDL-C level,cardiovascular risk factors,and use of lipid-lowering agents were collected.Results:In patients without history of cardiovascular disease (CVD),92.1% had one or more CVD risk factors,and the most common CVD risk factor was dyslipidemia.The overall average LDL-C level was 2.80 ± 0.88 mmol/L,which was 2.62 ± 0.90 mmol/L and 2.82 ± 0.87 mmol/L in patients with and without CVD history respectively.Only 38% of all patients met the target goal and 37.3% of patients who took lipid-lowering agents met target goal.Overall,24.5% of all patients were on lipid-lowering medication,and 36.3% of patients with a CVD history and 20.9% of patients without CVD history took statins for LDL-C management.The mean statin dosage was 13.9 ± 8.9 mg.Conclusions:Only a small portion of patients achieved target LDL-C level,and the rate of using statins to control LDL-C was low.Managing LDL-C with statins in patients with diabetes should be promoted,especially in patients without a CVD history and with one or more CVD risk factors. 展开更多
